OBJECTIVE To analyze the risk factors of pulmonary infection after liver transplantation and improve treatment strategy.METHODS Forty six adult patients who underwent liver transplantation were divided into two groups: pulmonary infection group and non-pulmonary infection group.An analysis was performed for the commonly used variables.RESULTS The frequency of pulmonary infection after liver transplantation was 43.5%,the mortality rate in the patients who developed pulmonary infection was 60%.Intraoperative total fluid perfusion, mechanical ventilatory time,serum creatinine,albumin,abdominal bleeding,and hydrothorax after liver transplantation were risk factors of pulmonary infection(P

Clinical data of 448 patients aged ≥60 years with osteoporotic hip fractures admitted in Shanghai Huashan Hospital between January 2010 and December 2014 were retrospectively analyzed.Among 448 cases,there were 143 males and 305 females;196 cases aged 80-89 years (43.8％) and 137 cases aged 70-79 years (30.6％);304 cases of femoral neck fracture (67.9％) and 144 cases of intertrochanteric femoral fracture (32.1％);the fractures caused by fall in 328 cases (73.2％) and comorbidity with hypertension and diabetes mellitus in 415 cases (92.6％).Two hundred and sixty eight patients were followed up for 2 years,among whom re-fractures occurred in 17 cases.The study indicates that health education is important for prevention of osteoporotic fractures in elderly people.

Objective To observe the survival of EAE rats after recombinant adenovirus NgR specific RNA interference(Ad-shRNA-NgR) transfected the brain tissue of the experimental autoimmune encephalomyelitis(EAE),and provide the basis for EAE intervention.Methods EAE rats were randomly divided into high,medium,low and control groups(20 rats in each group).The lateral ventricle of EAE rats were injected with a titer of 1×1011 pfu/mL,1×1010 pfu/mL and 1×109 pfu/mL Ad-shRNA-NgR.The survival of EAE rats at third and seventh days after injection was observed.Results The survival rate of EAE rats of the high titer group was significantly lower than those of the middle titer group and low titer group at third and seventh days after Ad-shRNA-NgR transfected into EAE brain tissue.There was no significant difference in survival rate in middle titer group,low titer group and control group.Conclusion The titer of Ad-shRNA-NgR is safe in the experiment of EAE rats from 1×1010 pfu/mL to the range of 1×109 pfu/mL.

Objective To detect the changes and the clinical significance in plasma protein-complement factor H-related protein 2 (CFHR2) in pulmonary arterial hypertension (PAH) associated with congenital heart disease (CHD) children.Methods Various types of 66 CHD patients with or without PAH and 16 healthy children(healthy control group) were studied,including 11 ventricular septal defects (VSD) with PAH (VSD-PAH),11 isolated VSD,11 atrial septal defects with PAH (ASD-PAH),11 isolated ASD,11 mixed type of heart defects [two or more defects of VSD,ASD and patent ductus arteriosus (PDA)] with PAH (Mix-PAH) and 11 cases without PAH (Mix).CFHR2 was validated by enzyme linked immunosorbent assay in the sample plasma.Results Compared with the healthy control group,the CFHR2 concentration in VSD-PAH patients [(189.10 ±24.01) μg/L vs.(42.99 ±4.53) μg/L,t =4.975,P ＜0.01] and VSD patients [(189.10 ±24.01) μg/L vs.(165.00 ±23.17) μg/L,t =2.661,P ＜ 0.05] were lower.The CFHR2 protein was also confirmed to be decreasing significantly in VSD-PAH patients compared with VSD patients (t =4.698,P ＜ 0.01).The plasma CFHR2 level in ASD-PAH patients [(189.10 ± 24.01)μg/L vs.(70.92 ± 8.27) μg/L,t =3.951,P ＜0.01] and ASD patients [(189.10 ±24.01) μg/L vs.(72.48 ± 8.99) μg/L,t =3.880,P ＜ 0.01] were significantly lower than those in the healthy control group,although there was no significant difference between ASD-PAH and ASD patients (t =0.128,P ＞ 0.05).The plasma CFHR2 level in Mix-PAH patients [(189.10 ± 24.01) μg/L vs.(83.23 ± 15.96) μL,t =3.314,P ＜ 0.05] was significantly lower than that in the healthy control group,while Mix patients [(189.10 ±24.01) μg/L vs.(170.40 ±33.15) μg/L,t =0.468,P ＞ 0.05] had no difference compared with the healthy control group,but had statistical significance with M ix PAH group (t =2.370,P ＜ 0.05).Conclusions The decrease of CFHR2 protein may demonstrate the deficiency of the immune system and coagulation mechanism in these patients and can be consi-dered as biomarker of CHD-PAH disease.

Objective To observe effect of environmental enrichment on the learning and memory ability and the expressions of hypoxia inducible factor 1α (HIF-1a) and vascular endothelial growth factor (VEGF) in rats with chronic cerebral hypoperfusion.Methods A total of 40 rats were randomly divided into 3 groups: bilateral vascular occlusion (2VO) of the common carotid arteries group (n=14, 2VO group), 2VO + enriched environment (EE) group (n=14, 2VO+EE group) and sham group (n=12, SHAM group).Morris water maze, novel object recognition test, real-time reverse transcription-polymerase chain reaction (real-time RT-PCR), immunohistochemistry methods and Western blotting were used to detect changes in learning and memory ability of rats and HIF-1α and VEGF expression levels in hippocampus.Results Morris water maze showed that the escape latency was longer in the 2VO group than in the SHAM group at 3, 4 and 5 day during the training (all P＜0.05), while the 2VO+EE group spent significantly less time in finding the platform as compared with the 2VO group at 4 and 5 day (both P＜0.05).The time for space exploration in target quadrant was less in 2VO group than in SHAM group (P＜0.05), while it was longer in 2VO +EE group than in 2VO group (P＜0.05).Novel object recognition test showed that the 2VO operation impaired the priority index (PI) of time spending at exploring the novel object (P＜0.05), and environmental enrichment could improve the PI in 2VO group (P＜0.05).The real-time RT-PCR and Western blotting showed that the HIF-1α mRNA expression was higher in 2VO group than in SHAM group (P＜0.05).The VEGF mRNA and protein expressions were higher in 2VO+EE group than in 2VO group (both P＜0.05).The expression of HIF-1α in hippocampal CA1 area was higher in 2VO group than in SHAM group (P＜0.05).Conclusions Environmental enrichment can alleviate the damages of spatial and non-spatial learning and memory ability which are caused by chronic cerebral hypoperfusion.And HIF-1α and its downstream gene VEGF may be involved in the restoration of cognitive function by enriched environment.

Objective To evaluate the overall failure rate of one or two-stage exchange arthroplasty for infections in total knee arthroplasty (TKA) and the predictors affecting the outcome of exchange.Methods Thirty-nine cases received one or two-stage exchange arthroplasty for periprosthetic joint infections after primary TKA in Shanghai Sixth People's Hospital affiliated to Shanghai Jiao Tong University School of Medicine and Southeast Hospital affiliated to Xiamen University from January 2012 to November 2014 were reviewed.Periprosthetic tissue and articular fluid of all patients were analyzed by bacterial culture.All patients were followed up for more than one year.C-reactive protein (CRP),erythrocyte sedimentation rate (ESR),procalcitonin (PCT) and blood routine were tested every four weeks,and the evaluation on pain,total periprosthetic function,range of motion and deformation of arthroplasty were conducted.Differences between groups were analyzed using chi-square test or Student's t test when appropriate.A stepwise selection approach in logistic regression analysis was used to screen key predictors for outcome of one or two-stage exchange for infections in TKA.Results There were 39 patients who had undergone one or two-stage exchange for infections in TKA,including 20 males (51.3％) and 19 females (48.7％) with an average age of (62.4±11.7) years.Among the 39 patients,18 gram-positive strains were isolated from specimens,and 6 gram negative strains,2 Mycobacterium tuberculosis and 1 candida albicans.Ten of 39 reimplantations developed reinfection.Between the success and failure groups,there were significant differences in the time from primary TKA to revision (P =0.023),operative time (P =0.029),multidrug resistant organisms (P =0.045),the preoperative and post-operative ESR (P=0.002 and P＜0.001,respectively) and post-operative CRP (P=0.018).Multivariable logistic regression analysis demonstrated that time from primary TKA to revision (OR =0.96,95％CI:0.92-1.00,P=0.025),preoperative ESR (OR=0.97,95％CI:0.95-1.00,P=0.045) and post-operative ESR (OR =0.94,95％ CI:0.91-0.98,P =0.002) were independent indicators associated with the outcome of one or two-stage revision.Conclusions The failure rate after revision for infected TKA is relatively high.The time from primary TKA to revision,preoperative and post-operative ESR could predict the outcome of one or two-stage revision effectively.

Objective To estimate the 10-year risk of coronary heart disease in patients with psoriasis,and to determine whether the risk profile is affected by disease duration,family history and clinical types of psoriasis.Methods Data were pooled from 322 inpatients with psoriasis and 297 inpatients with other skin diseases (151 cases of acute urticaria,33 drug eruption,113 herpes zoster) from December 2008 to December 2010 in the Department of Dermatology,West China Hospital of Sichuan University.Risks of coronary heart disease were estimated by using the Framingham risk score algorithm.Wilcoxon rank test,Kruskal-Wallis test and chi-square tests were used to evaluate the differences in risks and risk factors between the patients with psoriasis and controls.Results As far as the Framingham 10-year risk score was concerned,the difference was statistically significant between the patients with psoriasis and the control population (6％ vs.4％,Z =4.342,P =0.000),as well as between patients with different clinical types of psoriasis (6％ for psoriasis vulgaris vs.4％ for pustular psoriasis vs.7％ for psoriasis arthropathica vs.7％ for erythroderma psoriaticum,H =8.484,P ＜ 0.05),but was insignificant between patients with familial and sporadic psoriasis (6％ vs.6％,Z =0.406,P ＞ 0.05 ).No correlation was observed between the Framingham 10-year risk and psoriasis duration (P ＞ 0.05 ).Conclusions Increased risks of coronary heart disease exist in patients with psoriasis compared with those with acute urticaria,drug eruption or hepers zoster,and in patients with psoriasis arthropathica or erythroderma psoriaticum compared with those with pustular psoriasis.The Framingham 10-year risk of coronary heart disease is uncorrelated with disease duration and unaffected by family history of psoriasis.

Purpose　The aim is to study the inhibiting effect s of nineteen amino acids to E.coli,St.aureus and B.Subtilis.Metho ds　The qualitative inhibition of nineteen amino acids on E.coli ,St.aureus and B.Subtilis was determined using filter papers treated by saturated amino acids placed on the bacteria medium dish.The quantitative inhibi ting effects of the amino acids which were proved to have inhibition to these th ree bacteria were determined in the bacteria LB medium with different concentrations of amin o acids.Results　It was indicated that cysteine was used to in hibit St.aureus.The best inhibiting time was 6 hours,and the optimum of concentrat ion was 0.625% in which the inhibiting rate was 92.62%.Conclusion　 Cysteine could inhibit St. aureus.
