1.Effect of collaborative nursing on the quality of life and care ability of stroke patients

Objective To explore the effect of collaborative nursing on the quality of life and nursing of stroke patients. Methods Toally 84 care-givers for 84 stroke patients hospitalized during February 2014 to March 2016 were chosen. In the control group(hospitalized from Feburary 2013 to Feburary 2014), traditional nursing was carried out, while in the research group(hospitalized from March 2014 to March 2016), the collaborative care intervention was done. The comparisons were done between the two groups in terms of quality of life and care ability by the GHQ-28 quality of life scale assessment, family caregiver task inventory (FCTI) before the intervention and 4 weeks after the intervention. Results Before the intervention, the two groups had no significant differences in life quality and care ability (all P > 0.05). After the intervention, the scores on symptoms, anxiety, depression, insomnia and social dysfunction were all significantly higher than those of the control group (all P<0.05). The scores on the care role, strain, assistance, personal emotion control, family assessment and community resources, and adjustment of life to meet the care needs were all significantly lower than those of the control group (all P<0.05). Conclusions The collaborative care is effective in improving the quality of life of stroke patients. It can improve the care ability of the caregiver.
2.Reform and practice of comprehensive experimental teaching in clinical hematology laboratory course

To train the talents of clinical laboratory for the demand of clinical jobs,it is necessary to make the experimental teaching reform of clinical laboratory hematology.Comprehensive experimental teaching,which centers on case,has been explored in many ways like prefessional teachers' training,cases' choose,teaching method and experimental examination.And the practice has proved that the reform of comprehensive experimental teaching can give full play to the students' initiative and creativity,foster the students' ability to analyze and diagnose hematopathy.
3.Data mining-based analysis on rules of acupoints selection in RCT literature of acupuncture treatment of Bell palsy

Objective:To analyze the rules and characteristics of acupoints selection in acupuncture RCT literature on therapy of Bell palsy.Methods:Acupuncture Prescription Database was established through collecting RCT literature of acupuncture treatment of Bell palsy.The characteristics and rules of acupoints selection were investigated by means of data mining method.Results:After data mining-based analysis on acupuncture therapy of Bell palsy,we found that points on Yangming Meridians were frequently used,especially ST 4.Selection of local acupoints and distal points were important.Conclusion:According the means of data mining,the results revealed that acupoints selection along related meridian was the important principle for Bell palsy,and the special acupoints were the main part of prescriptions.
4.MRI characteristics of various pathological subtypes of focal cortical dysplasia

Objective To summarize MRI findings of focal cortical dysplasia (FCD), analyze MRI characteristics of various pathological subtypes of focal cortical dysplasia. Methods Forty-four patients with FCD were collected. Their MRI findings were analyzed retrospectively. According to pathologic findings, these patients were divided into FCD type Ⅰ group and FCD type Ⅱ group. The following MR signs were observed in the two types of FCD: ( 1 ) Focal thickening of the cortex. ( 2 ) Blurring of the gray matter-white matter junction. ( 3 ) Tapering of white matter signal intensity alteration toward the ventricle on FLAIR and on T2WI. (4)Focal brain hypoplasia. (5)Increased signal intensity of gray matter on FLAIR. (6)Increased signal intensity of gray matter on T2 WI. ( 7 ) Increased signal intensity of subcortical white matter on FLAIR.(8) Increased signal intensity of subeortical white matter on T2WI. (9) Decreased signal intensity of subcortical white matter on T1 WI. The χ2 tests and corrected χ2 tests were used for comparison between the two groups. Results In the 44 cases, there were 30 cases with FCD type Ⅰ and 14 cases with FCD type Ⅱ. FCD was identified by MRI in 32 cases. Blurring of the gray-white matter junction is the most common sign of FCD (23 cases). There were 21 cases identified by MRI in FCD type Ⅰ group. Focal brain hypoplasia is a typical sign of FCD type Ⅰ , which was found in 11 cases in FCD type Ⅰ group but none in FCD type Ⅱ group. There was statistically significant difference between the two groups (continuity corrected χ2 =5. 0286,P =0. 0249) . In FCD type Ⅱ group, 11 cases were identified by MRI. Increased cortical thickness was found in 10 eases in FCD type Ⅱ group and 11 cases in FCD type Ⅰ group ( χ2 =4. 6234 ,P =0. 0315). Increased signal intensity of subcortical white matter on FLAIR was found in 9 cases in FCD type Ⅱ group and 7 cases in FCD type Ⅰ group (χ2 =6.9180,P =0.0085). Tapering of white matter signal intensity alteration toward the ventricle was found in 4 cases in FCD type Ⅱ group and none in FCD type Ⅰ group ( continuity corrected χ2 = 6. 2883, P = 0. 0122). The above-mentioned three MRI findings showed statistically significant difference between the two groups and were features of FCD type Ⅱ.All of the other MRI findings showed no statistically significant difference between the two groups. Conclusions Different pathological subtypes of FCD have different MRI characteristics. It is helpful to make preoperative diagnosis and planning.
5.Magnetic source imaging in the preoperative localization diagnosis and prediction of curative effect of focal cortical dysplasia

Objective To assess the diagnostic value of magnetic source imaging(MSI) in the preoperative localization of focal cortical dysplasia(FCD). Methods Eighty-two patients with intractable epilepsy resulting from FCD undergone preoperative assessment including MSI, video electroencephalography(VEEG) and electrocorticography(ECoG)from February 2006 to June 2012.The consistency between pre- and intra-surgical assessment was evaluated. For patients who had consistent results,postoperative curative effect was also recorded. The accuracy and predictive values of noninvasive preoperative tests were compared by χ2 test. Results For MSI and ECoG, their results were consistent in 57 cases, partially consistent in 14 cases, and inconsistent in 11 cases. In the postoperative follow-up of 57 patients with consistent results, there were 46 cases with Engel class Ⅰ, 3 cases with Engel class Ⅱ, 4 cases with Engel class Ⅲ, and 4 cases with Engel class IV. For VEEG and ECoG, the results were consistent in 43 cases, partially consistent in 33 cases, inconsistent in 6 cases.The postoperative follow-up showed that 27 cases were graded as Engel classⅠ, 5 cases as Engel classⅡ,7 cases as Engel classⅢ, and 4 cases as Engel class IV. There were statistically significant differences(χ2=5.023,P=0.025)of the concordance rate with ECoG between MSI(69.51%,57/82)and VEEG(52.44%,43/82). In the postoperative follow-up of patients with consistent results, the curative effect was better in group MSI than in group VEEG (χ2=3.989,P=0.046). Conclusions Preoperativelocalization by MSI shows advantage over VEEG, which meanswhen it achieves an agreement with ECoG, patients with FCD may get better prognosis from the surgical process.

7.Distance Training for Medical Staff of Children's Rehabilitation

Objective To discuss the distance training for medical staff of children's rehabilitation. Methods 15 lessions were carried out using computer software from April, 2013 to November, 2014 in our centre. 7 cooperative hospitals participated the trainings. The implemen-tation effect was surveyed after training. Results 2693 person-times attended the trainings, including 2109 person-times with living distance training and 584 person-times with video distance training. In the following sampling survey, 92.8%staff thought that the distance training was helpful to their work. 98.6%staff thought that the living distance training was better than the video distance training and 88.6%staff thought their rehabilitation level improved after the distance training. Conclusion The distance training of children's rehabilitation has the advantages of economy, convenience and celerity. It has a wide developmental prospect.
8.Dosimetric influence of dwell weight standard deviation and applicator displacement in patients with cervical cancer

Objective To investigate the dosimetric influence of dwell weight standard deviation (DWSD) and applicator displacement in cervical cancer patients treated with three-dimensional brachytherapy.Methods A total of 20 cervical cancer patients who had completed radical treatment were selected in this study.The Fletcher applicator (Nucletron#189.730) was used for these patients.A new plan,based on the former CT images and structures,was designed for each patient.In former and new plans,dwell weight was recorded,and DWSD was calculated.Two groups,low-DWSD (LDWSD,0.141-0.299) and high-DWSD (HDWSD,0.211-0.337),were set according to the DWSD size for the two plans.Dosimetric effects from ± 1 mm displacement of tandem applicator or ovoid applicator were simulated with Oncentra (R) Brachy V4.3 treatment planning system.D100,D90,and V150 for clinical target volume (CTV)and D0.1cc,D1cc,and D2cc for the bladder,rectum,and sigmoid were evaluated.Dosimetric comparisons were made between the LDWSD group and HDWSD group to study the dosimetric effects of DWSD and applicator displacement in cervical cancer patients.Results The dosimetric effects from applicator displacement increased with increasing DWSD.If there was a 1 mm displacement of tandem applicator or ovoid applicator,D100,D90,and V150 of CTV were 3.0%,23.8%,and 4.8% higher or 0.5%,1.2%,and 5.2% higher in the HDWSD group than in the LDWSD group;D0.1cc,D1cc,and D2cc of the bladder and rectum were significantly higher in the HDWSD group than in the LDWSD group,particularly for the sigmoid (up 44.0%,22.8%,and 16.8%) and (up 10.3%,14.4%,and 12.4%).Conclusions DWSD should be considered in plan evaluation for cervical cancer patients treated with three-dimensional brachytherapy.The dosimetric influence from applicator displacement can be decreased by reducing DWSD properly.
9.Effects of sodium arsenite on metallothionein isoforms gene expression in cultured human lymphocytes

Objective To explore the effects of sodium arsenie on Metallothionein(MT) isoforms genes expression,and to study the relevance between the MT expression and cell survival percentage. Methods Healthy persons blood was extracted aseptically and the lymphocytes were separated. The lymphocytes were treated by 0 (control) ,2,5,10,15,30,60 μmol/L sodium arsenites, respectively. The cell survival percentage was determined by methyl thiazolyl tetrazolium(MTT) reduction assay at 24,48,72 h intervals, while the expression of MT-1 and MT-2 were examined by RT-PCR in 72 h. Results The cell survival percentage in 2,5μmoL/L groups were (115.50± 11.80)% and (130.49±8.28)%,which were all higher than those in the control group [(100.00±0.00)%,all P < 0.05]at any intervals. In 10,15,30,60 μmol/L groups,the cell survival percentage[(78.12±9.33)%,(71.62± 10.82)%,(52.06±3.05)%,(40.98±5.41)%]increased along with the decrease of concentration,which showed a significant difference between these groups and the control group and 2,5μmol/L groups(all P < 0.05). There was no significant difference on MT-1 expression after exposed to different concentration sodium arsenites (0,2,5,10,15,30,60 μmol/L) for72 h(0.925±0.123,1.082±0.504,1.103±0.170,0.927±0.056,0.730±0.307,0.604± 0.173,0.540±0.075,all P > 0.05). The expression of MT-2 in 2,5μmol/L groups(1.503±0.212,1.557±0.377) was up regulated compared with that in control group(0.702±0.112) and MT-2 also expressed more than that in other groups(all P < 0.05). However,the expression of MT-2 declined when the concentraion was 10,15,30,60 μmol/L(0.814±0.139,0.679±0.201,0.607±0.229,0.533±0.102). There was no significant difference among the groups (all P > 0.05). The expression level oE MT-1,MT-2 was positively correlated with the cell survival pereentage(r = 0.955,0.909,all P < 0.05). Conclusions The sodium arsenite at concentration of 10 μmoL/L might inhibit the expression of MT of lymphocytes and low concentration sodium arsenite (2,5 μmol/L) might stimulate the lymphocytes to regulate the expression of MT-2 to higher levels,which can increase the cell survival percentage and exert the function of protecting cells.
10.The value of 64-slice spiral CT angiography based on pre-contrasted raw data in diagnosing pulmonary arteriovenous fistula

Objective To evaluate the value of 64-slice spiral CT angiography based on pre- contrasted raw data in diagnosing pulmonary arteriovenous fistula.Methods 64-slice spiral CT plain scan and enhanced scan was performed in 16 patients with pulmonary arteriovenous fistula,pulmonary angiography based on pre-contrast and post-contrast raw data was performed respectively,including maximum intensity projection(MIP),shaded-surface display(SSD),and volume rendering(VR).According to the results of angiocardiography and surgical findings,comparson of the three methods was made in the display of PAVF in pre-contrast and post-contrast phase images.Results 8 of the 16 PAVF cases were single lesion,8 cases were multi-lesions.30 PAVF lesions were found in all the patients.MIP,SSD and VR based on pre-contrast raw data displayed PAVF lesions in 20,14,and 22,respectively.The combination of the 3 methods based on pre-contrast raw data could show 26 PAVF lesions.MIP,SSD,and VR based on post-contrast raw data displayed PAVF lesions in 24,18,and 30,respectively.The combination of the 3 methods based on post- contrast raw data could show 30 PAVF lesions.Conclusion 64-slice spiral CT angiography based on pre- contrasted raw data can clearly show the position,number,and shape of PAVF lesions,which is of great value in diagnosing PAVF.
