1.The random and comparative study on therapeutic effect and safety between Levetiracetam and Phenobarbitaladministration for neonatal seizures

Objective To randomly compare the therapeutic effect and safety between Levetiracetam (LEV) and Phenobarbital (PB) in the treatment of neonatal seizures.Methods A total of 61 infants with acute convulsion were randomly divided into 2 groups:LEV group (n =30) and PB group (n =31) during January 2013 to December 2014 in Urumqi Children's Hospital.All neonates received routine management including etiology treatment and adverse drug reaction monitoring.In the LEV group,subjects received oral formulation of LEV with initial loading dose 30 mg/kg,followed by 15 mg/kg twice a day.If the seizures were not controlled completely,PB treatment was added until seizures were completed controlled.If seizures were controlled quickly,the dose of PB was gradually reduced and LEV was used as monotherapy.The subjects in PB group received intramuscular or intravenous injection of PB with 10 mg/kg as the first dose,then 5 mg/(kg · d) oral PB was administered,if seizures were not controlled,LEV treatment was added,then dose of PB was gradually reduced until seizures were controlled completely,and then patients were switched to LEV monotherapy gradually.The drug adverse reactions were observed.Results (1) After LEV or PB monotherapy,66.7% (20/30 cases) and 54.8% (17/31 cases) of the subjects obtained sustainable seizure free respectively.Although,there was a higher control ratio in LEV group,but no significant difference was observed between the 2 groups (P >0.05).(2) LEV group (16/30 cases,53.33%) had higher rapid seizure control ratio with seizure controlled within 24 h after first dosage administration than that of PB group (8/31 cases,25.80%),and there was significant difference (x2 =4.841,P =0.028).Further more,if adding the cases who had to change to use another comparative one (LEV or PB) due to their seizures failure control with the first one treated,LEV group (21/44 cases,47.72%) still had higher rapid seizure control ratio in total patients than that of PB group(10/41 cases,24.39%),and there was significant difference (x2 =4.988,P =0.026).(3) Eight cases who changed to LEV after PB as the first treatment drug failed obtained sustainable seizure free.(4) One case in PB group with transient urinary retention was observed but the symptom disappeared 36 h after PB withdrawal,and no significant drug adverse reaction was observed in LEV group.Conclusion LEV is more rapid and safe for seizure control of neonates than PB.

3.Effects of hypertonic saline on CD14/CD16 expression by monocytes and the levels of anti-inflammatory cytokines in patients sustaining traumatic hemorrhagic shock

Objective To investigate the expression of CD14/CD16 by monocytes and the anti-inflammatory effects of hypertonic saline plus dextran (HSD) in adult blunt trauma patients in hemonhagic shock. Method A total of 30 adult patients were eligible for inclusion in the study if they sustained blunt trauma from March to October 2007 and had at least one recorded episode of hypotension (systolic blood pressure ≤ 90 mm Hg) with clear evidence of blood loss (external or internal including the thorax, abdomen or retroperitoneum). Patients were excluded if they refused to participate, were admitted ≥ 6 hours after injury, were pregnant, or had chronic disease. The enrolled patients were randomly divided in a double-blinded manner into an HSD group which was administered 7.5% Nad plus 6% dextran - 70, and a control group which was administered 0.9% NaCl. A single 250 ml dose of either HSD or NaO was immediately administered to the patients in each of the two groups while they were in the emergency room. The primary outcomes were to measure the changes in CD4/CD16 expression by monocytes and the levels of anti-inflammatory cytokines, including tumor necrosis factor-alpha (TNF-α), interleukin (IL)-lra and IL-10. Patient demographics, fluid requirements, organ dysfunction, infection and death were recorded. Results A total of 28 patients were enrolled with no significant differences in their clinical measurements. Hyperosmolarity was modest and transient. HSD altered the shock-induced monocyte redistribution pattern by reducing the drop in the "classic" CD14 ++ subset and remarkably affecting the expansion of the "pro-inflammatory" CD14+CD16+ subsets. In parallel, HSD significamly reduced pro-inflammatory TNF-α production while increasing anti-inflammatory IL-lra and IL-10 production. Conclusions This human trial demonstrates that HSD has anti-inflammatory and immunologic properties for trauma patients in hemorrhagic shock. HSD exerts profound immunomodulatory effects, promoting more balanced pro-/anti-inflammatory responses and reducing post-traumatic complications. Therefore, it could be useful in attenuating post-trauma multiorgan dysfunction (MOD).
4.Expression of PRR11 and its effect on proliferation of human osteosarcoma cells

Objective To explore the expression of PRR11(Proline-rich protein 11) in human osteosarcoma and investigate the effect of PRR11 on the proliferation of human osteosarcoma cells.Methods Immunohistochemical staining was applied to detect the PRR11 expression in 75 cases of osteosarcoma and corresponding normal tissues.Western blotting was used to examine PRR11 protein expression levels in osteosarcoma cell lines.We used siRNA to knock down the expression of PRR11 and tested the effects of PRR11 down-regulation on the proliferation in SaOS2 cells.Results PRR11 was overexpressed in osteosarcoma specimens compared to their paired normal tissues,the over expression rate of PRR11 in osteosarcoma and corresponding paracancerous tissues were 76%(57/75) and 9.33%(7/75) with statistical difference(P<0.05).The high expression of PRR11 was correlated with tumor pathological grade and lymphatic metastasis(P<0.05).PRR11 was expressed in 4 osteosarcoma cell lines which were SaOS2,143B,U2OS and MG63 respectively,the expression was highest in SaOS2 cells.Silencing PRR11 inhibited cell growth as compared with control cells(P<0.05).Conclusion PRR11 is overexpression in human osteosarcoma and promotes its progression by enhancing proliferation.The increased expression of PRR11 in osteosarcoma is a new target for treatment and early diagnosis of human osteosarcoma patients.
5.Screening of college students contacts among tuberculosis index patients in Haidian district, Beijing

Objective To screen college students contacts of tuberculosis index cases among colleges located in Haidian District,Beijing,so as to provide evidence for the modification of screening strategies for college students.Method The purified protein derivative test (PPD) and chest X-ray examination were carried out in colleges,located in Haidian District from March 2015 to March 2016.The basic demographic characteristics,tuberculosis infection and detection of patients,etc were analyzed.Results A total of 88 cases of tuberculosis index cases were diagnosed,and 3 604 college students contacts were detected 16 pulmonary tuberculosis cases were detected among close contacts,and all patients were negative,the detection rate was 443.95/100 000.Among the 3 604 close contacts,the positive rate of PPD was 8.32%.With the increase of the diameter of the PPD reaction,the detection rate of tuberculosis patients increased significantly (xtrend =8.06,P <0.001).During the period of outbreak,the PPD positive rate and the detection rate of patients were significantly higher than that of non-outbreak period,and the difference was statistically significant(all P <0.05).Conclusions It is necessary to strengthen the screening of tuberculosis among college students,and to find out the infected persons and patients as early as possible,so as to prevent the outbreak of tuberculosis.
6.The influence of lipid metabolic markers on hip osteoarthritis

Objective To investigate the influence of lipid metabolic markers on hip osteoarthritis (OA) and the correlation with the pain of hip.Methods In this retrospectively cross sectional analysis,393 subjects fufilling the inclusive criteria were continuously included from June 2013 to September 2014.The presentation of hip X-ray of subjects were classified into OA group and non-OA group in terms of Kellgren-Lawrence grading scale,the subjects with Kellgren-Lawrence (K-L)≥2 was classified into OA group.The average of age in total was (62±9) years.There were 183 males and 210 females,the number of patients with OA was 182.There were 210 patients with hypertension and 118 patients with diabetes while the number of smoking patients was 74.The demographic information,risk factors,blood pressure,blood glucose,lipid metabolic markers were analyzed.Differences in proportions were compared using the Chi-square test.Difterences in continuous variables were tested for statistical significance using t test analysis.VAS score was used to evaluate the severity of hip pain.Independent risk factors were confirmed by using multiple logistic regression analysis.The correlation between the risk factors and severity of hip pain was analyzed by Spearman correlation analysis.Results Statistically significance existed in age (t=-4.849),sex (x2=8.946),BMI (t=-4.794),hypertension (x2=4.751),smoking (x2=7.062),metabolic syndrome (x2=34.406),apolipoprotein A1 (t=2.352),apolipoprotein B (t=-6.870),high density lipoprotein cholesterol (t=2.519),triglyceride (t=-4.652) and ApoB/ApoA1 ratio (t=-4.901) between OA group and non OA group.Age [OR =1.060(1.033,1.086)],metabolic syndrome [OR=3.682(2.284,5.938)] and ApoB/ApoA1 ratio [OR=5.743(2.393,13.785)] were the independent risk factors.The ApoB/ApoA1 ratio had positive correlation with the severity of hip pain(r=0.379).Conclusion ApoB/ApoA1 ratio is the independent risk factor of hip osteoarthritis,and has positive correlation with the severity of hip pain.The ApoB/ApoA1 ratio should be monitored in hip osteoarthritis patients in clinical work,to get the relief of pain and improve hip osteoarthritis.

8.Observation of curative effect of avulsion fracture of anterolateral ligament of knee joint

Objective To explore the clinical effect of periarticular knee fractures combined with anterolateral ligament (ALL) avulsion fracture.Methods From June 2014 to November 2015,13 patients with ALL avulsion fracture in periarticular knee fractures were fixed by the star plate,cannulated or screws suture anchor,including 9 male and 3 female with the mean age of 39.5 years (range from 28 to 51 years).6 patients were diagnosed distal femoral fracture,4 tibial plateau fracture,and 3 tibial intercondylar eminence avulsion fracture which all combined with ALL avulsion fracture.Postoperatively,all patients were treated with the adjustable knee brace for 3 months.Results All patients were followed up for 4-20 months,the mean follow-up time was 13.5 months.The average surgical time was 130 min (range from 90 to 210 min).In all 13 patients,ALL was found and identified.Bone union was obtained in all patients,and the bone union time was 11.4 weeks (10-12 weeks).7 cases were fixed with star plate,5 cases were fixed with hollow screw,and 1 case was fixed with anchor.All follow-up patients underwent anteroposterior and lateral X-rays.Pain,swelling and functional recovery of the knee joint were observed.Patients were assessed postoperatively with International Knee Documentation Committee (IKDC) score,Lysholm score,and Tegner activity scale to evaluate the clinical effect.At the last follow-up,all patients had full range of motion of the knee joint which from 0° to approximate 120°.The Lysholm,objective IKDC were 86.5 and 84.6 respectively.Among the 13 patients evaluated,9 patients were graded A,and 4 patients were graded B by subjective IKDC.The Tegner activity scale at the last follow-up was 6.7.Incision of poor healing in 1 case,and got healed after 32 d cleaning dressing.Conclusion This study confirmed the presence of the ALL.The fixation of ALL avulsion fracture in periarticular knee fractures can be an effective procedure at a minimum follow-up of 13.5 months.
9.Application of peer education based on WeChat support in patients after laparoscopic radical prostatectomy

Objective:To investigate the influence of peer education based on WeChat support on the social support, mental flexibility and rehabilitation self-efficacy of patients after laparoscopic radical prostatectomy.Methods:A total of 82 patients after laparoscopic radical prostatectomy who were admitted from January 2018 to December 2019 were selected as the research objects. According to the operation time, they were divided into the treatment group (January 2019-December 2019) with 43 cases and the control group (January 2018-December 2018) with 39 cases. The control group was given regular health education, and the treatment group jointly applied peer education based on WeChat support. Followed up for 2 months, the two groups of patients were evaluated the degree of social support, psychological flexibility, and rehabilitation self-efficacy by Social Support Rating Scale, Connor-Davidson Resilience Scale, General Self-Efficacy Scale and compared.Results:There was no significant difference in the degree of social support, psychological flexibility, and rehabilitation self-efficacy before intervention between the two groups( P>0.05). The objective support, support utilization, and social support scores in the treatment group after intervention were (9.12±1.12), (10.45 ± 0.75), (32.49 ± 4.56) points, and the control group were (7.45 ± 1.36), (8.74 ± 1.43), (29.84 ± 4.45) points, and the differences were statistically significant ( t values were 6.091, 6.681, 2.658, P<0.01). The scores of toughness, self-improvement, optimism, and mental resilience in the treatment group after intervention were (28.21 ± 4.25), (20.32 ± 3.54), (9.36 ± 1.12), and (57.89 ± 7.21) points, and the control group were (24.36 ± 4.34), (17.14 ± 3.21), (7.84 ± 1.23), (49.34 ± 6.55) points, and the differences were statistically significant( t values were 4.056-5.857, P<0.01). The scores of physical exercise self-efficacy, coping self-efficacy, and rehabilitation self-efficacy in the treatment group after intervention were (43.43 ± 5.38), (54.45 ± 6.32), (97.88 ± 7.45) points, and the control group were (37.45 ± 5.42), (48.65 ± 6.45), (86.10 ± 9.12) points, and the differences were statistically significant ( t values were 5.009, 4.110, 6.430, P<0.01). Conclusions:Peer education based on WeChat support helps to enhance the degree of social support for patients after laparoscopic prostate cancer surgery, improve the level of mental flexibility, and promote the development of rehabilitation self-efficacy.
10.Comparison of the different formulas in intraocular lens power calculations on high myopia after LASlK with cataract patients

AlM: To compare the different formulas in intraocular lens(lOL) power calculations on high myopia after LASlK with cataract patients who received the phacoemulsification and intraocular lens implantation ( Phaco+lOL) .
METHODS:This was a retrospective study. Data from 102 eyes of 61 cataract patients with high myopia, who received Phaco+lOL after LASlK, were collected. Patients were divided into three groups by formulas in lOL power calculations before surgery: Holladay group ( 11 cases, 21 eyes), Haigis-L group (30 cases, 47 eyes) and SRK-T group (20 cases, 34 eyes). The ratios of equivalent lOL power after LASlK were predicted and the accuracy of their predictions were compared 3mo after surgery. lt was an index of reliability of the formulas outcomes.
RESULTS:The ratios of equivalent lOL power after LASlK using the Holladay, Haigis-L and SRK-T formulas were 0. 86 ± 0. 41D, 0. 43 ± 0. 30D and 1. 27 ± 0. 58D, respectively . There was no significant difference between Holladay group and SRK-T group ( t=-0. 271, P=0. 625>0. 05 ). However, significant difference was found between Haigis-L group and Holladay group ( t=2. 249, P=0.047<0. 05), Haigis-L group and SRK-T group (t=6. 012, P=0. 031<0. 05).
CONCLUSlON:The Haigis-L formula is more accurate than the SRK-T and Holladay formulas in predicting lOL power for cataract patients with high myopia after LASlK.
