Objective To explore the effect of home-based rehabilitation on cerebral palsy. Methods 60 children with cerebral palsy were assigned to hospital-based rehabilitation group and hospital-home-based rehabilitation group and treated for 1 year.Every child was evaluated before and after the treatment with Bayley Scales of Infant Development(BSID).The Mental Development Index(MDI) and Physical Development Index(PDI) were compared between these two groups. Results After one year, the MDI and PDI improved in both groups(P<0.05),while those of the hospital-home-based rehabilitation group was better than of the hospital-based rehabilitation group(P<0.05).Conclusion Home-based rehabilitation plays an important role in the rehabilitation of cerebral palsy children.

Objective To investigate the fatigue experience of the community-dwelling frail elderly to establish the conceptual model. Methods Grounded theory was adopted. With MAXQDA v10.4.15.1for 3 levels coding, the conceptual model of fatigue experience of community-dwelling frail elderly people was structured through constant comparison and in-depth analysis. Results Multi-response,selfmanagement,safe and belonging, hitting rock bottom, accepting and transferring formed the conceptual model. Conclusions Based on the conceptual model, nurses should pay more attention to concern about the fatigue experience of community-dwelling frail elderly people, and probe different assessment methods to enrich comprehensive geriatric assessment (CGA),then improve the community health service level and the quality of elderly people's life.

Objective To explore the opportunities and challenges which COC (continuity of care) has been facing, some strategies were proposed and improved continuously during the process of deepening medical reform in public hospital, and at the same time to supply reference for government to make relative policy. Methods By SWOT-analysis,the strengths, weaknesses, opportunities as well as threats of COC in medical reform were analyzed comprehensively. Results Depending on national relevant policies, supportive measures, the cooperation from multidisciplinary team was improved.Along with strategies of strengthening advantages, weakening disadvantages, meeting challenges and facing up to threat.COC had be approved and consented initially. Conclusions Public hospitals should demonstrate their various resources and make full use of their advantages, then guide and solid grass-roots in the new healthcare system which integrate medical, rehabilitation and elderly nursing out of the hospital. It must benefit the hospital, specialized subject, family and society.

A comprehensive analysis was carried out about the influencing factors of medication safety in the elderly with multiple chronic diseases, who lived in the community and stayed at their own homes.Applicable and simple preventive countermeasures should be established to ensure the medication safety outside medical agency. We should promote extending service of hospitals, cooperate with community and nursing facility in order to increase life quality of the elderly, and supply reference for professional continuous service model including rapid aging, empty nest with advanced age, disability and dementia and elderly people with chronic diseases.

Objective To explore the significance of NK cell activity,interleukin-2 receptors (sCD25) and glycosylated ferritin in the early diagnostic of acquired hemophagocytic lymphohistiocytosis (HLH).Methods 57 patients suspected of HLH from June 2005 to May 2008 and 25 healthy subjects were enrolled in the study. The patients suspected of HLH were divided into three groups i.e.(1) a group with diagnosis confirmed at first visit;(2) a group with diagnosis confirmed at subsequent visit and (3) a group with diagnosis unconfirmed according to HLH-2004 diagnostic criteria.Healthy subjects were enrolled as control.NK cell activity was determined with a released LDH assay. The percentage of glycosylated ferritin was determined with phytohemagglutinin adsorption assay,sCD25 was examined with ELISA double antibody sandwich assay. We compared the coincidence of each diagnostic index before and after diagnosis. Results The median percentage of NK cell activity was significantly lower in the first group ( 18.3±5.6) % and the second ( 16.7±6.7)% than that in the third group (33.4±6.8)% or in the controls (36.6±5.0)%.The median percentage of glycosylated ferritin was also significantly lower in the first group ( 15,4 ± 2.0)% and the second group (16.9 ± 3.4)% than that in the third group (40.4 ± 3.0)% or in the controls (45.2±2.2)%.Meanwhile,the median level of sCD25 was significantly higher in the first group (12 916±4328) ng/L and the second group (12 117 ± 5465) ng/L than that in the third group (4728±1482) ng/L or in the controls (3841 ± 993) ng/L.Furthermore,NK cell activity,sCD25 and glycosylated ferritin were abnormal in all the patients in the early stage of HLH.Conclusion NK cell activity,sCD25 and glycosylated ferritin may be helpful markers for the early diagnosis of HLH.

Objective:To prepare the production of TIGIT-Fc fusion protein using H22 cells stably integrated the gene by lentivirus vector , and to explore the immunoregulatory effect on macrophages by TIGIT-Fc.Methods: TIGIT-Fc fusion gene were constructed by molecular cloning. The fusion gene was then subcloned to plasmids contained the secretion signaling peptide .The secrected TIGIT-Fc fusion gene was inserted into the lentivirus backbone vector. The purified lentivirus vector was the used to infect the murine H22 cell line.TIGIT-Fc protein was purified by protein A column from the ascites of H 22-injected C57BL/6 mice.Macrophages stimulated by lipopolysaccharide ( LPS ) was challenged to TIGIT-Fc treatment or control.Cytokine levels was then detected by ELISA.Results: TIGIT-Fc protein was purified from the ascites of H 22-injected mice.PVR was upregulated in LPS-treated macrophages.IL-10 level was upregulated in TIGIT-Fc treated macrophages. Conclusion: TIGIT-Fc promotes the mature macrophages to secrete anti-inflammatory cytokine IL-10.

Objective To investigate the distribution characteristic and drug resistance of respiratory tract pathogens in ICU eld‐erly patients to provide the basis for clinical medication and control of nosocomial infection .Methods The isolation situation and drug resistance of pathogens in ICU elderly patients with respiratory tract infection from January 2012 to December 2014 were ret‐rospectively analyzed .Results Among 501 cases of respiratory tract infection ,350 cases were Gram‐negative bacilli infection ,which were mainly P .aeruginosa and A .baumannii;50 cases were Gram‐positive coccus infection ,which was mainly S .aureus ;101 cases were complicating fungal infection ,which was mainly C .albicans .The resistance of P .aeruginosa to imipenem showed upward trend (P<0 .05) ,A .baumannii had higher resistance to commonly used antimicrobial drugs ,but the drug resistance trend had no obvious change(P>0 .05) .Imipenem‐resistant A .bauman(IRAB) ,ESBLs‐producing E .coli and methicillin‐resistant S .aureus (MRSA) in the elderly patients with respiratory tract infection all exceeded 50% of each constitution ratio .Conclusion Multi‐drug resistant bacteria are usually isolated from ICU elderly patients ,their drug resistance rates maintain a higher level .Therefore clinicians should rationally select antibacterial drugs by combining with the laboratory reports ,increase the prevention and management of multi‐drug resistant bacteria and reduce the nosocomial infection occurrence .

Radiation?induced heart disease ( RIHD) is a common type of radiation?induced damages in chest radiotherapy. There are no obvious short?term symptoms in patients with RIHD. However, RIHD causes irreversible permanent damages to the heart over time, which undermines the quality of life. Patients with severe RIHD even have a risk of death from myocardial infarction caused by coronary atherosclerosis. This paper summarizes the research advances in epidemiology, diagnosis, mechanisms of radiation?induced injury in various parts of the heart, radiotherapy techniques, and treatment. Reduction in radiation range and dose, early diagnosis, and early treatment are recommended for patients to reduce heart injury and improve the quality of life.
