1.Research of the Value of MR Imaging in Diagnosing the Bucket Handle Tear of the Knee Meniscus

Objective To evaluate the value of MRI in diagnosing the bucket handle tear (BHT) of the knee meniscus.Methods Eighty four knees which underwent both MRI and arthroscopy (AS) examinations were analyzed. Results of AS were used as golden standard to evaluate the results of MRI. The evaluating criteria included primary reports, central displaced fragments of meniscus (CDFM), double posterior cruciate ligament sign (DPCLS) and the absent bow tie sign (ABTS). Results For MRI diagnosing BHT, the sensitivity and specificity of primary reports were 43% and 87%, respectively; the sensitivity and specificity of CDFM were 72% and 87%, respectively; the sensitivity and specificity of the DPCLF were 31% and 97%, respectively; and the sensitivity and specificity of the ABTS were 71% and 69%, respectively.Conclusions When MRI was applied to diagnose BHT, the CDFM had higher sensitivity and specificity, while the ABTS had higher sensitivity and lower specificity. The DPCLF had the highest specificity and the lowest sensitivity.
2. Expression of osteopontin in breast intraductal carcinoma and infiltrating ductal carcinoma and its clinical significance

Objective: To investigate the expression of osteopontin (OPN) in normal breast epithelium, intraductal carcinoma and infiltrating ductal carcinoma and its clinical significance. Methods: The expression of OPN was examined by S-P immunohistochemistry in all specimens, and the results were statistically analyzed. Results: The positive rates of OPN in normal breast epithelium, intraductal carcinoma and infiltrating ductal carcinoma specimens were 0%(0/20), 20.0%(5/25) and 43.1%(25/58), respectively, with significant differences found between the latter two groups (P<0.05). OPN expression in breast infiltrating ductal carcinoma was not correlated with the family history, the sizes of primary tumor or patient ages, and it was correlated with histological types, clinical TNM stages and axillary lymphatic metastasis(P<0.05). Conclusion: OPN may participate in the development, progression, metastasis of breast cancer, and it may be used for predicting the prognosis of breast cancer.

5.Simultaneous determination of six constituents in Qingre Zhili Pills by HPLC

AIM To develop an HPLC method for the simultaneous content determination of berberine hydrochloride,jatrorrhizine hydrochloride,palmatine hydrochloride,kaempferol,luteolin and apigenin in Qingre Zhili Pills (Berberidis Radix and Portulacae Herba).METHODS The analysis of methanol extract of this drug was performed on a 35 ℃ thermostatic Agilent TC-C18 column (4.6 mm × 250 mm,5 μm),with the mobile phase comprising of acetonitrile-0.02% potassium dihydrogen phosphate flowing at 0.9 mL/min in a gradient elution manner,and the detection wavelengths were set at 265 nm and 360 nm.RESULTS Six constituents showed good linear relationships within their own ranges (r≥0.999 3),whose average recoveries were 97.16%-99.90% with the RSDs of 0.62%-1.48%.CONCLUSION This simple method can be used for the rapid quality control of Qingre Zhili Pills.
6.The diagnosis and treatment of solid-pseudopapillary tumors of the pancreas

Objective To evaluate the clinical diagnosis and management of solid-pseudopapillary tumors of the pancreas.Methods A retrospective clinical analysis was made on 21 cases of solid-pseudopapillary tumors of the pancreas admitted from June 1994 to June 2003.Results Abdominal pain was the major complains in 13 cases , 7 cases had palpable abdominal mass.Imaging examination can show the abdominal mass clearly, but diagnosis can not be defined.There were 12 cases whose tumors were located in pancreatic head (4 underwent Whipple procedures and 8 tumor enucleations) and 9 cases with tumors in pancreatic tail (all underwent body and tail resection plus splenectomy).The average diameter of tumor was 9.5 cm (3-20).One case had liver metastasis.Diagnosis was proved by frozen pathology during operation in 14 cases, and other 3 cases initially diagnosed as insulinomas and other malignant tunors, had definite diagnosis by postoperative paraffin section and enzyme labeling.Enzyme labeling examinations showed that there were 14 cases of NSE positive in 15 cases, 8 cases of Vimentin positive in 10 cases and 7 cases of PAS positive in 8 cases.CK, EMA and CHG A were negative.Eighteen cases were followed up (85.7%) with a median period of 24 months(1~60) without tumor recurrence.Conclusion Huge pancreatic masses of pancreas in young females should suggest the diagnosis of solid-pseudopapillary tumors.Aggressive surgical therapy can result in good prognosis in these patients.
7.Effect of hydrocortisone on outcome of patients with cardiac arrest and cardiopulmonary resuscitation

Objective To observe the effects of hydrocortisone on the rate of return of spontaneous circulation(ROSC)and the outcome of patients with cardiac arrest(CA)and cardiopulmonary resuscitation(CPR). Methods A cohort study was conducted,78 non-traumatic patients with CA were divided into hydrocortisone group (31 cases)and control group(47 cases). Conventional treatments were given in the two groups after admission,and additionally intravenous 100 mg hydrocortisone was given to the hydrocortisone group during resuscitation. The ROSC rate and prognosis were compared between the two groups. Multivariate logistic regression analysis was used to predict the impact factor of ROSC. Results The ROSC rate and 24-hour survival rate in the hydrocortisone group were significantly higher than those of the control group(ROSC rate:58.1% vs. 40.4%,24-hour survival rate:48.4%vs. 36.2%,both P<0.05). There were no significant differences between the hydrocortisone and the control groups in duration of CPR〔minute:17.1(6-45)vs. 15.8(7-48)〕,acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score in survivors(37.2±8.2 vs. 36.1±8.2),the survival rate(12.9% vs. 12.8%)and hospital discharge rate(6.4%vs. 6.4%)in 7 days. Hydrocortisone〔odds ratio(OR)=3.12,95%confidence interval(95%CI)-1.18-8.29, P=0.017〕and witness(OR=4.24, 95%CI -1.87-12.43,P=0.008) were independent predictors for an increased ROSC rate after multiple logistic regression analysis. Conclusion Giving hydrocortisone during resuscitation may increase ROSC rate in CA patients.
8.Inhibitory effect of melatonin on murine foregastric carcinoma cells via membrane receptors MT2

Objective To investigate the inhibitory effect of melatonin on the proliferation activity of murine foregastic carcinomac ( MFC) cells via melatonin membrane receptors MT 2 and its relationship with the signaling pathways of mitogen-activated protein kinases ( MAPKs), phosphatidylinositol 3-kinase ( PI3K)-Akt.Methods Using siRNA technology to silence MT2 expression, we examined the ability of melatonin to inhibit the proliferation activity of MFC cells and its influence on the phosphorylation of ERK 1/2 and Akt.Results We found two interesting effects of SiRNA-mediated silencing of MT2 expression.Firstly, it significantly antagonized the inhibitory effect of melatonin on the proliferation activity of MFC cells .Secondly , it partially blocked the inhibitory effect of melatonin on the phosphorylation of ERK 1/2 and Akt.Conclusion Our results suggest that melatonin can inhibit the phosphorylation of ERK 1/2 and Akt via MT2 receptors , thereby inhibiting the proliferation of gastric cancer cells .
