2.Comparison of hollow compression screws and absorbable screws for the treatment of the fifth metatarsal fracture:ankle function and fracture displacement

BACKGROUND:The hol ow compression screws and absorbable screws can be applied for the treatment of the fifth metatarsal fracture. However, little evidence is obtained in China. OBJECTIVE:To compare ankle function and fracture displacement in patients with the fifth metatarsal fracture after interventions of hol ow compression screws and absorbable screws. METHODS:32 patients with the fifth metatarsal fracture were recruited from Zhongnan Hospital of Wuhan University between June 2012 and June 2014. According to the internal fixation method, the involved patients were divided into hol ow compression screws group (n=18) and absorbable screws group (n=14). Clinical outcomes of patients in the two groups were evaluated through observations of the mean healing time, postoperative infection, ankle activity and fracture displacement. RESULTS AND CONCLUSION:Al the 32 patients were fol owed up for 6-18 months and were al healed. Compared with the absorbable screws group, the healing time was shorter (P<0.05), incidence of postoperative screw rupture and fracture displacement was lower (P<0.05), and ankle activity was better in the hol ow compression screws group (P<0.05). It is necessary to enhance the strength, improve screw thread, increase anti-pul ing property, and al ow early ankle activity, thus reducing the risk of clinical application of the absorbable screws.
3.A vest-pocket instrument for the detection and treatment of the onset of epileptic seizures.

A new vest pocket medical instrument is here presented. It can automatically detect and identify the signals of the onset of epileptic seizures, and then as necessasy, will automatically generate an acoustic stimulation that may weaken and shorten the epileptic activity at the initial stage.

5.Influence of peroxisome proliferator activated receptor γ2 endogenous ligands on mRNA expression of bone metabolism related genes in osteoblastic cells

Objective To observe the effect of oxidized low-density lipoproteins (Ox-LDL),15-Deoxy-△ 12,14-prostaglandin J2 ( 15d-PGJ2 ),leukotrienes B4 ( LTB4 ) on mRNA expressions of peroxisome proliferator activated receptor γ2 ( PPARγ2 ),receptor activator of NF-κB ligand (RANKL),alkaline phosphatase ( ALP),and osteoprotegerin(OPG) in osteoblastic cells of rats; and to investigate the influence of these PPARγ2 endogenous ligands on bone metabolism.Methods Rat osteoblastic cells were cultured in vitro for 24 h in medium with different PPARγ2 endogenous ligands at various concentrations ( the final concentrations of Ox-LDL were 0,12.5,25,50μg/ml; the final concentrations of 15 d-PGJ2 were 0,10,20,30 μmol/L; the final concentrations of LTB4 were 0,0.1,1.0,10 μ mol/L).RT-PCR was performed to determine the mRNA expressions of PPARγ2,RANKL,ALP,and OPG in osteoblastic cells.Results RT-PCR analysis showed that Ox-LDL,15d-PGJ2,and LTB4 all down-regulated the mRNA expressions of RANKL,ALP,and OPG,while up-regulated the mRNA expressions of PPARγ2 in osteoblastic cells in a dose-dependent manner.Significant differences were found in interclass comparisons( P<0.05 or P< 0.01 ).Conclusions These findings suggest that Ox-LDL,15d-PGJ2,and LTB4 suppress the expressions of osteogenic genes through activating the transcription activity of PPARγ2,and this may be a plausible mechanism of senile osteoporosis.
6.Application of CT navigation in mandibular angle plastic osteotomy

Objective To summarize the use of the CT-guided mandibular angle plastic osteotomy.Methods The clinical data of mandibular angle plastic osteotomy were analyzed under the CT navigation in recent 3 years in our department.Thin-slice CT scans of the mandibular angle were performed before the operation.The CT data were input to the system of neuronavigation.The surgical procedures were then taken under the CT navigation.Results All of patients who received this new technology obtained good plastic effects and safe operation.The anatomic location accurately achieched with short operation time and less bleeding during the operation and distinct curative effect and fast recovery after operation.The effects of the treatment were fine.No visible complications occurred.Conclusions The CT navigation could fix accurately on position of important blood vessels and nerves which could be injured accidentally during operation and,of course,the safety of the operation is improved.The CT navigation can also determine the position and quantities of osteotomy from three dimensional angles,reduce effectively the surgical complications and the risk of surgery,reduce the psychological burden of patients efficiently and increase their confidence and credibility to the operation and surgeons.
7.Evaluation of the clinical efficacy of muscle regions of meridians needling method for refractory facial paralysis based on infrared thermal imaging technology

Objective: To explore the clinical efficacy of muscle regions of meridians needling method for refractory facial paralysis affecting different locations.Methods: A total of 110 participants were randomized into an observation group and a control group, with 55 cases in each group. The observation group was treated with muscle regions of meridians needling method, and the control group was treated with conventional facial three-line needling method. The clinical efficacy was evaluated four weeks after the treatment. And the infrared imaging spectra of the two groups were examined. Results: The total effective rate of the observation group was higher than that of the control group (P<0.05), and its curative effect for refractory facial paralysis located above the geniculate ganglion was better than that of the control group (P<0.05). The color scale distribution of different disease locations in the two groups varied significantly (P<0.05), the higher the disease location, the higher the occurrence rate of cool zone and low temperature zone. After treatment, the reductions of the facial and periotic temperature difference between the healthy side and the affected side in the observation group were statistically different from those in the control group (P<0.05). Conclusion: Muscle regions of meridians needling method has a better effect than facial three-line needling method for refractory facial paralysis. It can promote the microcirculation of the affected side of the face, improve the blood and oxygen supply to local tissues, and thus promote the repair of the peripheral facial nerve.
8.Effects of pioglitazone on differentiation and function of cultured osteoclast-like cells of rats

Objective To study the effect of pioglitazone on the differentiation and function of rat osteoclast-like cells (OLC), and to probe the relationship between activated PPARγ2 and osteoclasts. Methods On day 1 of OLC formation from nonadherent bone marrow ceils (BMC) obtained from rats induced by M-CSF and receptor activator of NF-кB ligand (RANKL), 1, 5 and 10μmol/L pioglitazone hydrochloride was added. RT- PCR was performed to determine the mRNA expressions of PPARγ2 and receptor activator of NF-кB (RANK) on day 3, 5 and 7 during incubation, the number of tartrate-resistant acid phosphatase (TRAP)-positive cells,the number of bone resorption pits and the ratio of its area on dentin slice were counted, the activity of TRAP and the mean fluorescence intensity of integrin β3 (CD61) of OLC were also measured. Results (1) The effect on the differentiation of OLC: The addition of pioglitazone at the start of the culture period induced a dose-dependent decrease in TRAP-positive OLC and the activity of TRAP (P < 0.01 or P < 0.05) ; the mRNA expression of PPARγ2 was up-regulated by 5 and 10 μmol/L pioglitazone in the early stage of incubation and attenuated with thematuration of OLC on the contrary, however, the expression of RANK was down-regulated by 5 and 10 μmol/L piolitazone in every stage of incubation (P < 0.05 or P < 0.01), combined with decrease in TRAP-positive OLC from day 3 by 10 μmol/L pioglitazone. (2) The effect on the function of OLC: the number of bone resorption pits and the ratio of its area on dentin slice were decreased in groups of 5 and 10 μmol/L pioglitazone (P < 0.01 orP < 0.05), no obvious change was noted in the group with 1 μmol/L pioglitazone compared with the control group; the mean fluorescence intensity of CD61 were down-regulated in groups of 5 and 10 μmol/L pioglitazone (P < 0.05 or P <0.01). Conclusion Activation of PPARγ2 pathway by pioglitazone could partially inhibit differentiation and function of OLC derived from rat BMC.
9.Effect of acupuncture on renal function and pathologic changes of kidney in rabbits with nephritis.

OBJECTIVETo observe the effect of acupuncture on cationized bovine serum albumin (C-BSA) nephritis model in rabbits and to explore its mechanism.
METHODSFifty rabbits were randomly divided into a blank group, a model group, a metoprolol group, a irbesartan group and an acupuncture group, 10 rabbits in each group. The model was established by ear vein intravenous injection with C-BSA. The positive control groups were treated by intragastric administrated with metoprolol and irbesartan, respectively. The acupuncture group was treated by acupuncture at "Fengmen" (BL 12) and "Shenshu" (BL 23). No interventions were added on the blank group and the model group. The changes of blood pressure (BP), heart rate (HR), plasma norepinephrine (NE), serum creatinine (Scr), blood urea nitrogen (BUN) and 24 hours urine protein (24 h UP) in rabbits at the time points of 3rd, 6th and 8th week of treatment were observed.
RESULTSAfter the model was established, the Scr of (194.30 +/- 20.09) micromol/L, BUN of (9.19 +/- 0.66) mmol/L and 24 h UP of (277.70 +/- 20.09) mg/24 h in the model group were all higher than the Scr of (66.03 +/- 4. 76) micromol/L, BUN of (4.11 +/- 0.71) mmol/L and 24 h UP of (14.28 +/- 1. 47) mg/24 h in the blank group (all P < 0.01), and the diffused mesenteria hyperplasia and the increase of intercapillary cells in the model group were showed in the pathological sections. After 3 weeks of treatment. The Scr of (99.82 +/- 9.29) micromol/L, BUN of (6.32 +/- 0.75) mmol/L and 24 h UP of (189.67 +/- 15.45) mg/ 24 h in the acupuncture group were all decreased significantly, furthermore, the decrease of BP, HR, NE were better than the other treatment groups (P < 0.05, P < 0.01). Except the level of 24 h up and HR at 8th week, other results were as same as the 3rd week.
CONCLUSIONAcupuncture can improve the function of kidney, decrease the content of 24 h UP and the underlying therapeutic mechanism could be correlated with that acupuncture can lower excitability of sympathetic nerve and alleviate the renal pathological lesion induced by nephritis.

10.Relationship of vascular endothelial growth factor gene polymorphisms with retinopathy of prematurity in pre-term infants

Background Statistic data revealed that different retinopathy of pre-term infants have different susceptibility to retinopathy of prematurity (ROP),which may be associated with polymorphism of vascular endothelial growth factor(VEGF) gene.Objective This study was to determine the association of polymorphisms of VEGF gene with the risk for ROP.Methods This research was approved by Ethics Committee of Hunan Children's Hospital,and written informed consent was obtained from the parents of patients.A prospective case-controlled study was designed.Ninety-nine ROP patients in Hunan Children' s Hospital and 88 pre-termed children without ROP were included from January,2006 to December,2009.Thirty-nine patients who received retinal photocoagulation or cryotherapy were included as the treatment group,and 60 untreated but spontaneously regressed ROP patients as the non-treatment group.No significant differences were seen in demography between with the ROP group and the without ROP group,or between the treatment group and the non-treatment group (all P>0.05).2 mL of peripheral blood was collected for the extraction of DNA.Gene polymorphisms of VEGF-A+405 and VEGF-A936 were detected using the pyrosequencing method.Results No significant difference was found in the frequencies of the VEGF-A+405 gene polymorphisms between the ROP group and without ROP group (P =0.071,OR =0.675,95 % CI =0.444-1.026).Also no significant difference was found in the frequencies of the VEGF-A936 gene polymorphisms between with the ROP group and without the ROP group (P =0.118,OR =0.768,95 % CI=2.823-4.614).However,the frequencies of the VEGF-A+405 gene polymorphisms were significantly higher in the ROP treatment group than the non-treatment group (P<0.01,OR--0.857,95 % CI =5.239-14.024),and VEGF-A936 gene polymorphisms was also significantly higher in the ROP treatment group than the non-treatment group (P =0.000,OR =3.609,95 % CI =0.711-0.829).Conclusions There is no association between the VEGF-A+405/VEGF-A936 single nucleotide polymorphism with the risk of ROP,but polymorphisms of VEGF gene may be correlated with the prognosis of ROP.The carrier of VEGF-A +405 /VEGF-A936 allele is more susceptible to ROP progression.
