2.Goals for the treatment of multiple myeloma in the era of novel agents

The treatment of multiple myeloma has become possible in the era of novel agents. Novel agents have dramatically improved the response rates of multiple myeloma (MM) and have extended the survival of patients even if MM remains an incurable disease. De-termining the treatment goals of MM has become a major challenge for clinicians in the era of novel agents. Many results show the correlation between response depth and patients' survival. Complete remission (CR), stringent CR, phenotypic CR, and CRs at the molecular and PET-CT levels are being explored and becoming the goals of MM treatment. Doctors should balance efficacy with adverse effects in daily clinical practice to avoid the blind pursuit of a high and deep response rate.
3.Comparative Study of Total Laparoscopic Hysterectomy for Complicated Hysteromyoma

4.Correlation of vascular endothelial growth factor with metastasis of non-Hodgkin lymphoma

Objective To investigate the relationship of vascular endothelial growth factor(VEGF)with the invasion and metastasis of non-Hodgkin lymphoma.Methods The levels of serum VEGF was measured by enzyme linked-immunoadsorbent assay(ELISA).Comparing the parameters between NHL group and the control group.Compared the parameters with different groups divided by clinical stage,the effect of the treatment,the level of plasma LDH and effect of treatment.Analyzed the correlation of the levels of serum VEGF and plasma LDH in patients of NHL.Results The levels of serum VEGF are remarkablely higher in patients than that in the control group(P<0.05).The levels of serum VEGF in the group of Ⅲ,Ⅳ clinical stage are higher than that in the group of Ⅰ,Ⅱ clinical stage,but no significant difference(P>0.05)were observed.The level of serum VEGF is higher in the group of none CR than the group of CR(P<0.05).The level of serum VEGF is higher in the group of non-normal level of plasma LDH than group of normal LDH level(P<0.05).The levels of VEGF and LDH are significantly correlated each other(P<0.01).Conclusion The angiogenesis are important for tumor metastasis in patients of NHL.The serum VEGF can be used as prognosis parameters in patients with NHL.
5.Expression of hepatocyte growth factor in placenta in PIH

Objective To study the expression of hepatocyte growth factor (HGF) in placenta from normal pregnancy and pregnancy induced hypertension (PIH) and the effect of HGF on angiogenesis in placenta. Methods Immunohistochemistry was used to determine the location and intensity of HGF staining in placenta from 20 cases of normal pregnancy and 58 cases of mild,moderate and severe PIH groups. HE staining was used in order to count the number of the vessles. Results HGF mostly were expressed in villous core mesenchymal cells,less in decidual cells . Intensity of HGF immnostaining in villous mesenchymal cells was significantly reduced in the severe PIH group compared with the normal group(H=7.395,P=0.003),while there were no statistical differences between the mild,moderate PIH group and the normal group (H=0.869,P=0.351,H=0.017,P=0.896) .The count of vessels reduced in severe PIH (F=10.676,P=0.008). There was positive correlation between the expression of HGF and the count of vessels in villous(r=0.246,P=0 030). Conclusion These results suggested that HGF is secreted mainly by villous core mesenchymal cells in human placenta and the reduction of HGF in PIH may be responsible for the impaired vascularization in PIH.
8.Identification and in vitro BrdU labeling of adult bone marrow mesenchymal stem cells

BACKGROUND: No particular marker molecule of bone marrow mesenchymal stem cells (BMSCs) is presently found, so its determination is difficult. BrdU marker has no radioactive pollution. Some studies have confirmed that BrdU marker has no damage to cell function without ultraviolet radiation. OBJECTIVE: To investigate in vitro identification and labeling methods and biological characteristics of adult BMSCs. DESIGN, TIME AND SETTING: The cell experiment was conducted at the General Hospital of Lanzhou Military Area Command of Chinese PLA between June and December 2007. MATERIALS: Bone marrow was collected from 5 healthy adult volunteers. BrdU was purchased from Sigma, USA. METHODS: 10 mL adult bone marrow was harvested to isolate mononuclear cells by density gradient. Cells were cultured in DMEM containing 10% fetal bovine serum and proliferated at a density of 2?108 L-1. At the third passage, BMSCs was inoculated in medium supplemented with osteoblast inductor and stained with alkaline phosphatase. Subsequently, BMSCs were inoculated in medium supplemented with lipoblast inductor and stained with oil red O. Cells were incubated with BrdU at different concentrations of 5, 10 and 15 ?mol/L for 12, 24, 48, 72 and 96 hours, and then detected by immunocytochemistry. MAIN OUTCOME MEASURES: Cell growth and proliferation were observed under an inverted light microscope. Cell phenotype, osteoblast and lipoblast differentiation were identified by flow cytometry. BrdU-positive labeling rate and cell growth after labeling were investigated. RESULTS: In vitro cultured BMSCs were homogenous population and exhibited a spindle-shaped fibroblastic morphology. BMSCs expressed CD44 and CD71, but did not express CD34 and HLA-DR. BMSCs can differentiate into osteoblasts and lipoblasts. Most BMSCs were labeled by BrdU. With the increase in labeling concentration and over time, BrdU positive rate gradually increased and exceeded 90% after labeling with BrdU (10 ?mol/L) for 72 hours, with the labeling identifiable in nine consecutive passages. At the third passage, 90% BMSCs were in G0/G1 phase, and 88.62% in G0/G1 phase after labeling. BrdU has little effect on morphous and proliferation of labeled cells. CONCLUSION: Adult BMSCs can be identified through morphological character, specific surface antigens and multipotential differentiation in vitro. BrdU labeling provides a feasible means for a dynamic observation of the survival, growth and differentiation of the implanted BMSCs. The optimal dosage and timing of BrdU labeling are respectively 10 ?mol/L and 72 hours.
9.Establishment of an animal model of sacral nerve root avulsion in rats

Objective To establish an animal model of sacral nerve root avulsion in rats and e-valuate its efficiency. Methods A total of 20 adult SD rats (either sex) were chosen at random to es-tablish the sacral nerve root avulsion model by avulsing the fight L4-6 nerve roots out of intervertebral fo-ramina without laminectomy. The left side was set as control group. The models were evaluated in aspects of survival rate, Basso-Beattie-Bresnahan (BBB) scores, somatosensory evoked potential (SEP), horse radish peroxidase (HRP) tracing, bilateral weight and cross section area (CSA) of muscle biceps femo-ris, fiber of triceps surae and anterior tibial muscle. Results Of all, 19 rats were survived but one died, with survival rate of 95.0%. The BBB score was (10.78+3.15) points in experimental group and 21 points in control group. The success rate of establishing animal model was 89.5% ,for there detected no SEP in bilateral cerebral cortex of the wounded extremity of 17 rats. HRP showed positive reaction in the spinal segment of L4-6 in two rats but negative in 17, with success rate of 89.5%. There was statistical difference in aspects of weight and muscle fiber CSA of double biceps femoris, triceps surae and anterior fibial muscle between experimental group and control group. Electron microscope found denervation chan-ges including muscle fiber atrophy, nucleus shifting to (center and muscle satellite cell. Conclusion A-vulsion of L4-6 sacral nerve root out of vertebral canal is a feasible and ideal method to establish the avui-sion model of sacral nerve root injury in rats.
10.Clinical observation on the effect of laparoscopic varicocelectomy for ejaculum quality in patients with varicocele

Objective To observe the clinical effects of laparoscopic varicocelectomy to ejaculum quality in patients with varicocele.Methods Fifty patients with varicocele were treated by laparoscopic varicocelectomy.The patients were grouped into Group A and Group B according to venous diameter.Semen parameters including semen volume,sperm density,sperm motilities (a + b + c),sperm motilities (a + b),sperm motilities (a),total sperms,normal spermmorphology rate and sperm survival rate were measured and recorded before operation and 1,3,6 months after operation.Results After the follow-up of 6 months above in 50 cases,there is no significant difference about semen volume preoperative and postoperative at different follow-up time(P > 0.05).In some semen parameters(sperm density,sperm motilities,total sperms,normal spermmorphology rate),there is no significant difference between preoperative and postoperative 1 month(P > 0.05),but there is significant difference between postoperative 3 months and postoperative 6 months(P < 0.05).There is no significant difference in preoperative semen parameters between the 2 groups(P > 0.05),but there is significant difference in postoperative 6 month(P <0.05).In Group A,there is significant difference in sperm density between preoperative and postoperative 1 month (P <0.05).In Group B,there is significant difference in sperm density,sperm motilities and normal spermmorphology rate between preoperative and postoperative 1 month(P < 0.05).Conclusions Laparoscopic varicocelectomy has a positive effect on sperm parameters,it can significantly improve postoperative semen parameters.Semen parameters improve to a relatively stable level by 3 months following operation.
