Fifty three Wistar rats were randomly divided into healthy control group, COPD model group and NAC group, PKC inhibitor H7 intervention group and TGF ? monocolonal antibody group(TGF ? MA group). To study the role of matrix metalloproteinases(MMPs) and the tissue inhibitor of MMPs(TIMP 1) in the airway extracellular matrix(ECM) remodelling of COPD rat models and to observe the effects of NAC, H7 and TGF ? MA intervention on the regulation of MMPs and TIMP 1 and on ECM remodelling of the airway walls. Compared with control group, the airway collagen, the hydroxyproline(Hy) content of lung homogenates ,the number of fibroblasts(Fb) , the protein and/or mRNA expressions of MMP 2, MMP 9, TIMP 1 and TGF ? Ⅰ, Ⅱ receptor and the enzyme activities of MMP 2 (72kD), MMP 9 (92kD) were significantly increased in COPD model group. In all the drug intervention groups, the expressions of the above parameters were significantly decreased than those in model group except for Hy and Fb in H7 group, protein expression of TGF ? I receptor in NAC group and MMP 9 in TGF ? MA group. The results suggested that increased MMPs as matrix degrading enzymes might be responsible for the excessive degradation of ECM in airway, whereas incresed TIMPs might promote excessive ECM synthesis and deposition. The imbalance of MMP 9/TIMP 1 was related to the airway ECM remodelling. An antioxidant NAC and TGF ? MA might regulate the MMPs/TIMP 1 expression and reduce the airway fibrosis.H7 had strong collagenase inhibitory action, resulting increse in Hy and Fbs. The data may be helpful for searching effective prevention and treatment of airway ECM remodelling.

OBJECTIVETo investigate the effect and safety of extracorporeal shock wave (ESW) in the treatment of pain symptom of type III B prostatitis.
METHODSWe treated 50 cases of type III B prostatitis by ESW once a week for 4 weeks. Then we evaluated the clinical effect and safety of the therapy based on the NIH-CPSI scores, visual analogue scale (VAS) scores, IIEF-5 scores, prostate volume and morphous, state of urination, color of urine, results of routine semen analysis, and changes of cytokines (IL-6, TNF-α and IL-1β) in expressed prostatic secretion (EPS).
RESULTSAll the patients successfully accomplished the treatment. Compared with the baseline, decreases were observed after 4 weeks of cytokine treatment in the pain scores (14. 61 ± 1. 82 vs 9. 36 ± 1. 47, P <0. 01), urination symptom scores (4. 59 ± 1. 01 vs.4. 66 ± 0. 89, P >0. 05) , quality of life scores (6. 51 ± 1. 03 vs 4. 56 ± 1. 02, P <0. 01), NIH-CPSI (25. 43 ± 1. 72 vs 18. 28 ± 2. 32, P <0. 01 ), and VAS (6. 59 ± 1. 10 vs 3. 02 ± 1. 07, P < 0. 01). The concentration of IL-6 in the EPS was significantly increased ([55.82 ± 6. 28] vs [86.59 ± 4. 55] ng/ml, P <0. 01) , while the level of TNF-α ([3.89 ± 0. 12] vs [3. 19 ± 0.22] ng/ml, P<0.01) and that of IL-1β ([3.21 ± 1.01] vs [1.48 ± 0.95] ng/ml, P< 0. 01) remarkably reduced after treatment. However, there were no statistically significant differences in IIEF-5 scores (18. 58 ± 2. 03 vs 18. 51 1. 89, P >0. 05) or various sperm parameters before and after treatment (P >0. 05). And no significant changes were observed in the prostate volume, morphous or internal echoes.
CONCLUSIONThe ESW therapy is effective and safe for the pain symptom of type III B prostatitis.

International Eye Science 2021;21(5):861-865
AIM: To systematically evaluate the effect and safety of cyclopentolate and atropine on ciliary muscle paralysis before optometry in myopic children.
METHODS: Relevant references published before April 2020, which concerned about cyclopentolate compared with atropine for ciliary muscle palsy in children with myopia, were obtained by searching PubMed, EMBASE, Web of Science, The Cochrane Library, CNKI and WanFang Database. For the selected studies, after data extraction and methodological quality evaluation of the included study, RevMan5.3 software was used for Meta-analysis.
RESULTS: Nine articles were finally included, containing 588 eyes using atropine and 592 eyes using cyclopentolate. Meta-analysis results indicated: comparing of cyclopentolate and atropine for cycloplegia in children with myopia before optometry, the diopter difference between the two is WMD: -0.01, 95%CI(-0.30, 0.27), P=0.93; the difference in residual accommodation power between the two is WMD: 0.22, 95%CI(-0.13, 0.58), P=0.22. In addition, the cyclopentolate is safer and has a lower incidence of adverse reactions.
CONCLUSION: Compared with atropine, cyclopentolate has equivalent effects on ciliary muscle paralysis in myopic children, and has higher security. Cyclopentolate could replace atropine for myopic children before optometry.

Objective To approach the evaluative effect of respiratory variation of superior vena cava peak flow velocity measured using transthoracic echocardiography (TTE) on fluid responsiveness in patients with mechanical ventilation. Methods A prospective cohort study was conducted. All mechanical ventilated critically ill patients whose fluid therapy was planned due to hypovolemia in Department of Critical Care Medicine of Beijing Tongren Hospital of Capital Medical University from April 2011 to April 2013 were enrolled.Volume expansion was performed with 500 mL Linger solution within 30 minutes.Patients were classified as responders if pulse pressure variation (PPV) increased ≥ 13% before volume expansion. The respiratory variation in superior vena cava peak velocity was calculated as the difference between maximum and minimum values of velocity in peak A,peak S and peak D over a single respiratory circle, and their variations (ΔA,ΔS,ΔD) were also calculated. The receiver operating characteristic curve (ROC curve) was plotted to assess the evaluative effect of respiratory variation of superior vena cava peak velocity on fluid responsiveness. Results Twenty-seven patients were enrolled in this study.Volume expansion increased PPV ≥ 13% happened in 14 patients (responders). The velocity of superior vena cava in peak A,peak S,peak D was significantly increased after volume expansion compared with that before volume expansion in responders [peak A (cm/s):34.6 ± 2.2 vs.31.3 ±2.1,t=-2.493,P=0.027; peak S (cm/s):39.1 ± 1.3 vs.35.3 ±2.1,t=-2.564,P=0.024; peak D (cm/s):28.1 ± 1.2 vs.23.3 ± 1.4,t=-4.995,P=0.000],but there was no significant difference in ΔA,ΔS and ΔD between before and after volume expansion. The ΔA,ΔS and ΔD were positively correlated with PPV (r=0.040,P=0.854; r=0.350,P=0.074; r=0.749,P=0.000).The area under ROC curve (AUC) of peak S was 0.36 [95% confidence interval (95%CI):0.11-0.52],but the AUC of ΔS was 0.68 (95%CI 0.47-0.89),the AUC of peak D was 0.41 (95%CI 0.19-0.63),but the AUC of ΔD was 0.95 (95%CI 0.86-1.00),so the aberration rate of superior vena cava in respiration was better than the flow rate in superior vena cava. When the cut-off value of ΔS was 20.7% for predicting fluid responsiveness, the sensitivity was 78.6% and the specificity was 61.5%.When the cut-off value of ΔD was 12.7% for predicting fluid responsiveness, the sensitivity was 92.0% and the specificity was 92.3%.Conclusion Respiratory variations in superior vena cava peak velocity measured by TTE could assess fluid responsiveness in patients with mechanical ventilation.
9. Continuous splenogonadal fusion: a case report and review of literature
Journal of Shanghai Jiaotong University(Medical Science) 2020;40(7):1002-1004
A 4-year-old boy with a painless mass for 3 years in the left scrotum was admitted to the hospital. Three testicle-like substances could be palpable one by one from groin to scrotum on the left side during physical examination. The upper and middle parts were slightly hard, while the lower part was slightly soft. The splenogonadal fusion (SGF) in the left side was found by laparoscopy after admission. Accessory splenectomy was performed with preservation of the left testicle. Postoperative follow-up was 18 months. The testicles were well developed without atrophy, and abdominal B-ultrasound showed no abnormality in liver, gallbladder and spleen. SGF was a rare congenital abnormality, which was difficult to diagnose preoperatively. During the operation, if the SGF was found, accessory splenectomy with testicle-sparing should be performed. Laparoscope can be used as an effective diagnosis and treatment method to improve the understanding of SGF, and avoid unnecessary orchiectomy.

OBJECTIVETo explore the effect of dexmedetomidine combined electrical stimulation on cognitive function of neurosurgical diseases patients treated by extracerebral intervention.
METHODSTotally 122 patients with neurosurgical diseases who underwent selective intervention were randomly assigned to the observation group and the control group, 61 cases in each group. Patients in the control group recieved anesthesia by dexmedetomidine. Those in the observation group received electrical stimulation at Baihui (DU20), Yintang ( EX-HN3), and Neiguan (PC6) before dexmedetomidine anesthesia. The cognitive function of patients at preoperative day 1 and postoperative day 1 was respectively evaluated by Mini-Mental State Examinations (MMSE). Serum NSE, S-100β, IL-1β, IL-6, and TNF-α levels were detected in the two groups before intervention and immediately after intervention using ELISA.
RESULTSMMSE scores of two groups were significantly reduced at post-intervention day 1, as compared with one day before intervention. MMSE score of the observation group at post-intervention day 1 was (23.15 ± 1.87) points, significantly higher than that of the control group [ (19.34 ± 1.64) points , (P < 0.05)]. The postoperative cognitive dysfunction (POCD) incidence rate of the observation group was 16.4% (10/61), significantly lower than that of the control group [39.3% (24/61); P < 0.05]. Compared with before intervention, NSE and S-100β protein levels, IL-1β, IL-6 and α-TNF levels of the two groups increased (P < 0.05). Post-intervention NSE and S-100β protein levels, IL-1β, IL-6 and α-TNF levels were significantly lower in the observation group than in the control group (P < 0.05).
CONCLUSIONDexmedetomidine combied electrical stimulation could effectively prevent the occurrence of postoperative cognition, and reduce levels of NSA, S-100β, IL-1β, IL-6 and TNF-α.
