1.Network of multiple factors and cascades: hotspots of mechanism studies for diabetic retinopathy
Chinese Journal of Ocular Fundus Diseases 2013;(1):5-8
The pathogenesis of diabetic retinopathy is complicated. The vast network of multiple factors including unifying mechanism, inflammatory reaction, neuron degeneration and metabolic memory of glucose, and the four established pathogenic molecular pathways are hotspots of mechanism research for diabetic retinopathy.Nevertheless, these researches may be only one corner of the iceberg of DR mechanism, and we still face enormous challenges in DR mechanism research.Collaboration with multiple disciplines to study the relationship between DR and diabetes and other systemic diseases, search novel therapy targets may increase the result in an unexpected windfall for DR basic research.

Background and purpose:Neoadjuvant chemotherapy is one of the hot studied area in breast cancer research. Our aim was to explore the relationship between the clinical effect, pathological changes and the neoadjuvant chemotherapy containing THP and docetaxel on breast cancer. Methods:The expression of ER, PR and CerbB-2 in breast cancer tissue of patients who had received neoadjuvant chemotherapy were detected by immunohistochemistry method. The relationship of age,ER,PR,CerbB-2,pathological stage and axillary lymph node metastasis with pathological complete response(pCR) was analysed. Results:For the patients after neoadjuvant chemotherapy, the pCR of the positive ER(0%),PR(9.38%) was lower than that of negative ER(20.27),PR(90.63%) respectively. The pCR of the positive CerbB-2(33.33%) was higher than that of negative CerbB-2(9.74%,P

Objective To evaluate the clinical efficacy of Xiaojianzhong Decoction plus Dachaihu Decoction in the treatment of chronic atrophic gastritis(CAG),and to provide scientific evidence for the clinical application of the formula. Methods The clinical observation was carried out in 80 CAG patients with spleen deficiency and stasis-heat syndrome of normal-people pulse type(the ratio of patients'pulse to the number of respirations within one minute being 4-5 evaluated by Changsangjun pulse-taking method)who attended the clinic of the Department of Gastroenterology of Shunde Hospital,Guangzhou University of Chinese Medicine, from January 2020 to December 2023.According to the treatment method, the patients were divided into the treatment group and the control group, with 40 cases in each group. The control group was given conventional western medicine treatment, and the treatment group was given Xiaojianzhong Decoction plus Dachaihu Decoction.Seven days constituted one course of treatment, and the treatment covered 6 months. The changes of traditional Chinese medicine(TCM)syndrome score, gastroscopy score, pathological score and gastric function indicators in the two groups were observed before and after the treatment. After treatment, the clinical efficacy and safety of the two groups were evaluated.Results(1)After 6 months of treatment, the total effective rate of the treatment group was 90.00%(36/40),and that of the control group was 60.00%(24/40).The intergroup comparison(tested by chi-square test)showed that the efficacy of the treatment group was significantly superior to that of the control group(P<0.05).(2)After treatment, the scores of TCM syndromes in the two groups were significantly lower than those before treatment(P<0.05),and the decrease of the scores in the treatment group was superior to that in the control group(P<0.05).(3)After treatment, the gastroscopy scores of the two groups were significantly lower than those before treatment(P<0.05),but there was no significant difference between the two groups after treatment(P>0.05).(4)After treatment, the total pathological scores of the two groups and the scores of the gastric mucosal atrophy and intestinal metaplasia of the gastric antrum, gastric angle and gastric body in the treatment group were significantly improved compared with those before treatment(P<0.05),and the improvement of the gastroscopy scores in the treatment group was significantly superior to that in the control group(P<0.05).No statistically significant differences were presented in the scores of the gastric mucosal dysplasia and chronic inflammation of gastric antrum, gastric angle and gastric body in the two groups and in the scores of the gastric mucosal atrophy and intestinal metaplasia of gastric antrum, gastric angle and gastric body in the control group when compared with those before treatment(P>0.05).(5)After treatment, the serum levels of gastric function indicators of pepsinogen Ⅰ(PGⅠ),pepsinogen Ⅱ(PGⅡ)and gastrin 17(G-17)in the two groups were significantly decreased compared with those before treatment(P<0.05),and the decrease in the treatment group was significantly superior to that in the control group(P<0.05).(6)There were no obvious adverse reactions occurring in the two groups during the treatment, with high safety. Conclusion Xiaojianzhong Decoction plus Dachaihu Decoction can significantly enhance the clinical efficacy of CAG patients with spleen deficiency and stasis-heat syndrome of normal-people pulse type, significantly improve the gastrointestinal function and pathological scores of the patients, and has high safety.

AIMIn order to seek the marks of the genes, the relation between the influence of endurance training on aerobic ability and ACE Gene I/D Polymorphisms were studied.
METHODS102 army recruits of Han nationality from North China for an 18 week en durance training of 5000m distance. Their VO2(max), VT and the left ventricular structure and function were measured before and after the training. We also tested their ACE Gene I/D Polymorphisms with PCR-AFLP method.
RESULTSThe compliance of VO2(max), VT and left ventricular structure and function had improved after the training; the deltaVO2(max) of ID and II type was obviously higher than that of DD type (P < 0.05); there was obviously diference of deltaVO2(VT) in different ACE genotype (P < 0.05), the deltaVO2(VT) of type II was obviously higher than that of DD type (P < 0.05).
CONCLUSIONI allele has obviously hereditary advantage on the sensitivity to aerobic training in VO2(max) and VT, and type II has relation on the sensitivity to aerobic training in VT; there is no relation between I/D polymorphism and the sensitivity to aerobic training on the structure and function of left ventricle.

OBJECTIVETo investigate the neurodevelopmental outcomes of extremely low birth weight (ELBW) and very low birth weight (VLBW) infants at a corrected age (CA) of 18 months and related factors influencing the outcomes.
METHODSThe ELBW and VLBW infants who were admitted to the neonatal intensive care unit, survived, and discharged between January 2013 June 2014 were enrolled. These infants were followed up at CAs of 40 weeks and 1, 3, 6, 12, and 18 months to evaluate the neurodevelopmental outcomes. According to the neurodevelopmental status, the infants were divided into normal and abnormal neurodevelopment groups. The differences in clinical data were compared, and the risk factors for abnormal neurodevelopment in ELBW and VLBW infants were analyzed.
RESULTSA total of 338 ELBW and VLBW infants were enrolled, and 15 died during hospitalization. At the CA of 18 months, 145 infants (44.9%) survived and had complete follow-up data, 75 (23.2%) died, and 103 (31.9%) were lost to follow-up. Of the 145 infants who survived and had complete follow-up data, 71 (49.0%) had neurodevelopmental impairment (NDI), and 3 (2.1%) had cerebral palsy. No infants experienced visual damage with blindness in one or both eyes or hearing loss with a need for hearing aid. The logistic regression analysis showed that bronchopulmonary dysplasia (BDP) (OR=3.530, P<0.001) and sepsis (OR=2.528, P=0.035) were independent risk factors for NDI in ELBW and VLBW infants, and the incidence of NDI increased with the severity of BDP.
CONCLUSIONSSepsis and BPD, especially severe BPD, are risk factors for NDI in ELBW and VLBW infants.
