1.Effect of different drug dosage to activate blood circulation and to nourish qi on cardiac function and structure of congestive heart failure rats after acute myocardial infarction.

OBJECTIVETo observe the effect of different drug dosage to activate blood circulation and to nourish Qi on cardiac function and structure of heart failure rats after acute myocardial infarction.
METHODThe congestive heart failure post cardiac infarction rats models subjected to left coronary occlusion were made and given different dosage of drug of Huoxue Yiqi and captopril (positive control) from 4 weeks to 8 weeks after operation. The cardiac function before and after using drugs were observed by impedance methods.
RESULTTreated by different dosage of drug of Huoxue Yiqi and captopil, cardiac function of heart failure rats after acute myocardial infarction improved, SV, CO, CI(P < 0.01); at the same time, prescription of Huoxue Yiqi and Qixue could improve the model rats' heart coefficient, VS SHAM(P > 0.05), but prescription of Yiqi Huoxue and captopril could not improve it.
CONCLUSIONDrug to activate blood circulation and to increase qi can treat heart failure rats after acute myocardial infarction, through improving their function and structure. At the same time, more dosage of drug to activate blood circulation and to increase qi(Huoxue/Yiqi drug) may improve the model rats' heart coefficient.

2.Study on differential proteomic expression in human liver cells stimulated by trichloroethylene with proteomics.

OBJECTIVETo explore the differential proteomic expression in human liver cells L-02 induced by different dosages of trichloroethylene (TCE).
METHODSHuman liver cells L-02 were treated with different concentrations of TCE and the solvent control (dimethylsulfoxide). The total cellular proteins were separated using 2DE and visualized with silver staining after TCE treatment. The images were analyzed with Image Master 2D Platinum 5.0 analysis software. The differentially expressed protein spots were identified by matrix assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF-TOF-MS).
RESULTSFifteen protein spots with significant difference were found, and went upward or downward or disappeared after the stimulation of TCE with different dosages, which indicated that TCE induced the change of the proteomic expression in the liver cells. The mass spectrum identification and the IPI human database retrieval were used for identifying 9 proteins related to the L-02 Liver cells induced by TCE.
CONCLUSIONThe result provides an insight to TCE-related molecular mechanism and which might be useful for further study of the TCE-associated proteins and molecular markers.

3.Neurodevelopmental outcomes of extremely low birth weight and very low birth weight infants and related influencing factors.

OBJECTIVETo investigate the neurodevelopmental outcomes of extremely low birth weight (ELBW) and very low birth weight (VLBW) infants at a corrected age (CA) of 18 months and related factors influencing the outcomes.
METHODSThe ELBW and VLBW infants who were admitted to the neonatal intensive care unit, survived, and discharged between January 2013 June 2014 were enrolled. These infants were followed up at CAs of 40 weeks and 1, 3, 6, 12, and 18 months to evaluate the neurodevelopmental outcomes. According to the neurodevelopmental status, the infants were divided into normal and abnormal neurodevelopment groups. The differences in clinical data were compared, and the risk factors for abnormal neurodevelopment in ELBW and VLBW infants were analyzed.
RESULTSA total of 338 ELBW and VLBW infants were enrolled, and 15 died during hospitalization. At the CA of 18 months, 145 infants (44.9%) survived and had complete follow-up data, 75 (23.2%) died, and 103 (31.9%) were lost to follow-up. Of the 145 infants who survived and had complete follow-up data, 71 (49.0%) had neurodevelopmental impairment (NDI), and 3 (2.1%) had cerebral palsy. No infants experienced visual damage with blindness in one or both eyes or hearing loss with a need for hearing aid. The logistic regression analysis showed that bronchopulmonary dysplasia (BDP) (OR=3.530, P<0.001) and sepsis (OR=2.528, P=0.035) were independent risk factors for NDI in ELBW and VLBW infants, and the incidence of NDI increased with the severity of BDP.
CONCLUSIONSSepsis and BPD, especially severe BPD, are risk factors for NDI in ELBW and VLBW infants.

4.Application of medical thoracoscopy in clinical diagnosis of tuberculous pleuritis

Objective:To evaluate the clinical diagnostic value of medical thoracoscopy in tuberculous pleuritis . Methods:The clinical data of patients with pleural effusion who received medical thoracoscopy from February 2012 to March 2016 was analyzed retrospectively . Results: Totally 136 patients with pleural effusions of unknown etiologies underwent medical thoracoscopy ,there were 46 cases confirmed to have tuberculous pleurisy .The appearances of tuberculous pleurisy under thoracoscopy were mainly different size of nodules on pleura (28 .3% ) ,pleural thickening or fibrous adhesion (56 .5% ) , hyperemia and edema of pleural or military nodules on the surface of pleura (10 .9% ) .ADA values of 46 tuberculous pleurisy patients were as follows :<40 U/L(19 .6% ) ,40‐45 U/L(4 .3% ) ,45‐60 U/L(39 .1% ) ,≥60 U/L(37 .0% ) .There were 45 cases had ADA more than 45 U/L , the pathological diagnosis was as follows :35 cases of tuberculosis , 1 case of adenocarcinoma ,3 cases of acute inflammation and 6 cases of chronic inflammatory .No serious complications were observed . Conclusions:Medical thoracoscopy is easy to operative and a relatively safe procedure ,and it can improve the accuracy in the diagnosis of tuberculous pleuritis and also reduce clinical misdiagnosis and missed diagnosis .
5.Study on the efficacy of hepatitis B virus vaccination and its influencing factors among children in rural area of Jiangsu province.

OBJECTIVETo evaluate the efficacy of hepatitis B viruse (HBV) vaccination and its influencing factors among children in rural area of Jiangsu province.
METHODSTwenty-five hundred and twenty-two children born after 1998 in rural area were selected as the study population using multistage cluster sampling method. HBsAg and anti-HBs were detected by enzyme linked immunoassay (ELISA) and radio-immunoassay (RIA), respectively. Anti-HBs negative children were boosted using different hepatitis B vaccines and the efficacy was compared. Factors causing HBV infection in HBsAg positive children were also investigated.
RESULTSHBsAg positive rates in 1-7 year olds were 0.28%-1.28%, and the anti-HBs positive rates decreased from 76.7% to 45.5%. The HBsAg positive rate in children not timely vaccinated was significantly higher than those with HBV vaccine injection within 24 hours after birth (1.4% vs. 0.5%, P = 0.031). More than 90% of the anti-HBs negative children had protective level of anti-HBs after boosted with HBV vaccine.
CONCLUSIONHBsAg positive rate in children born after 1998 in rural area of Jiangsu province decreased significantly, with an average of 0.8%. The reason for HBsAg carriage in children might be attributed to mother-to-infant transmission or not timely HBV vaccination.

6.Two-dimensional electrophoresis and mass spectrometry analysis on effects of trichloroethylene on protein of L-02 liver cells.

OBJECTIVETo study the effects of trichloroethylene (TCE) on the protein in L-02 cells in vitro.
METHODSThiazolyl blue and Trypan blue tests were used to investigate the cytotoxicity of TCE to L-02 liver cell. The 2-D electrophoresis was used to analyse the expression of proteins in L-02 liver cells. The differentially expressed protein spots were identified by matrix assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF-TOF-MS).
RESULTSWhen the concentration of TCE exceeded 30 micromol/L, there was distinct cytotoxicity to L-02 cell (P < 0.05). Selected 40 micromol/L to treat L-02 liver cells and analyze the differential proteome expression, the results showed that the expression level of 37 protein spots was up-regulated and 15 protein spots was down-regulated. And 15 proteins were identified by MALDI-TOF-TOF-MS.
CONCLUSIONTCE can change the proteome expression of L-02 liver cell. It should provide the fundamental information to identify proteins related to TCE in further study.

7.Study on cardiac gene expression microarray and recipe for activating blood circulation and supplementing qi on heart of rats with post-infarction heart failure.

OBJECTIVETo observe the effect of recipe for activating blood circulation and supplementing Qi (RAS) on cardiac functional structure in rats with post-infarction heart failure (PIHF).
METHODSRat model of PIHF was established by left coronary artery ligation. Left ventricular samples of model rats from infarcted or peri-infarcted area were obtained at PIHF formation stage and stable stage (10 days and 8 weeks respectively after operation), the total RNA extracted and detected using 6 pieces of rat's 40s gene microarray (4096 genes/microarray), the data were analyzed using software as Genespring, Treeview, Clustering and SOM. Besides, RAS was used to treat the model rats beginning from 4 weeks after modeling and lasted for 4 weeks, changes of heart function and cardiac coefficient before and after treatment were observed by impedance method with Captopril as positive control.
RESULTS(1) Genespring analysis showed thousands of genes differential expression (upper or down regulated), including 13 kinds of gene involving energy metabolism, myocardial cytoskeleton, fibrosis, etc. which, in the infarcted area at heart formation stage were 1086 genes and at the stable stage, 724 genes, while in the peri-infarcted area, formation stage 196 genes and stable stage 97 genes. (2) After RAS or Captopril treatment, the heart function improved significantly, with the stroke volume, cardiac output and cardiac index increased significantly (P < 0.01). RAS could also improve the cardiac coefficient of model rats, as compared with that in untreated model, P < 0.01, compared with that in the sham-operated rats, P < 0.05.
CONCLUSIONPIHF is a kind of overload heart disease with multiple genes abnormality. RAS could improve the heart function and histologic indexes, so as to treat the heart failure.

8.Effect of absorption combined with continuous venovenous hemofiltration on eliminating serum inflammatory medium and its nursing

Objective To discuss the effect of absorption (AP) combined with continuous venovenous hemofiltration (CVVH) on eliminating serum inflammatory medium for patients with systemic inflammatory response syndrome (SIRS) and its nursing essence.Methods Totals of 63 patients with SIRS were randomly divided into the treatment group (AP combined with CWH,n =31) and the control group (CWH,n =32).Data of serum inflammatory medium,such as TNF,CRP,IL-1,IL-6 and IL-10 were recorded and compared within and between two groups before and after the treatment.Results There was no statistically significant difference in TNF,CRP,IL-1,IL-6 and IL-10 between two groups before treatment (P > 0.05).After the treatment,levels of TNF,CRP,IL-1,IL-6 and IL-10 in 2,26 and 50 hours showed descending tendency in the treatment group,and the differences were statistically significant compared with before (P < 0.05).Levels of TNF,CRP,IL-1,IL-6 and IL-102,26 and 50 hours after the treatment in the control group also showed descending tendency,and the differences were statistically significant compared with before (P < 0.05) except data in 2 hours (P > 0.05).Levels of TNF,CRP,IL-1,IL-6 and IL-10 also had statistically significant differences between two groups at the same time (P < 0.05).For example,score of TNF was respectively (226.4±27.6),(165.3 ±24.5),(120.6 ± 19.2) ng/L 2,26 and 50 hours after treatment in the treatment group and (315.7 ± 38.3),(262.7 ± 29.4),(219.3 ± 25.6) ng/L in the control group,with statistically significant differences (t =-3.31,-7.19,-8.79,respectively; P < 0.05).Condusions AP combined with CVVH can effectively eliminate the serous inflammatory medium for SIRS patients,and close surveillance and nursing are key to smooth treatment and expectations achievement.
9.Protective effects of Salvianolic acid on myocardial ischemic injury of rats from the aspect of inhibiting inflammatory reaction

Objective To investigate the protective effects of Salvianolic acid on myocardial ischemic injury of rats from the aspect of inhibiting inflammatory reaction.Methods Sixty Sprague Dawley (SD) male rats were randomly divided into four groups (n =15per group):sham operation group,model group,experimental-high (36 mg · kg-1) group,experimental-low (18mg · kg-1) group with intraperitoneal injection dosing.Same amount of distilled water was injected to sham operation group and model group,lasting for 4 days.Myocardial ischemia model was established by coronary artery ligation of left anterior descending (LAD) branch after the last administration.Six hours after the operation,the blood of eye canthus was collected to assay the levels of creatine kinase(CK),MB isoenzyme of creatine kinase (CK-MB) and lactate dehydrogenase (LDH) in serum.The heart was collected 24 h after the operation,stained by hematoxylin eosin (HE)and checked the pathological change of the cardiac muscle tissue,assayed the cardiac troponin (cTnT),tumor necrosis factor α (TNF-α) in serum and myeloperoxidase (MPO) was measured in myocardial tissue homogenate.Results After 6 hours' myocardial ischemia,the serum CK in sham operation group was (529.61 ± 141.93) U · L-1,CK-MB was (708.12 ±385.93) U · L-1,LDH was (330.12 ±158.38) U · L-1 and cTnT was (294.12 ±55.10)pg· mL-1 after 24 hours' myocardial ischemia.The serum CK in model group was (996.42 ± 413.42) U · L-1,CK-MB was (1346.11 ±558.30) U · L-1,LDH was (520.12 ±154.76) U · L-1and cTnT was (513.00 ±69.71)pg · mL-1after 24 hours' myocardial ischemia.The serum CK in experimental-low and experimental-high groups were (499.45 ±159.33),(514.91 ±98.82)U · L-1,CK-MB were (831.42 ±385.11),(592.10 ±206.32)U·L-1,LDH were (462.62 ±229.68),(437.72 ± 175.80)U· L-1,and cTnT were (431.12 ±106.00),(338.80 ± 76.92)pg · mL-1after 24 hours' myocardial ischemia.Compared with the sham operation group,the CK,CK-MB and LDH value in the serum of model group increased significantly (P < 0.05).Compared with the model group,the salvianolic acid significantly both can decrease the CK and CK-MB in serum (P < 0.05).Compared with sham operation group,the cTnT value in the serum of model group increased significantly (P < 0.05).Compared with model group,high dosage of salvianolic acid could significantly decrease the cTnT value (P < 0.05).After 24 hours'myocardial ischemia,the TNF-α in serum of sham operation group was (118.90 ± 17.58) pg · mL-1,the MPO in heart tissue was (32.25 ± 3.75) U · L-1.The TNF-α in serum of model group was (156.00 ± 28.24) pg · mL-1,the MPO in heart tissue was (104.83 ±22.87) U · L-1.The TNF-α in serum of experimental-low and experimental-high groups were (136.05 ± 34.00),(125.06 ± 30.13) pg · mL-1,the MPO in heart tissue were (91.70 ±21.57),(68.00 ± 18.47) U · L-1.Compared with sham operation group,the TNF-α values in the serum and the MPO value in model group increased significantly (P < 0.05).Compared with model group,these values of experimental-high group decreased with significantly (all P < 0.05).Conclusion Salvianolic acid showed the protection of myocardial tissue,which also could significantly reduce the myocardial cell injury and inflammatory cell infiltration of cardiac myocytes,which could be related with the decreased levels of TNF-α and MPO.
10. Expression of serum Dickkopf-1 in gastric cancer patients

Objective: To explore the expression and clinical significance of DKK-1 protein in patients with gastric cancers. Methods: Enzyme linked immuno sorbent assay was used to detect expressions of serum DKK-1 protein in 90 cases of gastric cancers, 50 cases of gastric benign disease and 40 healthy cases. The dynamic change in serum DKK-1 protein of gastric cancer patients who accepted radical operation for a month was also observed. Results: The expression of serum DKK-1 protein in gastric cancer groups was significantly higher than that in gastric benign group's (P < 0.01) and in health control (P < 0.01). Serum DKK-1 level was increased gradually along with the progress of the disease. Serum DKK-1 levels were significantly higher in patients at TNM staging III and IV than patients at TNM staging I and II. Level of serum DKK-1 was related to microvascular invasion, differentiation degree and infiltration depth. Level of serum DKK-1 was significantly reduced in patients after radical surgery (P < 0.01). Conclusions: The expression of serum DKK-1 protein in gastric cancer patients is increased. Level of serum DKK-1 is related to TNM staging, microvascular invasion, differentiation degree and infiltration depth. DKK-1 detection can be used as a reference index in monitoring gastric cancer progress and biological behavior.
