1.Comparison of three kinds of surgical methods for cataract induced by silicone oil-filled eye

AIM:To compare the clinical efficacy of three combined surgical treatments for cataract induced by silicone oil -filled eye.
METHODS: The data of 75 eyes of 75 patients with cataract induced by silicone oil-filled eye were reviewed. Those patients were divided into group A, B, C. The patients in group A were treated by phacoemulsification combined with silicone oil removal through pupil and IOL implantation, the patients in group B were treated by phacoemulsification combined with silicone oil removal through 23G vitrectomy system and IOL implantation, the patients in group C were treated by phacoemulsification combined with silicone oil removal through 20G vitrectomy system and IOL implantation. The operation time, intraocular pressure, best corrected visual acuity ( BCVA ) , discomfort days and complications before and after the surgery were compared.
RESULTS: The differences of the intraocular pressure were not statistically significant between the three groups after the surgery. The BCVA of the three groups were all improved, but the differences were not statistically significant between three groups. The differences in postoperative complications showed no statistically significant. Otherwise, group A and B cost less operation time. The discomfort days of group A were less than other groups.
CONCLUSION: Three different surgical methods were safe and effective, appropriate surgical approach could be chosen according to the patients' clinical manifestations. For patients with cataract induced by silicone oil-filled eye whose retina recovered well, phacoemulsification combined with silicone oil removal through pupil and IOL implantation is a time-saving, safe and effective method.
5.Inhalation Pneumonia of Senile Patients:Analysis of Clinical Feature and Etiology

OBJECTIVE To investigate clinical features and etiology of inhalation pneumonia.METHODS Totally 108 cases of inhalation pneumonia during from Jan 2000 to Dec 2005 were completely surveyed and analyzed. RESULTS There were underlying diseases and susceptible factors, and it was not typical in their clinical signs and symptoms.Totally 177 pathogens were isolated from sputa. There were 96 strains of Gram-negative bacilli (54.2%), 41 strains (23.2%) of Gram-positive cocci, and 40 strains (22.6%) of fungi. The 45 cases (41.7%) were with polyinfections, and 19 cases (17.6%) with double infections.CONCLUSIONS We should enhance diagnosis of inhalation pneumonia, make rational use of antibiotic, and take vigorous precautions against inhalation pneumonia.
6.Clinical efficacy and safety of bevacizumab combined with TP chemotherapy in platinum recurrent ovarian cancer

Objective To study the clinical efficacy and safety of bevacizumab combined with TP chemotherapy in platinum recurrent ovarian cancer.Methods34 patients with platinum sensitive recurrent ovarian cancer were seleted as the objects.All patients were treated with bevacizumab plus paclitaxel and cisplatin.The short-term effects, overall survival(OS), progression free survival(PFS) and incidence of adverse reaction during chemotherapy were observed.ResultsThe patients with CR, PR, SD and PD were 3 cases, 24 cases, 5 cases and 2 cases, respectively.The median PFS and median OS of patiens were 12.7months and 26.2 months, respectively.The incidence of decrease in leukocytes, thrombocytopenia, neutrophils, gastrointestinal reactions, elevated levels of transaminase, hypertension, epistaxis, proteinuria, peripheral neuropathy, mucositis were 73.53%, 20.59%, 67.65%, 82.35%, 47.06%, 14.71%, 5.88%, 17.65%, 29.41% and 17.65%, respectively.ConclusionThe bevacizumab combined with TP chemotherapy in treatment of platinum sensitive recurrent ovarian cancer can better relieve the disease, improve the survival time of the patient, the adverse reaction is controllable, and the safety is good, which has the clinical popularization value.
7.Regulatory effects of CD44 antibody-A3 D8 on IL-3 Rαand downstream PI3 K/Akt pathway in NB4 cells

Aim To investigate the effect of CD44 anti-body-A3 D8 on the expression of IL-3 Rα and down-stream PI3K/Akt in NB4 cells. Methods The ex-pression of IL-3 Rα mRNA was detected by real-time quantitative RT-PCR, the IL-3Rα protein expression and changes of PI3 K/Akt signal pathway in NB4 cells treated with A3D8 were analyzed by Western blot. An-nexin-V-FITC/PI double staining flow cytometry was u-tilized to detect the apoptotic cells. The inhibitor of PI3 K/Akt signaling LY294002 combined with A3 D8 was used to inhibit the PI3K/Akt in NB4 cells. Re-sults After treated with A3 D8 , both the transcription-al level and translational level of IL-3 Rα were remark-ably reduced, and the PI3K/Akt pathway was inhibi-ted. LY294002 improved the inhibitory and apoptotic effects of A3D8 on NB4 cells. Conclusion CD44 antibody A3 D8 can downregulate the expression of IL-3Rα and inhibit the downstream PI3K/Akt pathway.
8.Research progress of oxidative stress in radiation-induced lung injury

Although it has been frequently used to treat thoraxic tumors,radiation induced lung injury (RILI) is the major factor of dose limitation in thoracic radiotherapy.Amount of endogenous and exogenous reactive oxygen / nitrogen species (ROS/RNOS) could be generated in the radiated organisms and further cause molecular damage of DNA,protein and membrane lipids,which results in celluar structure damage,dysfunction and RILI.In addition,a series of cytokines could also induce chronic oxidative stresses that contribute to increases in cell membrane permeability,tissue edema and extracellular matrix proteins accumulation and even further result in pulmonary fibrosis.Oxidative stress theory offers new clues and strategies for further understanding the mechanism of RILI,and some anti-oxidative stress drugs may have potential clincial application in RILI treatment.
9.Trends of demographic and primary disease in new end-stage renal disease dialysis patients of Zhejiang province from 2008 to 2013

Objective To analyze the changes of primary disease and demography of patients with end-stage renal disease (ESRD) who started dialysis between 2008 and 2013 in Zhejiang province, in order to provide statistical support for the improvement of dialysis quality.Methods The study retrospectively reviewed all the incident ESRD patients on dialysis registered in Zhejiang Dialysis Quality Control Center from 2008 to 2013.Their demographics and primary cause were investigated.Stratified analysis based on dialysis modality, duration and age were conducted.Results A total of 26 310 incident ESRD patients who started dialysis between 2008 and 2013 in Zhejiang were selected, among which 14 886 cases were male and 11 424 cases were female (male: female: 1.30: 1).The study gave priority to patients aged 45 and above (73.9%), with an average age of (55.7± 16.1) years.The top primary disease was chronic glomerulonephritis (51.3%), followed by diabetic nephropathy (17.3%) and hypertensive nephrosclerosis (6.4%).From 2008 to 2013, the incidence of dialysis was 46.3, 60.1, 71.2, 85.8, 104.1, and 119.9 per million population respectively.The average age of patients was increased year by year, from 52.4, 53.3, 54.8, 56.0, 56.9, to 57.6 years.The share of peritoneal dialysis, as well as the promotion of diabetic nephropathy in primary disease was on the rise.With the increase of age, the proportion of primary diseases such as diabetic nephropathy and hypertension nephrnsclerosis was increased.Conclusions In Zhejiang province, incident dialysis patients increase annually.More males than females are on dialysis.The average age and proportion of peritoneal dialysis are rising.The leading cause is chronic glomerulonephritis, but the incidence of diabetic nephropathy is on the increase, which requires attention.
10.The MRI findings of mitochondrial encephalomyopathy,lactic acidosis and stroke-like episodes

Objective To analyze retrospectively the MR features of MELAS patients,in order to improve the early diagnosis of MELAS.Methods MR data of 1 6 MELAS patients confirmed by clinical diagnose and muscle biopsy were retrospectively analyzed. MR features of plain scan(n=1 6),ASL(n=3),MRA(n=1 5),DWI(n=14)and MRS(n=3)were analyzed.Results MRI data demonstrated brain lesions mainly distributed in posterior cerebral hemisphere extending to subcortical area,which was not consistent with the vascular supply territories.The lesions were observed as low signal on Spin-echo sequence of T1 WI,while high signal on T2 WI, FLAIR and DWI.The focal lesions showed increasing Lac peak on MRS.Fifteen of 16 patients underwent MRA and one showed increased arterial branches in lesion zones.Three patients had ASL scanning demonstrating high irrigation in lesion zones which would wander once relapsed.Meanwhile,old lesions showed encephalatrophy and cerebromalacia.All patients’muscle biopsy pathology showed ragged red muscle fibers.Under electron-microscopic,bioblasts were bigger and more than average level and muscle fibers atrophied.Mitochondrial DNA sequence of 3 patients showed mtDNA A3243G transgenation.Conclusion The DWI,MRS and ASL sequences show good ability in MELAS diagnosis and differential diagnosis.
