1.The replantation method of upper limb's biplane mutilation in conjunction with soft tissue defect

Objective To explore the replantation method of upper limb's biplane mutilation in conjunction with soft tissue defect.Methods From January 2003 to January 2013,29 patients with upper limb's biplane mutilation in conjunction with soft tissue defect were treated.There were 21 males and 8 females with the average age of 28.6 years.The youngest one was 1 year old and the oldest one was 70 years old.All the cases underwent different replantation order and transplantation by using vascular and nerve and flap according to the lesion they suffered,including the defected plane and soft tissue.Results All replantation limbs were survived besides 2 cases with infection and 3 cases of delaying healed due to the part of tissue necrosis.Among all 29 cases,9 cases got excellent recovery,14 cases were in good condition,4 cases were poor and 2 cases were bad.The totally excellent and good rate of upper limb function was 79.3% according to the limb reattachment function evaluation criteria of Hand Surgery of the Chinese Medical Association.Conclusion It can obtain higher survival rate due to our different replantations and method of tissue replantation to cure double plane mutilation in conjunction with soft tissue defect of upper limb.
2.The diagnosis and treatment of solid-pseudopapillary tumors of the pancreas

Objective To evaluate the clinical diagnosis and management of solid-pseudopapillary tumors of the pancreas.Methods A retrospective clinical analysis was made on 21 cases of solid-pseudopapillary tumors of the pancreas admitted from June 1994 to June 2003.Results Abdominal pain was the major complains in 13 cases , 7 cases had palpable abdominal mass.Imaging examination can show the abdominal mass clearly, but diagnosis can not be defined.There were 12 cases whose tumors were located in pancreatic head (4 underwent Whipple procedures and 8 tumor enucleations) and 9 cases with tumors in pancreatic tail (all underwent body and tail resection plus splenectomy).The average diameter of tumor was 9.5 cm (3-20).One case had liver metastasis.Diagnosis was proved by frozen pathology during operation in 14 cases, and other 3 cases initially diagnosed as insulinomas and other malignant tunors, had definite diagnosis by postoperative paraffin section and enzyme labeling.Enzyme labeling examinations showed that there were 14 cases of NSE positive in 15 cases, 8 cases of Vimentin positive in 10 cases and 7 cases of PAS positive in 8 cases.CK, EMA and CHG A were negative.Eighteen cases were followed up (85.7%) with a median period of 24 months(1~60) without tumor recurrence.Conclusion Huge pancreatic masses of pancreas in young females should suggest the diagnosis of solid-pseudopapillary tumors.Aggressive surgical therapy can result in good prognosis in these patients.

4.Inhibition of stromal interaction molecule 1 and the expression of apoptosis-related proteins in prostate cancer PC-3 cells.

OBJECTIVETo explore the effects of stromal interaction molecule 1 (STIM1) on the expression of apoptosis-related proteins in prostate cancer PC-3 cells.
METHODSWe transfected the lentivirus vector STIM1-pGCSIL-GFP carrying STIM shRNA into human hormone-independent prostate cancer PC-3 cells, and 3 days later observed the transfection efficiency by fluorescence microscopy. At 7 days after transfection, we determined the expression of STIM1 in the PC-3 cells by RT-PCR and Western blot and those of apoptosis-related proteins Bcl-2, Bax, survivin and activated Caspase-3 by Western blot.
RESULTSAt 3 days, inverted microscopy revealed a transfection efficiency of > 80%. At 7 days, the STIM1 expression was significantly inhibited at both mRNA and protein levels. The Bcl-2/Bax rate was remarkably decreased as compared with that of the control group (0. 31 vs 1.24 ) , and the survivin expression was markedly reduced, 0. 14 times that of the relative expression in the control. However, the Caspase-3 cleavage was significantly activated, 1.52 times that of the control (P <0.05).
CONCLUSIONSTIM1 can be regarded as an oncogene in prostate cancer PC-3 cells. Inhibition of its expression can induce PC-3 cell apoptosis by reducing the Bcl-2/Bax rate, decreasing the survivin expression, and activating the Caspase-3 pathway.

5.Study progress in reversing multidrug resistance to breast cancer with Chinese herbs.

This article mainly reviewed the research progress in reversing multidrug resistance to breast cancer with Chinese herb. If the herb's effects and safety can be testified in vivo by further research, it will be effectively applied in clinic.

A colistin producing strain Paenibacillus polymyxa AS1.541 was treated by N-methyl-N-nitro-N-nitrosoguanidine(NTG) for increasing yields of the antibiotic colistin.High-yield strains were obtained by selection of deregulated mutant which grow on media containing colistin,a self second metabolite,and ethionine,an analogue of methionine.Some of these mutants have higher yield of colistin than that of the parent strain.

Objective To examine the effects of glucose on the expression of P38 MAPK and TGF?2 in cultured human umbilical vein endothelial cells.Methods Human umbilical vein endothelial cells(HUVECs)were incubated in a culture medium with 11.2 mmol/L,16.8 mmol/L and 33.6 mmol/L glucose concentrations for 24 h,48 h and 72 h respectively.The expression P38 MAPK and TGF?2 was measured by RT-PCR and Western-blot.Results When D-glucose concentrations rose,HUVECs exposed to high glucose concentration(11.2 mmol/L,16.8 mmol/L and 33.6 mmol/L)showd increase in cell expression of P38 MAPK and TGF?2 after 48 h and 72 h exposure as compared with those cultured in medium of low glucose concentration(5.6 mmol/L).ConclusionHigh concentration of glucose can arrest the proliferative response.Even a short-term exposure of endothelial cells(ECs)to high glucose concentration may lead to their activation associated with increased expression of P38 MAPK and TGF?2.High Glucose Concentration on P38 MAPK and TGF?2 expression may participate the pathogenesy of diabetic retinopathy.
8.An analysis on misdiagnosis and improper management of solid pseudopapillary tumor of the pancreas

Objective To improve the diagnosis and management of solid-pseudopapillary tumor (SPT) of the pancreas. MethodsTwenty-two SPT patients were retrospectively reviewed and divided into two groups, misdiagnosed group and those with preoperatively correct diagnosis. ResultsAbout one half (46%) SPT cases were misdiagnosed preoperatively. With time SPT tends to invade its capsule resulting in impossibility of radical resection, and increased medical expenses. ConclusionsAlthough SPT is of low degree malignant, and the prognosis after surgical resection is satisfactory, misdiagnosis and preoperative misdiagnosis and inappropriate management still cost the patients increased expenses and inhospital stay.
9.Efficacy evaluation on electroacupuncture for Alzheimer's disease

Objective: To observe the clinical efficacy of electroacupuncture (EA) on Alzheimer's disease (AD). Methods: A total of 50 AD patients were randomly allocated into a Western medication (WM) group (n=25) and an acupuncture plus medication (APM) group (n=25). Patients in the WM group took oral huperzine A capsules. In addition to huperzine A capsules, patients in the APM group also received EA at Shenting (GV 24), Baihui (GV 20), Dazhui (GV 14), Fengfu (GV 16), Mingmen (GV 4) and Yongquan (KI 1). The needles on the above points were connected to G6805-Ⅱelectric stimulator [3 pairs: Shenting (GV 24) and Baihui (GV 20); Dazhui (GV 14) and Fengfu (GV 16); and bilateral Yongquan (KI 1)]. The needles were retained 25 min. The treatment was done once a day, and 10 times made up a course of treatment. The patients received a total of 3 treatment courses. There was a 3-day interval between two courses. The mini-mental state examination (MMSE) and Hasegawa dementia scale revised (HDS-R) were conducted before and after treatment. The clinical efficacies were evaluated when the treatment was completed. Results: Before treatment, there were no between-group statistical differences in MMSE and HDS-R scores (both P>0.05). After treatment, the MMSE and HDS-R scores in the APM group were significantly higher than those in the WM group (both P<0.05). The total effective rate in the APM group was 88.0%, versus 76.0% in the WM group, showing a statistical difference (P<0.05). Conclusion: EA is effective for AD and can improve clinical symptoms in AD patients.
10.Multiple acute cerebral infarcts as initial manifestation of occult systemic malignancy with 12 case report

Objective To characterize the clinical manifestations, laboratory findings of patients with occult sys?temic malignant neoplasms, whose initial manifestation presented as multiple acute cerebral infarcts including coagula?tion function,radiological imaging and microembolic signals (MES) detection by transcranial Doppler sonography (TCD) and to explore the possible underlying mechanisms. Methods All clinical records, laboratory hematological tests includ?ing hypercoagulable states measured by D-dimer levels, brain MRI including DWI, and TCD monitoring MES, the treat?ment and prognosis were retrospectively reviewed in 12 patients with multiple acute cerebral infarcts as the first manifes?tation of occult systemic malignancy. Results The clinical manifestations presented as localized neurological dysfunction, e.g. hemiparesis, aphasia, hemiparesthesia, dysarthria, vertigo and seizures, etc. DWI revealed multiple disseminated acute cerebral infarcts in multiple arterial territories such as the bilateral anterior or anterior plus posterior cerebral circu?lation in all patients. Eleven of 12 patients tested had elevated D-dimer. TCD detected MES in 5 of 7 patients. There were 12 patients diagnosed with occult systemic malignancy including 5 lung cancer, 3 pancreatic cancer, 1 gastric can?cer, 1 colon cancer, 1 endometrial adenocarcinoma and 1 metastatic poorly differentiated mucinous adenocarcinoma with unknown primary. Ten patients already had remote metastasis at diagnosis. The prognosis was usually poor and there were 7 cases with ischemic stroke recurrence, 4 cases with acute myocardial infarction, 3 cases died during hospitaliza?tion. Conclusions When patients present with multiple disseminated acute cerebral infarcts involving multiple arterial territories as initial manifestation, the underlying occult systemic malignancy should be considered. Hypercoagulopathy and MES might provide the clues to the diagnosis.
