ObjectiveTo investigate osteopontin (OPN) level in pulmonary tuberculosis and its clinical significance.MethodsThe level of serum OPN was determined in 80 pulmonary tuberculosis patients before anti-tuberculosis treatment[patients group,simple pulmonary tuberculosis with 60 cases (simple pulmonary tuberculosis group),pulmonary tuberculosis combined with tuberculous pleurisy with 20cases (pulmonary tuberculosis combined with tuberculous pleurisy group)],and compared with that of 30healthy persons (healthy control group) and 20 patients having received effective anti-tuberculosis treatment and followed-up for 6 months (followed-up group).At the same time,the clinical significance of OPN was studied by other commonly used clinical indicators of tuberculosis.ResultsThe level of serum OPN in patients group [(843.49 ±569.23) ng/L] was significantly higher than that in healthy control group [(352.50 ± 185.02) ng/L,P ＜0.05].The level of serum OPN after treatment in followed-up group were significantly decreased compared with those before treatment [(494.11 ±352.40) ng/L vs.(1106.60 ± 628.39) ng/L,P＜ 0.05 ] and there was no significant difference compared with those in healthy control group (P ＞ 0.05 ).The level of serum OPN in pulmonary tuberculosis combined with tuberculous pleurisy group [(1179.80 ±606.33) ng/L] was significantly higher than those in simple pulmonary tuberculosis group [(727.53 ±511.66) ng/L,P ＜0.05],and they were both higher than those in healthy control group [ ( 352.50 ± 185.02 ) ng/L,P ＜ 0.05 ].Therefore,for further additional subgroup analysis in simple pulmonary tuberculosis group,the level of serum OPN in patients with cavity[ ( 836.51 ± 549.80) ng/L ] was significantly higher than that without cavity [ (535.52 ± 375.95 ) ng/L,P ＜ 0.05 ],while the levels of serum OPN had no statistical difference between pulmonary lesion involved ＜ 3 leaf patients and ≥ 3 leaf patients,sputum mycobacterium tuberculosis positive patients and negative patients,mycoDot test results positive patients and negative patients,erythrocyte sedimentation rate elevated patients and normal patients,PPD strongly positive patients and non-strongly positive patients (P＞0.05).ConclusionsOPN activity is closely related with pulmonary tuberculosis.OPN can serve as a judge of pulmonary tuberculosis and the clinical indicators of disease outcome.The level of serum OPN and the severity of pulmonary tuberculosis have some correlations.

Objective To investigate the condition of the professional identity and learning burnout in the college students majored in re-habilitation therapy, and discuss the correlation between them. Methods From October to November, 2014, students of grades 2 and 3 ma-jored in rehabilitation therapy were investigated with the professional identity questionnaire and the learning burnout questionnaire. Results A total of 127 questionnaires were sent out, and 119 returned. The score of professional identity was (3.36 ± 0.55), with the factors score of the cognition, emotion, behavior, appropriateness as (3.66±0.77), (3.25±0.60), (3.23±0.71), and (3.36±0.61), respectively. The score of learn-ing burnout was (2.90±0.46), the score of factors of low spirits, improper behavior and low achievement as (2.87±0.70), (3.10±0.60), (2.77± 0.52), respectively. There was negative correlation between score of professional identity and learning burnout (r=-0.271, P<0.01), as well as between some factors of the identity and the learning burnout (P<0.05). Conclusion The professional identity and learning burnout are in the middle level in students majored in rehabilitation therapy, which may interact each other.

Objective To analyze mutations of the STAT3 gene in a patient with hyper-IgE svudrome (HIES) mainly manifesting as multiple cold abscesses.Methods Clinical data were collected and blood samples were obtained from a 17-year-old male patient with HIES and his parents.Genomic DNA was extracted and subjected to PCR for the amplification of the entire coding region and splice sites of the STAT3 gene followed by bidirectional sequencing.Results A heterozygous missense mutation C1427T,which caused a codon change from TCC to TTC and resulted in the substitution of serine by phenylalanine at amino acid residue 476 (p.S476F), was found in exon 16 encoding the DNA-binding domain in the STAT3 gene in the patient, but not in either of his parents.The results of amplified ribosomal DNA restriction analysis were consistent with the findings mentioned above.Conclusion A novel missense mutation S426F was found in the STAT3 gene in the HIES patient with generalized cold abscesses as the prominent clinical manifestation.

Concurrent chemoradiotherapy is the optimal regimen for patients with inoperable locally advanced non-small cell lung cancer (LA-NSCLC).The application of new technologies such as positron emission tomography (PET)/CT and four dimensional CT (4D-CT) enhances the accuracy of radiotherapy and decreases adverse reaction.Induction chemotherapy and consolidation chemotherapy do not show benefit to survival.The targeted therapy and immunotherapy have the potential of improving the outcomes of inoperable LA-NSCLC.

Objective To observe the incidence of orthostatic hypotension and hypertension in the elderly hypertensive inpatients and investigate the effect of antihypertensive medication on the orthostatic blood pressure changes in the elderly hypertensive inpatients.Methods A total of 185 aged hypertensive inpatients was divided into elderly group(n =126,60～79 years old) and very elderly group(n =59,≥80 years old) according to the age.The orthostatic blood pressure was measured in supine position and after standing.Subjects were divided into 3 subgroups:orthostatic hypotension (OH),orthostatic hypertension (OHT),and orthostatic normotension (ONT) groups according to the standing blood pressure levels.Clinical characteristics of each subgroup were evaluated and analyzed according to the antihypertensive medication.Results Among 185 patients,the OH incidence was 31 (16.76％) cases,the OHT incidence was 23 (12.43 ％) cases,and the ONT incidence was 131 (70.81％) cases.There were 16 (12.70％) patients with OH and 10(7.90％) patients with OHT in elderly group,but 15 (25.42％) patients with OH and 13 (22.03％) patients with ONT in very elderly group.The prevalence of OH and OHT in each elderly group was both higher than elderly group (P ＜ 0.05).Antihypertensive medications such as Calcium channel blockers,angiotensin Ⅱ receptor-blockers,Beta-blockers,diuretics,and angiotensin converting enzyme inhibitors were not significantly different among three subgroups.Conclusions OH and OHT were common in the elderly hypertensive inpatients,especially in the very elderly people.The five-class antihypertensive medication that doctors commonly used at present might be not related to the orthostatic hypotension and hypertension.

Objective To evaluate the efficacy and safety of Qijialifei capsule,a kind of Chinese traditional medicine,as adjuvant chemotherapy for retreatment patients with smear positive pulmonary tuberculosis.Methods One hundred and sixty retreatment cases with smear positive pulmonary tuberculosis were divided into observation group (80 cases) and control group (80 cases).All patients received the therapy of 3SHRZE/6HRE.The patients in observation group took Qijialifei capsule.Then comparisons were made between two groups in the sputum tubercle bacillus conversion rate,focus improvement rate,cavity closing rate,the cellular immune function,and clinical symptom improvement.Results In 160 cases,77 cases in observation group and 78 cases in control group completed the treatment.At the end of intensified therapy period and at the end of the whole therapy the sputum tubercle bacillus conversion rate [79.2％ (61/77),92.2％ (71/77)] in observation group were significantly higher than those in control group [64.1％ (50/78),78.2％ (61/78)](P ＜ 0.05).The focus improvement rate,cavity closing rate in observation group [74.0％(57/77),26.0％(20/77)] were significantly higher than those in control group [52.6％ (41/78),11.5％ (9/78)] (P ＜ 0.05).At the end of intensified therapy period,the focus improvement rate,cavity closing rate in observation group [92.2％ (71/77),54.5％ (42/77)] were significandy higher than those in control group [76.9％ (60/78),29.5％ (23/78)] (P ＜ 0.05).By the end of the whole therapy,the number of CD3+,CD4+,CD4+/CD8+ of patients in observation group were significantly higher than those in control group (P ＜ 0.05).The disappearance of clinical symptoms in observation group was shorter than that in control group(P ＜ 0.05).The rate of side effects on Qijialifei capsule was only 1.3％(1/77).Conclusions Qijialifei capsule can improve the cellular immune function of the patients,can effectively alleviate the symptoms and can improve the sputum tubercle bacillus conversion rate,radiographic focus improvement rate and cavity closing rate.It is safe.

Objective:To compare the effects of distal movement of canines between elastic traction and canine lace back in the aligning stage of preadjusted appliance. Methods:50 cases with Angle I malocclusion were divided into two groups after first premolar extraction. Patients in both groups were treated by the same kind of preadjusted appliance in the aligning stage. Canine lace back was used in one group and elastic traction in another.Treatment was countinued for 12 weeks.Measurements of the dental models and the cephalograms were conducted before and after treatment.Results:Compared to the canine lace back, the decrease of the width between the cuspids of upper premolars and that between that of lower molars, the increase of the angle of the distally tipped canines and the mesially tipped lower molars and the lingually tipped incisors, the higher speed and larger distance of distal moving canines were achieved more easy with elastic traction. Objective To evaluate the early effect of hypoxemia on central executive function of working memory.Methods 24 patients with obstructive sleep apnea syndrome (OSAS) and 20 healthy matching controls were studied in probe retrieval task.Results In control group,both the low and high conflict identification tasks elicited the N270 component with right prefrontal [F4,(2.2?2.7) ?V]and posterior [P3,(7.0?3.0) ?V; P4,(6.9 ? 3.1) ?V] distribution. N430 was elicited in high conflict condition with right hemispheric lateralization. Objective To study the expression of PTEN et al and their significance in T lymphoblastic lymphoma/leukaemia (T-LBL/ALL). Methods Seventy-six cases of T-LBL/ALL were studied by using immunohistochemistry (EnVision method) for CD3, CD7, CD10, CD20, CD23, CD43, CD45RO, CD99, TdT, MPO and PTEN. Follow-up was included. Results In the Seventy-six cases of T-LBL/ALL, the percentages of tumor cells expressing TdT, CD99, CD3, CD7, CD45RO and CD43 were 93.42%, 94.74%, 67.12%, 92.11%, 36.85%,and 51.33%, respectivly while MPO, CD20 and CD23 were all negative. The index of Ki-67 expression higher than 80 % was found in 27 and ≤80% in 49 cases. The expression of PTEN (64.47 %) in T-LBL/ALL was lower than that of in reactivated lymphoid tissue (100%, P<0.05). PTEN was reversely correlated with stage,Ki-67 expression and LDH level (P<0.05). CD3-positive cases were more than CD3-negative cases in abnormal LDH level (P<0.05). Follow-up data was obtained in 50 cases, ranging from 1 to 108 months. The overall survival rate was 35.8 %, with median survival time 330 days. No prognostic significance was found in the expression of PTEN, CD3, CD7, CD10, CD43, Ki-67 and CD45RO (P >0.05). Conclusion The antibodies of CD3,CD7, CD10, CD20, CD43, CD45RO, CD99, TdT, MPO and Ki-67 were very helpful for the diagnosis of T-LBL/ALL.Down-regulation of PTEN may play an important role on the development of T-LBL/ALL.
