2.Goals for the treatment of multiple myeloma in the era of novel agents

The treatment of multiple myeloma has become possible in the era of novel agents. Novel agents have dramatically improved the response rates of multiple myeloma (MM) and have extended the survival of patients even if MM remains an incurable disease. De-termining the treatment goals of MM has become a major challenge for clinicians in the era of novel agents. Many results show the correlation between response depth and patients' survival. Complete remission (CR), stringent CR, phenotypic CR, and CRs at the molecular and PET-CT levels are being explored and becoming the goals of MM treatment. Doctors should balance efficacy with adverse effects in daily clinical practice to avoid the blind pursuit of a high and deep response rate.
3.Effects of traditional Chinese medicine for invigorating spleen to resolve dampness and dredging collaterals on patients with rheumatoid arthritis and anemia

OBJECTIVE: To observe the clinical effects of Xinfeng Capsules (XFC), a traditional Chinese medicine for invigorating the spleen to resolve dampness and dredging collaterals, on patients with rheumatoid arthritis (RA) and anemia, and to investigate its mechanism. METHODS: Forty patients with RA and anemia were divided into three groups: XFC-treated group (n=20), Tripterygium glycosides Tablets (TGT)-treated group (n=10) and methotrexate (MTX)-treated group (n=10). The patients in each group took corresponding medicine for three months. The response rates of the three groups were evaluated after treatment. The general symptoms and specific signs and symptoms of RA were observed before and after the treatment. The indexes of blood routine examination and some other laboratory indexes such as erythrocyte sedimentation rate (ESR), and blood levels of rheumatoid factor (RF), iron, C-reaction protein (CRP), immunoglobins (Ig), complements 3 and 4, tumor necrosis factor-alpha (TNF-alpha), interleukin-10 (IL-10) and erythropoietin (EPO) were all examined and compared before and after treatment. RESULTS: The total response rate in the XFC-treated group was similar to those both in the TGT-treated and MTX-treated groups. The effects on relieving specific symptoms of RA in the three groups were also similar. The ESR and serum levels of RF, CRP, IgG, IgA and IgM were all decreased after treatment as compared with those before treatment in the three groups, and there were no significant differences among those laboratory indexes in the three groups after treatment. The XFC displayed more obvious effects on improving the general symptoms of patients with RA and anemia, increasing the blood levels of hemoglobin, iron and IL-10, while decreasing the serum level of TNF-alpha and regulating the serum EPO level, as compared with those in the TGT-treated and MTX-treated groups. CONCLUSION: The XFC for invigorating the spleen to resolve dampness and dredging collaterals was developed on the basic principle of regulating spleen. It can obviously improve the symptoms and laboratory indicators of RA. Such effects may be related to maintaining the balance of cytokines, regulating the serum level of EPO and increasing the serum iron level in patients with RA and anemia.
4.Ventricular Endoscopy for Obstructive Hydrocephalus: Report of 33 Cases

Objective To discuss the techniques of ventricular endoscopy for obstructive hydrocephalus.Methods A total of 33 patients with obstructive hydrocephalus were treated by ventriculostomy using ventricular endoscopy from June 2003 to December 2006.The patients had received CT,MRI,and 99mTc-DTPA cistern SPECT before the surgery.Results Endoscopic third ventriculostomy was performed successfully in 32 patients,the other patient died of hemorrhage under the third ventricle fistula.Postoperative fever occurred in 13 patients.One patient developed bilateral chronic subdural hematoma 6 months after the operation,and was cured by drill and drainage.Three patients developed subdural hydroma 6 months postoperation without showing symptoms.They were monitored without any treatment.The 32 patients were followed up for 2-42 months(mean,16.7 months),all of them achieved clinical relieve.In the patients,the intracranial pressure(ICP) was 26(16-39) cm H2O before the operation,and was significantly decreased to 22(12-28) cm H2O 7 days after the operation(t=2.903,0.005

6.Correlation Study Between Serum Soluble ST2 Level and the Early Clinical Prognosis in Patients With Acute ST-elevation Myocardial Infarction

Objective: To preliminarily investigate the relationship between the baseline level of serum soluble ST2 (sST2) and 30-day MACE occurrence rate in patients with ST-elevation myocardial infarction (STEMI).
Methods: A total of 121 patients with confirmed diagnosis of STEMI in our hospital from 2015-05-01 to 2015-07-30 were consecutively enrolled. According to baseline sST2 level, the patients were divided into 2 groups:Low sST2 group, the patients with sST2≤56.68 ng/ml, n=61 and High sST2 group, the patients with sST2>56.68 ng/ml, n=60. Clinical condition and 30-day MACE (defined as death and new onset of congestive heart failure) occurrence rate were compared between 2 groups.
Results: ① The systolic blood pressure (SBP), Killip class≥II grade, blood levels of cTNI, NT-proBNP, hs-CRP and LVEF were different between 2 groups, all P<0.05. ② Baseline sST2 level was positively related to heart rate (r=0.271, P=0.003), Hs-CRP (r=0.359, P=0.000), cTNI (r=0.324, P=0.000) and NT-proBNP (r=0.425, P=0.000);negatively related to SBP (r=-0.226, P=0.013) and LVEF (r=-0.406, P=0.000).③30-day MACE occurrence rate was different between 2 groups (8.2%vs 30%, P=0.002). ④ Multivariate Cox regression analysis presented that sST2>56.68 ng/ml was the risk factor for 30-day MACE occurrence (HR=1.152, 95%CI 1.078-1.231, P=0.000).
Conclusion: Increased baseline level of sST2 implied the higher incidence of death and new onset of congestive heart failure in STEMI patients.
7.Impact of overweight and obesity on flow mediated dilation in adult

Objective To explore the impact of overweight and obesity on flow mediated dilation (FMD) in adult people.Methods Intact clinical data of 183 healthy people, aged 30~65 in our hosipital were analyzed.According to BMI, all people were divided into obesity group (48 cases), overweight group(75 cases) and normal group (60 cases).FMD was detected by E tracing of ultrasonic.Weight, WC, BFP, baPWV, BP,FPG,CHO,HDL-C,LDL-C,TG and UC were observed at the same time.FMD of three groups were compared and the risk factors of FMD were analyzed.Results The level of FMD in the obesity group and overweight group was significantly higher than that in the normal group[(5.63±2.76)%,(6.45±2.54)% vs.(8.31±3.04)%](F=13.77,P<0.001);there was no statistically significant difference between the obesity group and overweight group (t=1.68,P>0.05).Age(β=0.078,95%CI: 1.004,1.164),BMI(β=0.465,95%CI 1.148,2.208),LDL-C(β=2.294,95%CI: 9.918,1.612) and UC(β=0.007,95%CI 0.986,1.000) were independent risk factors of decreased FMD(P<0.05).Conclusion Age, BMI, LDL-C and UC are independent risk factors of decreased FMD.

10.Protection of the superior petrosal vein in microneurosurgery for acoustic neuroma

Objective:To explore the clinical signiifcance of the protection of superior petrosal vein (SPV) in the microneurosurgery for acoustic neuroma. Methods:From January 2009 to July 2011, 149 cases of acoustic neuroma microsurgery were observed. hTe difference in hematoma in surgical area, cerebellar hematoma and cerebellar edema were compared between a SPV without protection group (SPVWP group, n=8) and a SPV protection group (SPVP group, n=141). Results:In the 149 patients with acoustic neuroma, the SPV was reserved in 141 patients. In the SPVWP group (8 patients), hematoma in the surgery area occurred in 4 patients, cerebellar edema in 5, and cerebellar hemorrhage in 3. In the SPVP group (141 patients), hematoma in the surgery area occurred in 40 patients, cerebellar edema in 56, and cerebellar hemorrhage in 12. hTere was signiifcant difference in the incidence of cerebellar hemorrhage (χ2=3.84, P=0.05), no signiifcant difference in the incidence of hematoma in the surgical area (χ2=0.646, respectively, P=0.422), and no significant difference in the incidence of cerebellar edema (χ2=0.611, P=0.434) between the SPVWP group and the SPVP group. Conclusion:In acoustic neuroma surgery, the SPV should be protected, which may reduce the risk of cerebellar hemorrhage.
