3.Three-dimensional Finite Element Analysis of Biomechanical Effect of Rigid Fixation and Elastic Fixation on Lumbar Interbody Fusion.

This study was aimed to compare the mechanical characteristics under different physiological load conditions with three-dimensional finite element model of rigid fixation and elastic fixation in the lumbar. We observed the stress distribution characteristics of a sample of healthy male volunteer modeling under vertical, flexion and extension torque situation. The outcomes showed that there existed 4-6 times pressure on the connecting rod of rigid fixation compared with the elastic fixations under different loads, and the stress peak and area of force on elastic fixation were much higher than that of the rigid fixations. The elastic fixation has more biomechanical advantages than rigid fixation in promoting interbody lumbar fusion after surgery.

4.Simultaneous determination of six constituents in Qingre Zhili Pills by HPLC

AIM To develop an HPLC method for the simultaneous content determination of berberine hydrochloride,jatrorrhizine hydrochloride,palmatine hydrochloride,kaempferol,luteolin and apigenin in Qingre Zhili Pills (Berberidis Radix and Portulacae Herba).METHODS The analysis of methanol extract of this drug was performed on a 35 ℃ thermostatic Agilent TC-C18 column (4.6 mm × 250 mm,5 μm),with the mobile phase comprising of acetonitrile-0.02% potassium dihydrogen phosphate flowing at 0.9 mL/min in a gradient elution manner,and the detection wavelengths were set at 265 nm and 360 nm.RESULTS Six constituents showed good linear relationships within their own ranges (r≥0.999 3),whose average recoveries were 97.16%-99.90% with the RSDs of 0.62%-1.48%.CONCLUSION This simple method can be used for the rapid quality control of Qingre Zhili Pills.
5.Expression of microRNA in peripheral blood of patients with pancreatic cancer and its clinicopathological significance

Objective To measure plasma microRNAs dysregulated in patients with pancreatic cancer and to assess the potential of these miRNAs as biomarkers for pancreatic cancer.Methods Thirty-seven patients with pancreatic cancer who underwent pancreatic resection between June 2010 to July 2011 were enrolled in the Lihuili Hospital,and ten healthy volunteers were used as control in this study.The expression levels of miR-190,miR-196a,miR-221 and miR-222 were analyzed using quantitative real time polymerase chain reaction (qRT-PCR).U6 was used as an internal control.The relationships between clinicopathoiogic characteristics of pancreatic cancer and microRNA expression levels were analyzed.Results The relative abundances of plasma microRNAs were significantly higher in pancreatic cancer patients than in the control group.The highly expressed plasma miR-190,miR-196a,miR-221,miR-222 levels did not correlate with clinicopathologic characteristics of patients such as sex,age,tumor maximal diameter,and level of serum CA199.The plasma miR-196a levels showed a positive correlation with TNM stage in pancreatic cancer patients.Conclusions The plasma levels ofmiR-190,miR-196a,miR-221 and miR-222 were highly upregulated in pancreatic cancer patients.These microRNAs in plasma may provide a new method in the early diagnosis of pancreatic cancer.
6.Effect of hydrocortisone on outcome of patients with cardiac arrest and cardiopulmonary resuscitation

Objective To observe the effects of hydrocortisone on the rate of return of spontaneous circulation(ROSC)and the outcome of patients with cardiac arrest(CA)and cardiopulmonary resuscitation(CPR). Methods A cohort study was conducted,78 non-traumatic patients with CA were divided into hydrocortisone group (31 cases)and control group(47 cases). Conventional treatments were given in the two groups after admission,and additionally intravenous 100 mg hydrocortisone was given to the hydrocortisone group during resuscitation. The ROSC rate and prognosis were compared between the two groups. Multivariate logistic regression analysis was used to predict the impact factor of ROSC. Results The ROSC rate and 24-hour survival rate in the hydrocortisone group were significantly higher than those of the control group(ROSC rate:58.1% vs. 40.4%,24-hour survival rate:48.4%vs. 36.2%,both P<0.05). There were no significant differences between the hydrocortisone and the control groups in duration of CPR〔minute:17.1(6-45)vs. 15.8(7-48)〕,acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score in survivors(37.2±8.2 vs. 36.1±8.2),the survival rate(12.9% vs. 12.8%)and hospital discharge rate(6.4%vs. 6.4%)in 7 days. Hydrocortisone〔odds ratio(OR)=3.12,95%confidence interval(95%CI)-1.18-8.29, P=0.017〕and witness(OR=4.24, 95%CI -1.87-12.43,P=0.008) were independent predictors for an increased ROSC rate after multiple logistic regression analysis. Conclusion Giving hydrocortisone during resuscitation may increase ROSC rate in CA patients.

Objective To describe the clinical manifestation of the chronic atlantoaxial rotatory dislocation (AARD) with C1,2 lateral facets locked,and explore the effect of the operation combined with anterior retropharyngeal approach and posterior approach.Methods Data of 8 patients with chronic AARD with C1,2 lateral facet locked who had undergone open reduction with the anterior retropharyngeal approach and C 1,2 transpedicular screw fixation with autologous iliac bone graft from Oct.2010 to Jun.2013 were retrospectively analyzed.There were 4 males and 4 females with an average age of 31 years old (range,11-57 years old).The intervals from onset to diagnosis were from 29 to 180 days and the mean time was 70.6 days.5 cases were chronic AARD with right C1,2 lateral facets locked and 3 with left C1,2 lateral facets locked.Reduction was failed to obtain by traction for two to four weeks.Subsequently,after open reduction with the anterior retropharyngeal approach,the patients were performed C 1,2 transpedicular screw fixation with autologous iliac bone graft for one stage or two.Results 1 patient underwent open reduction with the anterior retropharyngeal approach in stage one and C1,2 transpedicular screw fixation in stage two because of an overall severe condition,and the other patients did anterior and posterior operation in one stage.The average operation time was 205 min (range,160-260 min).The mean blood loss was 210 ml (range,100-300 ml).There were no operation complications except one young girl reflected pain in autologous iliac donated area,and pain relieved when taking non-steroidal anti-inflammatory by oral for one week.All patients had been followed up for a mean period of 14.8 months (range,5-37 months).Three-dimensional computed tomography revealed C1,2 arthrodesis bone graft fusion from two to four months and the average was 3.1 months.Neither recurrence of symptoms nor dislocation was observed at the latest follow-up.Conclusion Open reduction through the anterior retropharyngeal approach with C1,2 transpedicular screw fixation is an effective treatment strategy for chronical AARD with C1,2 lateral facets locked,which reduces surgical complications with less operation difficulty.
10.Probability of premature death for four non-communicable diseases among residents at ages of 30 to 69 years in Xiacheng District from 2005 to 2020

Objective:
To investigate the mortality and probability of premature death due to cardio-cerebrovascular diseases, malignant tumors, diabetes and chronic respiratory diseases and their trends among residents at ages of 30 to 69 years in Xiacheng District, Hangzhou City from 2005 to 2020, so as to provide insights into the development of the chronic disease control strategy.
Methods:
The data regarding the mortality of cardio-cerebrovascular diseases, malignant tumors, diabetes and chronic respiratory diseases among residents at ages of 30 to 69 years in Xiacheng District, Hangzhou City from 2005 to 2020 were captured from the Zhejiang Provincial Chronic Diseases Surveillance Information Management System, and the crude mortality, standardized mortality by the sixth population census data in Zhejiang Province in 2010 and probability of premature death for these four chronic diseases were estimated. The trends in mortality and probability of premature death were analyzed in Hangzhou City from 2005 to 2020 using average annual percent change (AAPC).
Results:
A total of 6 436 deaths occurred due to cardio-cerebrovascular diseases, malignant tumors, diabetes and chronic respiratory diseases among residents at ages of 30 to 69 years in Xiacheng District, Hangzhou City from 2005 to 2020. The crude and standardized mortality rates were 183.33/105 and 152.88/105, which both appeared a tendency towards a decline from 2005 to 2020 (AAPC=-1.099% and -2.926%, both P<0.05), and the probability of premature death reduced from 10.40% in 2005 to 6.78% in 2020, which appeared a tendency towards a decline (AAPC=-2.827%, P<0.05). The probability of premature death due to malignant tumors, diabetes and cardio-cerebrovascular diseases all appeared a tendency towards a decline from 2005 to 2020 (AAPC=-2.681%, -6.571% and -2.286%, all P<0.05), while no significant changing trend was seen in the probability of premature death due to chronic respiratory diseases (AAPC=-4.263%, P>0.05). The probability of premature death for these four chronic diseases reduced from 13.55% in 2005 to 9.36% in 2020 among male residents (AAPC=-2.150%, P<0.05), and reduced from 7.18% in 2005 to 4.22% in 2020 among female residents (AAPC=-4.149%, P<0.05).
Conclusions
The mortality and probability of premature death due to chronic diseases appeared a tendency towards a decline in Xiacheng District, Hangzhou City from 2005 to 2020, and there was a significant decline in the probability of premature death for malignant tumors, cardio-cerebrovascular diseases and diabetes.
