1. Applicative value of CT angiography in emergent operation for ruptured intracranial aneurysm caused intracerebral hematoma

Objective: To investigate the applicative value of multislice spiral computed tomography angiography (MSCTA) in the microsurgery of ruptured intracranial aneurysm caused acute intracerebral hematoma. Methods: The imaging and clinical data of 15 patients with ruptured intracranial aneurysm caused intracerebral hematomas, whom underwent emergency surgery after preoperative CT angiography (CTA) were analyzed retrospectively. The surgical plan was determined according to the CTA imaging results. The hematomas were evacuated and the aneurysm were clipped. The applicative value of CTA were evaluated. Results: All the 15 patients were confirmed as intracranial aneurysms in the operation, which was essentially consistent with the preoperative diagnosis by CTA. The size and growth direction of the aneurysms were essentially consistent with the preoperative judgement, and all the aneurysms were clipped. The prognosis of the patients was assessed by the Glasgow Outcome Scale (GOS). All the patients were followed up by the outpatient department and/or telephone for 6 to 78 months (mean 24 months). The GOS score in 2 patients was 2, in 2 was 3, in 6 was 4, and in 5 was 5. Conclusion: Head CTA can briefly and quickly identify the ruptured intracranial aneurysm caused intracerebral hematoma and provide intuitive three-dimensional morphology and anatomical localization of the aneurysms. It is helpful to hematoma evacuation and clipping of the aneurysms in emergency operation.
2.Treating Elderly Coronary Heart Disease Patients by Different Approaches of Percutaneous Coronary Intervention: an Observation of Clinical Efficacy.

OBJECTIVETo observe thee efficacy of different ways of percutaneous coronary intervention (PCI) for treating elderly coronary heart disease (CHD) patients.
METHODSTotally 470 elderly CHD patients were classified to three age brackets (equal to or more than 85 years old, 60 to 74 years old, 75 to 84 years old). They were assigned to the transradial intervention (TRI) group (236 cases) and the transfemoral intervention (TFI) group (234 cases) according to different intervention pathways. Correlated indices and postoperative clinical efficacy were compared between the two groups.
RESULTSA higher successful rate of surgery was obviously got in patients 85 years old or older than 85 than in those 60 to 74 years old and 75 to 84 years old (P <0. 05). The incidence of major cardiovascular events (MACE) was reduced at post-operative 12 and 24 months in patients 85 years old or older than 85 (P <0. 05). The case number for changing intervention pathway were increased in the TRI group with statistical difference (P <0. 05). Compared with the TFI group, the case number for changing intervention pathway was increased; the time for arteriopuncture, the time for catheterization, and the time for X-ray exposure were prolonged; the time for postoperative bedding were obviously shortened; the incidence of vascular complications at the puncture site were lowered. The incidence of postoperative 12-month MACE was lowered, all with statistical difference (all P <0. 05). The incidence of MACE within postoperative 24-month MACE decreased in patients 60 to 74 years old and 75 to 84 years old (P <0. 05). The incidence of MACE within postoperative 24 months increased in patients 85 years old or older than 85 of the TRI group with statistical difference (P <0. 05).
CONCLUSIONTRI can be preferably chosen for PC in treating elderly CHD patients.

3.Analysis on iron metabolism indicators in children with various genotypes of thalassemia

Objective To study the value of iron metabolism indicatorsin thalassemia .Methods 218 cases of thalassemia ,57 ca‐ses of iron deficiency anemia (IDA) children and 70 healthy children(control group) were enrolled in the study ,and serum iron (SI) ,total iron binding capacity(TIBC) ,transferrin(Tf) ,serum ferritin (SF) were detected for them .Results β‐thalassemia double heterozygous(β0 ) or homozygous group :SI levels were significantly higher than the other groups(P<0 .05) ,while TIBC ,Tf levels were significantly lower than the other groups(P<0 .05);α‐thalassemia group (static α‐thalassemia ,standard α‐thalassemia):SI , TIBC levels compared with control group showed no significant difference(P>0 .05) ,but Tf level was higher than control group(P<0 .05);hemoglobin H disease group:when TIBC ,Tf levels compared with the control group ,the difference was statistically signif‐icant(P<0 .05) ,when SI compared with the control group ,the difference was not statistically significant(P>0 .05) .Conclusion Compared with Tf ,SI and TIBC are better indicators for monitoring iron loading in children with thalassemia .The increased SI level and decreased TIBC level are two indicators for the diagnosis of β0 thalassemia in children with cellule anaemia .
4.Breast-Conserving Therapy of Early-Stage Breast Cancer(a Report of 31 Cases)

Objective To investigate the principle and method of breast-concerving therapy of early-stage breast cancer.Methods The clinical data of 31 cases of early-stage breast cancer who were conserved breast were analysed retrospectively from January 1992 to April 2002. 14 cases performed wide lumpectomy plus axillary nodes dissection,5 cases performed wide lumpectomy, 11 cases performed lumpectomy and 1 case did not perform operation. After operation 17 cases received radiotherapy and chemotherapy,8 cases received only radiotherapy,3 cases received only chemotherapy and 3 cases did not receive radio-chemotherapy.Results Surviving time of all patients was 6 to 122 months.Mean surviving time was 32?5 months.Median of survival time was 22 months.Among 31 patients 1 case died of metastasis due to refusing chemotherapy,the other were alive now and 10 cases of all patients have survival over 3 years. Local recurrence was found in 1 case after 5 years,but he did not take chemotherapy and radiotherapy. Fairly good cosmetic outcome was obtained in 29 patients(93 5%).Conclusions It probably is ideal method that the wide lumpectomy plus axillary nodes dissection combines rationally with radiotherapy and chemotherapy in breast-conserving treatment of early- stage breast cancer.
5. Analyses of infection after liver transplantation in 113 patients

OBJECTIVE: To analyze the prevalence of bacterial infection and drug sensitivity after liver transplantation so as to provide reference to effective clinical therapy. METHODS: A retrospective survey of 113 patients who underwent orthotopic liver transplantation was carried out. RESULTS: The reported total incidence of bacterial infection after liver transplantation was 45.1%, while the verified incidence was 28.3%. The major bacterials were Klebsiella pneumoniae and Enterococcus faecom. Multidrug-resistant was observed. CONCLUSION: It is very important to prevent bacterial infection and use antibiotic rationaly. Selection of effective antibiotics according to etiologic data and resistance of bacterial can help prevent the occurance of drug resistant strains.

7.Empirical study on the interests claims of employees in public hospitals based on stakeholders

Objective To give an empirical study on the interests claims of employees in public hospitals. Methods By questionnaire investigation, to sum up data using factor analysis and pairedsamples T test, and to compare difference between different kinds of patients using ANOVA. Results The interests claims of employees can be summed up to seven factors and there is some significant difference between different kinds of employees. Conclusion At current stage, public hospitals need pay more attention to material interests claims of employees.
8.Relationship between serum matrix metalloproteinase-9 level and coronary heart disease in type 2 diabetic patients

Twenty-seven cases of simple type 2 diabetes mellitus,30 cases of coronary heart disease,and 32 cases of type 2 diabetes with coronary heart disease were enrolled in this study according to the results of coronary angiography.Meanwhile,32 healthy subjects were taken as a control group.The serum matrix metalloproteinase-9 (MMP-9) and other clinical and laboratory parameters were determined.The results showed that serum MMP-9 may play a minor role in the progression of coronary heart disease in type 2 diabetic patients.
9.Effect of Ultra-low Frequency Transcranial Magnetic Stimulation on Cerebral Blood Flow in Children with Cerebral Palsy

Objective To observe the effect of ultra-low frequency transcranial magnetic stimulation on cerebral blood flow in children with spastic diplegia cerebral palsy. Methods 36 cases of spastic diplegia cerebral palsy were divided into control group (n=21) and obsevation group (n=15), 19 healthy children were as normal group. The control group received conventional rehabilitation training. The obsevation group received the ultra-low frequency transcranial magnetic stimulation in addition. All the children received the check of transcranial color Doppler (TCD). Results Several arterials flow velocity was lower, several arterials pulsatility index and resistance index were higher in the obsevation group than in the normal group before treatment (P<0.05). Compared with the control group, the arterial flow of obsevation group improved, pulsatility index and resistance index decreased in some degrees in the obsevation group after treatment (P<0.05). Conclusion The cerebral blood flow is of low velocity and high resistance in spastic diplegia children, while ultra-low frequency magnetic stimulation could improve the condition of cerebral flow.
