Objective To study the effects of CBL teaching mode on treatment plan making process among orthodontic postgraduates. Methods Totally 42 junior and senior orthodontic post-graduates were randomly divided into 2 groups (21 per group)named as experimental group and test group. Each group was further divided in to 3 teams involved in CBL and traditional teaching. Six cases of the same type malocclusion were selected each time;after 4 teaching cycle,test scores of two groups were analyzed with SPSS 19.0. Results were shown as mean ± standard deviation and t test andχ2 test were applied to evaluate the rationality and accuracy of treatment plan made by students. Survey questionnaire were issued to teachers and students after class and the assessment results of CBL teaching mode were summed up. Results Test scores were (88.61±8.34)in experimental group and (76.13±8.59)in control group with significant differences (t=3.16,P=0.01). Pass rate of two groups were 96.12%and 87.51%with significant differences (χ2=4.76,P=0.02). During the first teaching program, accurate rates of experimental group and control group were respectively 31.00% and 36.70% and ra-tionality were 33.31%and 33.30%. During the second teaching program,accurate rates of experimen-tal group and control group were 68.80%and 45.70%and rationality were 62.72%and 43.37%. During the third teaching program,accurate rates of experimental group and control group were 89.70% and 51.20%,and rationality were 95.00% and 66.70%. During the fourth program,accurate rates of ex-perimental group and control group were respectively 97.80% and 55.30% and rationality were 98.30% and 75.60%. Conclusions CBL teaching mode is helpful to improve the students' ability of analyzing clinical cases and making more accurate and rational treatment plan,however the increasing teaching cost and the course load are also involved.

Objective To compare the difference of therapeutic effect of surgical operation and non-operation for acute damnification of elbow ulnar collateral ligament in rabbits.Methods A total of 81 New Zealand rabbits were randomly and equally divided into three groups(27 rabbits per group),ie,Group A(control group):the ulnar collateral ligaments of right elbow joint were only enclosed;Group B:the ulnar collateral ligaments of right elbow joint were severed and sutured;Group C:the ulnar collateral ligaments of right elbow joint were severed,without suture.The biomechanics was detected at time intervals of 3 and 6 weeks.Results Twelve weeks after transplantation,the displacements of the ligaments in Groups B and C were(6.06±0.4)mm and(7.72±0.44)mm,with statistical difference(P ＜ 0.05).The displacement of the ligaments in Group A was(5.87±0.46)mm,which was similar to that of Group B(P＞0.05).The maximum loads of the ligaments of Groups B and C were(68.23±5.64)N and (42.45±3.66)N respectively,with statistical difference(P＜0.05).The maximum load of the ligaments of Group A was(72.86±2.99)N,which was similar to that of Group B(P ＞ 0.05).The rigidities of the ligaments of Groups B and C were(11.33±1.52)N/mm and(5.52±0.67)N/mm respectively,with significant difference(P ＜ 0.05).The rigidities of the ligaments of Group A was(12.49±1.44)N/mm,which was similar to that of Group B(P ＞ 0.05).The power consumption of the ligaments of Groups B and C were(0.206±0.017)J and(0.163±0.013)J respectively,with statistical difference(P＜0.05).The power consumption of the ligaments of Group A was(0.213±0.010)J,which was similar to that of Group B(P＞0.05).Conclusion Surgical operation is more effective than the non-operation in dealing with acute damnification of the ulnar collateral ligaments of elbow ulnar collateral ligament in rabbits.

Objective To investigate the application of hepatitis C virus RNA and antibody detection method in population screening.Methods The colloidal gold rapid test method and the enzyme-linked immunosorbent assay (ELISA)were adopted to detect hepatitis C virus (HCV)antibodies,and the real-time quantitative PCR (RT-PCR)was adopted to detect HCV-RNA viral load.Results (1)Among 539 samples,266 cases were antibody negative and 263 cases were antibody positive.(2)Among 67 cases in the HCV-RNA viral load <103 IU/mL group,60 cases were HCV antibody positive by ELISA and 30 cases were HCV antibody positive by colloidal gold rapid test.Among 208 cases in the HCV-RNA viral load ≥ 103 IU/mL,199 cases were antibody positive by ELISA,but only 181cases were antibody positive by the colloidal gold rapid method.Other 6 cases of were 2 kinds of antibody negative had the HCV-RNA viral load ≥ 103 IU/mL.(3)208 cases of HCV-RNA viral load ≥ 103 IU/mL sample were divided in-to four groups.GGT,ALT and AST were statistically significantly different P <0.05),while ALB and S/CO values hadno statisti-cal difference (P >0.05).Conclusion In order to reduce the missed diagnosis rate and diagnose hepatitis C as early as possible,the above laboratory detection methods should be jointly applied and the comprehensive analysis should be conducted in population screening.

AIM:To study the effect of different surgeries for vision and postoperative complications in leprosy patients with lagophthalmos.METHODS:A retrospective analysis.Totally 68 leprosy patients (97 eyes) with lagophthalmos were selected who treated with different surgeries during May 2007 to September 2015 in our sanatorium.The surgeries included nylon thread correction surgery, tarsorrhaphy, temporalis transposition surgery.We observed and compared the best corrected visual acuity(BCVA) and the rate of postoperative complications, such as epiphora, lacrimal duct injury, hard to open eyes, infection at the inner and lateral canthus and scar formation of lacrimal apparatus.RESULTS:The effect on BCVA of patients treated with nylon thread correction surgery and tarsorrhaphy:the preoperative and postoperative BCVA was significantly different of the two groups (Z=-4.193,-4.213;P<0.05);the preoperative and postoperative BCVA was not significantly different of the patients underwent temporalis transposition surgery (Z=-1.532, P >0.05).The postoperative complications:(1) epiphora:there was 91% in patients underwent nylon thread correction surgery, 89% in patients underwent tarsorrhaphy, 59% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=12.198, P<0.05);(2) lacrimal duct injury:there was 66% in patients underwent nylon thread correction surgery, 95% in patients underwent tarsorrhaphy, 41% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=22.415, P<0.05);(3) hard to open eyes:there was 56% in patients underwent nylon thread correction surgery, 55% in patients underwent tarsorrhaphy, 22% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=8.795, P<0.05);(4) infections:there was 84% in patients underwent nylon thread correction surgery, 3% in patients underwent tarsorrhaphy, 11% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=60.858, P<0.05).CONCLUSION:There are certain clinical efficacy for lagophthalmos caused by leprosy after surgery, especially patients after temporalis transposition surgery can close eyes by oneself.And their vision does not change significantly, which suggests that this surgery may prevent exposure injury for cornea.But the iatrogenic injury, the vision decrease in patients underwent other surgeries and the ocular tissue damage, such as epiphora, lacrimal duct injury, hard to open eyes, infection at the inner and lateral canthus and scar formation of lacrimal apparatus should be pay more attention to.

Background As a newly developed refractive surgery,femtosecond laser in situ keratomileusis (LASIK) receives more and more attention.Changes in ocular stray light after femtosecond LASIK is an important problem that impacts visual quality.Some relevant research has been performed before,but the outcomes are conflicting.Objective This primary study was to investigate the change of stray light values before and after femtosecond LASIK and analyze the relevant factors underlying the change.Methods Clinical data from 109 eyes of 55 myopic patients who underwent femtosecond LASIK in Tianjin Eye Hospital from December,2010 through February,2011 were included in this study.Stray light values were measured and recorded by C-Quant stray light meter preoperatively,and 1 day,1 week,1 month and 6 months postoperatively.The stray light data were compared by repeated measures ANOVA,and the correlations among the stray light values with preoperative factors,such as age,equivalent sphere,pupil diameter and central corneal thickness(CCT),as well as with postoperative factors,such as residual bed thickness(RBT),RBT/CCT,ablation depth,ablation ratio,flap thickness,flap diameter flap base width and exiting energy,were analyzed statistically.Written informed consent was obtained from each individual at initial of this study.Results The stray light values were 0.90±0.19,1.10±0.19,1.02±0.18,0.96±0.16 and 0.94±0.15 pre-operation,and 1 day,1 week,1 month and 6 months after femtosecond LASIK.The stray light values increased significantly 1 day,1 week,and 1 month after surgery,showing significant differences among the various time points (F=17.699,P=0.000).The mean stray light value was higher in 1 week than that in 6 months after operation (t=2.412,P=0.017).No significant differences were found in the stray light values between 6 months and 1 month or pre-operation (t =0.779,P =0.437 ; t =-1.877,P =0.062).No significant correlations were seen between preoperative and postoperative stray light values with age,preoperative refractive diopter,pupil diameter or CCT (P＞0.05).The changes in stray light values 1 week,1 month and 6 months after surgery were negatively correlated with ablation depth(r=-0.226,-0.228,-0.241 ;P＜0.05) and ablation ratio(r =-0.149,-0.219,-0.255 ;P＜0.05).Conclusions Stray light values increase within 1 month and return to normal 6 months after femtosecond LASIK.Stray light values may be related to multiple setting laser parameters and factors during femtosecond LASIK.

Objective This study aims to evaluate the initial results of a hybrid procedure for treating descending thoracic aortic disease that involves distal aortic arch.It also intends to report our initial experience in performing this procedure.Methods A total of 45 patients(35 males and 10 females) with descending thoracic aortic disease underwent a hybrid procedure,namely,thoracic endovascular aortic repair(TEVAR) combined with supra-arch branch vessel bypass,in our center from April 2009 to August 2014.Right axillary artery to left axillary artery bypass(n =20) or right axillary artery to left common carotid artery and left axillary artery bypass(n =25) were performed.The conditions of all patients were followed up from the 14th month to the 77th month postoperative[mean(38.0 ± 17.1) months].Mortality within 30 days,complications such as endoleak after the hybrid procedure,and stenosis or blockage of the bypass graft during the follow-up period were assessed.Results One case of death and one case of cerebral infarction were reported within 30 days.Two patients underwent open surgery beacuse of endoleak.And a newly formed intimal tear was observed in one patient and the patient underwent a second TEVAR during the follow-up period.Condusion Initial results suggest that the one-stage hybrid procedure is a suitable therapeutic option for thoracic aortic pathologies that involve distal aortic arch.However,this procedure is not recommended for type-B aortic dissection,in which a tear is located in the greater curvature or near the left subclavian artery,because of the high possibility of endoleak occurrence.

The glutamate dehydrogenase (EC.1.4.1.4) gene which amplified from the genome of Brevibacterium flavum GDK-9 by polymerase chain reaction was linked with pUCm-T for sequence alignment. Analysis of gdh sequences revealed that the whole sequence is 1927 bp, only one ORF existed, which used ATG as the initiation codon and coded a peptide of 448 amino acids with a calculated molecular weight of 48 kD. The comparability between the cloned gdh sequence to the reported sequence is high to 99.55%. Only the 1190th base mutation (C→A) lead to the change of amino acid sequence (Thr→Asn), the others are not. The recombinant plasmid pXG was then transformed into E. coli XL-Blue and Brevibacterium flavum GDK-9 which was induced by IPTG. SDS-PAGE analysis revealed that there was a clear induced protein band with molecular mass of 48.7 kD on expected position. Standard glutamate fermentations indicated that although the level of GDH increases the intracellular glutamate pool, the level of GDH has no influence on glutamate secretion.

Objective To detect the change of cerebral perfusion in pediatric patients with Leigh's syndrome (LS)by using MR perfusion technique.Methods Twelve patients with Leigh's syndrome and thirteen normal children were scanned with the sequence of flow-sensitive alternating inversion recovery exempting separate T_1 measurement (FAIREST).Their relative cerebral blood flow(rCBF)values were obtained in regions of bilateral basilar nuclei and bilateral thalami.Student t-test was used to compare them between the two groups and receiver operating characteristic(ROC)curve analysis was carried out.Results Statistical analysis revealed significant difference between two groups in the regions of bilateral basilar nuclei and right thalamus(t =3.26,P =0.002;t =2.25 ,P =0.018 ;t =2.88 ,P =0.004,respectively).The rCBF values for LS group and control group were 0.432?0.158 and 0.619?0.125 for right basilar nuclear, 0.478?0.186 and 0.621?0.123 for left basilar nuclear,0.630?0.189 and 0.833?0.160 for right thalamus,respectively.The areas under the ROC curves were 0.833 and 0.756 for the rCBF of right and left basilar nuclear,respectively.Conclusion Relative CBF maps may reveal changes of cerebral blood flow in some specific brain regions in patients with Leigh's syndrome.It can provide additional information to the clinicians in the evaluation of the disease.

Objective To evaluate the clinical application of embolization therapy using N-butyl 2-cyanoacrylate(NBCA)for hepatocellular carcinoma(HCC)complicated with arterio-portal fistula(APF).Methods Eighty patients with HCC and APF underwent embolothempy with NBCA(NBCA group,26 patients)or absolute alcohol(alcohol group,54 patients).The APF wag first treated with liquid embolic agent during the TACE procedure.We used NBCA-Lipiodol mixtures in concentration of 20%-50% according to tlle different circulation times of these APF in NBCA group,Absolute alcohol plus gelfoam or other materisis were used for embolization of APF in alcohol group.The pain reaction during the procedure,influence for liver function(X2 test).occlusive suceessfxil rate after single embolization(Fisher's exact test)and the survival rate of 1 year(log-rank analysis)between two groups were compared after the embolotherapy.Results Four patients in NBCA group and 52 patients in alcohol group felt painful during the pmcedures.There wag statistically significant difference between two groups(x2=58.86,P＜0.001).The APF disappeared after only sin~e embolization in 24(92.3%,24/26)cages of NBCA group and in 37(68.5%,37/54)patients of alcohol group.There wag also significant difference between two groups (P=0.024).There were no statistical ditierence in the post.embolization liver function change and 1 year survival rate between two groups.Conclusions The embolization therapy using NBCA for HCC complicated with APF is safe.effective and more accurate.It can be used as a new technique for these patients.
