1.Effect of povidone iodine irrigation combined with levofloxacin eye drops on inflammatory indexes, NO and antioxidant indexes in serum and tear fluid of patients with diabetic cataract

Objective To investigate the effect of povidone iodine irrigation combined with levofloxacin eye drops on the inflammatory indexes, NO and antioxidant indices in serum and tear fluid of patients with diabetic cataract.Methods 50 cases of pati ents with diabetic cataract and admitted into the hospital during January 2013 to January 2015 were selected as the research objects all of whom received cataract surgery.According to the randomized double blind method, the patients were divided into two groups with 20 cases in each group.The control group received levofloxacin eye drops intervention before surgery while the observation group were treated withpovidone iodine irrigation, based on the control group.The inflammatory indexes (IL-2, sIL-2R, IL-6), NO, antioxidant indexes (TAC, SOD, CAT) and condition of postoperative infection were compared between the two groups before and after irrigation.ResuIts In the observation group, the sIL-2R, IL-6 and NO levels in serum and tear fluid after irrigation were significantly lower than those in the control group while IL-2, TAC, SOD and CAT were significantly higher than those in the control group.The differences were statistically significant (P<0.05).The postoperative infection rate of the observation group was 0.0% which was significantly lower than 16.0% of the control group.The difference was statistically significant ( P<0.05 ) .ConcIusion Povidone iodine irrigation combined with levofloxacin eye drops can significantly reduce the inflammatory reactions in patients with diabetic cataract and improve their antioxidant level.

Objective To review the causes of exudative retinal detachment(ERD),indications of vitrectomy for ERD and its outcomes.Design Retrospective case series.Participants 57 patients with ERD received vitrectomy in EENT Hospital,Fudan Uni- versity.Methods A retrospective analysis of clinical records were conducted.Main Outcome Measures Etiology,vitrectomy indica- tions and its outcomes.Results 57 cases(62 eyes)with ERD received vitrectomy.The disease distribution of vitrectomy were Coats dis- ease in 23 cases(45.2%),familial exudative vitreoretinopathy in 8 cases(12.9%),uveitis in 7 cases(11.3%),retinal hemangioma in 7 cases(11.3%),bullous retinal detachment in 6 cases(9.7%),endophthalmitis in 4 cases(6.5%)and pit of optic nerve in 2 cases(3.2%). Severe ERD and proliferative change were major indication for treatment with vitrectomy.During follow-up period,most patients gained useful vision in early stage.Follow up rate of 6 months or more was 64.9%.The recurrence of ERD was 5.4% and most patients also gained useful vision.Condusion Coats disease is the prominent cause of being treated with vitrectomy in ERD.When proliferation or vitreous hemorrhage happens or macula is involved,vitrectomy should be performed.Most patients can gain useful vision after vitrecto- my.(Ophthalmol CHN,2008,17:52-55)

6.Clinical Significance of the Special Signs of MRCP in Carcinoma of Pancreatic Head

Objective To research special signs of MRCP,its pathological and clinical significance in carcinoma of pancreatic head.Methods 35 patients with pancreatic head carcinoma confirmed by operation and /or pathology were examined by MR before operation,including SE sequences T_1WI,FSE sequences T_2WI,T_2WI FS and MRCP.Results The special signs of MRCP in pancreatic head cancer were showed in all cases(100%),these signs included:type C1+P1 in 31 cases,type C2+P1 in 2 cases and type C2+P2 in 2 cases.The volumes of pancreatic cancer shown by MRCP were bigger than ones in operated.Conclusion MRCP features of carcinoma of pancreatic head are special.
8. Effect of ferulate-heparin-poloxamer thermosensitive hydrogels on nerve regeneration in rats with spinal cord injury

Aim To investigate the effect of ferulate-heparin-poloxamer (SF-HP) thermosensitive hydrogels on nerve regeneration in ratswith spinal cord injury, and provide experimental basis for clinical application. Methods SD rats were randomly divided into five groups: sham group, SCI group, SCI + HP group, SCI + SF group and SCI + SF-HP group. Rats were performed moderate contusion injuries using a vascular clip for 2 min to establish SCI animal model, then rats were given BBB score and inclined plate scoring function test after SCI. BDA tracing was employed to observe the recovery of nerve conduction function. Moreover, immunohistochemical staining and Western blot-were used to measure GAP43, Nestin and GFAP levels. Results BBB score and inclined plane test score significantly increased in SF-HP treated group as compared with the model group (P < 0. 01). Staining data showed that the structure of spinal cord was void, and this phenomenon was reversed after SF-HP administration. Data showed that the level of GAP43 and Nestin increased after SF treatment (P < 0. 05), the expression of GFAP decreased (P <0. 05), and the treatment effect of SF-HP hydrogels were better than those of SF alone (P < 0. 0 5) . BDA tracing data showed that the number of positive neurons markedly increased and the repair of nerve conduction function was significantly improved in SF-HP hydrogel group compared with SF group. Conclusion HP hydrogels enhance the effect of SF on the nerve regeneration in damaged spinal cord in rats.

Objective To compare the manipulation of two different methods of percutaneous dilatational tracheostomy (PDT),the guidewire dilating forceps (Portex method) and the modified onestep dilation technique (Ciaglia Blue Rhino method,CBR method).Methods From March 2013 to February 2014,48 patients eligible to meet the criteria of tracheostomy in ICU were collected and were divided into two groups at random.The length of time consumed for operation,amount of blood lost during operation and the operation complications were compared between two groups.The participant surgeons were asked which method they preferred,Portex or CBR.Results Of them,the mean age was 76.7 ± 11.6 years old,and 25 patients (52.1%) were male.The reasons for tracheotomy were the length of time needed for mechanical ventilation support expected to be longer than 2 weeks (n =30),severe disturbance of consciousness (n =17),and upper airway obstruction (n =1).The patients enrolled in the study were randomly divided into Portex and CBR groups.There were no significant differences in age,gender,APACHE Ⅱ score,the reason of tracheotomy,the length of time for mechanical ventilation support before tracheotomy,and the preoperative coagulation function between two groups.CBR method can easily be done with less operation time required than Portex [(5.9 ±4.3) min vs.(9.9 ± 1.5) min,P <0.01],and had obviously fewer operation complications and less amount of blood lost than Portex method (P < 0.05).The male surgeons with a slew of surgical practice were in preference to Portex,whereas female doctors with less work experience and without surgical practice preferred CBR.Conclusions The CBR method can easier be done and was better than the Portex method in minimizing operative trauma and reducing the complications,especially it can easily be accepted by female doctors with less work experience and without surgical practice and it is worth using widely in clinic.
