1.Clinical value of serum tumor marker in early diagnosis ovarian cancer

To evaluate the value of serum human epididymis protein 4(HE4),cancer embryonic antigen(CEA), carbohydrate antigen(CA125,CA19-9,CA153),alpha fetoprotein(AFP) and β-human chorionic gonadotrophin (β-HCG) in ovarian cancer early diagnosis.Methods:Abbott Architect i2000SR was used to detect the levels of preoperative serum tumor markers (n=7) in ovarian cancer group,benign ovarian tumor group and the normal control (each n=60).Results:The levels of serum HE4,CA125, CA199,CEA,CA153,AFP and β-HCG in ovarian cancer group were markedly higher than the benign ovarian tumor group and the control group.There was statistically significant difference ( P<0.05 ).The diagnosis of ovarian cancer tumor markers were high specificity,but sensitivity was not high except HE4 (76.67%) and CA125 (68.33%).With Youden Index,the area under curve of ROC,the HE4 was demonstrated higher specificity ,sensitivity and accuracy.There was also medium diagnostic value of CA 125 ,CA199 and CEA for ovarian cancer.The sensitivity of ovarian cancer diagnosis is raised by combined assay with serum tumor marker ,but the specificity would be reduced accordingly.HE4, CA125, CA199 and CEA combined detection is considered the best.Conclusion:CA153,AFP and β-HCG have lowered diagnosis for ovarian cancer.HE4, CA125, CA199 and CEA combined detection could significantly improve specificity ,sensitivity and accuracy in the early diagnosis of ovarian cancer.
2.Hair follicle cell mixture forms hair follicle-like structures after subcutaneous implantation in nude mice.

OBJECTIVETo observe the hair follicle regeneration after subcutaneous implantation of hair follicle cell mixture in nude mice.
METHODSThe hair papilla cells, dermal sheath cells, outer root sheath and fibroblasts of human scalp were mixed with the hair follicle epithelial cells and implanted subcutaneously in nude mice to observe the regeneration of the hair follicle.
RESULTS AND CONCLUSIONFormation of intact hair follicle-like structures was observed in the skin sections of the recipient nude mice, suggesting the feasibility of this approach for hair follicle regeneration in vivo.

3.Diagnostic value of prostate imaging reporting and data system version 1 and 2 in detection of prostate cancer in transition zone

Objective To evaluate the diagnostic value of prostate imaging reporting and data system version 1 (PI-RADS V1) and version 2 (PI-RADS V2) for detection of prostate cancer (PCa) in the transition zone (TZ).Methods Seventy-seven patients with suspicious lesions in TZ on mpMRI were scored according to the PI-RADS system (V1 and V2) before MR-TRUS fusion guided biopsy prospectively.In all of the patients with suspicious tumors,respectively at least one lesion with a PI-RADS V1 assessment category of ≥3,was selected for biopsy.Independent sample t test was used to compare scores of PI-RADS V1 and V2 between PCa and benign prostatic hyperplasia (BPH).The diagnostic performance of PI-RADS V 1 and V2 for detection of PCa in the transition zone was compared by analyzing ROC basing on the results of MR-TRUS fusion guided biopsy.Results A cohort of 77 patients was performed including 31 cases of PCa (32 cores) and 46 cases of BPH (51 cores).PCa (V1:1 1.50±2.79;V2:4.28±0.99) had significantly higher scores of both PI-RADS V1 and PI-RADS V2 than BPH(V1:7.51± 1.63;V2∶2.61 ±0.67) (P<0.05).Using a PI-RADS V1 score cut-off ≥ 11,sensitivity and specificity in group PCa and BPH were calculated,which were 68.8%(22/32) and 96.1%(49/51) with a area under curve of 0.869;using a PI-RADS V2 score cut-off ≥4,which were 75.0% (24/32) and 90.2% (46/51) with a area under curve of 0.888,respectively.Conclusions PI-RADS system can indicate the likelihood of PCa of suspicious lesions in TZ on Mp-MRI.PI-RADS V2 perform better than V 1 for the assessment of prostate cancer in TZ.
4.Clinical efficacy and safety of docetaxel, oxaliplatin and S -1 neoadjuvant chemotherapy in the treatment of upper gastric cancer

Objective To evaluate the clinical efficacy and safety of docetaxel, oxaliplatin and S -1 neoadjuvant chemotherapy in the treat-ment of upper gastric cancer.Methods Nine cases with locally advanced upper gastric cancer recruited in this study.After recruited, the patients were given docetaxel 40 mg · m-2 through intravenous drip infusion in one hour on day 1 and 8, with anti-anaphylactic treatment, oxaliplatin 75 mg· m-2 through intravenous drip infusion in two hours on day 1, and S-1 40 to 60 mg po, twice a day on day 1 to day 14.All of the included patients received 2 cycles with 21 days percycle.After 2 cycles of neoadjuvant chemotherapy, the data of clinical efficacy and toxicity were evaluated.Results All of subjects finished the scheduled chemotherapy regiment.One case reached complete response ( CR, 11.1%) , 6 cases reached partial response ( PR, 66.7%) , and 1 patients had progress disease ( PD, 11.1%).The total response rate was 88.9%.Only 1 case of progress disease didn′t receive the surgery.And the 8 patients received surgery also received 4 cycles of post -surgery chemotherapy with the same regiment.Until October, all the 8 patients were alive without relapse or metastases.The general adverse reactions for neoadjuvant chemotherapy were myelosuppression, gastrointestinal toxicity and peripheral neuritis. No chemotherapy-related death was found in this study.Conclusion Docetaxel, oxaliplatin and S -1 neoadjuvant chemotherapy in the treatment of upper gastric cancer can improve the surgery rate and prognosis.
5.Effect of dopamine and metaraminol on the renal function of patients with septic shock.

BACKGROUNDVasoactive drugs are often necessary for reversing hypotension in patients with severe infection. The standard for evaluating effects of vasoactive drugs should not only be based on the increase of arterial blood pressure, but also on the blood flow perfusion of internal organs. The effects of dopamine and metaraminol on the renal function of the patients with septic shock were investigated retrospectively in this study.
METHODSNinety-eight patients with septic shock were divided into three groups according to the highest infusing rate of metaraminol, with the lightest infusing rate of (0.1 - 0.5, 0.6 - 1.0, > 1.0) microgxkg(-1)xmin(-1) in group A, B and C respectively. Urine output, mean arterial blood pressure (MAP), heart rate (HR), urine output, blood urea nitrogen (BUN), creatinine (CRE), urine albumin (U-ALB), urine beta(2)-microglubulin (Ubeta(2)-MG) and Apache III scores were recorded.
RESULTSBefore antishock therapy, hypotension, tachycardia and oliguria occurred to all the 98 patients with septic shock and CRE, BUN, U-ALB, Ubeta(2)-MG and Apache III scoring were abnormal in most cases. With the antishock therapy, MAP, HR, urine output, BUN and CRE in all patients returned gradually to normal (P < 0.05 or < 0.01 compared to those before antishock therapy). U-ALB, Ubeta(2)-MG output and Apache III scoring also reverted but remained abnormal (P < 0.01 compared to those before antishock therapy). No statistically significant differences in the changes of these indices with the time existed among the three groups (P > 0.05).
CONCLUSIONDopamine and metaraminol when applied to the patients with septic shock could effectively maintain the circulatory stability and promote restoration of renal function.

6.Design implementation and of ECG monitoring and management system based on smartphone platform

Objective To develop an ECG monitoring and management system based on the smartphone platform to transmit ECG data to the doctor in daily life or working environment.Methods The system was composed of an ECG signal acquisition device and a smartphone App.The ECG signal acquisition device used MSP430F149 microcontroller as the core unit,which executed ECG signal extraction by adopting right leg drive,active filters,dual instrumentation amplifier and moving average algorithm,applied differential threshold method to accurately detecting R wave,calculated heart rate and then sent ECG data to the smartphone through Bluetooth 4.0 module.The smartphone App was developed based on cross-platform mobile development framework Cordova,and used Cordova plugins,JavaScript,HTML5 and CSS to receive the data from the acquisition device,draw and store ECG,show heart rate and remind abnormalities in heart rate as well as to transmit ECG data to the doctor through the mobile medicine App and social App.Then the doctor provided advices on prevention,treatment and etc after analyzing the received data.Results Compared with the ECG machine,the system had the relative errors of heart rate,P duration,QRS duration and QT duration being 2.7%,4.5%,4.3% and 3.3% respectively.All the above four parameters having significant correlations,and the correlation parameters were 0.955,0.948,0.91 and 0.834 respectively (P<0.05).The system had 8-h endurance in case of being fully charged.Concluslon The system gains advantages in power consumption,accuracy,noise,cost,volume,weight,safety,reliability as well as easy operation and maintenance,and facilitates the ECG conditions to be mastered timely by the doctor.
7.The value of multiparametric MRI and transrectal ultrasound fusion guided biopsy in the detection of clinical significant prostate cancer

Objective:To evaluate the application of multiparametric MRI (mpMRI)-transrectal ultrasound (TRUS) fusion guided biopsy in the diagnosis of clinical significant prostate cancer (PCa).Methods:A prospective analysis was performed in 168 patients with suspected PCa from September 2015 to June 2017 in the First Affiliated Hospital of Soochow University. Suspicious areas on mpMRl were defined and graded using prostate imaging reporting and data system version 2 (PI-RADS V2) score. All the patients had the TRUS-guided systematic biopsy, 108 patients with PI-RAD V2 scores ≥ 3 had additional MRI-TRUS targeted biopsies. Taking pathologic results as golden standard, the detection rates were compared between the 2 methods using χ 2 test. Results:Initially, all of the 168 patients underwent TRUS biopsy. PCa was detected in 86 (101 niduses) of 168 patients (51.19%, 86/168), 82 (91 niduses) (48.81%, 82/168) were not prostate cancer. Seventy eight (46.43%, 78/168) cases of PCa were detected by TRUS biopsy, and 63 (58.33%, 63/168) cases of PCa were detected by MRI-TRUS fusion guided biopsy, the difference was statistically significant between TRUS biopsy and MRI-TRUS fusion guided biopsy (χ 2=3.73, P=0.035). The 168 patients were biopsied with a total of 2 300 cores, including TRUS biopsy 2 016 cores and MRI-TRUS fusion targeted biopsy 284 cores. Additionally, the detection rate for per cores for MRI-TRUS fusion targeted biopsy (51.76%, 147/284) was significantly higher than that for TRUS biopsy cores (19.64%, 396/2 016) (χ 2=142.38, P<0.05). Among patients with a positive biopsy for PCa, the biopsy cores for conventional TRUS biopsy was 1 032 comparing to 214 cores for MRI-TRUS biopsy. The suspicious MRI-TRUS fusion targeted biopsy (68.69%, 147/214) detected more PCa compared with TRUS biopsy (38.37%, 396/1 032) (χ 2=66.27, P<0.05). Among patients with a positive biopsy for PCa, MRI-TRUS fusion targeted biopsy [69.74% (106/152)] detected more significant cancer cores than TRUS biopsy [54.50% (351/644) ] (χ 2=11.67, P<0.05). Conclusion:MRI-TRUS fusion biopsy combined with PI-RADS V2 increases positive rate markedly and improves the detection rate of clinical significant PCa.
8.An investigation of vaccination in children with human immunodeficiency virus infection.

OBJECTIVE:
To investigate the status of vaccination in children with human immunodeficiency virus (HIV) infection.
METHODS:
A questionnaire survey was performed in 148 children in Hunan province, China who were registered in China's Acquired Immune Deficiency Syndrome Comprehensive Response Information Management System up to December 31, 2016 and were aged <15 years at the time of confirmed diagnosis of HIV infection. The information on vaccination, diagnosis of HIV infection, and diagnosis and treatment of related diseases was collected.
RESULTS:
Of the 148 children with HIV infection, there were 70 boys (47.3%) and 78 girls (52.7%); 140 children had an age of 3.8 (0.2-14.8) years at the time of confirmed diagnosis, and 8 children refused to answer this question. Mother-to-child transmission was found in 133 children (91.7%), blood transmission in 1 child (0.7%), and unknown in 14 children (9.5%). Of the 148 children, 129 (87.2%) received antiviral therapy and 19 (12.8%) did not receive such treatment. The vaccination rates of hepatitis B vaccine, bacille Calmette-Guérin vaccine, poliomyelitis live attenuated vaccine and diphtheria-pertussis-tetanus vaccine ranged from 70.9% to 77.7%, which was significantly lower than the national level (≥97%); the vaccination rates of the other vaccines in the National Immunization Program gradually decreased with age. No severe adverse effects were reported after vaccination.
CONCLUSIONS
Mother-to-child transmission is the main route of HIV infection in Chinese children. The diagnosis of children with HIV infection is significantly delayed, with low vaccination rates. Efforts should be made to strengthen early diagnosis, early treatment and vaccination in children with HIV infection, in order to improve their quality of life.

9.Craniotomy for posterior fossa and deep muscles incision and suture technique in suboccipital retrosigmoidal approach

Objective To probe the technical processes of the suboccipital retrosigmoidal craniotomy with replacement of bone flap and deep muscles incision and suture and its clinical application. Methods The data of 40 patients undergoing suboccipital craniotomy to the cerehello-pontine angle lesions, deep muscles incision and suture in our hospital from May, 2003 to May, 2005 were analyzed retrospectively. Results All the procedures of craniotomy including removal of the bone flap, deep muscles incision and suture were accomplished safely with an average operation time of 40-60 min. Two patients appeared post-operative subcutaneous hydrops without CSF leakage and the majority can move their head freely sixth day after the operation. Conclusion Suboccipital retrosigmoidal craniotomy with replacement of bone flap, and deep muscles incision and suture, being a safe and feasible method, may decrease the happening of postoperative CSF leakage and encephalocele, and accelerate the recover of head movement.
10.Logistic analysis on risk factors related to smear positive pulmonary tuberculosis.

OBJECTIVETo explore the risk factors on relapsing tuberculosis related to smear positive pulmonary tuberculosis which had been cured for five years.
METHODSPatients with smear positive pulmonary tuberculosis registered in 1995 from ten countries in Hubei province were studied and logistic regression was used for data analysis.
RESULTSThe 5-year relapse rate of smear positive pulmonary tuberculosis was 3.85 percent. Risk factors related to relapse would include being non-modeled county, negative smear after treated for three months, the class of retreatment, management of non-DOTS, method of chemotherapy and patients that did not get treated by the tuberculosis institute, with odds ratios of 0.15, 4.62, 3.68, 5.88 and 6.47, respectively.
CONCLUSIONEffect standard, regulation DOTS and the centralized management measure might have had effects on decreasing the relapse rate.
