1.Investigation in knowledge rate of 120 first aid and satisfaction rate with emergency services among Nanchong citizens

Objective To investigate in knowledge rate of 120 first aid and satisfaction rate with emergency services among Nanchong citizens. Methods 360 patients who received prehospital first aid from three hospitals and their accompanies were selected from January to June 2010. The investigation content included knowledge rate of 120 first aid and satisfaction rate with emergency services. Results Results showed that about half of those surveyed still did not know the emergency phone"120" and its function, lacked self-help and self-protection knowledge, some respondents felt quite helpless upon daily life of electric shock, gas poisoning, coma and traumatic accidents. The attitude of the majority of emergency personnel was good or better, but still very stiff or irresponsible attitude existed in those few. This undermined the "120" and the image of credibility and emergency personnel. Conclusions There is an urgent need for universal coverage of on-site first aid knowledge and enhance awareness of first aid. Full-time staff in emergency department must pay close attention to the knowledge of first aid training, mastering the technique of bleeding control and bandaging, fixing, handling and CPR, as well as the use of instruments and equipment equipped in vehicles. Only in this way can we continuously improve the treatment of critically ill patients and increase survival rate.
2.Observation of arterial compliance in patients with abnormal blood glucose metabolism

In this study, the effects of blood glucose and blood pressure on the compliance of large and small arteries were investigated. The results showed that arterial compliances of both large and small arteries were decreased in patients with diabetes mellitus. In non-diabetic patients with well controlled blood pressure, the compliance of small arteries was markedly improved. These results suggested that both blood glucose and blood pressure affected arterial compliance.
3.Effects of CBL mode on making treatment plan among orthodontic postgraduates

Objective To study the effects of CBL teaching mode on treatment plan making process among orthodontic postgraduates. Methods Totally 42 junior and senior orthodontic post-graduates were randomly divided into 2 groups (21 per group)named as experimental group and test group. Each group was further divided in to 3 teams involved in CBL and traditional teaching. Six cases of the same type malocclusion were selected each time;after 4 teaching cycle,test scores of two groups were analyzed with SPSS 19.0. Results were shown as mean ± standard deviation and t test andχ2 test were applied to evaluate the rationality and accuracy of treatment plan made by students. Survey questionnaire were issued to teachers and students after class and the assessment results of CBL teaching mode were summed up. Results Test scores were (88.61±8.34)in experimental group and (76.13±8.59)in control group with significant differences (t=3.16,P=0.01). Pass rate of two groups were 96.12%and 87.51%with significant differences (χ2=4.76,P=0.02). During the first teaching program, accurate rates of experimental group and control group were respectively 31.00% and 36.70% and ra-tionality were 33.31%and 33.30%. During the second teaching program,accurate rates of experimen-tal group and control group were 68.80%and 45.70%and rationality were 62.72%and 43.37%. During the third teaching program,accurate rates of experimental group and control group were 89.70% and 51.20%,and rationality were 95.00% and 66.70%. During the fourth program,accurate rates of ex-perimental group and control group were respectively 97.80% and 55.30% and rationality were 98.30% and 75.60%. Conclusions CBL teaching mode is helpful to improve the students' ability of analyzing clinical cases and making more accurate and rational treatment plan,however the increasing teaching cost and the course load are also involved.
4.Analysis of common gene mutations and cytogenetics in core binding factor related acute myeloid leukemia

Objective To assess the prevalence of c-Kit and FLT3 gene mutations in core binding factor related acute myeloid leukemia (CBF-AML) and analyze the karyotype characteristics of the CBF-AML patients. Methods Mutations of c-Kit, FLT3-ITD and FLT3-TKD were detected by genomic DNA PCR and sequencing, and the karyotype changes were analyzed in 48 newly diagnosed CBF-AML patients. Results c-Kit aberrations were detected in 13(27.1 %) out of 48 patients, including 5 cases with exon 8 mutation and 8 cases with exon 17 mutation. c-Kit was more prominent in t(8;21) AML patients than in inv(16) AML patients [(33.3 %(9/27) vs 19.0 %(4/21), P<0.05]. Only 1 case (2.1 %) had FLT3-ITD mutation (FLT3-ITD+) and 3 cases (6.3 %) had FLT3-TKD mutation (FLT3-TKD +). Prevalence of RUNX1-RUNX1T1 with additional chromosome abnormality was as high as 25.9 %(7/27), in which sex chromosome elimination was the most common one, while prevalence of CBFβ-MYH11 with additional chromosome abnormality was low. Conclusion c-Kit gene mutations and RUNX1-RUNX1T1 additional chromosome abnormalities are common in patients with CBF-AML and would be helpful for individualized treatment studies.
5.Nosocomial Invasive Fungal Infection:Clinical Features and Analysis of Pathogens

OBJECTIVE To investigate the current status,the clinical features and the pathogens of invasive fungal infections in hospital in order to provide clinical treatment based on identification and susceptibility test.METHODS The fungus-cultured positive cases among the discharged patients from Jan 2004 to Nov 2006,were analyzed according to their definite diagnosis of invasive fungal infections under the items,such as the patients age,underlying disease,sample,strain,and species distribution.RESULTS The rates of invasive fungal infections were 4.26%.There were 2221 fungus strains belonged to 8 species in all samples;the patients age was 7-96 years with 2 kinds of various underlying diseases;the age of 2221 cases was 60 years old,mainly senile patients with various diseases accounted for 68.29%.Lower respiratory tract was the most frequent infection site.The main pathogens of invasive fungal infections were Candida spp(93.38%).Strains of Candida albicans were the most frequent organism isolated accounted for 66.19% of all the isolates.C.glabrata,C.krusei and C.tropicalis accounted for 9.19%,8.10% and 4.50%,respectively,the others accounted for only 6.32%.The main infected sites were lower respirtory tract,urinary tract and digestive tract.CONCLUSIONS Candida spp are still the main pathogens of invasive fungal infections.The epidemiological properties of invasive fungal infections is changed.The incidence of non-C.albicans and the Aspergillus strains that arouse invasive infections is increasing recently.
6.Effect of magnetic labeled endothelial progenitor cell transplantation on renal function of diabetic rats:a MRI imaging verification

BACKGROUND:Endothelial progenitor cel s have been shown to play an important role in the pathogenesis of traumatic diseases in recent years. OBJECTIVE:To explore the effect of magnetic labeled endothelial progenitor cel transplantation on renal function of diabetic rats through a MRI imaging study.METHODS:Sixty Wistar rats were randomly divided into normal (no treatment), control and experimental groups. Intraperitoneal injection of 40 mg/kg streptozotocin was performed to make a rat model of type 1 diabetes in the control and experimental groups. Four weeks after modeling, rats in the experimental group were given intravenous injection of magnetic labeled endothelial progenitor cel s (0.15 mL, 1×109/L). Fasting blood glucose, serum insulin, serum creatinine, urea nitrogen and 24-hour urinary protein levels in rats were measured at 8 weeks after cel transplantation. MRI was used to trace transplanted cel s in vivo in comparison with renal biopsy findings, and rat body mass and kidney weight were measured to calculate kidney weight index. RESULTS AND CONCLUSION:After modeling, fasting blood glucose, serum creatinine, urea nitrogen and 24-hour urinary protein levels as wel as kidney weight index were increased significantly (P<0.05), while the insulin level decreased (P<0.05). Compared with the model group, the endothelial progenitor cel transplantation reversed these indices (P<0.05). Additional y, in the experimental group, there was slightly longer T1 and shorter T2 signals as wel as marked lesion edge, and the FLASH sequence became more remarkable compared with the T2-weighted RARE sequence. The other groups showed no significant low signal changes. Magnetic-labeled positive cel s in the experimental group showed by the MRI were consistent with the tissue biopsy results, while no positive cel s were found in the model and normal groups. To conclude, the magnetic labeled endothelial progenitor cel transplantation can improve renal dysfunction in diabetic rats to a certain extent.
7.Implement the major hospital indexes examination using clinical pathway as an opportunity

Objective To explore the method of the major hospital indexes examination using clinical pathway as an opportunity.Methods Based on the material of our hospital from 2008 to 2010,reference the data of the same level hospital in Xinjiang Uygur Autonomous Region,we set the benchmark and formulated the examination standard of three examination indexes:average length of stay (ALOS),average hospitalization expense and drug proportion.Meanwhile,we took the subentry incentives to give the corresponding reward to clinical departments according to the decreases rate compared with the benchmark monthly.Results ALOS,average hospitalization expense and drug proportion of department and the whole hospital were obviously decreased compared with the same period last year,and the differences were statistically significant (Medical system:t =27.479,14.462,11.362,all P < 0.01 ; Surgical system:t =18.944,16.029,12.071,all P < 0.01).Conclusion After using the examination methods and combining clinical pathway,the effects are obvious,which can effectively shorten ALOS,reduce average hospitalization expense and drug proportion,so as to relieve the patients' economic burden.
8.Value of hysteroscopy and laparoscopy in differential diagnosis of gestational trophoblastic neoplasia

Objective To determine the efficacy of hysteroscopy and laparoscopy in differential diagnosis of pregnancy-related diseases,including gestational trophoblastic neoplasia(GTN),incomplete abortion and ectopic pregnancy.Methods Twenty-seven patients with a suspected diagnosis of GTN were transferred to Peking Union Medical College Hospital from September 2003 to March 2006,and underwent hysteroscopy and laparoseopy.Clinical data of patients were reviewed retrospectively.Most patients had abnormal vaginal bleeding and persistently elevated plasma beta human chorionic gonadotropin(?-hCG) level for a median(53?37)days(range,15-125 days)after evacuation.Ultrasound revealed a lesion with affluent blood flow in intrauterine,unilateral horn of uterus,or myometrium.No positive findings were revealed by computerized tomography or X-ray of the chest in all patients.Eleven patients underwent evacuation under hysteroscope,10 patients were diagnosed and treated by laparoscopy,and 6 by hysteruscopy and laparoseopy.Results Choriocarcinoma was diagnosed in 4 patients,who achieved complete remission by chemotherapy later.The diagnosis of GTN was ruled out in the other 23 patients, including cornual pregnancy in 12,pregnancy in rudimentary horn in 1,and incomplete abortion in 10,who were cured by hysteroscopic and laparoscopic surgery and postoperative adjuvant single dose methotrexate.Conclusions The major causes of pregnancy-related abnormal bleeding include incomplete abortion,eetopic pregnancy,and GTN.Hysteroscopy and laparoseopy are effective alternative of diagnosis for differentiation of GTN from non-GTN and can also offer therapeutic treatment.
9.Clinical analysis of patients with lung metastasis of invasive mole before evacuation of hydatidiform mole

0.05).Compared with control group,significant decrease in positive group was found in the interval from first evacuation of HM to resolution of serum ?-hCG level,(83?18) days versus(126?31)days(P0.05).Conclusions Once HM is diagnosed,evacuation should be performed as soon as possible,the later the evacuation begins,the higher the risks of lung metastasis and chemotherapy are.It is not necessary to worry about lung metastasis before evacuation of HM,the outcome of post- chemotherapy is very good.
10.The clinical efficacy of early application of fluconazole combined sodium bicarbonate in oral fungal infections in elderly patients with AECOPD

Objective:To investigate the efficacy of the combined application of fluconazole and sodium bicarbonate in treatment of oral fungal infections among elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods:A total of 360 elderly patients with AECOPD treated in the Emergency Ward of our hospital during July 1, 2018 and December 31, 2019, were included for observation. After admission, the oral mucosal lesions were observed every day, and fungal smear and fungal culture were performed immediately once signs of fungal infection appeared. Meanwhile, fluconazole was given with 300 mg. oral. qd., followed by 150 mg. oral. qd., and 2.5% sodium bicarbonate was given three times a day as gargle.Results:The prevalence of oral fungal infection was 8.3% among which candida infection accounted for 83.2%. The mean treatment time was 5.0 ± 0.3 days. Daily observation of the oral cavity, early detection and application of fluconazole combined with sodium bicarbonate had significantly clinical effect in elderly AECOPD patients with oral fungal infection, with an effective rate of 16.7%, and a cure rate of 83.3%. No toxic side effects on liver and kidney function were found during the treatment. Obvious efficacy was found in relieving clinical symptoms, and there was no increase in hospitalization costs and time.Conclusion:Early combination of fluconazole and sodium bicarbonate has a significant clinical effect on control of oral fungal infection in elderly patients with AECOPD.
