2.Evaluation of coronary heart disease risks in patients with psoriasis

Objective To estimate the 10-year risk of coronary heart disease in patients with psoriasis,and to determine whether the risk profile is affected by disease duration,family history and clinical types of psoriasis.Methods Data were pooled from 322 inpatients with psoriasis and 297 inpatients with other skin diseases (151 cases of acute urticaria,33 drug eruption,113 herpes zoster) from December 2008 to December 2010 in the Department of Dermatology,West China Hospital of Sichuan University.Risks of coronary heart disease were estimated by using the Framingham risk score algorithm.Wilcoxon rank test,Kruskal-Wallis test and chi-square tests were used to evaluate the differences in risks and risk factors between the patients with psoriasis and controls.Results As far as the Framingham 10-year risk score was concerned,the difference was statistically significant between the patients with psoriasis and the control population (6% vs.4%,Z =4.342,P =0.000),as well as between patients with different clinical types of psoriasis (6% for psoriasis vulgaris vs.4% for pustular psoriasis vs.7% for psoriasis arthropathica vs.7% for erythroderma psoriaticum,H =8.484,P < 0.05),but was insignificant between patients with familial and sporadic psoriasis (6% vs.6%,Z =0.406,P > 0.05 ).No correlation was observed between the Framingham 10-year risk and psoriasis duration (P > 0.05 ).Conclusions Increased risks of coronary heart disease exist in patients with psoriasis compared with those with acute urticaria,drug eruption or hepers zoster,and in patients with psoriasis arthropathica or erythroderma psoriaticum compared with those with pustular psoriasis.The Framingham 10-year risk of coronary heart disease is uncorrelated with disease duration and unaffected by family history of psoriasis.
3.Treatment of Level 2 Hypertension by Diagnosis and Treatment Program of Integrative Medicine: a Multi-centre, Randomized Controlled Trial.

OBJECTIVETo explore the therapeutic effect of diagnosis and treatment program of integrative medicine (IM) on level 2 hypertension in the young and middle-aged patients and their ambulatory blood pressure.
METHODSA randomized, placebo parallel and controlled, multi-center clinical trial was performed. Totally 199 young and middle-aged level 2 hypertension patients were randomly assigned to the treatment group (99 cases) and the control group (100 cases). All received combined hypotensive treatment program by taking Nifedipine Sustained Release Tablet and Hydrochlorothiazide as basic drugs. Patients in the treatment group additionally took Western medicine (WM) combined Jiangyabao serial drugs (0.31 g per tablet, 2 tablets each time, twice daily), while those in the control group additionally took WM combined simulative agents of Jiangyabao serial drugs (0.31 g per tablet, 2 tablets each time, twice daily). The treatment course was 8 weeks for all, and 24-week follow-ups performed. 24 h ambulatory blood pressure and casual blood pressure, and their efficacies were compared between the two groups, and safety assessed as well.
RESULTSCompared with before treatment in the same group, daytime and night casual blood pressure, as well as 24 h ambulatory blood pressure were all obviously improved in the two groups (P < 0.01). Average diastolic and systolic blood pressures at night decreased more in the treatment group than in the control group with statistical difference (P < 0.05). There was no statistical difference in total efficacies of daytime casual blood pressure or ambulatory blood pressure (P > 0.05).
CONCLUSIONJiangyabao serial drugs combined WM in treating young and middle-aged level 2 hypertension patients showed obvious effect in improving night blood pressure, especially for night diastolic blood pressure.

5.Immunoregulation effect of TIGIT-Fc on macrophages

Objective:To prepare the production of TIGIT-Fc fusion protein using H22 cells stably integrated the gene by lentivirus vector , and to explore the immunoregulatory effect on macrophages by TIGIT-Fc.Methods: TIGIT-Fc fusion gene were constructed by molecular cloning.The fusion gene was then subcloned to plasmids contained the secretion signaling peptide .The secrected TIGIT-Fc fusion gene was inserted into the lentivirus backbone vector.The purified lentivirus vector was the used to infect the murine H22 cell line.TIGIT-Fc protein was purified by protein A column from the ascites of H 22-injected C57BL/6 mice.Macrophages stimulated by lipopolysaccharide ( LPS ) was challenged to TIGIT-Fc treatment or control.Cytokine levels was then detected by ELISA.Results: TIGIT-Fc protein was purified from the ascites of H 22-injected mice.PVR was upregulated in LPS-treated macrophages.IL-10 level was upregulated in TIGIT-Fc treated macrophages.Conclusion: TIGIT-Fc promotes the mature macrophages to secrete anti-inflammatory cytokine IL-10.
7.Molluscicidal experiment of endophytes from Pseudolarix kaempferi Gord

Objective To screen 18 endophytes from Pseudolarix kaempferi Gord for molluscicidal effect and identify them by morphology. Methods Molluscicidal tests were performed according to the immersion test suggested by WHO and the strain screened was identified by the slide culture. Results The mortality rates of snails immersed by JJ18 broth salified (pH=7) were 26.7%, 76.7% and 100.0% for 24,48 h and 72 h, respectively, and 53.3% and 86.7% in 5% and 10% concentrations of JJ18 broth, respectively. The active components were extracellular moiety of the broth which had no acute toxicity to fish, and JJ18 strain belonged to Aspergillus. Conclusion Extracellular moiety of endophyte JJ18 from Pseudolarix kaempferi Gord is a new resource of molluscicide.

Evidence-based medicine(EBM) emphasizes that the clinical practice should be based on the combination of the best available clinical evidence,medical experiences and patient desire to cope with the demand of medical decision-making.Guidelines for good clinical(research) practice,conduct of more trials as multicentre trials,and the cochran collaboration may all help apply the best research evidence to clinical practice.China is challenged by variety of health problems.The implementation of EBM will improve the decision-making and health care services and protect the doctors and patients rights.
9.Analysis of Nosocomial Infection Pathogens from Lower Respiratory Tract in Senile Patients in Different Season

OBJECTIVE To investigate the distribution and drug resistanc of nosocomial infection pathogens from lower respiratory tract in senile patients in different season.METHODS The sputum samples from lower respiratory tract infection in senile patients in two years,were collected to identify pathogens and drug sensitivity test j udged according to NCCLS standard.RESULTS The gram-negative bacilli accounted for 85.7%.The gram- posutive bacilli accounted for 14.3%.The predominant pathogens were Pseudomonas aeruginosa(20.3%),Kleb- siella pneumoniae(14.5%),Acinetobacter lwof fi(9.8%),coagulase-negative staphylococci(9.0%).The distribution and antibiotic-resistanc of main pathogens had different characteristics in different season. CONCLUSIONS Understanding the characteristics of local pathogen spectruma and the antibiotic-resistanc of main pathogens in different season were signincant on prevention and therapy of the lower respiratory tract infection in senile patients.
10.Nosocomial Infection Pathogens from Lower Respiratory Tract in Senile Deceased Patients

OBJECTIVE To investigate the distribution and drug resistance of nosocomial infection pathogens from lower respiratory tract in senile patients. METHODS The sputum and lower respiratory tract secretion in the senile patients with lower respiratory tract infection were collected nearly five years,and identified.The drug sensitivity test,the results of examination were judged according to NCCLS standard. RESULTS The Gram-negative bacilli accounted for 79.5%.The Gram-positive bacteria accounted for 20.5%.The predominant pathogens were Pseudomonas aeruginosa(19.6%),coagulase-negative Staphylococci(CNS)(18.7%),and acinetobacter lwoffi(11.6%).The resistant bacteria were markedly increasing. CONCLUSIONS The Gram-negative bacilli are the major pathogens in the senile deceased patients.The incidence of CNS infection is markedly increasing.The isolating rate of meticillin-resistant CNS is 100.0%.
