Objective To research the validity of PET/CT in clinic for detecting lung cancer. Method There were 20 subjects who may have lung cancer according to the result form other detecting way, including 15 males and 5 females with an average age from 54 to 86. Results Malignant lesions were found in 15 cases and benign lesions in 5 cases. Discussion The examination of chest with PET/CT is suit for early diagnosis of lung cancer. The number of patient of false postive and false negative is decreased by controlling some factor.
4.Control study in 0.45% sodium chloride solution and sterile water for injection in airway humidification of patients with mechanical ventilation

Objective To observe the two different wetting fluid in airway humidification of patients with mechanical ventilation.Methods 40 patients with mechanical ventilation after cardiac surgery in our hospital from January to April,2009,were divided randomly into group A and group B,group A was given 0.45% sodium chloride solution 100ml plus ambroxol 15mg as airway humidification fluid,group B was given sterile water for injection100 ml plus ambroxol 15mg as airway humidification liquid.The amount of sputum aspiration,color,viscosity,the body temperature of patients,the lungs auscultation and chest X-ray were observed.Results No statistical difference was seen in sputum volume,color,viscosity,the body temperature of patients,lungs auscultation as well as chest X ray after mechanical ventilation for 4 hours,8 hours,16 hours,24 hours,1~2 d,3~5 d,6~7 d.No statistical difference was also seen in auscultation of the lung after mechanical ventilation for 4 hours,16 hours,24 hours,1~2 d,3~5 d,6~7 d.But auscultation of the lung in group B was better than that of group A after mechanical ventilation for 8 hours.Conclusions No sufficient fact can prove that different effect exists between 100 ml 0.45% sodium chloride solution plus ambroxol 15mg and 100 ml sterile water for injection plus ambroxol 15mg as airway humidification fluid during mechanical ventilation.
5.An anatomical study and clinical application of free transfer of foot tissues pedicled with plantar metatarsal artery

Objective To explore a new method of functional reconstruction of hand digits and joints with free transfer of foot tissues so as to increase the success rate of the operation.Methods After micro-anatomic study of the plantar and dorsal metatarsal arteries,retrograde and free grafts of foot tissues pedicled with plantar metatarsal arteries were designed and applied in transplantation to treat 76 cases of hand digital or joint defects.The surgeries included 58 cases of transfer of the second toe,four cases of transfer of composite tissues of the second toe, eight cases of transfer of proximal interphalangeal joint,and six cases of nail flap transfer.Results The mi- cro-anatomic study found that the first plantar metatarsal artery was anatomically constant and the diameter of its branch to the second toe was larger than that of the first dorsal metatarsal artery.Flaps survived in 75 of the 76 patients(98.7%),with fine appearance and significantly improved function.One patient who had received free transfer of the second toe to reconstruct the thumb function had to undergo a second repair with infraclavicula skin tube because of refractory arteriospasm of anastomosed vessels.Conclusion Transfer with free retrograde grafts of foot tissues pedicled with plantar metatarsal artery to reconstruct hand functions can effectively improve the success rate of the operation,because it is free of the shortcomings of great anatomic variation of blood vessels and time-consuming and complex procedures in conventional transfer.
6.Clinical Observation of Zuo’s Yang-warming Acupuncture Therapy for Treatment of Allergic Rhinitis

Objective To observe the clinical efficacy of yang-warming acupuncture therapy established by Zuo Changbo for the treatment of allergic rhinitis. Methods Sixty patients with allergic rhinitis were randomly divided into observation group and control group, 30 cases in each group. The observation group was given Zuo’s yang-warming acupuncture therapy and the control group was given conventional acupuncture therapy. Acupuncture was performed once every other day, and 10 times constituted one course of treatment. After treatment, the therapeutic effect was evaluated, and the symptom scores of nasal itching, stuffy nose, sneezing and nasal discharge as well as their visual analog scale ( VAS) scores were observed. Results ( 1) Total effective rate of the observation group was 93.3%, and that of the control group was 63.3%, there was significant difference between the two groups ( P<0.01). ( 2) After treatment, the scores of the four symptoms were decreased obviously ( P<0.05 or P<0.01) , and the decrease in the observation group was superior to that in the control group ( P<0.01) . ( 3) After treatment, the VAS scores in both groups were decreased ( P<0.01) , and the decrease was more obvious in the observation group (P<0.01). Conclusion The curative effect of yang-warming acupuncture therapy on allergic rhinitis is better than that of the routine acupuncture group.
7.Major lung resection for the treatment of lung cancer in octogenarian patients

Objective To explore the clinical characteristics of bronchial carcinoma in elder patients and determine the operative indication, surgical technique, and management in the perioperative period. Methods 16 patients with bronchial carcinoma aged 80 and over received major pulmonary resection through muscle sparing mini-thoracotomy. The operative procedures were lobectomy(8 cases), bilobectomy (3 cases), sleeve lobectomy (2 cases), pneumonectomy (1 case), and lobectomy plus wedge resection (2 cases). Clinical data were analyzed retrospectively. Results Of the 16 patients, 6 with stage Ⅰ disease, 3 with stage Ⅱ and 7 with stage Ⅲdisease. There were no perioperative death occurred. The most popular postoperative complications were cardiac arrhythmia, pulmonary infection and hypoxemia. Conclusion Major pulmonary resection is a rational choice for part of the octogenarian patients with lung cancer.
8.3D-CT guided percutaneous radiofrequency ablation against advanced lung cancer: short term effect assessment

Objective Under the guidance of CT and three dimensional reconstruction, we made therapeutic plan for lung cancer by radiofrequency ablation (RFA) and observed its effect. Methods From February 2007 to January 2009, we used RITA radiofrequency therapeutic equipment to treat lung cancer under the guidance of 64-slice spiral CT (Siemens) and three dimensional reconstruction of the image. The target temperature is 90℃. Results We performed 29 RFA on 25 patients. In the follow-up, 10 of them showed tumor shrinkage by CT scan. 23 showed lack of tumor-uptake value by SPECT scan and 2 showed lower tumor-uptake value. Conclusion CT-guided percutaneous RFA is safe and practical for lung cancer. It has satisfactory short-term effect to reduce tumor burden.
9.Evaluation of Three-Dimensional Reconstruction CT in Percutaneous Radiofrequency Ablation (RFA) of the Unresectable Lung Tumor with a Clustered Electrode

Background and objective Radiofrequency ablation (RFA) of lung tumours has recently received much attention for the promising results achieved. Here, to evaluate the value of three-dimensional reconstruction CT in radiofrequen-cy ablation (RFA) of advanced non-small cell lung cancer. Methods Sixty-six cases of advanced non-small ceil lung cancer with 68 lesions (2 patients had 2 lesion treated in one session) were underwent three-dimensional reconstruction CT-giuded percuta-neous RFA therapy. To evaluate short-term therapeutic effect of lung tumors using spiral CT scanning in 1-3 months after RFA to investigate the alterations of minor size and density pre-and post-procedure, and complications, to observe the short-term cura-tive effect. Results Our experiences have shown an initial increase in lesion size at immediate follow-up CT. The density of 64 lesions was lowered (94.1%) and 4 lesion is increased (5.9%) at immediate and one month follow-up CT. SPECT scan findings that 82.4% (56/68) cases of FDG uptake in tumors after RFA with tumor/non-tumor of lower than 2.5 at one month follow-up. The change in treated lesion size over time, radiologically assessed through measurements of the lesions on axial CT scans in the lung window setting no lesions had complete response, 50 lesions (73.5%) had partial response, 2 lesions with stable disease, 6 lesions showed progressive disease at 3 month follow-up CT. SPECT scan findings that 79.4% (54/68) cases of FDG uptake in tumors after RFA with tumor/non-tumor of lower than 2.5 at 3 month follow-up. Conclusion The percutaneons RFA therapy under the guidance of three-dimensionsl reconstruction CT scan is safe and effective, with few complications, and can serve as a new method to the treatment of advanced non-small ceil lung cancers.
10.Modified Superior Mesenteric Artery Approach Totally Laparoscopic Radical Resection for Right Colon Cancer

Objective To explore the safety and feasibility of a modified superior mesenteric artery(SMA)approach in totally laparoscopic complete mesocolic excision(CME)and D3 lymphadenectomy for right colon cancer.Methods A retrospective analysis was performed on clinical data of 77 cases of totally laparoscopic radical surgery for right colon cancer from April 2021 to April 2023.Before August 2022,42 cases underwent traditional SMA approach(control group,only marking with the ileocolic vascular pedicle as the tail of SMA),while after August 2022,35 cases underwent modified SMA approach(modified group,marking with the Treitz's ligament and ileocolic vascular pedicle as the head and tail of SMA,respectively).There was no statistically significant difference in general information between the two groups(P>0.05).The intraoperative conditions,postoperative recovery,and postoperative complications were compared between the two groups.Results Compared with the control group,the modified group had a shorter surgical time[(147.3±35.8)min vs.(173.4±29.9)min,t =-3.428,P =0.001].There were no statistically significant differences in the number of lymph node dissection,number of positive lymph nodes,drainage volume,exhaust time,postoperative hospital stay,and incidence of complications between the two groups(P>0.05).Conclusion The modified SMA approach for totally laparoscopic radical resection of right colon cancer shortens the surgical time,reduces the difficulty and risk of surgery,and has high safety and feasibility.
