40 rabbits are used to study the effects of phaco-emulsification and extracap-sular lens removal or; corneal endothelial cells and corneal thickness.With the specular microscope, the preoperative cell morphology, cell density and thickness of cornea are revealed. The endothelial cell counts decrease 16% (at central) and 14% (at the periphery) after the surgery with phaco-emulsification and 25% (central) and 22% (periphery) with extracapsular lens removal. Corneal thickness increases in both groups. Scanning electron microscopy shows some alterations in morphology of endothelial cell injury significantly different in the 2 groups. According to these results the causes of corneal thickness increasing and mechanism of endothelial cell injury are approached and a new procedure preventing the cell injury and decreasing cornea edema in both groups is designed.

Dendritic cells (DCs)can capture HIV from the mucosal site and carry it to the draining lymph nodes to promote CD4 + T cell infection.DC-SIGN(CD209)plays a vital role during this process. Heroin,methamphetamine and cocaine are the most abused drugs used by drug abusers,which can increase the opportunities of HIV infection.Drugs acting on the corresponding receptors activate the downstream signaling pathway and regulate the expression of DC-SIGN on the surface of DCs,weakening the inhibited effects of immune cells on HIV and promoting HIV infection in many aspects.Given the multiplex effects of drugs on immune function,this article reviews the relationship between abused drugs,DCs and HIV infection,particularly in the signal pathway on immune modulation.

Objective To observe the therapeutic effect of Shenqi Fuzheng injection combined with EP chemotherapy in the treatment of non small cell lung cancer (NSCLC),and to improve the quality of life and side effects.Methods 60 cases of NSCLC were randomly divided into observation group and control group,30 cases in each group.The two groups were treated with EP,the observation group was treated with Shenqi Fuzheng injection for two courses.The life quality and adverse effects were observed.Results The quality of life of the two groups after treatment was compared,the difference was statistically significant (93.3% vs 70.0%,χ2 =2.83,P <0.05),the quality of life in the observation group was significantly higher than that in the control group.The white blood cell reduction,anemia and liver and kidney function damage between the two groups had statistically significant differences (P =0.011,0.025,0.016),the adverse reaction of the observation group was less than the control group.Conclusion Shenqi Fuzheng injection combined with EP regimen in the treatment of NSCLC can protect bone marrow hematopoietic function and improve the quality of life of patients with NSCLC chemotherapy.It can be used as one mean of Chinese and western treatment for patients with NSCLC in clinical application.

Objective To determine the effects of the early enteric nutrition (EEN) up to scratch on the outcomes of the critical care patients with different degrees of severity of illness. Method There were 192 critically ill patients eligible for enrollment for study during the past 18 months in our ICU. They were classified by using APACHE Ⅱ scores. The aim of this retrospective analysis of the early enteric nutrition was to see if the enteric nufore, the patients were divided into two groups: up to scratch group and not up to scratch group. According to APACHE Ⅱ scores, the patients of each group were further divided into three sub-groups in terms of scores below 15, between15 and 25, and above 25, respectively. Results There were 62 patients in the group of EEN up to scratch, and 130 patients' EEN did not up to scratch. When the scores of APACHE Ⅱ were below 15, the length of hospital stay (LOS) was significantly shorter in group of EEN up to scratch in comparison with that of EEN not up to scratch (t = 6.453, P = 0.000). When the scores of APACHE Ⅱ were between15 and 25, the LOS in ICU (t = 3.966, P = 0.000), in hospital (t = 8.165,P = 0.000), The cost of medical care (t = 4.812,P= 0.000) and the mortality (x2 = 5.421,P = 0.038) were all significantly less in patients with EEN up to scratch. However, when the scores of APACHE Ⅱ were above 25, only the cost of medical care ( t = 7.364, P = 0.000) was significantly lower in patients of EEN up to scratch than that of EEN not up to scratch. Conclusions The EEN up to scratch can significantly improve the outcomes of critical patients and the clinical value of EEN up to scratch depends on the severity of illness.

Objective To discuss the practice of nursing management model of stroke unit based upon the international standards implemented in the neurological unit.Methods The experiences of the stroke unit management were summarized and applied to instruct the clinical practice.Results After the implementation,the average hospitalization days,the proportion of drugs,medical cost,and mortality rate decreased.The satisfactory degree of patients with the stroke unit increased.Conclusions The nursing management model of stroke unit not only improved the quality of nursing service to stroke patients,but also decreases the mortality and disability rate,shortened the length of hospital stay,and enhanced the satisfaction degree of patients and their family members.

Objective To evaluate metamorphopsia and vision-related quality of life (VRQoL) and its influencing factor after vitrectomy for idiopathic macular hole (IMH).Methods This is a prospective and nonrandomized clinical cohort study.Thirty eyes of 30 IMH patients who received vitrectomy and inner limited membrane (ILM) peeling were included.The best-corrected visual acuity (BCVA),medical refraction test and optical coherence tomography (OCT) were performed.BCVA was recorded as logarithm of the minimum angle of resolution (logMAR).The macular hole index (MHI) was measured using OCT.The average logMAR BCVA and MHI in suffering eyes at baseline were 1.02± 0.07 and 0.47 ± 0.02,respectively.The uncorrected visual acuity of the fellow eyes was less than 0.1.Follow-up period was longer than 6 months.At 6 months after surgery,the central retinal thickness (CRT) was measured by OCT;vertical and horizontal metamorphopsia were measured by metamorphopsia charts;VRQoL was evaluated by Chinese VRQoL-25.Spearman correlation analysis was performed to analyze the relationship of VRQoL and postoperative BCVA,metamorphopsia and preoperative MHI.Results At 6 months after surgery,macular hole closure was confirmed by OCT in all patients.The vertical and horizontal metamorphopsia were (0.17± 0.03)° and (0.11 ± 0.03)°,respectively.The VRQoL-25 composite score was 79.81 ± 1.29.The average BCVA was 0.59±0.05.The average CRT was (155.10± 6.27) μm.The postoperative VRQoL was positive correlated with preoperative MHI (r=0.491,P=0.002),and negative correlated with preoperative BCVA (r=-0.445,P=0.014),postoperative BCVA (r=-0.530,P=0.003) and postoperative metamorphopsia (r=-0.532,P=0.006),but not correlated with the postoperative CRT (r=0.231,P＞0.05).Conclusions IMH patients improved their visual acuity after surgery,but still have metamorphopsia.VRQoL was negative correlated with metamorphopsia,positive correlated with preoperative MHI.

OBJECTIVE: To investigate the molecular mechanism of Huanglian Jiedu Decoction (HLJDD), a traditional Chinese compound herbal medicine, in treating type 2 diabetes mellitus (T2DM) by observing its effects on glucose transporter 4 (GLUT4) protein expression and translocation in adipose and skeletal muscle tissues of rats with T2DM. METHODS: T2DM was induced in rats by intravenous injection of a small dose of streptozotocin (STZ, 30 mg/kg) plus high fat and high caloric laboratory chow. Then animals were divided into untreated group, aspirin-treated group and HLJDD-treated group. Normal rats fed with common chow were designated as normal control group. Ten weeks later, the oral glucose tolerance test (OGTT) was performed in all animals, and the changes of murine body weight, fasting blood glucose (FBG), fasting serum insulin (FINS) and the expression of GLUT4 protein in skeletal muscle and adipose tissues before and after insulin treatment were routinely determined. RESULTS: Compared with the untreated group, the result of OGTT of HLJDD-treated group was improved. The levels of the body weight and FBG were decreased, while the GLUT4 protein expression and translocation were elevated obviously. CONCLUSION: It is suggested by the present results that the therapeutic effects of HLJDD on T2DM might be related to its ability of increasing GLUT4 protein expression and translocation in adipose and skeletal muscle tissues of T2DM rats.

BACKGROUND: The early symptoms of amyotrophic lateral sclerosis (ASL) are usually located at a certain part of the body, and are similar to the clinical manifestations of cervical spondylotic myelopathy (CSM). However, their treatment and prognosis differ greatly. Therefore, multiple-part electromyography and electroneurography possess an important value of reference, particularly electromyography of thoracic paraspinal muscles can be used as an objective parameter for differentiation between ASL and CSM.OBJECTIVE: To investigate the electrophysiological changes in ASL and CSM.DESIGN: A retrospective clinical analysis.SETTING: Neurological Department of Second Hospital Affiliated to Jiangxi Medical College.PARTICIPANTS: Totally 30 inpatients and outpatients with ASL and 30 patients with CSM were selected from the Neurological Department of the Second Hospital Affiliated to Jiangxi Medical College between December 2001 and November 2004.METHODS: Totally 30 inpatients and outpatients with ASL as well as 30 inpatients and outpatients with CSM were subjected to routine electromyographic and electroneurographic examination. Routine electromyography examining three limb muscles + sternocleidomastoid + thoracic paraspinal muscles intended to observe the spontaneous potential at resting stage, time limit and wave-amplitude of motor unit potential, and recruit-ment phase during energetic contraction. Electroneurography was used to detect motsr conduction velocity and sensory conduction velocity,as well as the terminal latency and wave-amplitude of motor evoked potential.limb muscles, sternocleidomastoid and thoracic paraspinal muscles in paASL and SCM.RESULTS: Data of all the 60 patients entered the final statistical analysis.in patients with ALS and CSM, and were more severe in the former. The abnormality rate of spontaneous potential of thoracic paraspinal muscles reached 93.3% (28/30) as compared to 3.3% (1/30) in patients with CSM (P ＜ 0.001). The average time limit and abnormal increment of wave amplitude of motor unit potential were significantly different in patients with conduction velocity: The decrease rate of motor evoked potential amplitude of complex muscles was obviously higher in ASL than in CSM (75.6% and 86.7%, ~=7.25, P ＜ 0.01). So was the decrease rate of motor conduction sory nerve conduction velocity: It was not affected either in ASL or CSM.ALS and CSM, but the abnormality rate of spontaneous potential of thorography indicates that nerve conduction velocity was not affected in both playing decreased conduction velocity and wave amplitude than those with CSM.
