1.The long term results of thoracolumbar burst fractures treated with short segment pedicle instrumentation

Objective To analyze the long term results of thoracolumbar burst fractures treated with short segment pedicle instrumentation. Methods Sixty eight cases of thoracolumbar burst fractures fixed with short segment pedicle instrumentation were followed up for at least 5 years (5-13 years, average 8 years). All of them were checked with radiography pre and post operation and before implant removal. At final follow up assessment, all were examined with radiography, 18 cases were checked with CT scan as well. Results 1) Neurological status improved at least 1 Frankel grade in 90.3% of the patients who had preoperative incomplete paraplegia, while no improvement was obtained in those who had preoperative complete paraplegia. Low back pain was evaluated according to Denis Pain Scale, the results showed P1 41 cases (60.3%), P2 24 cases (35.3%), P3 3 cases (4.4%). 2) The shortest distances between the upper and lower pedicle screws tips shortened for 0-6.5 mm (average 2.5 mm) before removal in contrast to that at post operative immediately. Of the 68 cases, nuts loosened in 1 case, pedicle screws loosened in 1 case, bent in 2 cases and broken in 5 cases. Of the 8 cases whose implants were not removed timely, screw broke in 2 cases, and seepage occurred through incision in 2 cases. 3) At final follow up, the correction of anterior vertebral body height averaged 30.5%, and Cobb angle 5.8?, despite a mean correction loss of Cobb angle 12.1?. Correction loss was most evident at the above disc spaces, then the below disc spaces. Correction loss of vertebral body height averaged 1.9% in the anterior, and 6.0% in the middle part. 4) The collapse of vertebral body was most serious in the middle part. The deformity of "codfish vertebrae" occurred in the superior part of vertebrae body in 21 cases, among them the upper adjacent vertebral body sank into the injured body in 5 cases, 3 of whom engendered kyphoses greater than 25?. Evident intra corporeal gaps were found in 16 of 18 patients who were checked with CT, and the gaps communicated with the above disc space. The fractured body could have a deformity of the "cup" after reduction. 5) Degeneration and narrowing of the disc spaces next to fractured vertebrae were very common, and the above space disappeared in 27 cases, while the lower space disappeared in 14 cases. The upper disc space adjacent to fixation levels degenerated in 1 of 8 cases whose implants were not removed timely. Five patients had Cobb angle greater than 20?, three of them had low back pain, and one had mild symptom of nerve compromise. Low back pain was not correlated with degeneration of discs. Conclusion Short segment pedicle instrumentation provides satisfying stability for thoracolumbar burst fractures. It helps physiologic postural contour restoration and facilitates neural recovery, but is associated with relatively high rates of implant failure and correction loss which is most evident at adjacent discs spaces. Deficiency of bone grafting and delayed implant removal are the most probable causes for correction loss.
2.Animal experimental study on incorporation of silk fibroin reducing the risk of cardiovascular complication during vertebroplasty

[Objective]To develop an animal pulmonary cement embolism model,by which the effect of polymethylmethacrylate(PMMA),calcium phosphate cement(CPC) and silk fibroin/calcium phosphate cement composite(SF/CPC) on the hemodynamic change,respiratory function and the antithrombin activity were measured after the pulmonary embolism caused by cement injection.The efficiency of SF reducing the risk of cardiovascular complication was evaluated.[Method]Twenty-four pigs were assigned to 3 groups randomly,with 8 pigs in each group.After general anesthesia,animals were endotracheal intubated and mechanically ventilated by a respirator.The pulmonary trunk was exposed via sternotomy and each kind of cement was injected into the main pulmonary artery.Hemodynamic parameters were continuously monitored.Blood samples were drawn at certain time points for blood gases analyzing and measurement of AT Ⅲ.Postmortem lungs were subject to computer tomography(CT) examination along with three-dimensional reconstructions of cement casts.[Result]The injection of CPC caused severe pulmonary hypertension and arterial hypotension,with a maximum elevation of 20.93?5.04 mmHg in PAP(20 min post-injection) and a maximum decrease of-26.59?6.8 mmHg in MABP(50 min post-injection).The changes in SF/CPC group were relatively slighter,which is similar with PMMA group,having a statistically significant difference from CPC groups(P
3.Simultaneous Determination of Resveratrol and Polydatin in Polygonum Cuspidatum by Quantitative Nuclear Magnetic Resonance Spectroscopy

A quantitative nuclear magnetic resonance spectroscopic ( qNMR) method was established for the simultaneous determination of resveratrol and polydatin in Polygonum Cuspidatum traditional Chinese herb cuts and granule. The 2_step ultrasonic extraction method using 80% alcohol and acetone was used for fully extracting these two components in samples before qNMR determination. The qNMR experimental conditions were investigated and deuterated dimethyl sulphoxide_deuterium oxide (10∶1, V/V) was selected as solvent, the pulse delay time was 5 s, the scan number was 32, 2,3,5_triiodobenzoate was used as internal standard which was calibrated with primary standard substance of potassium hydrogen phthalate. The 1 H_NMR peaks atδ6. 388-6. 391 ( d, 2H) of resveratrol and δ 6. 322-6. 330 ( t, 1H) of polydatin were chosen as the quantitative peaks. Method validation was performed in terms of precision ( RSD<0. 6%), linearity (correlative constants R2>0. 999), limit of detection (0. 23 g/L for resveratrol and 0. 24 g/L for polydatin) and limit of quantitation ( resveratrol 0. 69 g/L, polydatin 1. 57 g/L), recovery ( resveratrol 97. 7% -103 . 5%, RSD=2 . 4%, polydatin 94 . 5%-99 . 2%, RSD=1 . 6%, including the sample extraction and preparaton process) . The results showed the reliability of qNMR for traditional Chinese medicine assay. The resveratrol and polydatin in Polygonum Cuspidatum real cuts and granule samples were experimental determined as 3. 57-5. 69 mg/g and 12. 73-24. 07 mg/g, respectively.
4.?-Lactam Genes in Clinical Isolates of Pseudomonas aeruginosa

OBJECTIVE To investigate the genes associated with the drug-resistance to ?-lactam antibiotics in Pseudomonas aeruginosa(PAE) isolated from clinical patients in Huaibei,Anhui Province.METHODS Antibiotic sensitivity test was performed by VITEK-AMS.The three-dimensional method was taken to differentiate various beta-lactamases.The ?-lactamases and the outer membrane protein D2(oprD2) genes were detected by polymerase chain reaction(PCR) in 36 PAE strains.RESULTS In 36 PAE strains,the positive rates of genes of TEM,DHA,CTX-Ge and CARB were 77.8%,69.2%,27.8% and 25.0%,respectively,that of the OXA-1,OXA-10,VIM-2 and IMP-1 were 16.7%,16.7%,13.9% and 2.8%,respectively.The rate of lacking oprD2 gene was 58.3%,and none possessed VEB,SHV,GES,PER,GIM and SPM-2 genes.CONCLUSIONS P.aeruginosa carries various beta-lactamase genes in clinical patients of Huaibei,and the deletion ratio of oprD2 gene is higher.
5.Curative effect of long versus short Inter Tan intramedullary nailing of senior intertrochanteric hip fracture in conjunction with early total care

Objective To compare the clinical outcomes of long and short InterTan intramedullary nailing of senior intertrochanteric hip fracture in conjunction with early total care (ETC).Methods In this prospective study, records of 30 elder patients with intertrochanteric fracture hospitalized between January 2011 and September 2013 were included.Using the concept of ETC, the patients were treated with long (long-nail group, n =15) and short InterTan intramedullary nails (shortnail group, n =15) within 72 hours postinjury.The two groups were compared for operation time, intraoperative blood loss, fracture healing time, Harris hip score, time to pre-injury mobilization and hardware failure rate.Results Operation time was (77.25 ± 7.38) minutes in long-nail group versus (72.10 ± 6.90) minutes in short-nail group (P < 0.01).Time to pre-injury mobilization was (7.45 ± 1.61) months in long-nail group versus (8.57 ± 2.18) months in short-nail group (P < 0.05).There were no significant differences between long-and short-nail groups regarding the blood loss [(180.75 ± 38.26)ml vs (178.62 ± 34.79)ml], fracture healing time [(4.82 ± 1.12)months vs (4.76 ± 1.04) months], and Harris hip score [(83.75 ± 1.71) points vs (82.57 ± 2.18) points] (P > 0.05).Conclusions Either long or short InterTan intramedullary nailing combined with ETC is effective in treatment of intertrochanteric hip fracture in the elderly.However patients treated with long nails return to pre-injury mobilization relatively earlier.Long InterTan intramedullary nail system is better for the patients combined with serious osteoporosis.
6.The regulatory effect of miR-101 and miR-125a-5p on autophagy in lipopolysaccharide derived THP-1 macrophages

Objective To investigate the role of micro-RNAs (miR-101 and miR-125a-5p) in autophagy of lipopolysaccharide (LPS) derived human THP-1 macrophages.Methods Human monocytic leukemia cell line THP-1 was cultured in vitro,and it was differentiated into macrophages after being induced with phorbol (50 μg/L) for 48 hours.THP-1 macrophages were stimulated with LPS in 0,250,500,1 000 μg/L respectively for 12 hours,miR-mimic was transfected into THP-1 macrophages as induced by Lipofectamine RNAiMAX,and the transfection efficiency of miRNA was determined with fluorescence microscopy.Enzyme linked immunosorbent assay (ELISA) was used to determine the levels of tumor necrosis factor-α (TNF-α) and monocyte chemotaxis protein-1 (MCP-1) in the supernatants of culture.Western Blot was used to detect the protein expressions of autophagy proteins ATG4D,Beclin1,and LC3 Ⅱ.The expression levels of miR-101 and miR-125a-5p were determined by quantitative reverse transcription-quantitative polymerase chain reaction (RT-qPCR).Results ① The releasing levels of TNF-α and MCP-1 induced by LPS with 250,500,1 000 μg/L were significantly increased as compared the cells without LPS stimulation [TNF-α (ng/L):1 336.1 ± 18.5,1 340.6±24.8,1 364.5± 14.9 vs.47.6±4.4;MCP-1 (ng/L):996.3 ±51.3,934.6±84.3,974.2±69.5 vs.21.3±6.5,all P < 0.01],but no significant differences were found among the three LPS stimulation groups.The protein expressions of ATG4D,Beclin1 and LC3 1Ⅱ were up-regulated in the presence of different LPS concentrations (0,250,500,1 000 μg/L) for 12 hours in THP-1 macrophages (when compared with the cells without LPS stimulation,t value of ATG4D was 8.103,38.410,52.020,P value was 0.015,0.001,< 0.001;t value of Beclin1 was 3.026,5.328,3.482,P value was 0.047,0.034,0.037;t value of LC3 Ⅱ was 3.836,6.200,4.665,P value was 0.018,0.003,0.010),and the optimal concentration was 500 tg/L LPS.When THP-1 macrophages were stimulated with 500 μg/L LPS for 12 hours,the expression levels of miR-101 and miR-125a-5p were down-regulated significantly as compared with the cells without LPS stimulation [miR-101 (2-△ △Ct):0.68 ± 0.08 vs.1.95 ±0.26,t =8.047,P =0.001;miR-125a-5p (2-△ △Ct):0.23 ± 0.06 vs.1.69± 0.42,t =5.975,P =0.004].② The higher transfection efficiency was showed under fluorescence microscope.Westem Blot results showed the protein expressions of ATG4D,Beclin1 and LC3 Ⅱ were down-regulated as induced by an over-expression of miR-101 or miR-125a-5p in THP-1 macrophages,and more obviously down regulated by co-transfected with miR-101 and miR-125a-5p (compared with negative control group,t value was 14.550,5.855,14.180,P value was 0.005,0.014,< 0.001).Conclusion miR-101 and miR-125a-5p can inhibit the autophagy in LPS challenged THP-1 macrophages,and the potential mechanism might be related to target regulation of ATG4D.
7.Determination of Gallic Acid in Ninbitai Capsules by HPLC

Objective: To establish the determination method of gallic acid in Ninbitai Capsules.Methods: HPLC with Shimadzu Shim-pack VP-ODS(250mm?4.6mm,5??m) column was used. Methanol-water-dimethylformamide-acetic acid glacial (1∶81∶15∶3) was used as mobile phase. The flow rate was 0.5mL/min. The detection wavelength was at 272nm.Results: The linearity of this method was well. The recovery of the added sample was 98.16%. RSD was 1.18%.Conclusion: This method is convient with a good separating degree and can be used for the quality control of Ninbitai Capsules.
8.Genotyping of aminoglycoside-modifying enzymes in clinical isolates of aminoglycoside-resistant Pseudomonas aeruginosa

Objective To investigate the genotypes of aminoglycoside-modifying enzymes (AMEs) produced by Pseudomonas aeruginosa isolated from Huaibei Miner's General Hospital.Methods Minimum inhibitory concentration of 36 strains of P. aeruginosa to 3 aminoglycoside antibiotics was determined. AME genes were amplified by polymerase chain reaction (PCR). Results The P. aeruginosa isolates were resistant to multiple antibiotics. Most (62.2% to 81.1%) were resistant to aminoglycosides. And 27 strain (75.0%) carried one or more types of AME genes, including ant(3″)-Ⅰ (63.9%), aac(6')-Ⅱ (58.3%), aac(6')-Ⅰ (50.0%), aac(3)-Ⅱ (38.9%) and ant(2″)-Ⅰ (36.1%). The aac(3)-Ⅰ, aac(3)-Ⅲ, aac(3)-Ⅳ, aph(3')-Ⅳ genes were not identified.Conclusions The study indicates that the P. aeruginosa isolates in Huaibei are multi-resistant to aminoglycoside antibiotics. The prevalence of AMEs-positive strains is high.
9.Comparison of MRI manifestations and histopathologic findings of the elderly carotid arteries in ex vivo

ObjectiveTo evaluate the ability of high resolution magnetic resonance (MR) imaging at 3.0 tesla to depict the characterization of human carotid arterial vessel wall and detect atherosclerotic lesions ex vivo in comparison to histopathologic results. MethodsEighteen carotid arteries obtained from 9 elderly donors underwent fat-suppressed T1 - and T2-weighted MR imaging at 3.0 tesla MR system with a mouse coil. Corresponding histological sections were obtained for the comparison. Correlation between MR images and histopathologic slices was obtained by Pearson or Spearman correlation coefficient.Cohen K was computed to quantify the agreement between MRI and histopathologic findings.Results Lumen area,intima and media area measured on fat-suppressed T2-weighted images showed stronger correlation with the corresponding histopathologic slices [ MRI vs.histopathology:( 27. 53 ± 6. 77 ) mm2 vs.( 25.83 ±6. 69 ) mm2, r2 = 0. 91,P2 = 0. 024, ( 12.31 ± 3. 31 ) mm2 vs.( 12. 28 ± 3.71 ) mm2, r2 = 0. 70, P2 = 0. 020,Median 12. 29 mm2(Min 1.12 mm2, Max 33.18 mm2) vs.Median 11.62 mm2(Min 0.89 mm2, Max 32. 84 mm2 ), r2 = 0. 74, P2 = 0. 016, respectively]. The Cohen K score between the MR imaging and American Heart Association classifications was 0. 74, which corresponds to a good agreement. Conclusions 3.0 T high-resolution multi-sequence MRI can clearly show the structure of ex vivo carotid artery wall and allow quantitative assessment.Fat-suppressed T2Wimaging has a greater advantagein presenting atherosclerotic lesions.
10.Effect of endostar combined with radiotherapy on expression of integrin αvβ3 mRNA and MVD in lung cancer of Lewis rats

Objective To observe the MVD and integrin αvβ3 mRNA expression changes in tumor -bearing rats by using endostar combined with radiotherapy ,and to explore the potential synergy mechanism .Meth-ods We randomly divided the tumor -bearing rats into four groups:control group(NC),endostar group(ES), radiation treatment group ( RT) and endostar combined radiotherapy group ( ES +RT) .Tumor inhibition rate was calculated every other day .Microvessel density and αvβ3mRNA were texted by immunohistochemical or real time PCR in each group.Results (1)ES+RT group showed the most obvious inhibition rate;(2)Compared with NC group,microvessel density of RT was increased ,but decreased in ES group and ES +RT group significantly ( P<0.05);(3)Compared with NC group,αvβ3 mRNA expression of RT group was increased,while decreased in ES group and ES+RT group,and ES +RT group displayed a greater decrease when compared to ES group ( P<0.05).Conclusion Endostar combined radiotherapy can inhibit the growth of cancer and the expression ofαvβ3mRNA,improve the disorders of tumor vascular network .It may be one of the mechanisms of increasing radi-osensitization.
