1.Study of relationship between renal function and respiratory tract infections in the elderly

Objective To study the relation between kidney function impairment and respiratory tract infections(RTI) in the elderly.Methods Two hundred and twenty-four patients above 65 years old were divided into five groups according to stages of chronic renal disease and estimated glomerular filtration rate (e GFR):(A group,n =65):≥90 ml/(min · 1.73 m2),(B group,n =55):60-89 ml/(min · 1.73 m2),(C group,n =27):30-59 ml/(min · 1.73 m2),(D group,n =28):15-29 ml/(min · 1.73 m2) and(E group,n =49):< 15 ml/(min · 1.73 m2).Taking A group as the control baseline.Logistic regression was employed to analyze the relationship between kidney function and RTL Results Compare with A group(10.8%,7/65),the incidences of RTI in three groups(C group,D group and E group were 63.0% (17/27),85.7% (24/28) and 100% (49/49) respectively.The incidence in these groups was higher than that in A group(x2 =26.95,49.46 and 89.00 respectively,P < 0.05).We identified the risk factors with RTI were smoking (Waldx2 =17.66,P < 0.01),respiratory underlying diseases(Waldx2 =14.38,P <0.01),invasive operation history(Waldx2 =4.50,P < 0.05),diabetes (Waldx2 =3.86,P < 0.05),stay in hospital for above 2 weeks (Waldx2 =18.18,P < 0.01),Gender(Waldx2 =4.12,P < 0.05),eGFR (Waldx2 =13.57,P < 0.01).After adjusting for other risk factors of RTI,Logistic regression analysis showed that the risk of RTI still increased with the decrease of eGFR (eGFR≥90 ml/(min · 1.73 m2):(Waldx2 =0.053,P >0.05);60-89 ml/(min · 1.73 m2):(Waldx2 =0.046,P>0.05) ;30-59 ml/(minl · 1.73 m2):(Waldx2 =7.61,P<0.05),15-29 ml/(min · 1.73 m2):(Waldx2 =4.38,P<0.05); <15 ml/(min · 1.73 m2):(Waldx2 =13.57,P<0.01)).Conclusion Kidney function impairment in patients above 65 years old is an independent risk factor for RTI.

Acquired aplastic anemia is a refractory blood disease for children,and severe acquired aplastic anemia can be a great threaten for life.China is a high incidengce area,children at a period during which this disease can mostly occur.In China,except some congenital aplastic anemia,most children who have this diease is acquired aplastic anemia.In 2009,the Britain society of hematology recommended Guidelines for the diagnosis and management of aplastic anaemia,which represented the latest diagnosis and treatment principle and mainstream view of the clinical methodology about aplastic anemia.They make it clear that the main treatment for the disease are allogeneic hematopoietic stem cell transplantation(allo-HSCT) and immunosuppressive therapy(IST).This article collects the documents in recent 5 years,and provides a overview about the latest advance of the allo-HSCT and IST in the word.
3.Status and perioperative management of liver transplantation in treatment of portal hypertension

Objective To explore the status and perioperative management of liver transplantation in treatment of portal hypertension. Methods Clinical data of 56 patients with portal hypertension who underwent (orthotropic) liver transplantation (OLT) from February 2000 to April 2004 were studied retrospectively.(Incidence) of complications,and in-hospital mortality and survival rate were analyzed. Platelet count after OLT was measured and liver blood flow was monitored by Doppler. Results Among the 56 OLT patients, 6 cases(10.7%) underwent simultaneous splenectomy. Incidence of complications was 40.3%,and in-hospital mortality and 1-,2-,and 3- year survival rate after OLT were 12.5% and 87.46%,85.19% and 81.58% respectively. Platelet count reached a nadir at post-transplant day 3. Conclusions OLT is an (effective) method for radical treatment of portal hypertension. Perioperative management is of vital importance for success of the operation and patient′s prognosis.
4.Study of the Sleep Quality of Frontier Armymen Located in the Desert and Its Correlated Factors

Objective: To investigate the the sleep quality of frontier armymen located in the Gobi desert and its corre- lated factors. Methods: The sleep quality of 302 frontier armymen located in the Gobi desert was assessed with Pittsburgh Sleep Quality Index (PQSI). Results: The total scores of PQSI was 6.79?3.39,and 49.7% of the armymen suffered from sleep problems.The total scores of PQSI were correlated with health status,satisfaction with work,influence by the relation- ship with opposite sex,pressure from study and work,et al. Conclusion: The sleep quality problem of the frontier armymen located in the Gobi desert was more than normal people, and special measures should be taken to improve their sleep quality.
5.Application and Significance of MELD Scoring System in Liver Transplantation

Objective To introduce the birth and development of model of end-stage liver disease(MELD) and evaluate its effect on liver transplantation(LT) as a new scoring system.Methods Literatures of MELD applied in LT were analyzed retrospectively.Results MELD scoring system was used for predicting the prognosis of patients with end-stage liver disease and the death risk of candidates on waiting LT extensively and the order of organ sharing was determined by its predicable results.Conclusion MELD has been had a successful initial implementation for predicting the short-term survival probability and mortality in patients with end-stage liver disease,and meeting the goal of providing a system of allocation that emphasizes the urgency of the candidate while diminishing the reliance on waiting time,which has been proven to be a powerful tool for auditing the liver allocation system.
6.Regulation of licorice flavonoids on cytokines mRNA expression and oxidation reaction in mice with lung inflammation

Objective To study the mechanism of anti-inflammatory effects of licorice flavonoids(LF) isolated from the roots of Glycyrrhiza uralensis(licorice) on lipopolysaccharide(LPS)-induced lung inflammation in mice.Methods Mice were intratracheally instillated with either LPS(4 mg/kg) or saline.At 6 or 24 h after LPS intratracheal instillation,pathological examination and bronchoalveolar lavage were performed and the lung wet/dry weight ratio as an index of acute lung injury was assessed.Then,the numbers of total leukocytes,neutrophils and the activity of superoxide dismutase(SOD) and TNF-? protein in bronchoalveolar lavage fluid(BALF) were measured.LPS-induced myeloporoxidase(MPO) activity and expressions of TNF-? and IL-1? at the mRNA levels and TNF-? at the protein level in lung tissues were determined by RT-PCR and ELISA.Results LPS caused a severe lung inflammation,as indicated by the pathological findings and the lung wet/dry weight ratio.However,oral LF could attenuate these LPS-induced abnormalities.LF could decrease the numbers of total leukocyte cells and neutrophils,and increase the levels of SOD and TNF? BALF.In addition,LF significantly suppressed the MPO activity,TNF-? and IL-1? mRNA expression levels,and TNF-? protein level in the lung tissues.Conclusion Inhibition of inflammatory cytokines expressions and regulation of oxidation/antioxidation of LF may be its anti-inflammatory mechanism in LPS-induced lung inflammation in mice.
7.Prevention of surgical complications of renal transplantation operation

Objective To investigate the potential complications of renal transplant patients, to develop appropriate treatment programs to reduce the incidence. Methods Clinical data of 42 patients with renal transplantation were retrospectively analyzed. Blood type of the donors and recipients were same in 32 cases; Blood type of the donors was 0 and recipients blood type was A,3 cases;Donors blood type was 0 and recipients blood type was B,2 cases; Donors blood type was O and recipients blood type was AB,4 cases; Donors blood type was B and recipients blood type was AB,lease. Results In alll cases, there were acute rejection in 2 cases,pulmonary infection in 2 cases, wound infection in 1 case,urinary leakage in 1 case,urinary tract infection in 2 cases.42 cases were all cured up. One patient lost kidney function and required dialysis therapy 4 years after kidney transplantation, the remaining 40 patients were good. Conclusion Early detection and timely management of complications of renal transplantation played a very important role in improving the survival rate.
8.Clinical observation on the treatment of chronic glomerulonephritis with Bushen Yiqi Qingluo decoction

Objective To observe the efficacy of Bushen Yiqi Qingluo decoction treating chronic glomerulonephritis.Methods 84 patients were randomly recruited into a control group and a treatment group.The control group was treated with western medicine;while the treatment group was treated with Bushen Yiqi Qingluo decoction based on the control group.The efficacy and the change of lab indexes were observed.Results The total effective rate was 90.48%and 76.19%in the treatment group and the control group respectively,showing significant difference(P<0.05).After the treatment,the levels of urinary protein,Scr,and BUN in the treatment group were significantly decreased compared with the control group and the levels before the treatment.The levels of Ccr were increased significantly.Conclusion Bushen Yiqi Qingluo decoction Combined with basic western medical treatment was effective in treating chronic glomerulonephritis.
9.Analysis of 30 cases with diabetic foot treated mainly by Simian Yongan Decoction anti Xuefu Zhuyu Decoction

Objective To observe the efficacy of the treatment of diabetic foot mainly by Simian Yongan Decoction and Xuefu Zhuyu Decoction. Methods 60 patients were randomly recruited into a control group and a treatment group. The control group was treated by conventional western medicine. The treatment group was treated with Simian Yongan Decoction and Xuefu Zhuyu Decoction on the basis of western medicine. Results The total effective rate was 86.6% and 66.6% in the treatment group and the control group respectively, showing significant difference (P<0.05) . Conclusion The efficacy of Simian Yongan Decoction and Xuefu Zhuyu Decoction combined with westeru medicine was better than exclusive western medicine in treating diabetic foot.
