1.Study on Exhale CO_2 Recovery and Resting Energy Expenditure in Patients with Cirrhosis

Objective To explore the recovery rate of exhale CO 2 and energy expenditure in the patients with cirrhosis.Methods The recovery rate of exhale CO 2 and energy expenditure in fourteen patients with cirrhosis and eleven healthy subjects were measured by using NaH 13 CO 3 as tracer.After overnight fast,all subjects were intravenously given NaH 13 CO 3 at a primed constant rate of infusion for two hours after a saturating dose infusion.Amount of 13 CO 2 in exhale air was measured using stable isotope mass spectrometry analysis,and the recovery rate of exhale CO 2 and energy expenditure were calculated.Results The recovery rate of exhale CO 2 in patients with Cirrhosis was (80 7?0 26)%,and in healthy control subjects was (78 3?0 84)%,which showed a significantly different between the two groups(P0 05)[(30 2?2 4)Kcal?kg -1 ?d -1 and (28 4?3 5)Kcal?kg -1 ?d -1 ,respectively].Conclusions The results provide theoretical basic to calculate meals for nutritional therapy in the patients with cirrhosis.
2.Content Determination of 2,3,5,4'—tetrahydroxystilbene—2—O—?—D—glucoside in Yangxueshengfa Capsules by RP-HPLC

OBJECTIVE:To establish the method for the content determination of2,3,5,4'—tetrahydro-xystilbene—2—O—?—D—glucoside in Yangxueshengfa capsules by RP-HPLC.METHODS:Alltima-C 18 was used as the chromatographic column;the acetonitrile-water(15∶85)was taken as the mobile phase with flow speed at1.0ml/min and detection wave?length at320nm.RESULTS:In the range of0.45?g~2.26?g,2,3,5,4'—tetrahydro-xystilbene—2—O—?—D—glucoseide were in direct proportion to the peak area value(r=0.9996),the mean recovery rate was98.32%(RSD=1.58%,n=6). CONCLUSION:This method was simple,convenient and accurate,which can be used for the content determination and the quality control of Yangxueshengfa capsules.
3.Therapeutic Effect Evaluation of Acupuncture and Chinese Patent Medicine on Rheumatoid Arthritis

Objective: To evaluate the therapeutic effect of combining use of acupuncture and drugs on rheumatoid arthritis(RA) in the early and the active stages of by the imaging of bone and joint with 99m TC-methlene diphosphate(MDP). Methods: 102 cases of RA in the early and the active stages were randomly allocated into the compound Fengshining group (Group A),acupuncture group(Group B), and compound Fengshining plus acupuncture group(Group C).They were treated for 30 days. All patients were examined before treatment and after treatment by 99m TC-MDP imaging of the whole body, hands and feet, and analyzing method of random interested region was used to count the ratio of radioactivity in the affected area of RA to radioactivity in adjacent normal bone(T/TN). Meanwhile, joint swelling index, joint tenderness index, morning stiffness, gripping force, and erythrocyte sedimentation rate (ESR) were also observed. Results: T/TN of the Group A after treatments had no significant difference comparing with that before treatment(t = 1.35, P>0.05), while the Group B and C had significant differences(t = 5.31, t=8.97, P<0.01). The changes of clinical and experimental examination in the Group C had more significant differences than those in the Group A and B(P<0.05).Conclusion: Acupuncture therapy could promote RA recovery, and combining use of acupuncture and compound Fengshining had better effect.
4.Risk factors of depression in coronary heart disease patients who underwent revascularization therapy

Objective To explore the incidence and risk factors of depression in coronary heart disease patients who underwent revascularization therapy. Methods A total of 493 patients who were admitted in Tianjin chest hospital from April 2012 to February 2013 were enrolled, among whom 258 patients acceptted coronary artery bypass grafting (CABG) and the rest 235 patients underwent percutaneous coronary intervention (PCI). Self-rating depression scale (SDS) was employed to assess the state of patients at both1 day before and 7 days after the operations. According to the postopera?tive scores, CABG group was divided into the depression group (n=90) and non-depression group (n=168) while PCI group was also divided into depression group (n=54) and non-depression group (n=181). Basic clinical datum of patients were col?lected and analyzed and independent risk factors of depression was analyzed though logistic multi-variant regression. Results The incidence of postoperative depression among CABG patients was significantly higher than that in PCI patients (P<0.05).(1)In the CABG group, age, ratio of female gender, alcohol intake, rate of past depression, length of anaesthesia, length of staying in ICU and incidence of postoperative cognitive dysfunction(POCD)were all higher in depression subgroup than those in non-depression subgroup. Female and preoperative depression were both independent risk factors for postoper?ative depression in patients underwent CABG.(2)In PCI group, ratio of female gender, blood pressure, incidence of Diabe?tes Mellitus, the rate of past Myocardiac infaction (MI), length of intervention therapy and the number of planted stents were all higher in depression subgroup than non-depression subgroup. Female, past MI and length of intervention therapy are all independent factors of post-operative depression in patients underwent PCI. Conclusion Incidence of depression in pa?tients underwent revascularization is high. Female is the dependent risk factor in both CABG group and PCI group. Com?pared with PCI, CABG had greater influence on development of depression in postoperative patients.
5.The relationship between vascular lesion and serological changes in patients with coronary heart disease and obstructive sleep apnea hypopnea syndrome

Objective To investigate the relationship between vascular lesion and serological changes in patients with coronary heart disease (CHD) complicated with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods According to the standard, a total of 168 patients of OSAHS complicated with CHD were selected in this study. Those patients were divided into 3 groups according to the apnea hypopnea index (AHI) level:light group (AHI, 5-14/h), moderate group (AHI, 15-30/h) and severe group (AHI,>30/h). Syntax scores were performed on three groups according to coronary angiography results. The data of hemoglobin (Hb), platelet count (PLT), fibrinogen (FIB), D-Dimer (DD), total cholesterol (TC), low density lipoprotein (LDL), high density lipoprotein (HDL), triacylglycerol (TG), alanine aminotransferase (ALT), aspartate transaminase (AST), uric acid (UA), creatinine (Cr) and echocardiographic examination index were collected and analyzed in three groups. Results The Syntax score was significantly higher in severe group than that in mild and moderate groups (P<0.05). There was no significant difference in the Syntax score between mild and moderate groups. The levels of Hb, FIB, LDL, pulmonary arterial pressure (PAP), and right ventricle transverse diameter (RVD) were significantly lower in the mild and moderate groups than those in the severe group (P<0.01). The left ventricular ejection fraction (LVEF) was significantly lower in the severe group than that in the mild and moderate groups (P < 0.01). There were no significant differences in levels of Hb, FIB, LDL, PAP, RVD and LVEF between mild group and moderate group (P > 0.05). Conclusion The serology and cardiac structure can change gradually in severe OSAHS patients, and the coronary artery lesion will be more complex. Therefore, the clinical treatment should pay attention to screening for OSAHS in patients with coronary heart disease.
6.Therapeutic effect of complex radix sophorae flavescentis injection in treating cancer pain

Objective To evaluate the clinical effect of complex Radix Sophorae Flavescentis injection in treating advanced cancer pain. Methods 90 patients with advanced cancer and pain were selected randomly. The patients' general information was analyzed, their pain was ranked, and the score of quality of life was estimated. Results Among the patients with gentle pain,ratio of CR is 28.57 % (10/35), ratio of PR is 54.29 % (19/35), ratio of slight response is 5.71 %, total effective rate is 88.57 % (31/35). Among the patients with medium pain, the ratio of CR is 10% (3/30), ratio of PR is 23.33 % (7/30), ratio of slight response is 36.67 % (11/30), total effective rate is 70 % (21/30). Among the patients with severe pain, the ratio of CR is 0, ratio of PR is 4.00 % (1/25), ratio of slight response is 12.00 % (2/25), total effective rate is 16.00 % (4/25). The score of quality of life was improved from (21.04±4.92) to (33.52±5.98). The difference was statistical significant (t =14.07, P =0.00). TCM type: in all the 90 patients, the total effective rate of mixture of phlegm and blood phlegm are the highest (88.46 %), and the total effective rate of yang asthhenia of the spleen and kidney is 20.00 %. Rash was found in only one patient and no other adverse effect, such as in digestive tract, bone marrow, heart, liver or kidney was found among all patients. Conclusion Complex Radix Sophorae Flavescentis injection has significant analgesic effect in treating light and medium cancer pain and could improve the patients' quality of life, and it has significant therapeutic effect especially in the mixture of phlegm and blood stasis patients. So Complex Radix Sophorae Flavescentis injection is safe and effective traditional Chinese medical drug.
7.Clinical analysis of surgical treatment of 46 cases with spontaneous cerebellar hemorrhage

Objective To evaluate the surgical indications and treatment of spontaneous cerebellar hemor-rhage.Methods The clinical data and surgical methods of 46 patients with spontaneous cerebellar hemorrhage were retrospectively analyzed.Results Follow up 6 months,4 cases were dead.Outcomes were assessed by the ADL:17 cases wereⅠ,10 cases were Ⅱ,9 cases were Ⅲ,4 cases were Ⅳ,2 cases were Ⅴ.Conclusion The surgical way should be selected according to degree of patients,conditions severity and CT imaging of hemorrhage in the patients with spontaneous cerebellar hemorrhage.The curative of timely proper surgery on the patients with spontaneous cerebellar hemorrhages is good.

9.Clinical significance of serum omentin-1 in patients with obesity and essential hypertension

Objective To investigate the clinical significance of serum omentin-1 levels in obesity and essential hypertension patients and its related influencing factors. Methods One hundred and thirty patients with obesity admitted to the Cadre Health Care Department of Navy General Hospital from Jun. 2011 to Dec. 2013 were enrolled in this study. According to the blood pressure,130 obesity patients were divided into simple obesity group( n=64 )and obesity hypertension group( n=66 ). Sixty non obesity patients with normal blood pressure were enrolled as control group. Serum omentin-1 levels were assessed by enzyme linked immunosorbent assay ( ELISA). Association between omentin-1 and biochemical parameters( including systolic blood pressure( SBP), diastolic blood pressure(DBP),body mass index(BMI),waist hip ratio(WHR),fasting blood glucose(FBG), triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol( HDL-C),fasting insulin( FINS),insulin resistance index( HOMA-IR)and high sensitive C reactive protein(hs-CRP))were analyzed. Results Serum omentin-1 levels in control group was(26. 7 ± 5. 1)μg/L, significantly higher than that in simple obesity group and obesity hypertension group((22. 1 ± 3. 2)μg/L,(18. 3 ± 3. 4)μg/L;F=33. 7,P﹤0. 01),and serum omentin-1 levels in obesity hypertension group was significantly lower than that in simple obesity group( t =21. 5,P ﹤0. 05). Pearson correlation analysis showed that serum omentin-1 negatively correlated with BMI,WHR,SBP and HOMA-IR( r = - 0. 447,- 0. 340,- 0. 350,-0. 397;P﹤0. 01). There were no significant relations between omentin-1 and age,DBP,FBG,FBG,TG,TC, LDL-C,HDL-C,FINS and hs-CRP( P ﹥0. 05 ). Multiple stepwise regression analysis showed that HOMA-IR, BMI and SBP were independent influencing factors of serum Omentin-1. Conclusion The serum omentin-1 level decreases in the obesity hypertension patients. Omentin-1 is negatively correlated with BMI,SBP,and HOMA-IR, which may contribute to the underlying pathophysiology of obesity and essential hypertension.
10.Effects of spike wave electric stimulation on the transplantation of autogenous marrow mesenchymal stem cells in rats with spinal cord injury

Objective To investigate whether the implanted spike wave electrical stimulation (ES) can improve survival of mesenchymal stem cells (MSCs) after transplantation in spinal cord injury (SCI) rats. MethodsIn the study,60 male SD rats were used to prepare the SCI induced by a digital weight drop apparatus.The MSCs from each rat' s femurs were collected and then labeled by Bromodeoxyuridine (Brdu) seven days after SCI.The rats were randomly assigned into four groups,ie,group A (receiving a transplantation of MSCs only),group B (receiving electro-stimulant therapy only),group C ( receiving a transplation of MSCs and electro-stimulant therapy) and the control group ( receiving a single injection of phosphate buffer solution,PBS).Functional status was assessed regularly before and after SCI by using the Basso-Beattie-Bresnahan (BBB) locomotor rating score and somatosensory evoked potential (SEP).The recovery of spinal cord was checked by diffusion tensor imaging (DTI) and the survivorship of the labeled MSCs was observed with immunohistochemical method. Results BBB score of group C demonstrated significant difference in comparison with that of the other three groups ( P < 0.05 )and was significantly higher than that of the control group (P <0.01 ) four weeks after surgery,which indicated an obvious functional improvement of the extremity mobility in group C.At the 8th week postoperatively,SEP study displayed significant difference in group C from the other three groups in terms of latency and wave amplitude (P <0.05 ),indicating the fast neurofunctional recovery of the spinal cord in group C.MR images of the spinal cords by DTI showed the same outcome.Immunohistochemistry confirmed a large number of the labeled MSCs positive cells in the lesion site of group C.ConclusionTheimplanted spike wave ES promotes the bioactivity and survival of MSCs and improves the therapeutic effect in combination with MSCs transplantation.
