ObjectiveTo analyse the prognostic factors of acute respiratory distress syndrome (ARDS) of children by comparing mortality in ARDS. MethodARDS type ,treatment process if there were multiple organ dysfunction syndrome (MODS), and whether family members actively cooperated with the treatment factors in 38 children with ARDS were analyzed. ResultsARDS of the original mortality of sepsis was the highest[ 66.67％ ( 14/21 )]; treatment process combined with MODS in 8 cases, 6 died, the mortality up to 75.00％ (6/8), significantly higher than combined without MODS [40.00％ (12/30)],there was significant difference (P ＜ 0.05); family members actively cooperated with the treatment could reduce mortality of children (P ＜0.05); PaO2 and PaO2/FiO2 value:pulmonary-exterior type was lower than pulmonary-interior type, combined with MODS was lower than combined without MODS, family members actively cooperated with the treatment was lower than family members inactively cooperated with the treatment (P ＜ 0.05 );PaCO2 value:pulmonary- exterior type was lower than pulmonary-interior type, family members actively cooperated with the treatment was lower than family members inactively cooperated with the treatment (P ＜ 0.05 ) ; pH: pulmonary-exterior type was higher than pulmonary-interior type, combined with MODS was higher than combined without MODS, family members actively cooperated with the treatment was higher than family members inactively cooperated with the treatment (P ＜0.05);the critical illness score in family members with a positive attitude was higher than that in family members with a negative attitude (P ＜ 0.05).ConclusionThe primary disease of sepsis, associated with MODS, the family members with a negative attitude toward the treatment affect the prognosis of children with ARDS.

Objective To compare the clinical application value between Leep knife operation and Vaginal ligation operation in submucous uterine myoma during the basic-level hospital.Methods According to digital table method,63 cases with submucous uterine myomas were randomly divided into the trial group and control group,33 cases in the trial group used the Leep knife,30 cases in the control group used vaginal ligation operation.Results The amounts of the trial group such as operation time,complications bleeding,operation stump bleeding rate and menstrual disorder improvement rate were (8 ± 3)min,(7 ± 3)mL,0.0％,91.0％,while the amounts of the control group were (10 ± 3) min,(10 ± 4) mL,13.3％,70.0％.The difference was statistically significant (t =2.642,3.387,x2 =4.698,4.455,all P ＜ 0.05).Conclusion Leep knife operation is better than Vaginal ligation operation,because it has shorter operation time,less complications bleeding,lower operation stump bleeding rate and higher menstrual disorder improvement rate.It is worthy of applying in the basic-level hospital.

ObjectiveTo evaluate the clinical efficacy of acupuncture plus moxibustion in treating chronic superficial gastritis (due to colddeficiency of spleen and stomach).MethodTotally 112 patients with chronic superficial gastritis (due to cold deficiency of spleen and stomach) were randomized into a treatment group of 56 cases and a control group of 56 cases. The treatment group was intervened by acupuncture plus moxibustion, while the control group was by using Ranitidine, both for 2 treatment courses (20 d).ResultAfter 2 treatment courses, the total effective rate of the treatment group was significantly higher than that of the control group (P<0.01); in comparing the gastroscopic results after treatment, the total effective rate of the treatment group was significantly superior to that of the control group (P<0.01).ConclusionAcupuncture plus moxibustion can produce a significantly higher therapeutic efficacy than orally taking Ranitidine in treating chronic superficial gastritis.

Objective To observe the clinical efficacy of qi-based acupuncture treatment in treating chronic gastritis due to liver-stomach disharmony. Methods Totally 120 patients with chronic gastritis due to liver-stomach disharmony were randomized into group A, group B, and group C, 40 in each group. Group A was intervened by qi-based acupuncture treatment; group B was by medication;group C was by sham acupuncture by selecting points 1 cm away from the points selected in group A. The therapeutic efficacies were evaluated after 4-week treatment. Results The total effective rate was 95.0%in group A, versus 75.0%in group B and 45.0% in group C, and the total effective rates in group A and B were significantly different from that in group C (P<0.01, P<0.05). The total effective rate in group A was significantly different from that in group B (P<0.05). Conclusions Qi-based acupuncture treatment is an effective approach in treating chronic gastritis due to liver-stomach disharmony.

Objective To compare effects between the laparoscopic surgery and open surgery for the bigger adrenal adenoma(＞4cm) patients in Qinghai.Methods Altogether 57 cases of bigger adrenal adenoma patients(＞4cm)in the affiliated hospital of Qinghai university and Qinghai People's Hospital from 2012 to 2016 were selected and divided into two groups (A and B) according to surgical programs difference.Group A (23 cases)wcre givcn laparoscopic surgcry and Group B (34 cases)were given open surgery (And 2 cases in group A turned into open surgery were out of statistics).The operative time,blood loss,hospital stay and drainage tube removal time were compared.Results The operative time,blood loss,hospital stay and draiuage tube removal time in Group A were significantly better than those in Group B,with statistical significance (P ＜ 0.05).Conclusion The use of laparoscopic surgery for the bigger adrenal adenoma(＞4cm) patients is safer and more effective than open surgery.

Objective To investigate the causes of intra-abdominal hernia and improve the level of early (diagnosis) and therapy of intra-abdominal hernia. Methods A retrospective analysis of the clinical data of 68 cases of intra-abdominal hernia was made. Results The postoperative confirmed diagnosis was para-occal hernia in 1 patient;herniation of efferent jejunal loop through the space between the afferent jejunal loop and the mesocolon after Billroth II gastrectomy in 6 patients,one of which had bowel necrosis,all of the 6 patients were cured after reduction of the hernia or enterectomy after and enteroanastomsis;internal henia though (ruptured) pelvic (peritoneum) after Miles operation in 4 patients;internal hernia though the space between the (descending) colon and the lateral abdominal wall after colostomy of descending colon in 6 patients;internal (hernia) caused by (adhesion) of omentum to the intestinal wall of sigmoidostomy in 1 patient;internal hernia through hiatuses caused by postoperative or post-peritonitis adhesions to the peritoneum,ovaries,urinary (bladder),uterus,(intestinal) wall or between loops of intestine in 50 patients(73.5%),including 3 cases of bowel (necrosis).All of those patients were cured after reduction of the hernias,repair of the hiatuses,release of (adhesions) or enterectomy and enteroanastomosis. Conclusions Preoperative diagnosis of internal abdominal hernia is difficult.The possibility of internal abdominal hernia should be cansidered in patients with a history of operation,and who complain of frequent abdominal pain or bowel obstruction.Early diagnosis and prompt (operation) is necessary to prevent the occurrence of bowel strangulation and bowel necrosis.

OBJECTIVE To conclude and analyze epidemiology,etiology,diagnostic,treatment and prognostic factors of local adult hospitalized patients with severe pneumonia.METHODS A retrospective analysis with case-control study was designed.All the medical records of consecutive patients aged ≥18 yrs with diagnosis of pneumonia from Sep 2000 to Sep 2005 in the First Affiliated Hospital of Sun Yat-sen University were searched.Contribution of the factors to prognosis was determined by multivariate analysis with Logistic regression.RESULTS In the severe community-acquired pneumonia(SCAP) subgroup,mortality was 45.0%;and in the severe hospital-acquired pneumonia(SHAP) subgroup,mortality was 54.8%.In the SCAP subgroup,there were five factors associated with prognosis on multivariate analysis: age(≥75yrs),heart failure,intubation and septic shock were risk factors;initial combined antibiotic therapy was protective factors.In the SHAP subgroup, factors associated with prognosis on multivariate analysis were heart failure,septic shock and heart rate ≥100bpm;all were risk factors.CONCLUSIONS Septic shock and heart failure are associated with mortality both in SCAP and SHAP,which suggest that severe systemic inflammatory response associated with the pulmonary infection be an important prognostic factor in the outcome of severe pneumonia.Initial appropriate antibiotic empiric therapy was important to the prognosis.

With the existence of capsules,lens regeneration occurs in some mammals after extracapsular lens extraction.It is usually thought that lens regenerates from resident lens epithelial cells (LECs) in the capsule.However,lens regeneration dose not mean simple redupilication of development,and transparency of the lens is affected by irregular growth,migration and transdifferention of the resident LECs.Previous studies mainly focus on the dysplasia of LECs,but update theory about lens regeneration is proposed to be associated with stem cells.Some views and suggestions in lens regeneration are concerned in current researches to better illuminate the mechanism and therapy of posterior capsular opacity.
