1.Isoflurane inhibits neural stem cell proliferation in the hippocampus and promotes its differentiation into neurons

BACKGROUND:Isoflurane cannot only induce a wide range of large neuronal apoptosis, but also inhibit hippocampal neurogenesis in neonatal rats, thereby resulting in hippocampus-dependent learning and memory defects.
OBJECTIVE:To investigate the isoflurane effect on proliferation and differentiation of the hippocampal neural stem cels.
METHODS:Twenty-six Sprague-Dawley rats were randomly divided into air group and isoflurane group (n=13 per group). Rats in the isoflurane group were subjected to 2.5% isoflurane inhalation for 3 minutes folowed by 1.5% isoflurane inhalation for 4 hours. Rats in the air group only breathed in air. After the intervention, blood glucose and arterial blood gas changes were detected in the two groups. Additionaly, rats in the two groups were given intraperitoneal injection of 5-bromodeoxyuridine before and after intervention. At 24 hours after the last injection of 5-bromodeoxyuridine, brain tissues were taken to make frozen sections for immunofluorescence staining.
RESULTS AND CONCLUSION:There were no significant difference in pH, PaO2, PaCO2, HCO3, BE and SaO2 levels between the two groups (P> 0.05). Compared with the air group, the number of BrdU+ cels was significantly less in the isoflurane group (P < 0.05), while the number of NeuroD+/BrdU+ cels was significantly higher in the isoflurane group (P < 0.05). The incidence of adverse reactions was 23% in the isoflurane group, which was significantly higher than that in the air group (7.7%;P < 0.05). These findings indicate that isoflurane can inhibit the proliferation of neural stem cels in the hippocampal dentate gyrus, and promote their differentiation into neurons.
3.A Randomized Clinical Study of Geiftinib and Pemetrexed as Second Line Therapy for Advanced Non-squamous Non-small Cell Lung Cancer

Background and objective Geiftinib and Pemetrexed are drugs used as second-line therapy for ad-vanced non-small cell lung cancer (NSCLC), although studies comparing the two drugs are limited. hTe aim of this study is to explore the effects, safety, and quality of life (QoL) of Geiftinib and Pemetrexed on patients with advanced non-squamous NSCLC. Methods Forty-six advanced non-squamous NSCLC patients who failed to ifrst-line therapy were randomly divided into two groups with 23 patients each, one using oral Geiftinib (Geiftinib group) and the other using intravenous injection Pemetrexed (Pemetrexed group). hTe effects, safety, and QoL were determined and analyzed. Results For the Pemetrexed group, objective response rate (ORR) was 13.0%(3/23), disease control rate (DCR) was 30.4%(7/23), and median progres-sion-free survival (mPFS) was 3.1 months. In the Geiftinib group, ORR was 17.3%(4/23), DCR was 39.1%(9/23), and mPFS was 4.4 months. Compared with the Pemetrexed group, the ORR, DCR, and mPFS in the Geiftinib group exhibited no statisti-cal signiifcance (P>0.05). Furthermore, the most common toxicities in the Pemetrexed group were neutropenia (n=9, 39.13%) and fatigue (n=8, 34.78%), whereas those in the in Geiftinib group were skin rash (n=8, 34.78%) and diarrhea (n=4, 17.39%). Compared with baseline, the QoL improved in both groups but to different degrees. Likewise, emotional, functional well-being, and QoL aspects speciifcally related to lung cancer were better improved in the Geiftinib group than in the Pemetrexed group (P<0.05). Conclusion hTe effects of Pemetrexed and Geiftinib as second line therapy were similar, although with dif-ferent AEs. Both drugs could improve the QoL, but Geiftinib showed better overall results than Pemetrexed.
4.Effect of dexmedetomidine on postoperative outcomes in pediatric patients undergoing open reduc?tion and internal fixation following upper limb fracture performed under sevoflurane anesthesia combined with brachial plexus block

Objective To evaluate the effect of dexmedetomidine on postoperative outcomes in ped?iatric patients undergoing open reduction and internal fixation following upper limb fracture performed under sevoflurane anesthesia combined with brachial plexus block. Methods Sixty American Society of Anesthe?siologists physical status Ⅰ pediatric patients of both sexes, aged 6-12 yr, with body mass index <24 kg∕m2, scheduled for elective open reduction and internal fixation following upper limb fracture, were ran?domly divided into ropivacaine group(group R)and dexmedetomidine mixed with ropivacaine group (group DR), with 30 cases in each group. Interscalene brachial plexus block was performed under ultra?sound guidance after sevoflurane anesthesia, 0.25% ropivacaine plus 1 μg∕kg dexmedetomidine(0.3 ml∕kg)was given in group DR, and 0.25% ropivacaine 0.3 ml∕kg was given in group R. The emergence time was recorded, and the development of agitation was observed immediately after emergence from anes?thesia. Sensory block duration, motor block duration, consumption of sedatives and analgesics during post?anesthesia care unit(PACU), consumption of rescue analgesics within 24 h after operation, length of stay in PACU and adverse reactions in the perioperative period were observed and recorded. Results Compared with group R, the incidence of agitation immediately after emergence from anesthesia was significantly de?creased, sensory block duration was prolonged, the consumption of sedatives and analgesics during PACU and rescue analgesics within 24 h after operation was reduced, and the emergence time and length of stay in PACU were prolonged in group DR(P<0.05). There was no significant difference in the incidence of bradycardia between the two groups(P>0.05). No adverse reactions were found in the perioperative period in the two groups. Conclusion Dexmedetomidine can promote postoperative outcomes in pediatric patients undergoing open reduction and internal fixation following upper limb fracture performed under sevoflurane anesthesia combined with brachial plexus block.
5.Effect of mindfulness meditation training on anxiety, depression and sleep quality in perimenopausal women.

OBJECTIVE:
To explore the effect of mindfulness meditation training for improving anxiety, depression and sleep disorders in perimenopausal women.
METHODS:
Intervention by menopause meditation training was delivered in 121 perimenopausal women with anxiety, depression or sleep disorders in Baiyun District, Guangzhou. Before and after the intervention, the Self-rating Anxiety Scale, Self-rating Depression Scale and Pittsburg Sleep Quality Index were used for assessment of changes in the conditions of the women.
RESULTS:
After menopausal meditation training, the perimenopausal women showed significant improvement in the mean scores of Self-rating Anxiety Scale (48.26 ± 6.47; =3.865, < 0.01), Selfrating Depression Scale (50.27 ± 6.54; =4.541, < 0.01) and Pittsburgh Sleep Questionnaire (10.64 ± 4.38; =5.596, < 0.01). The symptom remission rates differed significantly among the women with different self-practice frequencies ( < 0.01). The remission rates of anxiety, depression and sleep disorder increased significantly with the frequency of self-exercise ( < 0.01).
CONCLUSIONS
Mindfulness meditation training can effectively alleviate the symptoms of anxiety and depression and improve the quality of sleep in perimenopausal women, and the frequency of the exercise is positively correlated with the improvements. Mindfulness meditation training can be an effective intervention for improving the mental health of perimenopausal women.

6.The evaluation of blood glucose and blood lipid changes and non-pharmacological intervention effects in gestational diabetes mellitus patients at different gestational weeks based on insulin sensitivity index differences of homeostasis model assessment 2

Objective:To investigate the changes of blood glucose and blood lipid in gestational diabetes mellitus (GDM) patients with different insulin sensitivity during pregnancy and the effect of non-drug intervention.Methods:Data of 240 pregnant women with GDM and 240 healthy pregnant women were collected from July 1 to September 1, 2023 in Shijing People′s Hospital in Baiyun District and other five hospital. The insulin sensitivity index (ISI) was calculated by homeostasis model assessment 2(HOMA2) model, according to the 25th percentile of ISI of normal pregnant women, GDM patients were divided into insulin sensitive group (group A) and insulin sensitive deficiency group (group B), and group A and group B were divided into two groups according to 36-week blood glucose control: group A1 with good blood glucose control (group A1 and group B1) and group A2 with bad blood glucose control (group A2 and group B2). The age, body mass index (BMI) before pregnancy, fasting plasma glucose (FPG), blood lipids and blood glycated hemoglobin (HbA 1c ) in the first trimester, blood glucose and blood lipids in the second trimester were compared at the 28th, 32nd and 36th weeks of gestation, the number of cases, blood glucose, blood lipids and non-drug intervention were measured. Results:There were 166 cases in group A and 74 cases in group B. Blood glucose and blood lipid were normal in early pregnancy. There was no significant difference in blood glucose between group A and group B during the second trimester. The levels of blood lipids were significantly higher than those during the first trimester, and the levels of triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) were significantly higher than those during the first trimester. The number of pregnant women in group A1 was significantly more than that in group A2 in the third trimester ( P = 0.01), and the number of pregnant women in group B1 was more than that in group B2 at 28 weeks ( P = 0.01). At 32 weeks, the number of pregnant women in group B1 and group B2 was similar ( P = 0.31). At 36 weeks, the number of pregnant women in group B1 was significantly lower than that in group B2 ( P = 0.01). In the third trimester of pregnancy, the levels of blood glucose in group B2 were higher than those in group A2 ( P<0.05). The levels of TG and LDL-C in group A2 and group B2 were higher than those in group A1 and group B1 respectively, high-density lipoprotein cholesterol (HDL-C) was lower than that in group A1 and group B1( P<0.05), and there was no significant difference in TC between group A2 and group A1 at 28 and 32 weeks ( P>0.05), but it was significantly higher at 36 weeks ( P = 0.01). In the third trimester of pregnancy, diet control was the most common (91.7%, 87.7%, 81.6%, respectively) in group A ( P>0.05). The proportions of diet-only and diet-plus exercise interventions were similar in group B1 at 28 weeks and 32 weeks (52.9% vs. 47.1%, 45.7% vs. 54.3%)( P = 0.072, 0.113). At the 36 weeks, the main intervention was diet combined with exercise (73.3%). In group B2, dietary intervention (69.6%, 71.8%, 69.5%) was the main cause of poor control of blood glucose. Conclusions:In GDM patients with insulin sensitivity deficiency, the blood glucose and blood lipids in the second trimester are obviously increased, and the abnormality in the third trimester is even greater.
7.Reference value of lumbar spine bone mineral density and regional differences based on quantitative CT examination in healthy adult female in China

Objective:To establish the normal reference value of lumbar bone mineral density (BMD) under quantitative CT (QCT) in Chinese healthy adult females and to explore the regional differences.Methods:Total of 35 431 healthy women who met the inclusion criteria of Chinese health quantitative CT big data program were selected in this study. The BMD of the central plane of L 1 and L 2 vertebrae was measured by Mindways′s QCT system, and the mean value was taken. One-way analysis of variance was used to compare the BMD differences of lumbar vertebrae in women of different ages and regions. The subjects were grouped by an age interval of 10 years, and the level of BMD in different regions of the same age group were compaired. Results:The peak BMD of Chinese healthy adult women appeared in the age group of 20-29 years (Northeast China(183.01±24.58) mg/cm 3, North China (188.93±24.80) mg/cm 3, East China (187.54±27.71) mg/cm 3, South China (186.22±33.72) mg/cm 3, Central China (176.33±24.91) mg/cm 3, Southwest China(182.25±28.00) mg/cm 3), and then it decreased with age. The level of BMD in different regions decreased with the age. Before the age of 70 years, BMD in Central and Southwest China was always at a low level((176.23±24.91) to (90.38±28.12) mg/cm 3, 182.25±28.00 to (88.55±25.68) mg/cm 3), lower than those in Northeast China ((183.01±24.58) to (99.69±27.85) mg/cm 3), North China ((188.93±24.80) to (95.89±26.12) mg/cm 3), East China ((187.54±27.71) to (95.65±27.86) mg/cm 3). After 70 years of age, BMD tended to be the same in different regions ( P>0.05). The BMD values in Central China and Southwest China were similar in the age group of 40-60 years ( P>0.05). The BMD values in the health adult femles in the age group of 60 years in different regions of Chinawere all lower than those of bone mass abnormality (all P<0.05). The detection rate of osteoporosis in females over 50 years was the highest in Southwest China (25.65%) and it was the lowest in North China (17.30%). Conclusions:This study establishes reference values of BMD under QCT in healthy Chinese women, which can be used as a reference basis for identifying women with low BMD who are at risk of osteoporosis. The BMD value is the lowest in Southwest China and the highest in South China.
8.Improvement and Anti-inflammation Mechanism of Shenfu Injection on Lung Tissue in Endotoxin Shock Model Rats

OBJECTIVE: To study the improvement and anti-inflammation mechanism of Shenfu injection on lung tissue of endotoxin shock model rats. METHODS: Totally 48 rats were randomized into control group,model group,dexamethasone group (positive control,1 mg/kg) and Shenfu injection low-dose,medium-dose and high-dose groups (5,10,15 mL/kg),with 8 rats in each group. Except for normal group, other groups were given intraperitoneal injection of lipopolysaccharide (LPS) to induce endotoxin shock model. After modeling, each group was given relevant medicine once intraperitoneally. 24 h after medication, HE staining was used to observe pathological changes of lung tissue in rats and pathological scoring was conducted. RT-PCR was used to determine mRNA levels of P65 and P50 proteins related to NF-κB signaling pathway. Western blot assay was used to determine the expression levels of P65 and P50 proteins in lung tissue, and the expression levels of P65 protein in nucleus and cytoplasm of lung tissue were also determined. The level of TNF-α in plasma in rats were determined by ELISA. RESULTS: Compared with control group, alveolar septum became thicker, obvious vascular engorgement was found, and a large number of neutrophils infiltrated the interstitium in model group. Histopathological score, mRNA and protein expression levels of P65 and P50 in lung tissues were increased significantly (P<0.01 or P<0.001); the protein expression of levels P65 in nucleus and cytoplasm and level of TNF-α in plasma were increased significantly (P<0.001). Compared with model group, alveolar structure of rats in dexamethasone group and Shenfu injection medium-dose and high-dose groups was complete, no obvious bleeding was observed, and the degree of inflammatory cell infiltration was improved significantly. Histopathological score, mRNA and protein expression levels of P65 and P50 in lung tissue and level of TNF-α in plasma were decreased significantly (P<0.05 or P<0.01 or P<0.001). The protein expression level of P65 in nucleus and cytoplasm of lung tissue were decreased significantly in dexamethasone group and Shenfu injection low-dose and medium-dose groups were decreased significantly (P<0.05 or P<0.01 or P<0.001). CONCLUSIONS: Shenfu injection can decrease mRNA and protein expression levels of P65 and P50 in lung tissue, level of TNF-α in plasma, and protect lung tissue of endotoxin shock rats.
9.Intervention Effect of Shenfu Injection on the Nuclear Translocation of HMGB 1 in LPS-induced RAW 264.7 Cells

OBJECTIVE:To investigate the intervention effect of Shenfu i njection(SFI)on the nuclear translocation of high mobility group box 1(HMGB1) in lipopolysaccharide (LPS)-induced RAW 264.7 cells. METHODS : Using LPS-induced RAW264.7 cells as objects ,the histone deacetylase inhibitor RGFP 966 as positive control ,CCK-8 assay was used to screen drug dosage,and the effects of low ,medium and high doses (3,6,12 μL/mL)of SFI on HMGB 1 nuclear translocation in RAW 264.7 cells were observed by immunofluorescence method ;mRNA expression of HMGB 1 in RAW 264.7 cells were detected by real time fluorescent PCR. Western blotting assay was used to determine protein expression of HMGB 1 and Toll-like receptor 4(TLR4);the expression of HMGB 1 were compared between nucleus and cytoplasm. The levels of HMGB 1,IL-1β and TNF-α in supernatant of cells were detected by ELISA. RESULTS :In blank control group ,HMGB1 was mainly located in the nucleus ;after LPS induction, HMGB1 migrated from nucleus to cytoplasm. Compared with blank control group , mRNA and protein (No.81760738) expression of HMGB 1, protein expression of TLR 4 in RAW264.7 cells as well as the levels of HMGB 1,IL-1β and TNF-α in supernatant of cells were increased significantly in LPS group (P<0.01). The protein expression of HMGB 1 was decreased significantly in nucleus while was in creased significantly in cytoplasm (P<0.01). After SFI treatment ,the nuclear translocation and secretion of HMGB 1 were inhibited in different degrees ;compared with LPS group ,mRNA and protein expression of HMGB 1 in administration groups ,protein expression of TLR 4 in RAW 264.7 cells of positive control group ,SFI medium- and high-dose groups as well as the levels of HMGB 1,IL-1β and TNF-α in supernatant of cells in administration groups were decreased significantly (P<0.01). In positive control group ,SFI medium- and high-dose groups ,the protein expressions of HMGB1 in nucleus were increased significantly ,while protein expressions of HMGB 1 in cytoplasm were decreased significantly (P<0.01). CONCLUSIONS :SFI may inhibit the nuclear translocation and secretion of HMGB 1 in RAW 264.7 cells,thus avoiding the activation of inflammatory pathways and the production of inflammatory factors ,so as to reduce the inflammatory response induced by LPS.
