CALR mutations are discovered in classic myeloproliferative neoplasms (MPN) as a new biomarker very recently.CALR mutations occur in about 20 ％-35 ％ essential thrombocythemia and primary myelofibrosis and are associated with some clinical features and favorable prognostic impact.This article reviews biological characteristics and clinical implications of CALR mutations in classic MPN.

OBJECTIVE: To evaluate the anticoagulation therapy regimen in one patient with dilated cardiomyopathy and severe heart failure. METHODS: Literatures and clinical practice guidelines were consulted, referring to the management of anticoagulation therapy in heart failure with sinus rhythm. RESULTS: Clinical pharmacist participated in the process of drug treatment and decided on optimal anticoagulation therapy. CONCLUSION: Individual bleeding and thrombotic risk should be evaluated to optimize the treatment of individual patients with heart failure.

Objective:To study the clinical effect of Qinghao Fuzheng Jiedu decoction on systemic lupus erythematous （SLE）. Method:A total of 109 SLE patients admitted to the Rheumatology and Immunology Department of Wuhan No. 1 Hospital from December 2019 to October 2020 were selected and divided into an observation group （55 cases） and a control group （54 cases） using the random number table. Two cases in the observation group dropped out， leaving a total sample of 53， and one case in the control group dropped out， with 53 cases finally included. Patients in the control group were treated with prednisone tablet and azathioprine. On this basis， those in the observation group further received Qinghao Fuzheng Jiedu decoction. The clinical efficacy， traditional Chinese medicine （TCM） syndrome score， TCM syndrome efficacy， immunoglobulin （Ig） G， IgA， IgM， and complements C3 and C4 of the two groups were compared. The conversion of positive antinuclear antibody （ANA） and anti-double-stranded deoxyribonucleic acid antibody （DS-DNA） titers to negative in two groups after treatment was analyzed. Result:The total clinical efficacy rate of the observation group was significantly higher than that of control group （92.45% vs 73.58%，χ²=6.692，P<0.05）. Before treatment， there were no significant differences in IgG， IgA， IgM， complements C3 and C4， and serum ANA and ds-DNA titers between two groups. After treatment， the levels of IgG， IgA， and IgM and serum ANA and ds-DNA titers in both groups obviously declined， whereas the levels of complements C3 and C4 rose （P<0.05）. Besides， the levels of IgG， IgA， and IgM and serum ANA and ds-DNA titers in the observation group were lower than those in the control group， while the levels of complements C3 and C4 were higher （P<0.05）. The negative rates of ANA and ds-DNA in observation group were significantly higher than those in control group （χ²=8.040，P<0.05）. TCM syndrome scores were decreased in both groups after treatment （P<0.05）， and the score in observation group was lower than that in control group （P<0.05）. In terms of TCM syndrome efficacy， the total effective rate of observation group was significantly increased as compared with that of the control group （94.34% vs 50.94%，χ²=25.112，P<0.05）. Conclusion:Qinghao Fuzheng Jiedu decoction is effective in treating SLE and has a certain clinical application value.

AIMTo observe the direct effect of urotensin II (U II) on the release of nitric oxide (NO) and expression of inducible nitric oxide synthase (iNOS) mRNA in human umbilical vein endothelial cells (HUVEC).

METHODSHUVEC were cultured with different concentrations of U II (10(-9)-10(-7) mol/L) for 24 hours. Then the supernatant was collected to detect the level of NO and the activity of iNOS, the expression of iNOS mRNA of HUVEC was measured by semi-quantitative reverse transcription polymerase chain reaction (RT-PCR).

RESULTSIn comparison with controls, the level of NO, the activity of iNOS and the iNOS mRNA expression increased significantly (P < 0.05).

CONCLUSIONU II may up-regulate the expression of iNOS mRNA and increase NO generation in HUVEC, it suggests that U II may relax blood vessel by activating iNOS/NO pathway.

Translational medicine is an emerging idea in current medical research area. Typically, for the purpose of bridging the gap between basic and clinical research, it not only emphasizes the urgency and necessity to break the traditional working formats, including single subject centered research team and limited cooperation among different scientific groups, but also highlights a more close and frequent interaction between basic scientist and clinician. In order to reach this goal, the theory and method of systems biology should be employed. This paper mainly focused on a central issue that how to carry out an investigation on early clinical diagnosis of xenobiotic-induced intrauterine growth retardation (IUGR) by using research concept of translational medicine and method of systems biology. Briefly, a hypothesis of common mechanism of IUGR was first proposed and subsequent validation was performed via integrating--omics (e.g. genomics, proteomics, cytomics, metabonomics/metabolomics) and molecular biology techniques. Metabonomics was further utilized to explore IUGR biomarker and establish preliminary forecasting model by bioinformatics and computational biology, which is available for early diagnosis of IUGR and make a complement to current evaluation criteria.

Studies have shown that most of the sequence in the mammalian genome is transcribed into long noncoding RNAs (lncRNAs). Their crucial roles in gene regulation are becoming a hotspot in current biomedical research. LncRNAs can control gene activities through multiple mechanisms such as: 1) direct or indirect regulation of gene expression via cis-/trans-action or function as protein baits in the nucleus; 2) affecting the stability and the translational process of mRNA; 3) functioning as competitors to regulation of microRNA; 4) binding to transcription factors. Recent studies have highlighted the significance of lncRNAs in development and diseases, and their potentials in future clinical application.

Lectures showed that the VR technique would impact and prompt the teaching, experiment, research and application of forensic pathology with the development of operation guiding system, virtual autopsy, micro-imaging technique. Because of the limitation of software, hardware and the expense, the VR technique needed to be improved and perfected.

Traditional drug development and pre-clinical tests are based on animals and involve large numbers of animals,costs and long periods. Meanwhile,inter-species differences are difficult to overcome. Human embryonic stem cells (hESCs),which can self-renew and directly differentiate to types of cells,have become a new tool for toxicity alternative testing. hESC-Based alternative testing models,such as the reproductive toxicity test system,neuro development toxicity test system and metabolic model,can be used to predict target organ toxicity and toxic mechanisms of chemicals, analyze metabolic pathways and to search for potential toxicity biomarkers, when combined with omics such techiniques as metabonomics , proteomics and genomics. Therefore, hESC-based alternative testing models have extensive application to toxicology.

Objective To evaluate the utility of flow cytometry (FCM) in diagnosis and subclassification of non-Hodgkin lymphoma (NHL). Methods The samples of lymph nodes biopsy from 59 cases clinically suspected of NHL were detected by flow cytometry; and clonal lymphocytes and their immunophenotypes were identified analyzed. The concordance between the results of flow cytometry and histopathology was analyzed. Results Among the 59 cases, flow cytometry was able to identify aberrant clonal lymphocytes in 24 of 28 NHL cases identified by histopathology, the neoplastic lymphocytes ranged from 4.28 % to 89.10 %; 23 cases were diagnosed as B-NHL and 1 case was diagnosed as T-NHL. Compared with histopathology, the accuracy of FCM was 85.71% in diagnosis of NHL. The specificity and sensitivity of FCM was 100 % and 92% in diagnosis of B-NHL. The accuracy of flow cytometry immunophenotyping in classification of 24 cases of NHL was consistent with that of histopathology. Conclusion Flow cytometry could be an ancillary technique in diagnosis of NHL by identifying aberrant clonal lymphocytes, and enable identification of B-NHL subtype.
