Objective To evaluate non operative and simple operative managements of the calcaneal fractures. Methods 97 patients with calcaneal fractures were treated by non operative management or percutaneous reduction with wires or screw fixation from April 1997 to December 2001 in our department. They were followed up for more than 2 years. 64 patients were treated with non operation, 27 were fixed with wires and 6 with screws. Results 23 patients had excellent results, 67 good, 5 fair and 2 poor. The total good and excellent rate was 92.7%. 2 patients experienced secondary arthrodesis 1 year after injury. Conclusion Although the calcaneal fractures can be treated satisfactorily with non operative management or percutaneous reduction with wires or screws fixation, further prospective and comparative studies should be done in clinic to determine whether the calcaneal fractures should be treated with open reduction and fixation or with simple operative management.

Histone deacetylase (HDAC1) has a high expression in many cancer cells and curcumin can inhibit the growth of cancer cells. This paper was designed to investigate the expression of HDAC1 of Raji cells and the effect of curcumin on their proliferation and apoptosis. Raji cells were treated with 3.125-50 micromol/L curcumin for 8-48 h and the growth inhibition rates of Raji cells were measured by MTT. The expression of HDAC1 on Raji cells were examined by mRNA, Western blot at 24 h various concertrations (1.6-50 micromol/L). Curcumin could selectively inhibit the proliferation of Raji cells in a dose- and time-dependent manner, with the inhibition rate being 52.47 %-82.18 % (P<0.01). The up-regulation of HDAC1 expression was observed within 24 h after the treatment with curcumin as shown by RT-PCR and Western blot. With the increase of concentration, the expression was down-regulated in a dose- dependent manner. It is concluded that the expression of HDAC1 plays an important role in the proliferation and apoptosis of Raji cells and curcumin can inhibit the growth of Raji cells at various concentrations and promote the apoptosis of Raji cells.

Objective To explore clinical indications and curative effects of microwave coagulation combined with submucous 5-fluorouracil (5-FU) injection in the management of early-stage lower rectal cancer. Methods Nine patients with early-stage lower rectal cancer underwent microwave coagulation with submucous 5-FU injection from January 1996 to December 2002. After the microwave treatment, a retention enteroclysis with 100~200 ml normal saline containing 1.0 g 5-FU was performed once daily for 3 days. Results In 3 patients, levels of carcinoembryonic antigen (CEA), which were preoperatively increased, were reduced below normal limits after microwave treatment. No hemorrhage or other intra- or post-operative complications was observed. Follow-up observations to January 2006 showed that all the 9 patients survived. One patient survived for 3 years and 5 months, 1 patient survived for 3 years and 8 months, and 1 patient experienced a tumor relapse at 1 year and 4 months and survived for 2 years and 9 months after a Miles procedure. Among 6 patients who survived longer than 5 years, 1 survived for 5 years and 1 month, 1 for 6 years and 3 months, 1 for 6 years and 8 months, 1 for 7 years and 6 months, and 2 for 9 years. Conclusions Surgical indications for microwave coagulation combined with submucous 5-FU injection for rectal cancer included early-stage cancer, lesions located within 7 cm away from the anus, the mass type cancer, tumor diameter at 0.5~3.0 cm or less than one third of the rectum’s circumference, well differentiated tumor, and patient’s strong anus-sparing demand. This method is minimally invasive, safe, reliable, and cost-effective.

Objective In order to probe the possible biomechanical mechanism of lumbar vertebral injuries, we studied the nonlinear stress distribution on lumbar vertebrae at 1500N axial compression force and 15Nm bending moments during hambar flexion, extension, lateral bending and axial torsion using an available nonlinear finite element (FE) model of functional spine unit. Methods Three-dimensional nonlinear FE model was created based on CT data of L3-L4 motion segment. The segment mechanical responses to different loadings were evaluated by the range of disc bulging, the stress level of fibrousrings, facet loadings and equivalent stress distribution of posterior structure. Results Compression and flexion induced disc bulging in anterior region, whereas extension and lateral bending induced disc bulging posteriorly and posterolaterally. Axial torsion caused not only higher unilateral facet stress but also the concentrated stress on posteroleteral fibrousrings stress. The loadings in extension and axial compression were lower than in torsion but higher than in lateral bending and anterior flexion. Higher equivalent stress was found when compression and extension loadings were applied to the pedicle of vertebral arch and interarticularis. Conclusion Intervertebral disc beared higher compressive than tensile. The posterolateral regions of lumbar vertebrae were prone to be deformity and at higher risk of fibrousring failure. Stress failure could be occured when facets were overloaded with axial torsion, extension and extension force due to the importance of facets during lumbar movement.

Objective To investigate the feasibility of laparoscopy assisted Meckel’s diverticulum resection in children. Methods Meckel’s diverticulum resection was performed under laparoscope in 12 children from October 2000 to April 2002.Two 3 mm incisions were made on the McBurney point and the midpoint between the umbilicus and the pubis, respectively.Three 3 mm trocar were introduced. When the diverticulum was found, the incision was prolonged to 2 cm long along the inferior border of the umbilicus. The lesion was removed externally and the intestine was anastomosed. Results All operations were completed smoothly under laparoscope. The intraoperative blood loss was 2~3 ml and the operation time was 1 2~2 5 h.The diverticulum was 1 cm?2 cm~3 cm?4 cm in size. The patients were dismissed 5~7 days after surgery, without obvious scar formation. Conclusions Laparoscopic Meckel’s diverticulum resection is feasible, with the advantages of the minimal invasion, rapid recovery and unobvious scars.
