According to the property of military health service positioning , armed forces all over the world invariably put military health service into combat team ranks .They want to strengthen the ties between the health system and the military system by adjusting the health command and management system in order to meet the needs of multi -level and multi-directional wartime support in the future .This paper summarizes the development of foreign ( the US.Germany and Russian Federation) and our military health command and management system , and make a detailed comparison .It may be a practical and valuable research for the current military health reform .
2.Clinical Observation of Continuous Hypoperfusion of Ozone Water Combined with VSD in Deep Bedsore

Objective To explore the clinical effects and application value of VSD combined with continuous hypoperfusion of ozone water in treating deep bedsore.Methods 40 patients with deep bedsore admitted to our hospital from June 2014 to December 2016 were randomly divided into the observation group (n=20) and the control group (n=20).The control group was treated with VSD while the observation group was treated with VSD combined with continuous hypoperfusion of ozone water.Results Compared with the control group, the wound healing time in the observation group was shortened significantly (P<0.001).The positive rate of bacteriology inspection about the wounds was decreased significantly (P<0.05).The density of capillary blood under microscope was increased significantly (P<0.05).Conclusion The clinical effects of VSD combined with continuous hypoperfusion of ozone water in treating deep bedsore is better than VSD alone apparently.It is worth clinically popularizing.
3.Study on Willingness to Pay for Typhoid Vi Vaccine in Lingchuan County,Guangxi Zhuang Autonomous Region

Objective To assess willingness to pay(wtp)for typhoid vi vaccine in typhoid epidemic area in Guangxi Zhuang Autonomous Region and to provide evidence for introduction of the vaccine.Methods Applying the method of wtp was investigated in typhoid epidemic area,the curve of "price-accept ratio model"was drawn up with Probit regression.Results The formula of "price-accept ratio model" was described as following:Probit(p)=0.88952-0.46296X.The WTP for typhoid vi vaccine was $10.41,with a 95% confidence interval of $6.67~16.24.Conclusions WTP for typhoid vi vaccine was around $10 in Guangxi typhoid epidemic area.The Contingent Valuation Method was applicable in the investigation of WTP.
4.Radiation-Induced changes in expression of genes related to DNA damage response in lymphocytes of human peripheral blood

Objective To detect the expression of DNA damage response genes induced by radiation in human peripheral blood lymphocyte,and to explore the new biomarkers of radiation.Methods The human peripheral blood cells were irradiated to X-rays at different doses of 0,1,2,3,4,and 5 Gy.The quantitative real.time qPCR wag used to detect the expressions of cyclin-dependent kinase inhibitor l a gene(Cdknl a)and growth arrest and DNA damage inducible gene(Gadd45a)in lymphoeytes at 4 and 24 h post-irradiation,respectively.The method of CB mieronucleus was used to determine the change of micronucleus ratio.Results The expression of Cdknl a in peripheral blood lymphocytes wag increased significantly at 4 and 24 h post-irradiation to 0-5 Gy.reached the peak at 4 Gy and began to decrease at 5 Gy,which showed a dose-dependent manner(r=0.946,0.975,P<0.05).Similarly,the expression of Gadd45α in human peripheral blood lymphocytes was also increased significantly at 4 and 24 h post-irradiation to 0-5 Gy in a dose-dependent manner,while the expression of Gadd45a at 4 h wag higher than that at 24 h(r=0.936,0.797,P<0.05).The ratio of micronuclei wag increased significantly at 4 and 24 h post-irradiation to 0-5 Gy(r=0.990,0.984,P<0.05).Conciusions Cdknl a and Gadd45α expression could be increaged significandy at 4 and 24 h post-irradiation to 0-5 Gy,showing a good linear relationship.which might be candidate for radiation biological dosimeter.
5.High frequency oscillatory ventilation and conventional mechanical ventilation in the treatment of infants with severe respiratory syncytial virus pneumonia: a randomized controlled trial

Objective:To compare the clinical efficacy of high frequency oscillatory ventilation (HFOV) and conventional mechanical ventilation (CMV) in the treatment of infants with severe respiratory syncytial virus (RSV) pneumonia.Methods:A prospective randomized controlled trial was conducted. The infants with severe RSV pneumonia who received invasive mechanical ventilation admitted to intensive care unit (ICU) of Children's Hospital Affiliated to Capital Institute of Pediatrics from January 2018 to December 2019 were enrolled. According to the order of admission, each infant was assigned to HFOV group or CMV group by random number table. The basic data, pediatric critical score, blood gas analysis, ventilator parameters, oxygenation index [OI, OI = mean airway pressure (Pmean)×fraction of inspired oxygen (FiO 2)/arterial partial pressure of oxygen (PaO 2)×100], duration of mechanical ventilation, length of ICU stay, complications, prognosis, use of muscle relaxants and vasoactive drugs and other clinical indicators of the two groups were recorded. Results:A total of 28 infants were enrolled in the analysis, including 15 infants receiving CMV and 13 infants receiving HFOV. There were no significant differences in age, body weight, pediatric critical score and OI before enrollment, type Ⅱ respiratory failure, multiple organ dysfunction, basic diseases and laboratory examination indexes before enrollment between the two groups. Six hours after enrollment, compared with CMV group, heart rate (HR), respiratory rate (RR), case of transcutaneous oxygen saturation (SpO 2) decrease, case of HR decrease, case of cardiopulmonary resuscitation (CPR) and OI in HFOV group were significantly decreased [HR (bpm): 130 (125, 138) vs. 144 (140, 160), RR (times/min): 35 (34, 38) vs. 40 (35, 45), SpO 2 decrease (case: 1 vs. 10), HR decrease (case: 0 vs. 6), CPR (case: 0 vs. 4), OI: 6.5 (4.4, 8.9) vs. 9.3 (8.0, 12.8)], while case of use of muscle relaxants (case: 3 vs. 0) and volume of 7-day positive fluid balance [mL/kg: 167.1 (113.8, 212.6) vs. 90.8 (57.8, 112.7)] were significantly higher, the differences were statistically significant (all P < 0.05). There was no use of blood purification treatment, no severe complications such as pneumothorax and intracranial hemorrhage, and no death within 28 days in the two groups. Conclusion:Compared with CMV, HFOV in the treatment of infants with severe RSV pneumonia can improve the oxygenation level and clinical physiological indexes earlier, reduce the incidence of adverse events such as HR, SpO 2 decrease and CPR, increase the use of muscle relaxants and the positive fluid balance, and do not increase the incidence of severe complications such as pneumothorax and intracranial hemorrhage, so its clinical application is safe.
6.Evaluation of fractionated stereotactic radiotherapy for residual lesion after the first course of radiotherapy for nasopharyngeal carcinoma

Objective To analyze the long-term results of fractionated stereotactie radiotherapy(FSRT)for the local residual lesion after the first course of radiotherapy for nasopharyngeal carcinoma.Methods From July 1997 to July 2002,46 patients were treated with FSRT.According to the 1992 Fuzhou staging system,the number of patients was 1,6,30 and 9 with stage Ⅰ,Ⅱ,Ⅲ and Ⅳ disease,respectirely;3,11,27 and 5 with T1,T2,T3 and T4 tumor,respectively;14,16,12 and 4 with N0,N1,N2and N3 disease.Radiotherapy was delivered to tumors with the total of dose 68-70 Gy in 7-8w.Chemotherapy(2 cycles of PVF or POF)was given to the patients with stage Ⅲ and Ⅳ a disease.FSRT was given to the residual disease with the total dose of 18-24 Gy in 3 fractions with an interval of 3-7 days.The reference dose line was 70%-90%.Resuits CR and PR rates in this group were 61%and 39%,respectively.The overall survival rates of each year from 1- to 5-year were 100%,87%,83%,78%and 76%.The 1-,3- and 5-year disease-free survival rates were 100%,93%and 89%;The distant metastasis-free survival rates were 100%,85%and 79%;The local-regional control survival rates were 100%,94%and 91%.Seventeen patients who died during the follow-up period were 1 for local cervical lymph node recurrence,2 for fatal nasopharyngeal hemorrhage,4 for local nasopharynx recurrence,and 10 for distance metastases. Conclusions Fraetionated stereotactic radiotherapy is safe and effective for the patients with residual lesion of nasopharyngeal carcinoma at the primary site after radiotherapy.The optimized fractionation and total dose requires the further investigation.

Immunoglobulins ( Ig ) , also called antibodies, are important components in humoral -mediated immunity. Ig can bind with their receptors, called immunoglobulin receptors ( IgR ) , trigger biologic activities respectively. Different sub-types of Igs show different function. And IgRs have been treated as therapeutic targets in inflammation and immunity related dis-eases for many years. This article reviewed the recent progresses in the study of IgR function and its therapeutic role in inflamma-tion and immunity related diseases.
8.Deep venous thrombosis after greater saphenous vein ligation and stripping

Objective To report deep venous thrombosis (DVT) after greater saphenous vein ligation and stripping and to evaluate diagnosis,treatment and prophylaxis. Methods The clinical characteristics, diagnosis and treatment of 12 inpatients with postoperative DVT were analyzed retrospectively. Results Of these 12 cases there were 7 cases of central type DVT,2 cases of peripheral DVT,and 3 cases of mixed type DVT.Secondary pulmonary embolism was complicated in 2 cases.Clinical symptoms in these 10 cases of proximal DVT were all severe.Catheter-directed thrombolysis(CDT) through the ipsilateral popliteal vein with protective(IV)CF was applied for these 10 cases.Of 10 cases,femoral vein was found ligated in 1 case,which was repaired under open surgery. Residual greater saphenous vein thrombus extending into deep vein was proved and managed by successful thrombolectomy in one case.Cockett syndrome were found as the causes in the other 8 cases,6 cases were treated with balloon dilatation angioplasty and endovascular stenting of the iliac vein.The 2 cases of with distal DVT were treated by anticoagulation therapy.All patients were cured. Conclusions Cockett syndrome,femoral vein injury and too long residual greater saphenous vein are common causes of DVT after greater saphenous vein ligation and tripping.Careful selection of cases,correct diagnosis,standard operative manipulation,early ambulation were all important in the prevention of DVT after great saphenous vein varix procedure.
9.Real-time Control and Information Management in Monitoring of Invasive Procedure

OBJECTIVE To use the software for hospital infection control in our hospital to improve monitoring of nosocomial infection in invasive nursing procedure and reduce incidence rate of nosocomial infections.METHODS The software for hospital infection control was used in this prospective investigation to collect common information of invasive nursing procedure,institute intervention measure to invasive nursing procedure,and observe the(occurrence) of nosocomial infection.RESULTS Element administration,process administration,and monitoring(administration) were used to real-time control in invasive nursing procedure,thus fasten information transfer and optimize(performance) flow-sheet of nosocomial infections administration.The software usage could accurately(provide) the(information) of nosocomial infection in-time,and feed-back rapidly.CONCLUSIONS Whole process(control) in invasive nursing procedure can discover and solve problems,thus improve efficiency and effectiveness for preventing and controlling nosocomial infection.
10.Application of Software for Drug-resistance Message Input System in Pathogen Detection

OBJECTIVE To improve the level of pathogen detection with the software for drug-resistance message input system. METHODS The software which manufactured by the First Affiliated Clinical hospital of General Hospital of PLA,can detect, analyze,calculate and inquire to samples of inpatients. RESULTS The software may classify the drug-resistance with different partner statistic means.Moreover,it can calculate the sample detection rate and positive rate. CONCLUSIONS The software may provide efficiency and effectiveness for drug-resistance detection.
