1.Anti-thrombopoietin antibodies in systemic lupus erythematosus with thrombocytopenia

Objective To investigate the existence and significance of circulating autoantibodies to thrombopoietin (TPO) in sera from patients with systemic lupus erythematosus (SLE).Methods Fifty-six consecutive patients with SLE,twenty patients with idiopathic thrombocytopenic purpura (ITP),and twenty normal individuals were involved in this study.The characteristics of all patients with SLE were analyzed.Antibodies to TPO were detected using an enzyme-linked immunosorbent assay (ELISA).For normal distribution count data,x2 test or Fisher exact test was used,t test was used for measurement data,and Wilcoxon's rank test for non-normally distributed data which was represented by M(Q).Results A higher frequency of antibodies to TPO were observed in SLE patients than those in healthy controls (39.3% vs 0,x2=11.058,P=0.001).Moreover,anti-TPO antibodies were detected in 15 (57.7%) of 26 SLE patients with thrombocytopenia,compared with that in 7 (23.3%,x2=6.894,P=0.009) of 30 patients without thrombocytopenia.Furthermore,the patients with antibodies to TPO exhibited more severe thrombocytopenia (t=3.010,P=0.004).Finally,anti-TPO antibodies seemed more likely to occur in patients with arthritis (x2=5.959,P=0.015),anti-dsDNA antibodies (x2=5.959,P=0.015).Conclusion The high incidence of antibodies to TPO in SLE patients with thrombocytopenia suggests that anti-TPO antibodies might play a vital role in SLE patients developing thrombocytopenia.Thus,there might be a clinical value by detecting anti-TPO antibodies in SLE patients with thrombocytopenia.
2.Combination of OCT and FFA for the research of pathogenesis of central serous chorioretinopathy

Background The diagnosis of central serous chorioretinopathy (CSC) is mainly dependent onfluorescine fundus angiography (FFA). However, the combination of optical coherence topography (OCT) with FFA offers a new approach to the research of the pathogenesis of CSC. Objective This clinical study was designed to study the combined application of the FFA and OCT for the research of the pathogenesis of central serous chorioretinopathy (CSC). Methods Forty-four eyes of 44 patients with CSC were included in this study with 36 cases of males and 8 cases of female. The patients were aged 39.3 ± 5.3 years and the visual acuity was 0. 64 ±0. 27. FFA and OCT examinations were performed in all patients and the FFA images were imported into the Topcon 3D OCT 1000 device to locate the conformity of OCT lesions with the leakages of FFA. The neuroepithelial layer thickness at the fovea and the height of the neuroepithelial layer detachment were measured using 3-D OCT. Results OCT showed serous REP detachment in 34 eyes (77.3%) and rough surfaces of RPE in 10 eyes (22. 7% ). In thirtyfour eyes with RPE detachment, the OCT lesions and FFA leakage spots conformed to the same locations in 31 eyes, but the other three eyes did not. The mean foveal neuroepithelial thickness was (138.5±19.4) μm in CSC eyes and that of normal eyes was ( 131.35±5. 01 ) μm ,showing a significant difference between them( t=0. 39 ,P>0. 05 ). The mean height of neuroepithelial detachment was (263.3 ± 126.7 ) μm in CSC eyes. Conclusion RPE detachment occurs in CSC eyes and further induces macular neuroepithelial detachment. Leakage lesion of fluorescine corresponds to RPE detachment. CSC without RPE detachment may be related to the increase in RPE permeability. OCT can accurately measure the thickness of the macular neuroepithelial layer and the height of the neuroepithelial detachment.
4.Clinical significance of serum myocardial enzymes and CSF LDH detection in adults with intracranial infection

Objective To investigate the changes and clinical significance of serum lactate dehydrogenase (LDH),creatine kinase (CK),glutamate pyruvate transaminase (AST),and cerebrospinal fluid lactate dehydrogenase (CSF LDH) in adult patients with acute central nervous system infection.Methods The levels of myocardial enzymes (AST,LDH,and CK) in serum of 96 adult patients with acute intracranial infection in 7days and 39 healthy people were measured by Beckman automatic biochemical analyzer and enzyme rate assay,and CSF LDH level in 96 patients were measured simultaneously.Results (1) The serum myocardial enzymes (LDH,CK,and AST) of intracranial infection group (47 cases with viral encephalitis,30 cases with tuberculous meningitis,and 19 cases with purulent encephalitis) were significantly higher than those of normal control group (P <0.01).(2)The myocardial enzymes (LDH,and AST ) of patients with cerebral functional disorder were significantly higher than those of patients with normal cerebral function (P <0.05).(3)The levels of serum AST,LDH,and CK in the virus encephalitis group,serum AST and LDH in the purulent encephalitis group,and serum LDH in the tuberculous meningitis group were significantly higher than those in the control group (P < 0.01).The CSF LDH level in the viral meningitis group was prominently lower than that in the tuberculous encephalitis group and purulent encephalitis group,respectively (P <0.01).(4) No correlations were found between CSF LDH and serum myocardial enzymes (P >0.05).Conclusions (1)There is significant change in the levels of serum LDH,CK,AST,and CSF LDH of adult patients with acute intracranial infection,especially in infected patients with cerebral functional disorder,and the change of LDH is the most obvious.(2)The levels of serum myocardial enzymes and CSF LDH are helpful to the differential diagnosis of intracranial infection in early stage,and judging the severity of the illness.
5.Treatment of tibial plateau fracture with minimal invasion spinal surgery in 36 cases

Objective To investigate the clinical results of minimal invasion spinal surgery (MISS) in treating tibial plateau fractures to provide best procedure for such treatment.Methods The study involved 36 cases with tibial plateau fractures resulted from traffic injury,fall-from-height injury and strain injury including 24 males and 12 females at age of 19-65 years (average 42.6 years).According to Schatzker classification,there were seven cases of typeⅠ,five typeⅡ,five typeⅢ,five type IV,sev- en typeⅤand seven typeⅥ.All cases were treated by different methods of MISS.Results Follow up for 8-24 months in 35 cases showed fracture healing within 3-4 months,without severe complications such as poor wound healing,infection or osteofascial compartment syndrome.Meanwhile,no traumatic knee osteoarthritis,inversion or eversion of the knee were found.The mean range of knee motion was 94?,with fine knee function in 32 cases (91.4%) according to Merchant score.Conclusions Treatment of tibi- al plateau fractures should adopt limit incision,indirect reduction and biofixation to take place widespread exposure and rigid plate fixation.MISS is safe and effective for tibial plateau fractures.
6.Efficacy of chronic disease management in the treatment and quality of life for rheumatoid arthritis

Objective To explore the effects of chronic disease management in the treatment and quality of life for rheumatoid arthritis (RA).Methods A total of 160 patients with active RA were selected from June 2011 to August 2012 and randomly assigned into experimental and control groups (n =80 each).Based upon oral medication,the experimental group had chronic disease management for 6 months.Before and after therapy,DAS28 score,erythrocyte sedimentation rate (ESR),ACR50 and quality of life score were measured.Results The number of regular medication in experimental group was significantly more than that in the control group (68 vs.37,x2 =26.625,P =0.000).The inter-group difference of DAS28 score reduction was statistically significant (2.46 ± 1.09 vs.3.68 ± 1.65,t =1.655,P =0.002).After intervention,ESR significantly decreased in experimental group (66.09 ± 41.98 vs.21.27 ± 19.61,t =1.655,P =0.029) and the reduction of control group was not statistically significant (t =1.976,P =0.059).The number of ACR50 in experimental group was more than that in control group (60 vs.44,x2 =4.314,P =0.038).The score of life quality of experimental group increased dramatically (56.58 ± 20.24 vs.63.71 ± 18.35,t =1.655,P =0.013) and it was significantly higher than control group (63.71 ± 18.35 vs.48.76 ± 19.88,t =1.906,P =0.020).Conclusion Chronic disease management can improve the life quality and clinical indices of RA patients.Thus it may be widely applied clinically as an adjunct treatment.

8.Effect of Underwater Treadmill Training on Hemiplegics after Stroke

Objective To investigate the effect of underwater treadmill training (UWTT) on patients with hemiplegia after stroke. Methods Based on routine rehabilitation, 15 hemiplegic patients after stroke were treated with UWTT system 20 min/d, 5 times a week for 4 weeks. Walking distance and speed in water were tested before and after UWTT. Dynamic balance function and static balance function were also assessed by Active Balancer assessment. Results All the items improved significantly after treatment (P<0.01), except deflection center displacement of X and Y axis (P>0.05). Conclusion UWTT can improve the walking ability and balance function of hemiplegic patients after stroke.

Doctor WANG Zhi-zhong in the Southern Song Dynasty proposed the acupoint view of "location of disease", which explained the connotation of acupoints from the angle of clinic. Its meaning included two levels, one level meant pathological change on the body surface, that was the location of acupuncture diagnosis-treatment, and the other one indicated that the body surface which was the reflecting point of pathological change on the distal area or inside the body was the location of acupuncture diagnosis-treatment. The specific connotations of clinical acupoints were: location of pathogenic factors or reflection of pathogenic factors, regularity between acupoints un- der disease and specific organ, morphological differences and positioning variability after acupoints under disease, and acupoints examination, diagnosis and treatment.
