3.Research Progress in Studies on Antitumor Mechanisms of Leech

Leech has a broken blood stasis effect and has been used to eliminate a variety of stasis diseases. With the development of modern pharmacology study, in recent years, leeches are widely used in clinical cancer patients, and its anti-tumor effects have been confirmed by numerous clinical and experimental studies. Based on the latest domestic and foreign literature analysis and summary, this article discussed the antitumor mechanisms of leech from the following aspects:inhibition of tumor growth and proliferation, induction of tumor cell apoptosis, inhibition of tumor angiogenesis, anti-platelet aggregation and anti-clotting effect, improvement of immunity, and anti-tumor multi-drug resistance. It also pointed out deficiencies and controversy in current studies, with a purpose to provide ideas for further researches and clinical applications.
4.The effects of autoimmune regulator(AIRE) TLRs on TLRs expression

Objective:To explore the effect of autoimmune regulator to TLRs expressions on peripheral antigen presenting cells(APC).Methods:①pEGFPC3-AIRE plasmid was transfected with liposome.②Confocal microscopy was used to observe the effect of transfection.③RT-PCR assay was used to detect the expressions of AIRE and TLR1-9 in RAW264.7cells at 36,48,72,96 h after transfection.Results:①The plasmid was transfected into RAW264.7 cells successfully,and the efficiency of transfection was 60~70%.②AIRE transfected RAW264.7 cells were achieved,and the best time was 72 h.③At 72 h after transfection,the expressions of TLR1,4,5,9 increased,and TLR3,7,8 reduced.The expression of TLR2,6 increased at 96 h.Conclusion:AIRE may regulate the immune response by control TLR expression in APC.It maintain the effective response to pathogen and tolerance state to self tissues through the effects to different TLRs.
5.Isolation and purification of breast cancer stem cells by suspension culture combined with chemotherapeutic agents

Background and purpose:The scarcity in quantity and continuous differentiation ability severely compromise the isolation and purification of cancer stem cells(CSCs).The existing sorting methods can only enrich CSCs as a heterogeneous population.However,more primitive CSCs closer to the source still have not been identified.Based on the CSC hypothesis,we hereby explored the possibility of isolation,culture,purification and identification of murine breast cancer stem cells in suspension culture combined with anticancer regimens. Methods :TM40D murine breast cancer cells were cultured in serum-free medium.The expression of CD44 + CD24-was measured by flow cytometry.Cells from the passage of TM40D cells with the highest expression of CD44 + CD24-were treated in combination with anticancer agents pacilitaxel and epirubicin at different peak plasma concentrations(PPC) for 24 hours,and then maintained under suspension culture.The rate of apoptosis was examined by flow cytometry with Annexin-V FITC/PI double staining method.Selected cells in different amounts were injected subcutaneously into BALB/C mice to observe tumor formation. Results :Cells of passage 10 in suspension culture had the highest percentage of CD44 + CD24-(about 77 percent).As few as 100 cells in 0.35 PPC could generate tumors in BALB/C mice. Conclusions :TM40D cell line contains breast cancer stem cells.This study suggested that suspension culture combined with anticancer regimens is a feasible approach for screening tumor stem cells.
6.A study on the anatomy of the transcorpus callosal ventricle approach to the thalamic region using the mi- croscope and neuroendoscope

Objective To investigate the anatomic landmarks during the exposure of thalamus via the transcorpus callosal ventricle approach between microscope and endoscope to provide an anatomic foundation for clinical application. Methods The transcorpus callosal ventricle approach to expose the thalamus was simulated in selected 6 (12 sides) red and blue latex-perfused cadaver head specimens. The anatomic structures of four stages-interhemispheric, septum pellu?cidum cavity, lateral ventricle and third ventricle were examined by microscope and endoscope and relevant anatomic date was obtained during the process. Results Both microscope and endoscope could show the anatomic structures clear?ly during the interhemispheric and septum pellucidum cavity stages. The major landmarks of the interhemispheric cavity included callosal margin artery, cingulate sulcus, pericallosal artery and corpus callosum, and landmarks of septum pellu?cidum cavity included the septum pellucidum and body of fornix. Lateral ventricle stage-the major landmarks contained foramen of monro, septum vein, thalamus striatum vein, choroid plexus, body of fornix and body of caudate nucleus. The blind field under microscope such as anterior part of frontal horn (25.7mm±1.7mm vs. 14.2mm±1.2mm, P<0.05), lateral part (1/3) (12.1mm ± 0.7mm vs. 7.0mm ± 0.9mm, P<0.05) and posterior part (2/5) (28.8mm ± 1.4mm vs. 18.7mm ± 1.4mm, P<0.05) of thalamus could be made up by endoscope. Third ventricle stage-neither microscope nor endoscope could show the medial part of thalamus effectively due to the restriction of fornix and internal cerebral vein. Conclusions The anatomic landmarks can be identified during the exposure of thalamus via the transcorpus callosal ventricle approach. The coordination of microscope and endoscope can be helpful to identify eloquent structures and make up blind surgical field.
7.Treatment of comminuted tibia and fibula fractures with one arm external fixator with skeletal traction of calcaneus

Object To report the one arm external fixator with skeletal traction of calcanus in treatment of comminuted fractures of tibia and fibula. Method 186 cases of fractures of tibia and fibula were treated by the one arm external fixator with skeletal traction of calcaneus. 42 cases were open fractures, and 142 cases were closed fractures.185 cases were followed up. Results 183 cases of fracture united, and the average healing time was 6 months. By Johner Wrichs score 113 were rated as excellent, 69 good, 2 poor. The infection rate was 17.5%. Conclusion It is convenient and effective to use one arm external fixator with skeletal traction of calcaneus to treat comminuted tibia and fibula fractures.
8.Application of multiplex amplification of STRs for monitoring survival of engraftment after bone marrow transplantation

Objective To evaluate the application of fluorescent multiplex amplification of short tandem repeat(STR)for monitoring survival of engraftment after bone marrow transplantation(BMT).Methods Three STR loci named D12S391,D18S865 and D20S161 in 56 cases were detected by fluorescent multiplex amplification.PCR products were separated and typed by DNA Sequencer.Results The genotypes of STR in 52 recipient after bone marrow transplantation were completely identical with those of the donors.In another 4 cases the evidences of mixed chimerism were observed.Conclusion The system of fluorescent multiplex amplification of STRs exhibited high capacity of discrimination and low cost.Its application in the detection of STR after BMT is reliable,sensitive and simple.Combined with the clinical manifestation it can be used to evaluate the effect of BMT.
9.Advances in Study on Helminths in Treatment of Inflammatory Bowel Disease

Inflammatory bowel disease (IBD) is a group of intestinal chronic non-specific inflammatory disease with unknown etiology, and includes ulcerative colitis (UC) and Crohn''s disease (CD).Studies have indicated that environmental factor, genetic factor, disorder of immunity are associated with the pathogenesis of IBD.More and more basic and clinical studies focus on treatment with helminths in IBD.The accumulated evidences have shown that helminths infection has beneficial effects on the alleviation of IBD and is relatively safe.This article reviewed the advances in study on helminths in the treatment of IBD.
10.The impact of pregnancy on differentiated thyroid cancer

Objective To study the impact of pregnancy on differentiated thyroid cancer. Methods Retrospective comparative analysis were made between pregnancy-related thyroid cancer patients ( including nine months before delivery and one year after delivery) and the age-matched non-pregnancy-related thyroid cancer patients treated in the Department of head and neck, Fudan University Shanghai Cancer Center. Results There were 125 cases of pregnancy-related thyroid cancer patients from 1998 to 2008, including 28 cases (22.4%) of neck recurrence and metastasis after the surgery of thyroid cancer, meanwhile there were 691 cases of the agematched non-pregnancy thyroid cancer patients, including 90 cases ( 13% ) of neck recurrence and metastasis after the surgery of thyroid cancer. The rate of neck recurrence and metastasis after the surgery of thyroid cancer in the pregnancy-related group was higher than that in the non-pregnancy related group ( P < 0.05). The maximum diameter of tumor in the pregnancy-related group and the non-pregnancy group is 2. 1 cm and 2.6cm respectively.There were 19 cases of multi-focal cancer ( 19.6% ) in the pregnancy-related group and 58 cases (9. 6% ) in the non-pregnant group, P <0.05. There were 23 patients (23.7%) of extrathyroid invasion in the pregnancy-related group and 65 patients ( 10.8% ) in the non-pregnant group, P <0.05. There were 67 cases (53.6%) and 35 cases (28%) of the central and lateral cervical lymph node metastasis in the pregnancy-related group respectively and 296 cases (42.8%) and 117 cases (16. 9% )in the non-pregnant group, P <0.05. There were 19 cases (15.2%) of extralymph invasion in the pregnancy-related group and 32 cases (4.6%) in the non-pregnant group, P < 0.05. Conclusions The incidence of Extrathyroid invasion, multi-focal cancer, cervieal lymph node metastasis and extralymph invasion of the differentiated thyroid cancer is higher in pregnancy-related patients.Pregnancy may deteriorate the prognosis of thyroid cancer. For patients after the surgery of thyroid cancer, pregnancy may increase the risk of neck recurrence and metastasis. However, pregnancy does not affect the stage of thyroid cancer and the prognosis is still good.
