4.Validity and reliability of the simplified Chinese version of modified Barthel index for Chinese stroke patients

Objective To study the validity,reliability and sensitivity of the simplified Chinese version of modified Barthel index (MBI) in evaluation of activities of daily living(ADL) of Chinese stroke patients. Methods Forty Chinese stroke patients (26 males and 14 females,aged 59.65±11.68 years) participated in the study.The duration of the disease averaged 21.20±19.44 days.All the patients were evaluated for three times with MBI.Barthel Index (BI) and the motor component of Functional Independence Measure (M-FIM)independently by twophysiatrists.Spearman correlation coefficients between MBI and Barthel index (BI),between MBI and M-FIM were used to test the validity of MBI.Intraclass correlation coefficients (ICC) were used to test both interrater and intra rater reliability of MBI.Sensitivity of MBl was analyzed with the Wilcoxon signed rank test and paired t-test.Results MBI was highly correlated with BI in each item and total score(r=0.816~1.000,P<0.01),and with M-FIM in total score(r=0.935~0.981,P<0.01).There were high reliability with MBI in the results collected by same assessor and different assessors(ICC=0.866~0.997).Furthermore,MBI was more sensitive than BI in majorities of its items and its total scores as well.Conclusions MBI has good validity and reliability as well as sensitivity in assessment of ADL of Chinese stroke patients.
5.Changes of AQP2 expression in the kidney of rat sepsis model

Objective To investigate the expression and the significance of AQP2 in the kidneys of septic rats. Methods Sixty-four Wistar rats of 6 weeks were randomly divided into two groups: sham group and the septic model group. Septic models were made by cecum ligation perforation (CLP). Fach group was divided into 3-hour,6-hour,12-hour and 24-hour subgroups,with 8 rats in each. The urine,blood and kidney sampies were collected. The urine volume ,urine osmotic pressure and renal function were observed. The expressions of AQP2 protein in rats' kidneys were determined by using immunohistochemical method and the expressions of AQP2 mRNA with real-time PCR. The pathological changes of the kidneys were observed under light microscopy. Results After CLP,urea volume of Wistar rats in septic model group decreased and urine osmotic pressure increased at 3 hours; urea volume gradually increased and urine osmotic pressure decreased at 6 hours; Serum Cr and urea nitrogen began to increase at 6 hours and reached the peak at 24 hours. The longer the time lasted after CLP,the more serious the injury of kidney became,which was mainly manifested as that the space between the epithelial cells of the tubular wall disappeared,nucleus disappeared,glomerular loops fused,and cell structure was unclear. Renal mesenchymal was infiltrated with inflammatory cells. AQP2 mRNA expression increased at 3 hours,decreased at 12 hours and 24 hours ,AQP2 protein expression decreased at 12 hours,with the lowest at 24 hours,which were significantly different from those of the sham groups(P<0.05). Conclusion AQP2 may regulate the water metabolism of septic rats' kidney. The decreased AQP2 protein expression is one of the main pathological mechanisms that results in dysfunction of urine concentration in sepsis.
6.Validity and reliability of Wolf motor function test on assessing the upper extremities motor function of Chinese stroke patients in acute stage

Objective: To study validity and reliability of Wolf motor function test (WMFT) on assessing the upper extremities motor function of acute stroke patients so as to guide clinical application. Method: Twenty-three stroke patients in acute stage participated in the study. Patients were assessed twice with WMFT and the upper extremities motor function test of Fugl-Meyer movement assessment (U-FMA) within one week. The correlation between the assessment results of WMFT and U-FMA were analyzed to study the validity and reliability of WMFT. Result: WMFT was highly correlated with U-FMA in total scores (r =0.922, 0.929, P<0.001). There were high correlation in all items of WMFT between two assessment sessions. The intraclass correlation coefficients (ICC) were 0.989, 0.997 for intrarater and 0.980, 0.991 for interrater. Conclusion: The WMFT is valid and reliable on assessing upper extremities motor function of stroke patients in acute stage.
7.ANTIINFLAMMATORY, ANALGESIC & ANTICANCER EFFECTS OF FERULOFEN

Antiinf lammatory effects of Ferulofen ( FL ) were shown by im or ip on xylene-induced swellingof mice ear, carrageenin produced edema of hind paw in rats, and decreased volume of pleural exud- ate on carrageenin-induced acute pleurisy of rats, FL also inhibi-tied the proliferating granuloma by croton oil. The above dose r-ange in 50-100mg/kg im or ip which presented a good dose-effect relationship, FL also inhibited the mice writhing caused by H AC & tail-flick response to 1 light irradiation, suggesting its strong analgesic effect. In addition, its anticancer effect in vitro ( carcinoma of Ehlrich ascitis)was better than that in vivo ( Sl80 sarcoma) in mouse. So FL may be considered to be a promising new type of non-steroidal antiinflammatory & anticancer drug ,worthy of further reseaching & exploring in the future.
8.The Long-term Efficacy Observation of Whole-breast External Beam Radiation Therapy after Breast Conserving Surgery Coupled with Brachytherapy or Local Electronic Boost Radiation

Objective To summarize the long term effect of the whole-breast external beam radiation therapy after breast conserving surgery coupled with brachytherapy or local electronic boost radiation. Methods The survival rate and cosmetic results were retrospectively analyzed for 96 patients with early stage breast cancer received breast conserving surgery and hormone therapy from January 1995 to December 2003 in Yunnan Tumor Hospital. The enrolled conditions included single lesion, tumor maximum diameter ≤ 3cm, microscopically negative surgical margin, negative axillary lymph node dissection, and pathological type of invasive ductal carcinoma. 53 cases with comprehensive follow-up data were selected. Among them, 23 cases were treated with whole breast tangent conventional fractionated irradiation: 46-50 G/23-25 fractions, local tumor bedarea with high dose-rate brachytherapy: 8-10 Gy/1-2 fractions in postoperative,and 30 cases in control group were treated with electron beam irradiation of 10 Gy/5 fractions. The efficacy and cosmetic results were observed in every three months within two years,in every six months from three to five years, and in every year five years later. Results All the patients were followed up for 217 months (average follow-up 165 months), of which 2 patients died of distant metastasis 62 months and 158 months later after treatment. The excellent cosmetic result was more than 78%, and no local recurrence was observed in the group. The 3-,5-,10-and 15-years survival rates were 100%,100%,95.7%and 88.3%,respectively. In control group,3 patients were died of distant metastasis 46 months,97 months and 87 months later after treatment,also with excellent cosmetic results. There was only one case of local recurrence,and the local control rate was 97.0%. The 3-,5-,10-and 15-years survival rates were 100%,96.7%,93.6%and 82.3%, respectively. There was no significant difference in local recurrence and survival rates between the two groups ( >0.05) . Conclusion Both radiation brachytherapy and electronic boost have the same effect and good cosmetic results. The radiation brachytherapy shows a high quality of life and a short treatment time.
9.Sustained negative pressure drainage for treatment of severe maxillofacial and neck space infection.

OBJECTIVEThis study observed the curative effect of sustained negative pressure drainage application on treatment of severe maxillofacial and neck space infection.
METHODSIncision and drainage were performed to treat 18 patients with severe maxillofacial and neck space infection. A small incision was made on the site of the most obvious swelling or fluctuations, and localized negative pressure was applied with a drainage device on the wound during suturing.
RESULTSAmong the 18 patients, 14 were healed, whereas 4 underwent dehiscence of the wound after the operation. Negative pressure was lost as the drainage tubes were removed, and non-negative pressure drainage method was used instead. During the negative pressure treatment, swelling and pain did not increase after the operation. Other complications, such as asphyxia, septic shock, or mediastinal abscess, did not occur. All the patients were healed and eventually discharged from the hospital.
CONCLUSIONSustained negative pressure drainage, which is a modified version of the traditional method of incision and drainage, is an alternative treatment for severe maxillofacial and neck space infection. Such treatment reduces patient pain and eases doctor exertion. Thus, this method provides a new therapeutic strategy for severe maxillofacial and neck space infection.

10.Expression and significance of keratinocyte growth factor-1 in kidneys of septic rats

Objective To investigate the expression and significance of keratinocyte growth factor-1(KGF-1) in the kidneys of septic rats.Methods Eighty male Wistar rats were randomly divided into the sepsis group and sham operation group.Either group was divided into 3-hour,6-hour,12-hour,24-hour and 36-hour subgroups,with 8 rats in each.Sepsis models were established by cecum ligation perforation (CLP).The kidney and blood samples were respectively taken for pathological study.Serum creatinine (Cr) and blood urea nitrogen (BUN) level determination and leukocyte counting were performed.KGF-1 protein was detected by immunohistochemical method and the expression of KGF-1 genes by real time PCR.Results The renal tubules in sepsis group were injured most at 24h after operation,which were infiltrated by considerable neutrophilic granulocytes,leukomonocytes and mononuclear cells,with vacuolar degeneration,focal hemorrhage and necrosis under the light microscopy.Serum Cr and BUN increased from 6h after operation,and reached the peak at 24h after operation in sepsis group,which were significantly different in those of sham operation group (P< 0.01).Compared with sham operation group,the expression of KGF-1 protein and genes of sepsis group increased significantly and reached the peak at 6h after operation.KGF-1mRNA was positively correlated with BUN and Cr (r=0.504,P<0.01;r=0.382,P<0.05).Conclusion KGF-1 was also expressed in normal kidneys,mainly in tubule epithelial cells of renal medulla and also in that of renal cortex.The expression of KGF-1 in sepsis group was obviously higher than that of sham operation group,and was positively correlated with the degree of kidney damage after infection.KGF-1 was speculated to be one of the protective factors affecting the repair of impaired kidneys caused by infection.
