1.Rehabilitation of Talipes Valgus in Children with Cerebral Palsy

Objective To explore the clinical significance of the footprints and lateral X-rays in weight loading in evaluating the rehabilitation of talipes valgus in the children with cerebral palsy.Methods One hundred footprints of 56 children who were cerebral palsy with talipes valgus in the hospital-community-family-based rehabilitation mode were divided into light,middle and heavy types,and they were counted with two footprint-based approaches,and the two approaches were compared with chi-square criterion,and the dependability on consequence of the footprint and lateral X-rays in weight loading were analyzed.Results One hundred feet evaluated by the method of footprint were flatfeet radically,and the occurrence of flatfoot evaluated by three define lines were 11,57,32,respectively,and evaluated by footprint ratio were 6,7,87,respectively,but the degrees were different with two footprint-based approaches.There was a close correlation between two footprint-based approaches and lateral X-rays in weight loading.Conclusions Flat foot caused by talipes valgus can be evaluated by footprint in static state,while the development of bones of foot and the constitution of instep can be monitored on the lateral X-rays in weight loading.
2.Effects of Levothyroxine on the Follicular Helper T Lymphocyte Level of Patients with Hashimoto's Thyroid-itis

OBJECTIVE:To explore the effects of levothyroxine on the follicular helper T lymphocyte level of patients with Hashimoto's thyroiditis. METHODS:51 patients with Hashimoto's thyroiditis were selected as levothyroxine group,and another 30 healthy people were control group. Levothyroxine group was orally given 25-50 ug Levothyroxine sodium tablet,once a day,the dose was adjusted based on thyroid function,and the dose maintained when thyroid function turned normal,for continuous 90 d. Free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH)and the percentage of Th17 cells account-ed for peripheral blood CD4+ T cells(Th17/CD4+)in levothyroxine group before and after treatment were observed and compared with control group,the correlation of Th17/CD4+ and thyroid function. RESULTS:After treatment,Th17/CD4+ level in levothyrox-ine group were significantly lower than before and higher than control group,the differences were statistically significant(P<0.05). TSH in levothyroxine group were significantly lower than before,FT3 and FT4 were significantly higher than before,the differenc-es were statistically significant(P<0.05);compared with control group,the differences were not statistically significant(P>0.05). Th17/CD4+ showed positive correlation with thyroid function in levothyroxine group. CONCLUSIONS:Levothyroxine can signifi-cantly reduce the Th17/CD4+ and TSH,increase FT3 and FT4,and Th17/CD4+ is involved in the occurrence and progression of Hashimoto's thyroiditis.
3.Efficacy and safety of sibutramine in treatment of patients with overweight and simple obesity

Objective: To evaluate the efficacy and safety of sibutramine,an anti obesity drug, in patients with overweight and simple obesity. Methods: Forty six patients with overweight and simple obesity were randomly assigned to sibutramine or placebo group according to the rule double blind. Results: After a 2 week wash out followed by a 24 week treatment periods, body weight, BMI, waistline and hip of sibutramine group decreased significantly compared with placebo group. The effect was significant at week 4 and advanced at week 8. In sibutramine and placebo group, the total effective rates were 81.82% and 16.67% respectively. The incidence of total adverse effects (thirsty, constipation, headache, agrypnin and so on) in sibutramine group was 27.27% and 11.11% in placebo group. Conclusion: The short term obeservation shows that sibutramine is an effective and safe anti obesity drug in overweight and simple obesity treatment, and the patients compliance is good.

5.Infection Rate of HBV among High School Graduates in Jingjiang City of Jiangsu Province

Objective To investigate the positive rates of HBsAg and abnormal rates of Alanine aminotransferase among high school graduates in Jingjiang city.Methods HBsAg were detected by ELISA and ALT was detected by rate assay among 20 988 high school graduates during 2004—2007 in Jingjiang city.Results The HBsAg positive students were 1754.The total positive rate of HBsAg was 8.36%.The positive rate of HBsAg of male(9.46%)was higher than that of female(6.95%).The HBsAg positive rate of rural students(9.44%)was higher than that of urban students(7.28%).HBsAg positive rates of urban students showed a decreasing trend during 2004—2007,the rates were 9.31%,8.08%,6.74% and 5.24%,respectively(P0.05).There were 155 cases(8.4%)with increased ALT among 1754 HBsAg positive students.Conclusion The total positive rate of HBsAg among high school graduates in Jingjiang city from 2004—2007 was 8.36%.The HBsAg positive rate of male students was higher than that of female.The HBsAg positive rate of rural students was higher than that of urban students.8.4% students were found ALT abnormal.We should intensify Hepatitis B vaccine in time to prevent the spreading of HBV.
6.Clinical observation of dexmedetomidine given intranasally for day surgery recovery period

Objective To evaluate the clinical efficacy of dexmedetomidine given intranasally for day surgery recovery period.Methods 60 pediatric patients who were treated for elective day surgery were selected as study objects,and they were randomly divided into control group and observation group according to the digital table,30 cases in each group.During operation, after induction of 8% sevoflurane inhalation at 8 L/min,3% of sevoflurane was maintained at 1 L/min.After intubation,0.5 g dexmedetomidine was given intranasally in the observation group, 0.9% NaCl saline was given to the control group.Mean arterial pressure (MAP),heart rate (HR),oxygen saturation (SpO2) were observed at different time points.The recovery time, number of cases of agitation, Aono agitation scale, children anesthesia delirium PAED score and the incidence of adverse reactions were observed in the two groups.Results As the time passed by, the MAP,HR,SpO2 in the two groups stabilized, except HR of the observation group at T0 time was higher than the control group[(92.92±3.22)beats/min vs.(89.32±3.22) beats/min], the difference was statistically significant(t=2.34,P<0.05).The children of the observation group recovered more slowly than the control group[(20.55±0.97)min vs.(18.62±1.03)min,t=2.17,P<0.05)],the number of cases of agitation, Aono restless score, anesthesia delirium PAED scores were lower than children in the control group (t=5.38,3.72,2.56,all P>0.05).The overall incidence of adverse reactions was similar in the two groups, there was no statistically significant difference (13.33% vs.10.00%,x2=0.12,P>0.05).Conclusion Dexmedetomidine given intranasally for the children underwent day surgery can significantly improve the quality of children awakeness, reduce the incidence of postoperative delirium, and without significant side effects.
7.The analysis of mitochondrial DNA point mutation at position 3243 in age-related macular degeneration

Purpose To detect whether a 3243 point mutation existed in age related macular degeneration (AMD). Methods Twenty six cases of wet form AMD patients, ten cases of dry form AMD patients were selected,and compared with twenty nomal controls. After collecting anti coagulated blood samples, total cellular DNA were extracted and purified. Using polymerase chain reaction and restriction fragment long polymorphism techniques, the mtDNA A→G point mutation at position 3243 were detected. Results After cleaveded by restriction endonuclease Apa I, a 294 bp fragment remained only in all detected DNA samples including twenty six wet form AMD, and ten dry form AMD. No any other fragment appeared. The result showed that there was no A→G mutation at position 3243 found in AMD. Conclusion It is suggested that mtDNA 3243 point mutation due to maternal inheritance might be not concerned with both wet form AMD and dry form AMD.
8. Pulmonary histone deacetylase is involved in phenotype transformation of airway smooth muscle cells in chronic asthmatic mice

Objective: To investigate the changes of histone deacetylase (HDAC) activity during chronic asthma and its effects on the phenotype and biological activities of airway smooth muscle cells. Methods: Chronic asthma model was established with mice using repeated sensitization and challenge with OVA, and the lung was homogenated for total HDAC activity assay. Rat airway smooth muscle cells and human bronchial smooth muscle cells were stimulated with trichostatin A (TSA), an HDAC inhibitor. The effect of HDAC inhibition on phenotype switching was detected by Western Blotting analysis, and its effects on proliferation, migration and contraction of smooth muscle cells were examined by MTT, Transwell and gel contraction, respectively. Results: The total HDAC activity was significantly decreased in chronic asthmatic mice compared with saline-treated controls ([0.371±0.054] vs [0.603±0.034] μmol/(L·μg), P<0.01). Incubation of the rat trachea ring and human bronchial smooth muscle cells with TSA (0.5 μmol/L) for 12-24 h increased the expression of α-sm-actin and SM22-α. TSA significantly decreased cell number after 24 h treatment ([1.719±0.044)×10 4 vs ([1.911±0.048]×104, P<0.05) and 48 h [1.808±0.009]×104 vs [2.537±0.01]×10 4, P<0.05) compared with DMSO controls. TSA treatment for 24 h also significantly decreased the migrated cells compared with the PDGF-treated cells (52±7.5 vs 88±7.632, P<0.05). Furthermore, the gel contraction rate was significantly lower in the TSA-treated cells than in the DMSO or PDGF-treated cells ([9.885±7.084]% vs [44.844±3.808]% and [41.315±7.943]%, P<0.05). Conclusion: HDAC activity is decreased in the lung of chronic asthmatic mice, which may be related to switching of the airway smooth muscle cells to a "contractic" phenotype, but with no increase of their contraction capability.
9.In vitro anti-proliferation effect of peptides from cinobufacini injection.

Cinobufacini is an aqueous extract of Bufo bufo gargarizans Cantor dried skin, which has been widely used for cancer therapy in China. So far, its active components are still not very clear. In previous reports, bufadienolides with low-concentration were usually studied because of their anticancer effects. However, the high polarity constituents in cinobufacini are less investigated. The present study found that more than 50% contents of cinobufacini were water-soluble peptides. Then, in vitro anticancer experiments were carried out, including human stomach cancer cell lines BGC823 and MCG803, human colon cancer cell lines DLD-1 and HT-29, and human pancreatic cancer cell line MIAPACA-2. The IC50 for these cell lines model were ranged from 25-123 microgmL(-1). The results indicated that these peptides showed similar activity with cinobufacini injection. As a conclusion, this study provides a new and further understanding of anticancer components in cinobufacini injection.
10.Positive alarming time of blood culture and distribution of pathogens

Objective To investigate the distribution of pathogens and positive alarming time of blood culture,and provide basis for laboratory diagnosis and clinical treatment. Methods Blood specimens from clinical departments in a hospital in May-November 2015 were collected,positive alarming time of blood culture was recorded,species of pathogens were identified. Results A total of 157 pathogenic strains were isolated from blood culture specimens, gram-positive cocci,gram-negative bacilli,and fungi accounted for 31 .85% ,57.32% ,and 10.83% respectively. The median positive alarming time were as follows:Enterobacteriaceae 0.50 day,non-fermenting bacteria 0.63 day, Enterococcusspp. 0.60 day,Streptococcusspp. 0.80 day,Staphylococcusspp. 1.01 days,and fungi 1.44 days, respectively. Conclusion Positive alarming time of blood culture specimens from early to late are as follows:Enter-obacteriaceae,Enterococcus,non-fermentative bacteria,Streptococcus spp.,Staphylococcus spp.,and fungus. Positive alarming time of pathogens causing bloodstream infection are all within 4 days,and most of them are within 1 day.
