Chinese Journal of Current Advances in General Surgery 1999;0(03):-
Objective: To study the efficiency and prognosis of preoperative transcatheter arterial chemoembolization(TACE)treatment in patients with hepatocellular carcinoma(HCC)after hepatectomy. Methods: The clinical pathological data of 479 patients with resectable HCC from 1998 to 2007 were analyzed retrospectively. Fifty patients had preoperative TACE, 379 cases had not and others were lost of follow up or dead. Clinical pathological data, tumor-free survival rate and overall survival rate after hepatectomy were analyzed. Results: There was a significantly difference in the age and the tumor grade between the groups, other clinical pathological factors were no difference. The preoperative TACE group was younger than non-TACE group (?2=6.228, P=0.012), And had lower tu-mor grade, more tumor necrosis (?2=53.874, P=0.000). The disease tumor-free survival rate and overall survival rate were no difference between the groups (?2=0.736, P=0.391;?2=0.000, P=0. 995). Conclusions: For the unresectable HCC patients by one-stage operation, the preoperative TACE treatment has no adverse effect. However, tumor-free survival rate and overall survival rate are similar to non-TACE group.

Objective To obtain the recombinant urease B subunit (ureB) of Helicobacter pylori (Hp) and apply it in serological detection of Hp infected patients. Methods Urease B subunit gene was amplified from the complete genome of Helicobacter pylori by PCR, and cloned into the PinPoint TM Xa Ⅲ fusion expression vector, then was sequenced. The protein of urease B subunit was expressed in E.coli JM109 and purified with affinity chromatography. 113 serum samples of peptic ulcer patients were detected by ELISA combined with purified UreB protein. Results DNA sequence analysis showed the nucleic acid sequence homology of ureB gene was 96.44% and the putative amino acid sequence homology was 99.65%. The recombinant UreB protein was composed of 571 amino acid residues and kept original immunologic reaction with corresponding antibody, and its purity was over 90% after affinity chromatography. The results of ELISA associated with recombination UreB antigen showed the sensitivity and specificity was 92%, 98.5%. Conclusion The recombinant UreB protein will be of value for clinical serodiagnosis and epidemiological study of Helicobacter pylori.

Objective To obtain and identify dendritic cell (DC) from mouse bone marrow in vitro. Methods Recombinant mouse granulocyte and macrophage colony stimulus factor (GM-CSF) induction was used to induce mouse bone marrow cells to form dendritic cells in vitro. The morphological changes were observed with light inverted microscope and scanning electron microscope (SEM). CD11c, MHCⅡ, and CD86 were identified with flow cytometry. The biological function was studied with antigen phagocytosis test and mixed lymphocyte reaction (MLR). Result The cultured DC from mouse bone marrow displayed the typical morphological characteristics of DC. Immature DC, which had high expression in CD11c and low expression in MHCⅡ and CD86, could phagocytize antigen. Mature DC, which had high expression in CD11c, MHCⅡ and CD86, could stimulate allogenic mixed lymphocyte proliferation. Conclusion DC can be generated from mouse bone marrow cells through cytokine induction in vitro and be used for further study associated with DC.

Objective: To develop the Chinese version of the Neuropsychiatric Inventory Questionnaire (NPI-Q) and evaluate its reliability and validity. Methods: Three investigators assessed 10 dementia cases together with the NPI-Q to evaluate the inter-rater reliability. Eighty-six dementia cases and 30 healthy controls were assessed with the Behavioral Pathology in Alzheimer's Disease (BEHAVE-AD), the Brief Psychiatric Rating Scale (BPRS) and NPI-Q to evaluate the criterion validity. A subgroup of 30 dementia cases was tested with the NPI-Q again 24 h later to evaluate the test-retest reliability. Results: (1) Internal consistency. Cronbach's a coefficient of the severity subscale was 0.589, mean inter-item correlation coefficient of the severity subscale was 0.107. Cronbach's α coeffi-cient of the distress subscale was 0.612, and the mean inter-item correlation coefficient of the distress subscale was 0.114. (2) Inter-rater reliability. Intraclass correlation coefficient (ICC) of the severity subscale was 0.97, and ICC of the distress subscale was 0.94. (3) Test-retest reliability. The test-retest correlation coefficient of the severi-ty subscale was 0.89, and the test-retest correlation coefficient of the distress subscale was 0.86. (4) Criterion va-lidity. The severity subscale total score was correlated significantly with BEHAVE-AD (r=0.70, P <0.001) and BPRS(r=0.40, P<0.001) total score; the distress subscale total score was correlated significantly with the global ratings in part 2 of BEHAVE-AD (r=0.76, P<0.001) . (5) The severity subscale total score in demen-tia group was significantly higher than that in control group (median: 10 vs.1.5, P <0.001). The distress sub-scale total score in dementia group was significantly higher than that in control group (median: 10 vs 0; P< 0.001) . (6) Exploratory factor analysis identified 4 common factors, including psychosis, abnormal behavior, discontrol and affective symptoms, which explained 58.3% of total variance. All of the items loaded were between 0.596 and 0.803 on their conrresponding factor. Conclusion: The reliability and validity of the Chinese version of NPI-Q meet the psychometrics properties, and deserves to conduct further study in larger samples.

Objective To research the relationship between binocular acuity and monocular acuity, and their effect on the evaluation of visual impairment and disability. Methods 1006 patients, aged between 18 and 60 years, are measured monocular and binocular acuities after the best lens correction. Results Binocular vision is higher than, equal to and less than monocular vision, three accounted for 27.14%, 70.78% and 70.78% respectively. Interocular difference has certain correlation with binocular interactions (χ2=54.939,P<0.05,r=0.228). Thirty-three patients have same binocular acuity but different monocular acuity. Conclusion Binocular acuity associated with the monocular acuity and the better eye vision plays a special role in visual disability evaluation.
