1.Mechanical and biodegradable properties of regenerated fibroin fibers

BACKGROUND: The molecular weight of fibroin is inevitably reduced in the course of the dissolution of regenerated fibroin fiber, and this is why we have not entered the stage of the practical application of the regenerated fibroin fiber till now.OBJECTIVE: To obtain biodegradable regenerated fibroin fiber.DESIGN: A single sample experiment.SETTING: Jiangsu Provincial Key Laboratory of Silk Project of Material Engineering College of Soochow University.MATERIALS: Leftover silk pieces were provided by Suzhou Silk Import & Export Corp.METHODS: This study was performed at Jiangsu Provincial Key Laboratory of Silk Project of Material Engineering College of Suzhou University between January 2003 and December 2004. By controlling the molecular of abandoned natural fibroin fiber during neutral salt dissolution process, wet spinning technique and draft method were applied to spin regenerated fibroin fibers that have certain mechanical and biodegradable properties.The specific procedure is as follows: ①fibroin preparations. ② Spinning solution preparations. ③ Regenerated fibroin fibers were spun with wet spinning method. ④Sodium lauryl sulphate-polyacrylamide gel electrophoresis method was used to detect the molecular weight of LiBr dissolved fibroin solution. ⑤To detect the crystallinity and orientation of fibroin fibers. ⑥ To detect the mechanic properties of regenerated fibroin fibers. ⑦ To detect the in vitro enzyme degradation rate of fibroin fibers.MAIN OUTCOME MEASURES: ①The relative molecular weight of LiBr dissolved silk fibroin. ② Result of X-Ray diffraction. ③The in vitro enzyme degradation rate of regenerated fibroin fibers.RESULTS: ①After LiBr dissolution, the molecular weight of fibroin spinning solution was mainly below 100 000. ②During the coagulation and pulling process of regenerated fibroin fiber wet spinning, fibroin fiber conformation changed from random convolution to the co-existence of β-folding and random coi1/α-spiral conformation in molecular chain. ③ Actinozyme was used to assess the in vitro enzyme degradation rate of hexafluoroisopropanol spun regenerated silk fibroin, and the 30 day-degradation rate of regenerated fibroin fibers was 37.16%, compared with 10.70% of natural fibroin fiber.CONCLUSION: Wet spinning technique and draft method can be used to spin regenerated fibroin fibers possessing certain mechanical and biodegradable properties by controlling the molecular weight of abandoned natural fibroin fibers during neutral salt dissolution process.
2.Study of community nursing intervention on medication compliance of hypertension patients

Objective To discuss the effect of community nursing intervention on medication compliance and control rates of blood pressure in patients with essential hypertension.Methods Community nursing intervention was conducted to 73 community patients with essential hypertension. The patients' medication compliance was compared before and after intervention and their control of blood pressure was also observed. Results The difference was statistically significant in the comparison of patients' knowledge of antihypertension and the proper use of antihypertensive drugs before and after intervention. Blood pressure control rate was 80.82%.Conclusions Community nursing intervention can improve patients' medication compliance and effectively control the blood pressure of the patients.

Autophagy is a vital phenomena which widely exists in eukaryotic cells.It runs through the cellular normal growth,development,and physiological pathology processes.Studies in recent years have shown that autophagy involves in the pathological process after stroke,and affects the survival and death of nerve cells,This article reviews the classification,formation process,molecular mechanism and detection of autophagy as well as its relation with stroke.Our purpose is to provide a new direction for clinical treatment of stroke.
4.Application of a reinforced laryngeal mask airway with drain tube in elderly patient undergoing endoscopic sinus surgery

Objective To investigate the safety and efficiency of a reinforced laryngeal mask airway(LMA) with drain tube in elderly patient undergoing endoscopic sinus surgery.Methods Sixty elderly patients were selected and randomly divided into two groups:a reinforced LMA group (group R,n=30)and a reinforced endotracheal tube group(group T,n=30).Observation items include the difficulty degree of device insertion,the time for successful insertion,the number of attempts required for successful insertion,fiberoptic view of glottis,the difficulty degree of gastric tube insertion,oropharyngeal leak pressure(OLP),ventilatory indicators at a certain time,haemodynamics during insertion,the blood stain on the mask were compared between the two group.The postoperative complications 1,6,24 hours after operation were recorded.Results There were no significant differences in general conditions,the successful insertion rate at first attempt,the total success insertion rate,airway quality and the anesthetic drug dosage between the two groups.The insertion time was shorter in group R than in group T [(34.1± 13.0 s)vs (45.4± 15.6 s),t =5.274,P=0.001].The incidence of cough during extubation was less in group R than in group T [0% (0/30) vs.53.3%(16/30),x2 =21.222,P=0.000].Conclusions The reinforced LMA with drain tube is safe and effective for elderly patients during endoscopic sinus surgery.
5.Investigation into the current status of the drug resistance of enterococci

Objective To analyze the current status of the drug resistance of enterococci and monitor the prevalence of drug-resistant strains of enterococci in hospital infection. Methods 318 strains of enterococci separated from infectious samples in nine general hospitals in Hubei were assayed and tested with regard to drug sensitivity and the software of WHONET4 was used to analyze the data. Results The separation ratio of ARE, HLARE, VRE and ARE complicated with HLARE was respectively 16.98% , 37.76% , 0 and 5.66% . Conclusion The drug resistance of enterococci shows the tendency of increasing and consequent hospital infection shows the tendency of scattered prevalence. Laboratories of clinical microorganisms ought to closely watch the detection and spread of VRE.
6.Effects of isoflurane on water-maze performances in scopolamine-treated rats

Objective To investigate the effects of isoflurane (Iso) on the water-maze performances in scopolamine (Sco) treated rats.Methods Fifty-five male SD rats weighing 150-200 g were randomly divided into 8 groups :group I control (Con)/1 d ( n = 6); group II Iso/1 d ( n = 7);group III Sco/1 d ( n = 9);group IV Iso + Sco/1 d ( n = 9) ; group V Con/7 d ( n = 6); group VI Iso/7 d ( n = 6); group VII Sco/7 d ( n = 6) and group VIII Iso + Sco/7 d ( n = 6) . In Iso and Iso + Sco groups the animals inhaled 1.5% isoflurane 2 h per day for four consecutive days. The Morris water-maze (MWM) test was performed for 3 consecutive days starting from the first day (group I -IV ) or the 7th day (group V -VII) after the 4 day isoflurane inhalation. Scopolamine 0.8 mg.kg-1 was given ip 20 min before the beginning of the MWM test in Sco and Iso + Sco groups. 7.Minimally Invasive Surgery Treatment with Small Incision on Hypertensive Intracerebral Hemorrhage

Objective To explore the principle and methods of normative minimally invasive surgery treatment on hypertensive intracerebral hemorrhage. Methods 49 cases of hypertensive intracerebral hemorrhage were treated by microsurgery with small incision. Results 46 cases survived and 3 cases died after surgery. 42 cases were followed up. Adopting GOS, 19 cases were normal, 12 cases were disable and could care for self, 5 cases were severely disable, sober and could not care for self, 3 cases were vegetable survival, and 3 cases were dead. Conclusion Application of minimally invasive surgery idea and normative treatment according to the patients' condition can obviously improve the efficacy of patients with hypertensive intracerebral hemorrhage.
8.Comparision of rivaroxaban and warfarin on preventing post-operation deep vein thrombosis in elderly hip fracture patients with diabetes mellitus

Objective To compare the clinical effects between rivaroxaban and warfarin on preventing deep vein thrombosis (DVT) in elderly hip fracture patients with diabetes mellitus (DM) after operation. Methods One hundred and sixty-eight elderly hip fracture patients with DM were randomly divided into the observation group and the control group. The observation group (88 cases) were treated with rivaroxaban (10mg, po, qd) after surgery, and the control group were treated with warfarin and low molecular weight heparin calcium for 14 days. Respectively after 7, 14, 21 and 28 days, the color doppler ultrasonography was used to examination vein blood flow and the DVT of affected lamb. In addition, the levels of D-dimer and bleeding were observed during treatment. Results The incidence of DVT in observation group (2 cases, 2.23%) was significantly lower than that in control group (7 cases, 8.75%), and DVT of control group was happened on 7th day after surgery. In addition, the level of D-dimer on 7th day post-surgery in observation group significantly decreased, while it significantly decreased on 14th day post-surgery in control group (P<0.05). However, the incidence of bleeding events in observation was less than that in control group (P<0.05). Conclusion Rivaroxaban can be more effective compared with warfarin in preventing diabetes mellitus elderly hip fracture patients with postoperative DVT formation and significantly reduce the bleeding risk, with more safe and convenient.
9.Effects of midazolam on muscarinic receptor of brain in healthy and scopolamine-treated rats

Objective:To investigate the recent and permanent effects of pretreatment with midazolam on muscarinic receptor of brain in healthy and scopolamine-treated rats. Methods:(1) In recent effect group, thirty-eight male SD rats were randomly divided into 4 groups: control group (Con, n=9), midazolam group (Mid, n=9), scopolamine group (Sco, n=10), midazolam+scopolamine group (Mid+sco, n=10). On the 1st, 2nd, and 3rd days, Mid group and Mid+sco groups were treated with intraperitoneal injection of 50 mg/kg midazolam per day while Con group and Sco group were treated with intraperitoneal injection of the same voluminal physiological saline per day. On the 4th, 5th, and 6th days, Scopolamine 0.8 mg/kg intraperitoneal injection per day was administered in Sco group and Mid+sco group while same voluminal physiological saline was administered in Con group and Mid group. (2) In permanent effect group, thirty-six male SD rats were randomly divided into 4 groups: control group (Con, n=9), midazolam group (Mid, n=9), scopolamine group (Sco, n=9), midazolam+scopolamine group (Mid+sco, n=9).
