1.Unusual management of thoracoabdominal impalement injury to the right hemiliver and diaphragm.

Laparotomy or thoracolaparotomy is a traditional management approach for thoracoabdominal impalement injury associated with major liver and diaphragmatic injuries. We successfully treated the impalement injury with minimally invasive management. A male was brought to our trauma centre with the 15 cm long handle of the knife protruded from right lateral thoracoabdominal region. CT scan revealed that the knife blade traversed through the right costophrenic recess into segment 8 of the liver. There was an intraparenchymal haematoma and a collection of fluid in the abdominal cavity. The conservative management plan consisting of removing the impaled knife, observing, monitoring and managing complications was undertaken. A multidisciplinary approach to manage a patient with less invasive techniques yielded a good outcome. This management option may be considered as an alternative for open surgery for hemodynamically stable patients in experienced centres.

4.The challenges faced in the field of trauma care in China.

With the continuous development of China's urbanization and socio-economic, death and disability caused by trauma has increased prominently, and trauma has become the first cause of death in the people younger than 45 years old. Compared with the treatment of other diseases, China are facing many problems and challenges in terms of trauma treatment, specifically focused in three areas: First, the medical profession and society had not attach enough attention to trauma; Second, trauma centers in our country is insufficient; The level of trauma care vary greatly between different regions, the doctors and nurses commitment to trauma care lack of standardized training; Third, scientific treatment process and treatment system that compliance with international standards and meet the geographical features of China is insufficient. In view of the above situation and existing problems, learn from the successful experience of foreign countries, we think we should proceed from the following three aspects to change the status of China's treatment of trauma gradually: First of all, we should establish Traumatology that deals with the treatment of serious wounds and injuries. Secondly we should establish the right trauma care system that suitable for China's conditions; The third point is the establishment of trauma specialist training system composed by "basic training" and "targeted training"; Final we should establish severe trauma multidisciplinary treatment team model in the process of trauma care. Thereby improving our overall level of trauma treatment through above means, reduce the disability and mortality caused by trauma, thus promoting the development of trauma forward.

7.Impact of subtropical climate on frequency of ambulance use for trauma patients in a coastal area of China.

PURPOSETo explore the impact of subtropical maritime monsoon climate on the frequency of ambulance use for trauma patients in a coastal region in China.
METHODStatistical analysis of data on ambulance use from the 120 Emergency Command Center in Shantou City, Guangdong Province, from January to December 2012 as well as daily meteorological data from a Shantou observatory was performed to determine how climatic factors (seasons, time, and weather) affect the frequency of ambulance use for trauma patients.
RESULTSThe daily ambulance use for trauma patients differed between spring and summer or autumn (p<0.05), between sunny and rainy days (p<0.05), and between cloudy and lightly or moderately rainy days (p<0.05).We found a linear correlation between daily maximum temperature and daily ambulance use for trauma patients (R² =0.103, p<0.05). In addition, there was significant difference in ambulance use between good and bad weather (p<0.05).
CONCLUSIONFrequency of ambulance use for trauma patients is affected by the subtropical maritime monsoon climate in the coastal region. Better weather contributes to increased daily frequency of ambulance use, which is the highest in autumn and lowest in spring.

8.Translation medicine in wound healing: successful cases and personal deliberation.
