1.To survey on the bacterial cause infected wound and the sensitiveness with antibiotic of isolate microbial strain at surgical department, Can tho General Hospital

Wound infection is one of the most important surgical infection and is a high percentage of hospital infection. In general surgical deparment at Can Tho General Hospital from 2001-2002, 110 samples of wound infection were isolated and tested antibiogram. Total of 133 microbial strains were isolated, multi infection is 19.09%, the highest of number of bacterial strain in a sample is 3. E. coli is the most common isolated (26.34%), Klebsiella sp 16.55%, Staphylococcus aureus: 9.78%. The result of antibiogram showed that among cocci gram positive group, lowest resistance antibiotic are vancomycin (6.24%), ciprofloxacin (55.3%); among bacilli negative group most of antibiotic tested were resistant in more than 50%, antibiotics nearly completely resistant were ampicillin (95.92%) and amoxicillin+clavulanic acid (82.35%), amikacin (75.29%). For Pseudomonas, antibiotica are still good effect are ciprofloxacin (90%), colistin (80%). Pseudomonas are still reristant to other antibiotics (50-70%).

2.Heart and major blood vessel lesions in abdominal - thoracic injuries.

From September 1994 to August 2000, there were 206 patients with abdominal-thoracic injuries who admitted the Popular Hospital. Among these, there were 22 patients (10.7%) included 20 males and 2 females had heart and major blood vessel injuries. Mean age of patients was 30 years. All of them were stabbed with knife. 2/22 patients had treated previously in the other hospital. 20 patients had the mean time from accident to admission was 1 hour and 14 minutes (ranged from 15 minutes to 3 hours). Among 12 deaths, 4 cases occurred before admission, 2 before surgical intervention, 5 during operation, and 1 in second day post-operation. So, there were only 11 patients underwent surgical treatment, included 2 patients with heart injuries and 9 with major blood vessel injuries. 10/11 patients gained good outcome after operation. 1 in 11 patients was dead from abdominal aortic injury.

3.Surgical treatment of popliteal artery injury due to trauma

From Jan/2001 to Nov/2001 at the Cardio–vascular ward Cho Ray Hospital there were 20 patients who were popliteal artery injured. The causes are that motorbike accident: 18 cases; block ferrous compressed: 01 case; jumping: 01 case. The treatment is that cutting the arterial damaged and transplantation by saphenous vein: 06 cases; amputation 1/3 inferior femoral: 04 cases (two cases for premarie amputation, two cases for 24 hours after operating). Result: 16 patients were checked after operating (ECHO Doppler: 12 cases, DSA: 04 cases). All them have good results: the anastomosis is not stenosis or obstruction.

4.The duodenal after the traumatic operation and the duodenal injuries: the clinical characteristics and attitude of management

Background: Duodenal fistula is severe complication and complicated in management. Morbidity and mortality were high. The aims of study were to determine the clinical characteristic of duodenal fistula after surgical treatment of duodenal blunt and penetrating trauma, and attitude of treatment. Methodology: Retrospective study was done. Data of the patients with post-operative duodenal fistula after surgical procedure who were admitted in Cho Ray Hospital from 6-1996 to 6-2000 were analyzed. Results: There were 15 posttraumatic duodenal fistulas. Locations of fistula were often in D2 and D3 (80% of cases). Time of appearance of fistula was 10,257,25 days (2-29 days) after operative management of duodenal trauma. Mean of fistula output was 573626ml/24 hours. Total of energy of parenteral nutrition was 800130,23 Kcalo. Post-operative mortality was 26%. Only one case or 6.7% was spontaneous closure with medical treatment. 67.3% of cases was healed by surgical treatment. Conclusion: Main treatment for post-operative fistula of duodenal trauma is surgical procedure. Need of selection of appropriate time for operation is proposed. Pre- and post-operative management such as withholding oral taking, replacing fluid and electrolytes, and administering total parenteral should be paid attention

5.Results of 45 cases of termino-terminal urinary tract cutting and anastomosis in the urinary tract rupture and anterior urinary tract stenosid due to trauma

45 patients suffering the urinary tract rupture and anterior urinary tract stenosid due to external trauma received the termino-terminal urinary tract cutting and anastomosis. 14 cases of which had complete anterior urinary tract rupture indicated an operation involving blood clot removal, necrotis tissue removal and cutting and anastomosis of two head of urinary tract. 31 cases of which had an anterior urinary tract indicated an operation involving the cutting of the stenosid part and termio-terminal anastomosis. The results have shown that the successful rate was high. This was a physiological operation.

7.The vitrectomy in the treatment of the dislocation and luxation of lens after trauma

A study on 40 patients in the Traumatic Department of the National Institute of Ophthalmology was carried out. The results: 16 patients with ages of under 40, with the soft nuclear 1+, 2+ received a vitrectomy by pars-plana. 16 patients with the solid nuclear 3+, 4+ received an anterior segment vitrectomy in intravitreal surgery. 8 patients with the luxation of lens in to vitreous cell received the method of penetration for moving the lens to the anterior room and lens was removed via cornea combined with vitrectomy. 8.Surgery of the parotid gland with protection of facial nerve from injury

Surgery of the parotid gland is usually indicated for the relief of an inflammatory disorder or for control of neoplasia. In all these procedures, the overriding critical factor is the management of the facial nerve. The facial nerve is embedded within the substance of the gland and is at risk of injury during any parotid gland operation. The various surgical approaches described in our study comprised a common thread, the surgeon’s awareness of facial nerve anatomy and efforts to protect the nerve from an inadvertent injury.

10.Antibiotic resistance of some bacteria isolated from infectious lesion of operation at the Hospital No 175

157 samples were isolated from infectious operating lesion they were cultured into Biomerieus culture media and undergone an antibiogramme on Muller-Hilton medium. S. aureus and E. coli are the most common organisms, which are isolated. Occasional infections occurred increasingly.
