2.Lesion in the Sacro-iliac Joint

Sacro-iliac joint is, as other joints, the prediIection site of tuberculous or pyogenic arthritis as well as primary or metastatic tumor. The writers reviewed twenty cases of sacro-iliac lesion performed at the Department of Orthopedic Surgery, Seoul National University Hospital, for the period of five years from June 1972 till May 1977. 1. They are tuberculous arthritis eleven, pyogenic arthritis five, ankylosing spondylitis two(one associated with spinal stenosis, the other with hip invoIvement), osteosarcomatosis one, metastatic tumor from adenocnrcinoma of prostate one 2. Infection is the predominant Iesion in sacro-iliac joint, and tuberculous arthritis is twice than pyogenic arthritis. 2. Early diagnosis of sacro-iliac lesion is difficult, but S-I stress test is a valuable finding. 4. Two of eleven cases of tuberculous arthritis, two of five cases of pyogenic arthritis with mild involvement were treated conservatively without sequele, but in more advanced cases, surgical intervention was preformed.

3.Intramedullary Nailing for Pathological Fractures of the Proximal Humerus.

BACKGROUND: Endoprosthetic reconstruction is widely applied for pathological fractures of the proximal humerus; however, functional impairment is usually unsatisfactory. The aims of the current study are to evaluate the reliability of interlocking intramedullary (IM) nailing with cement augmentation as a fixation method in proximal humeral lesions and to assess functional outcomes. METHODS: We reviewed 32 patients with pathological fractures of the proximal humerus who underwent interlocking IM nailing and cement augmentation. Functional scores and pain relief were assessed as outcomes. RESULTS: The mean follow-up period was 14.2 months. The mean Musculoskeletal Tumor Society functional score and Karnofsky performance status scale score were 27.7 and 75.6, respectively. Improvement of pain assessed using the visual analogue scale was 6.2 on average. Thirty-one patients (97%) experienced no pain after surgery. The mean ranges of forward flexion and abduction were 115° and 112.6°, respectively. All patients achieved stability and had no local recurrence without failure of fixation until the last follow-up. CONCLUSIONS: Proximal interlocking IM nailing with cement augmentation appears to be a reliable treatment option for pathological or impending fractures of the proximal humerus in selected patients with metastatic tumors, even with extensive bone destruction.

4.A Rapid and Simple flow Cytometric Method for Measuring Cell Viability Using Propidium Iodide Staining and Forward Scatter Measurement.

BACKGROUND: The importance of the determination of cell viability has prompted the development of several assays of viability that utilize the exclusion of certain dyes by viable cell membranes. Recently, flow cytometry has been adapted to estimate cell viability by using fluorescent dye which is excluded by living cells on the basis of altered dead cell properties. OBJECTIVE: We have developed a flow Cytometric method for measuring cell viability after staining with propidium iodide (PI) and have compared it with the classical colorimetric method, MTT assay, which is currently widely used in cytotoxicity assays in the research field. METHODS: We performed flow cytometry and MTT assay for the comparison of the sensitivity of the assessment of cell viability. RESULTS: Decrease of cell viability was measured by flow cytometry with the addition of as little as 0.002% Triton-X 100 in comparison to MTT assay which could only reveal a similar decrease of cell viability with the new method to 0.008% Triton-X 100. CONCLUSION: Our results demonstrate this new method to be more sensitive and simple for the assessment of cell viability.

8.Diagnostic Challenge of Hemorrhagic Fever with Renal Syndrome on Admission before its Serological Confirmation.

PURPOSE: The purpose of this study was to look for possible changes from typical clinical features of 5 sequential clinical phases described in 1950s. METHODS: The clinical features and laboratory data of hemorrhagic fever with renal syndrome (HFRS) with acute renal failure (ARF) and its subdivided 2 groups of correct and incorrect impression of HFRS on admission before the serological confirmation were reevaluated in 35 patients admitted at a single tertiary center from 1995 to 2000. RESULTS: We experienced the high failure rate (74%, 26/35) to recognize HFRS as the cause of ARF on admission. On admission, fever was present in all patients (100 %), and thirty three (94%) had either single or combined gastrointestinal symptoms. However, no one had clinical shock and only 4 patients (11%) had hypotension. Also, oliguria was noted only in 5 patients (14%) during the first 24 hrs on admission. When compared between 2 groups of correct (n=9) and incorrect impression of HFRS (n=26), febrile (100% vs 100%) and gastrointestinal symptoms (89% vs 96%) were present high both but hematologic (67% vs 23%) and hemodynamic signs (67% vs 4%) were much less present in the latter. The patients of incorrect impression on admission were admitted more than half (14/26, 54%) under non-renal care, and were exposed to antibiotics (46%) and even steroids (15%), respectively. In addition, these patients with incorrect impression on admission showed the unwanted outcomes of longer hospitalization than those with correct impression of HFRS (mean+/-SD, 21+/-3 vs. 12+/-4 days, p<0.01). CONCLUSION: Its well awareness of the changing clinical features in endemic area of HFRS would avoid overlooking Hantaviruses as a causal agent of ARF on the initial admission stage of HFRS leading to unnecessary treatments and longer hospitalization.

9.A case of uterine tumor resembling ovarian sex-cord tumor with clinical review.

Uterine tumor resembling ovarian sex-cord tumor (UTROSCT) were first described by Clemen and Scully in 1976. They are a heterogenous group of uterine neoplasm characterised by pure or predominant histological patterns that resembled sex cord tumors of ovary. Clemen and Scully divide the tumors into two groups according to sex-cord-like structures within tumors (the so-called group I tumors with 50% of sex-cord elements, the so-called group II tumors with almost completely sex-cord-like structures). The histogensis of the sex-cord-like elements is uncertain, the group I tumor appears to be more malignant. We experienced a case of UTROSCT in a 44 years old woman underwent LAVH under tantative diagnosis of uterine myoma and report the case with brief review of literature.
