2.Paehydermoperiostosis: A Case Report

Pachydermoperiostosis (idiopathic hypertrophic osteoarthropathy) ia an unusual syndrome of familial nature and characterized by clubbing of the digits, periosteal new bone formation especially over the distal ends of the long bones, and coarsening of the facial features with thickening and furrowing of the skin of the face and forehead. The case of a 46 year old male exhibiting the above characteristics is reported, The same tendencies were exhibited by his father before him. A review of literature is included.

3.MR Imaging Findings of Clonorchiasis.

OBJECTIVE: The purpose of this study was to evaluate the MR spectrum and MR cholangiographic imaging findings of clonorchiasis. MATERIALS AND METHODS: We reviewed 26 patients with confirmed clonorchiasis by either stool tests (n=24) or surgery (n=2). MR imaging was performed on a 1.5 T system (GE Medical Systems, Milwaukee, WI) with a torso coil. Axial T1-and T2-weighted, gadolinium-enhanced dynamic images and MR cholangiography were obtained. Image analyses were used to identify abnormalities of the intrahepatic and extrahepatic bile ducts and the presence of hepatobiliary malignancy. All MR examinations were reviewed by the consensus of two abdominal radiologists. RESULT: Intrahepatic bile duct abnormalities were seen in 23 (89%) of the 26 patients. The most common finding was mild dilatation of the intrahepatic bile duct which was found in 21 (81%). "Too many intrahepatic ducts" were found in 16 (62%), wall enhancement and thickening in 21 (81%) and filling defects and ductal stricture in the intrahepatic bile duct in 6 (24%) and 3 (12%) patients, respectively. Extrahepatic ductal dilation was found in 5 (19%) and 9 (35%) revealed hepatobiliary malignancy. CONCLUSION: MR imaging revealed various findings of clonorchiasis, including dilatation, wall enhancement, stricture of the intrahepatic ducts and filling defect within the intrahepatic bile duct.

4.Multidetector-row CT Angiography of Hepatic Artery: Comparison with Conventional Angiography.

PURPOSE: To determine the usefulness of three-dimensional CT angiography using multidetector-row CT (MDCT) for delineating the arterial anatomy of the liver. MATERIALS AND METHODS: Hepatic arterial three-dimensional CT angiography was performed using MDCT (Lightspeed Qx/I; GE Medical Systems, Milwaukee, Wis., U.S.A.) in 45 patients with HCC undergoing conventional angiography for transcatheter hepatic arterial chemoembolization. The scanning parameters during the early arterial phase were 2.5 mm slice thickness, 7.5 mm rotation of table speed, and a pitch of 3. Images were obtained by one radiologist using maximum intensity projection from axial CT images obtained during the early arterial phase. Two radiologists blinded to the findings of conventional angiography independently evaluated the hepatic arterial anatomy and the quality of the images obtained. RESULTS: Compared with conventional angiography, reader A correctly evaluated the hepatic arterial anatomy depicted at three-dimensional CT angiography. Reader B's evaluation was correct in 40 of 45 patients. Interobserver agreement was good (kappa value, 0.73), and both readers assessed the quality of three-dimensional CT angiography as excellent. CONCLUSION: Three-dimensional CT angiography using MDCT was accurate for delineating the arterial anatomy of the liver, and interobserver agreement was good. The modality may provide, prior to conventional angiography, valuable information regarding a patient's hepatic arterial anatomy.

5.Effects of Preoxygenation during induction of General Anesthesia.

After the description of the necessity of denitrogenation in rebreathing anesthesia circuits some decades ago, many methods were recommended for techniques of ventilation with oxygen before induction of anesthesia for the purpose of maintaining arterial oxygenation during laryngoscopy and tracheal intubation . In this point of view, we compared PaO2, MAP, PaCO2, pH of non-preoxgenation group(oxygen was given after succinylcholine was injected) with those of preoxygenation group(owygen was given after thiopental sodium was injected). The cases were selected randomly who blongs to ASA class l or ll(without cardiopulmonary abnormalities) and ages betweens 27 years and 66 years old. Before induction of general anesthesia, we cannulated radial artery after Allen's test and sampled arterial blood before anesthesia, after thiopenthal and succinylcholine were injected respectively, when tracheal intubation was done in the two groups and measured PaO2, PaCO2, MAP, pH respectively. PaCO2, MAP, pH changes in the two groups were not significant and of no clinical significanses. PaO2 after tracheal intubation showed marked increase in preoxygenation group compared to non-preoxygenation group but mean PaO2 was maintained within normal range in non-preoxygenation group with the apnetic period of tracheal intubation. WE found that non-preoxygenated patients also had normal range of PaO2 during the whole process of induction of anesthesis. But I think preoxygenation technique of any method can reserve more time and will do more effectively, especially when times consuming events of difficult intubation of any reason occurs.

6.Radical Nephrectomy with Extended Retroperitoneal Lymphadenectomy in Renal Cell Carcinoma: Review of 19 Cases.

A clinical observation was made on 19 patients of renal cell carcinoma who were treated by radical nephrectomy with extended retroperitoneal lymphadenectomy in the Department of Urology, Catholic Medical College during the period from May, 1980 to August, l985. Results were as follows; 1. Clinical stages by Robson`s staging system were 7 cases of stage I, 6 cases of stage II, 5 cases of stage III and 1 case of stage IV. Acuracy rate of clinical staging was 68.4%. 2. Pathologic stages were 8 cases of stage I, 5 cases of stage II, 4 cases of stage HI and 2 cases of stage IV. 3. Metastatic sites were perirenal fat(5 cases), regional lymphnode (2 cases), vessel(4 cases) and lung (2 cases). The rate of regional lymphnode involvement was 10.5%. 4. Postoperative complications were colocutaneous fistula (1 case), prolonged ileus (1 case) and pleural effusion (1 case). 5. Out of 19 cases 13 cases were alive without recurrence, 2 cases were alive with metastasis, 3 cases died with recurrence and 1 case died with other cause. 1 year survival rate was 77.8%.

8.A Case of Intracranial Arachnoid Cyst.

The intracranial arachnoid cyst is distinctly uncommon and they have unusual clinical course. The plain skull series were usually shown bulging of the overlying skull due to progressive collection of cerebrospinal fluid in cystic cavity. Pre-operative diagnosis is unlikely to be made. The predilection sites of this lesion are usually convexity of hemisphere, cisterna chiasmaticus, especiaily forebrain along the axis of main cerebral artery. The authors report a case of intracranial arachnoid cyst arisen along the Sylvian fissure combined chronic subdural hematoma follwoing sustained mild head injury. The pathologic diagnosis was arachnoid cyst, since the removed cyst wall was normal arachnoid tissue.

9.Antibiotic Sensitivity to the Causative Organism of Acute Simple Urinary Tract Infection for Recent 3 Years.

BACKGROUND: We studied the antibiotic sensitivities to the causative microorganisms of acute simple urinary tract infection for recent 3 years. METHODS: We analyzed 112 microorganisms and their antibiotic sensitivities of the 104 patients who were admitted to or visited the Department of Urology, Catholic University St. Marys Hospital and had more than 10cfu/mL on urine culture from June 1996 to January 1999 retrospectively. RESULTS: The chance of gram negative and positive as causative microorganisms was 72.3% and 27.7% respectively. The most common pathogenic microorganisms were Escherichia coli (67.0%) followed by a-hemolytic streptococci, Entercoccus. In gram negative acute UTI, imipenem, sulperanzone showed relatively higher sensitivity, while cotrimoxazole, ampicillin showed relatively lower sensitivity. In gram positive, vancomycin, penicillin showed relatively higher sensitivity, while ampicillin, imipenem showed relatively lower sensitivity. CONCLUSIONS: We consider that gram negative microorganisms, especially E. coli, is the main cause of acute simple UTI. But, we should be concerned about the increase of gram positive organisms and other gram negative organisms besides E. coli. Regarding to the choice of adequate drug in the treatment of UTI, it is necessary to consider the change of pathologic microorganisms.

10.Extent of Necrosis of Renal Cut Margin on Three Types of Partial Nephrectomy in Dogs.

Partial nephrectomy frequently accompanies serious complications such as secondary bleeding and urinary fistula and these complications result from necrosis of renal cut margin after partial nephrectomy. We have performed experimental partial nephrectomy to compare the extent of necrosis of renal cut margin after the wedge resection(Group l), the transverse polar resection(Group 2) and the chromic ligature resection(Group 3). In order to investigate which type of 3 partial nephrectomies cause less complications, 15 adult mongrel dogs were divided into three groups and partial nephrectomy was performed at the lower one quarter of 10 kidneys of 5 dogs in each group. The gross and microscopic findings of renal cut margin were observed at 1, 2, and 3 weeks after the operation. The results were as follows; 1. Mean depth of necrosis of the renal cortex at cut margin in Groups 1, 2 and 3 were 1.Ocm, O.2 cm and O.3cm, respectively. 2. Mean depth of necrosis of the renal medulla in Groups 1, 2 and 3 were 1.1cm, O.9cm and O.8cm, respectively. 3. On microscopic examination of the renal cortex at the 3rd week after operation, Groups 2 and 3 showed complete healing of necrosis but Group 1 showed delayed healing with remained necrotic tissue. From the present experiment it was found that both the transverse polar and chromic ligature resection cause less complications than the wedge resection.
