1.The Effects of Aprotinin on ACT and the Total Amount of Heparin for Open Heart Surgery.

BACKGROUND: Aprotinin, which is a nonspecific serine protease inhibitor, has an antiinflammatory and thrombogenic effect. However, it has an antithrombogenic effect during the cardiopulmonary bypass. This study was performed to evaluated the effects of aprotinin on the activated clotting time(ACT) and the total amount of the heparin used during the cardiopulmonary bypass. MATERIAL AND METHOD: From December 1998 to November 1999, 82 consecutive patients electively underwent open heart surgery at Gachon medical school. The patients were older than 18 years. Eighty two patients were classified into a control group(group C, n=36) and a aprotinin-treated group(group A, n=46). Body weight, height, body surface area(BSA), pump time(PT), aortic cross clamping time(ACCT), and body temperature(BT) were determined. Total amount of heparin and protamine during the CPB were also measured. ACT was determined before heparin administration, at 20, 40 and 60 minutes after heparin administration, and after protamine administration. RESULT: No significant differences were noted in either group in body weight, height, BSA, BT, and the total amoun of heparin and protamine. Group A demonstrated a significant(p<0.05) increase in age, PT, ACCT, and ACT at 20, 40, and 60 minutes after heparin administration. CONCLUSION: In summary, the use of aprotinin prime resulted in an increase in ACT. The total amount of heparin in aproinin-treated patient was similar to that of the control group in spite of having the prolonged pump time. Therefore aprotinin may reduce the requirement of heparin.

2.Renal Blood Flow in Chronic Glomerulonephritis.

Renal blood flow was measured by single injection technique of HippuranI13 in 13 patients with chronic glomerulonephritis and 11 control subjects. There was a significant decrease in renal blood flow in chronic glomerulonephritis particulary in those with elevated blood urea nitrogen. Renal blood flow was inversely proportionate to renal vascular resistance but no correlation was noted between renal blood flow and creatinine clearance.

6.Inflammatory Factors Predicting Dry Eye Syndrome in a Model Using Osmotic Pressure

Purpose:
To identify inflammatory factors that may serve as biomarkers for dry eye syndrome using a hyperosmotic dry eye syndrome model.
Methods:
Cultured human conjunctival epithelial cells were subjected to 400, 450, 500, and 550 mOsm/L NaCl, and cell viability was assessed in response to osmolarity. The relative expression of inflammatory factors was evaluated by real-time polymerase chain reaction of interleukin (IL)-1β, IL-6, IL-8, tumor necrosis factor (TNF)-α, metalloproteinase (MMP)-2, and MMP-9 according to the NaCl concentration.
Results:
An increase in the concentration of NaCl led to a notable decrease in cell viability (p < 0.05). IL-8, TNF-α, and MMP-9 expression levels increased significantly in proportion to NaCl concentration (p < 0.05).
Conclusions
IL-8, TNF-α, and MMP-9 may serve as effective biomarkers in dry eye studies.
7.Radiological Findings of Angiosarcoma in Breast: A Case Report .

Angiosarcoma is a rare primary malignancy occurring anywhere in the body, but most often in the skin, liver or breast. The radiological findings of angiosarcoma of the breast have rarely been reported. We encountered a case in which the condition involved a 32-year-old woman in whom mammography revealed a obscuved-marginated and lobular-shaped high-density lesion, and ultrasonography demonstrated an ovalshaped, microlobular-marginated, inhomogenous hypoechoic mass which metastasized to the whole body after surgery. We report the radiological findings of this case of angiosarcoma of the breast, and review the literature.

8.Effectiveness of Intramuscular Stimulation Therapy in Patients with Osteoarthritis of the Knee Joint.

BACKGROUND: Osteoarthritis(OA) has become one of the most important medical problem among the old age because it causes severe functional disability, but, up to date, the therapeutic methods for OA have not been so satisfactory to the patients with OA. Authors have investigated the effectiveness of intramuscular stimulation(IMS) therapy as an substitutive therapy for the relief of pain and dysfunction in patients with OA of the knee. METHODS: Eighteen patients who had visited the department of family medicine, Korea university anam hospital, with symptomatic OA of the knee during the period from March 1 to May 31. 2001 were included in this study and treated with IMS therapy on the quadriceps and hamstring muscles four times during two weeks. Patients self-scored Korean Western Ontario and McMaster universities Osteoarthritis Index (KWOMAC) at baseline and after the last therapy. The result was analysed by paired t-test and ANOVA. RESULT: Patients improved on KWOMAC scales for pain, stiffness, function and total scores after IMS therapy(p<0.05). The demographic characteristics such as body mass index (BML), duration of disease, affected site, radiologic finding did not have influence on the result of this study. No adverse effects of IMS therapy were reported. CONCLUSION: In this study, the improvement of pain, stiffness, and function of knee joint after treatment was reported. This study suggest that IMS is an effective and safe substitutive therapy for patients with OA of Knee.

9.Hemodynamic Effects of Propofol Anesthesia on Cesarean Section Patients.

BACKGROUND: It is already known that systemic vascular resistance (SVR) is decreased during pregnancy. In addition, one of the large hemodynamic changes when using propofol is also a decrease in SVR, more profoundly than is found with enflurane. It might therefore be suggested that hemodynamic changes are more prominent in cesarean section during propofol anesthesia, compared with enflurane anesthesia. This study was designed to investigate these possible changes by propofol anesthesia. METHODS: One hundred thirty six women for elective cesarean section were involved in this study. They were divided into 2 groups: group E (n = 74), anesthesia with thiopental, enflurane, and N2O, and group P (n = 62), anesthesia with propofol and N2O. All patient were given glycopyrrolate as premedicants. The hemodynamic variables (MAP, HR, CO, CI, SVR, SVRI, SI, and EF) were measured by bioimpedence at the five different time points: at preoperation, after induction, after intubation, during push abdomen, and after delivery. RESULTS: The variables of hemodynamics did not change significantly when both groups were compared, except that MAP, SVR and SVRI at after induction and HR during push and after delivery in group P were lower compared with respective variables in group E. CONCLUSION: From these results, we concluded that there were no significant changes in cardiovascular system (CVS) by propofol anesthesia, compared with enflurane anesthesia in cesarean section patients. It is therefore suggested that propofol anesthesia is safe in considering hemodynamics for cesarean section.

10.Expression of p53 and MDM-2 Protein in Cervical Neoplasia.

OBJECTIVES: MDM-2 is an oncoprotein that inhibits p53 tumor suppressor protein. Amplication and over- expression of its protein have been observed in human malignancies, and these abnormalities have a role in tumorigenesis through inactivation of p53 function. To elucidate the role of p53 and MDM-2 protein in cervical neoplasia we investigated the expression rates of MDM-2 and p53 protein in surgically resected specimens. METHEDS: Immunohistochemical studies using anti-p53 and anti-MDM-2 protein in the paraffin embedded section of 62 cases including cervical intraepithelial neoplasm(CIN) and invasive cervical cancer were performed. RESULTS: Expression rates of p53 protein were 25% in CIN I& CINII, 20% in CINIII, and 44% in invasive carcinoma, respectively. The MDM-2 protein were 33% in CIN I & CIN II, 16% in CIN III, and 48% in invasive carcinoma, respectively. There was no evident correlation between p53 positivity and MDM-2 positivity(p>0.05). However, correlation between MDM-2 negativity and p53 negativity was statistically significant(p=0.002) CONCLUSION: These data suggest that the expression of p53 protein is presumed to be necessarily correlated with MDM-2 protein expression in cervical neoplasia.
