Pachydermoperiostosis (idiopathic hypertrophic osteoarthropathy) ia an unusual syndrome of familial nature and characterized by clubbing of the digits, periosteal new bone formation especially over the distal ends of the long bones, and coarsening of the facial features with thickening and furrowing of the skin of the face and forehead. The case of a 46 year old male exhibiting the above characteristics is reported, The same tendencies were exhibited by his father before him. A review of literature is included.

4.2 Cases of Dual Left Anterior Descending Coronary Artery.

"Dual LAD" was defined as the early bifurcation of the proximal LAD into two vessels : a short LAD which remained in the anterior interventricular sulcus and does not reach the apex, and a long LAD which leaves the anterior interventricular sulcus only to return to the distal sulcus and continue to the apex. Recognition of "Dual LAD" is essential to prevent errors of interpretation of the coronary arteriogram and for planning of optimal surgical therapy. We report 2 cases of "Dual LAD" with the review of the literatures.

5.A Cses of Total Occlusion of the Left Main Coronary Artery.

A patient had total occlusion of the left main coronary artery that was proved by coronary arteriography. Patients with total occlusion of the left main coronary artery have a varying clinical presentation and may have prolonged survival. In patients with good collaterals, left ventricular function may be preserved. This report reveiws the clinical and angiographic findings of a patient with occlusion of the left main coronary artery with symptoms of unstable angina pectoris but without congestive heart failure or EKG evidence of myocardial infarction.

7.The Growth Inhibition Effect on the Causative Bacteria of Bacterial Vaginosis by Bacterial Strains Isolated from the Vagina of a Healthy Woman.

Two Gram-positive rod strains isolated from the healthy vagina of a woman were tested for the possibility as probiotics. One strain was identified as Steroidobacter denitrificans (YH1) and the other as Lactobacillus crispatus (YH2) by 16S rRNA partial sequencing. The Casman agar and Man-Rogosa-Sharpe (MRS) agar were mixed in same quantity, supplemented with 5% human rbc lysate (CMB agar). The Wilkins-Chalgren agar and MRS agar were mixed in same quantity (WCM agar). Gardnerella vaginalis was cultured in Casman broth, supplemented with 5% human rbc lysate and 1,000 x-diluted with normal saline. Bacteroides fragilis, Mobiluncus mulieris and Peptostreptococcus asaccharolyticus were cultured in Wilkins-Chalgren anaerobe broth and 2,000x-diluted. S. denitrificans YH1 and L. crispatus YH2 were cultured in MRS broth anaerobically and 100x-diluted. The diluted suspensions of B. fragilis, M. mulieris and P. asaccharolyticus were inoculated on WCM agar and G. vaginalis on CMB agar by cotton swabs. Ten microl aliquots of YH1 and YH2 were inoculated on the center of WCM agar and CMB agar. The growth inhibition zone diameters of B. fragilis, G. vaginalis, M. mulieris and P. asaccharolyticus by YH1 were 35 mm, 35 mm, 25 mm and 60 mm. The inhibition diameters by YH2 were 25 mm, 30 mm, 20 mm and 40 mm, respectively. These results implicate that S. denitrificans YH1 can be the stronger probiotics for the treatment of bacterial vaginosis than L. crispatus, compared inhibition zone diameters by YH1 and YH2.

The investigation of systolic time intervals and hemodynamics was performed in 42 patients with liver cirrhosis by noninvascive methods. The patients were divided into four groups according to the presence or absence of anemia and/or ascites: i.e. group I; cirrhosis without anemia and ascites, group II; cirrhosis with ascites only, group III; cirrhosis with anemia only, and group IV; cirrhosis with ascites and anemia. In the resting state of the patients, the systolic time intervals and hemodynamic data were measured by the high speed simultaneous recordings of electrocardiogram, phonocardiogram, carotid and femoral arterial pulse tracings, and compared with those obtained from 155 normal adult subjects. 1. The pulse were increased considerably in group IV, and diastolic blood pressure was elevated in group II with significance. 2. The stroke volume and cardiac output were increased significantly in group III. 3. The peripheral resistance was reduced particularly in group III, and the volume elasticit coefficient was decreased in group IV. 4. The QS1 interval was prolonged significantly in group II and IV, but QS1 interval corrected by multiple regression equation proposed by our laboratory (illustrated in the text) did not show significant difference compared with that of normal subjects. 5. The left ventricular ejection time(LVET) and total electromechanical systole(QS 2) appeared to be shortened in group II and IV, but the corrected LVET and QS 2 were not different significantly from those of the normal subjects

10.The Dental Factors in Oral Squamous Cell Carcinoma.

The oral cavity has frequent contacts with many carcinogenic compounds and its soft tissue is continuously stimulated by numerous dental factors. We have examined the detailed dental factors and its correlation with oral squamous cell carcinoma, and denture-wearing effects to analyze the effect of the dental factors on the genesis of oral squamous cell carcinoma. We have studied clinical contributing factors and the dental factors in the genesis of oral squamous cell carcinoma when the effects of smoking and drinking are controlled. The study cases are 100 patients (75 males and 25 females) who were diagnosed histo-pathologically as squamous cell carcinoma at the Yonsei Medical Center. The control group was 154 patients who have no systemic malignant tumors. The effects of 6 dental factors were analyzed in this study. They were divided into the smoking group, the non-smoking group, the drinking group, and the non-drinking group. The effects of dental factors were analyzed in each group. In this study, we have drawn some conclusions on the relationship between the dental factors and oral squamous cell carcinoma using x2-test. 1. The repaired teeth have statistical significance on the genesis of squamous cell carcinoma. This is probably due to the combining effects of past poor oral hygiene and continuous stimulation due to poor prosthesis. 2. There is statistical significance of the lost teeth in the smoking group, and the repaired teeth and the degree of alveolar bone resorption had statistical significance in the non-smoking group. 3. Smoking and drinking by-itself have no statistical significance in the genesis of oral squamous cell carcinoma. However, in combination, they have statistical significance. In this study, dental factors had a synergistic effect with smoking and drinking. Together with avoidance of smoking and drinking, appropriate restoration and oral hygiene control are most important factors in the preventive aspects of the oral squamous cell carcinoma.
