3.Operative Treatment of Medial Epicondylitis: A Comparative Analysis of the Clinical Outcomes between the Suture Anchor Group and the Non-suture Anchor Group.

BACKGROUND: The purpose of this study was to make a comparative analysis of the clinical outcomes after the operative treatment of refractory medial epicondylitis between the suture anchor group and the non-suture anchor group. METHODS: We enrolled 20 patients (7 men and 13 women) with recalcitrant medial epicondylitis who were able to receive operative treatment in a minimum of an 18-month follow-up. The mean age was 48.6 years (range, 36-59 years). The patients were allocated into either the suture anchor group (7 patients) or the non-suture anchor group (13 patients). We evaluated clinical outcomes using the visual analog scale (VAS), the pain grading system of Nirschl and Pettrone, and postoperative grip strength. RESULTS: The VAS score decreased from 8.8 to 2.0 for the suture anchor group and from 8.6 to 1.3 for the non-suture anchor group (p=0.16). The postoperative grip strength was 95%, 93% of the non-treated arm in both groups (p=0.32). The postoperative satisfaction level was good in 5 patients and fair in 2 for the suture anchor group and excellent in 5 patients, good, in 4, and fair, in 4 for the nonsuture anchor group (p=0.43). The clinical outcomes did not show a statistically significant difference between the two groups. CONCLUSIONS: We found that patients with recalcitrant medial epicondylitis were treated reliably with satisfactory clinical outcomes whether or not suture anchors were used. We believe the use of suture anchors when more than 50% of the tendon origin is affected provides an effective and favorable treatment modality.

5.The use of Methylmethacrylate as an Adjunct in the Internal Fixation of metastatic Femoral Shaft Fracture

This report deals with a 49-year-old male farmer who had a pathological fraeture. of the right femoral shaft due to metastatic carcinoma from the middle lobe of the right lung. The lesion of the femur was resected en-bloc and bone stabilization of the fractured bone was obtained by an intramedullary Kiintscher nail plus cement fixation. Two weeks later, a right middle and lower lobectomy was done. Microscopic examination of both specimens from right femur and lung revealed poorly differentiated squamous cell carcinoma. The postoperative course was satisfactory with no recurrence for period of 26 months.

6.Treatment of Supracondylar Fractures Following Total Knee Arthroplasty.

Supracondylar fractures of the femur following total knee arthroplasty are rare complication with reported rates ranging from 0.3% to 2.5%. Union of the fracture in proper alignment, without disruption of prosthetic component fixation and with maintenance of 90 of knee motion, is impotant in treatment of this type of fracture. Modalties of treatment are conservative or operative methods and operative treatment are open reduction 4 internal fixation, intramedullary nailing, revision arthroplasty using a prosthesis with a long stem and closed reduction and external fixation, but there are controversies in the method of treatment. Between Apr. 1989 and Jul. 1994, 10 patients were treated for supracondylar fracture of the ipsilaterai femur following total knee atthroplasty and 2 cases of them had arthrop]asty at other hospital. Average age of patients was 53 years (29-68 years), nine patients were women and one was man. The preoperative diagnosis was rheumatoid arthritis in four, degenerative osteoaithritis in three, posttraumatic osteoarthritis in one and tuberculosis sequelae of the knee in two. In nine cases mechanism of injury were slip down and in one was fall down. The interval hetween arthroplasty and fracture was an average of 22 months (9-79months). Two cases we.re treated with closed reduction and cast immobilization, one case was treated with open eduction and in1ernal fixation and seven patients of' ten cases were treated with closed intramedullay Ender nailing. The average foIlow-up period after fracture was 25 months (3 - 74months). We evaluated the results as the range of motion, bony union at simple radiograph and Knee Rating Score of Hospital for Special Surgery at last follow up. Eight cases of supracondylar fracture occuned among 350 patients, 514 cases of total knee arthroplasty which were operated at Kyung Hee Univcrsity Hospital between Apr. 1989 and Jul. 1994 and incidence was 1.6%. At the last follow up, all 10 cases achieved bony union and 7 cases using Ender naiIing achieved clinical bony union which enabled patients to do weight-bearing without pain at postoperative 6-8 weeks and radiological union was achieved at postoperative 12 weeks. Range of motion were average 105 degrees (80 J.20 degrees) hefore fracture and 86 degrees (6S 120 degrees) at the last follow up and HSS Knee rating score were average 80.3 points (59 94 points) before fracture and average 78.8 points (66-89 points) at the last follow up. At 7 cases using Ender nailing, range of motion were average 106 degrees (90 11S degrees) hefore fracture and 91 degrees (6S-105 degrees) at last follow up and HSS Knee rating score were average 80.6 points (59-94 points) before fracture and average 80.3 points (72-89 poiints) at the last follow up. Closed iintramedullary Ender nailing is a gecommendable operative method in the treatment of supracondylar fracture following total knee arthroplasty considering to good bony union, satisfactory functional result, easy operative technique and less postoperative complication.

8.Clinical and Mycological Study of Tinea Faciale.

BACKGROUND: Tinea faciale usually has been classified as tinea corgoris. Recently it has often been classified as a seperate disease recently because of its various clinicel pesentations. OBJECTIVE: Our purpose was to evaluate the clinical characteristics and mycological findings of tinea faciale, METHODS: Clinieal and mycological study was done with 42 eases of tinea faciale among out-patients examined for eight years from January 1985 to December 1992 at Yeungnam University Hospital, Taegu, Korea. RESULTS & CONCLUSION: 1. The average of annual distribution of patients was five and there a as no significant difference according to year. The age distribution of patients was highest under age 10(33.4%). The ratio of male to female patient was 3.1: 1. Monthly or seasonal distribution if patients was not significantly different. 2. Of 42 cases, causative organisms were isolated in 31 cases. They are Trichophyton(T.) rubrum in 19 cases (61.3%), Microsporum(M.) canis in 6 cases (19.4%), T. mentcgrophytes in 5 cases (16.1%) and M. gypseum in 1 case(3.7%) in tbe order of decreasing frequncd M. canis was the main causative organisrn in children under age 10. In the age group after 10, T. rubrum was predominated. T. rubrum infection was most frequently seen in spring, T. mentagrphates infection was seen more in winter and M. canis infectior in autumn. 3. Classic annular types were seen most frequently in the cases caused by T. rubrum and T. mentagrophytes. Papular types were seen most commonly in the case caused by M. canis. Multiple facial lesions were seen in T. rubrum infection (2 cases) and Of canis infection (1 case). Tinea faciale simulated several other dermatoses. These were contact, dermatitis, lupus erythematosus, photosensitive dermatosis, rosacea, granuloma annulare and acne. Of these, tinea faciale simulated contact dermatitis most frequently. Coexisting fungal infections were found in 7 patients and tinea corporis was most frequently present. Topical antifungsl agent was tried alone in 22 cases, and topical and systemic antifungal agent were combined in 20 cases.
