2.Change of Plasma Glucose Levels in Term Neonates during the First 72 hours using the New Reagent Strip Method.

Blood glucose levels were measured in 89 healthy term neonates during the first 72 hours using the SureStep, a newly developed reagent test strip method by LifeScan. The blood samples were obtained by heel-stick puncture and blood glucose concentrations were monitored at birth(0), 2, 4, 6, 12, 24, 48, and 72 hours after birth. Mean and standard deviation of their measurement were compared according to postnatal hours and type of delivery. Comparison of significance between mean plasma glucose levels were made with the Wilcoxon rank sum test and significance level of 0.05 was used to determine which pair-wise comparisons were significantly different. The mean plasma glucose concentrations of first 6 hours were significantly lower than those of 12, 24, 48, 72 hours. This finding indicates that plasma glucose stabilization in healthy fullterm neonates can be expected with the feedings. The mean plasma glucose concentration at birth in the neonates of cesarean section (64.5+-8.06 mg/dl) was significantly lower than that of vaginal delivery (80.3+-20.7 mg/dl), but there were no significant differences after 2 hours. This may be due to the difference in prenatal care of obstetric department of Horamae hospital (C/S: midnight NPO and Hartmann solution, V/D: NPO with labor pain and 5% dextrose solution intravenously). In summary, the changes in perinatal care, especially prenatal fluid therapy and time of first feeding should be considered in defining neonatal hypoglycemia. Our data suggest that hypoglycemia should he defined as below 40 mg/dl during the first 6 hours and below 55 mg/dl thereafter.

3.Identification of Oocyte-Specific Diva-Associated Proteins using Mass Spectrometry.

OBJECTIVE: We previously described that Diva is highly expressed in matured metaphase II (MII) oocytes compared to immature germinal vesicle (GV) oocytes in mouse.1 We report here that the expression of Diva transcript as well as protein is oocyte-specific. To elucidate its physiological role in oocyte, the binding partner(s) of Diva has been identified by using immunoprecipitation (IP) followed by Mass Spectrometry. METHODS: NIH/3T3 cells were transiently transfected for 24 h with either empty vector for control or FLAG-tagged mouse Diva construct, and IP was performed with anti-FLAG antibody. The immuno-isolated complexes were resolved by SDS-PAGE on a 12% gel followed by Coomassie Blue staining. For in-gel digestion, 15 bands of interest were excised manually and digested with trypsin. All mass spectra were acquired at a positive reflector mode by a 4700 Proteomics Analyzer (Applied Biosystems, Framingham, MA). Proteins were identified by searching the NCBI nonredundant database using MASCOT Peptide Mass Fingerprint software (Matrixscience, London). RESULTS: Diva-associated complexes were formed in FLAG-tagged mouse Diva-overexpressed NIH/3T3 cells via IP using anti-FLAG-conjugated beads. Among the excised 15 bands, actin and actin-binding proteins such as tropomyosin, tropomodulin 3, and alpha-actinin were identified. Binding between Diva and actin or tropomyosin was confirmed by IP followed by Western blot analysis. Both bindings were also detected endogenously in mouse ovaries, indicating that Diva works with actin and tropomyosin. CONCLUSIONS: This is the first report that immuno-isolated Diva-associated complexes are related to actin filament of the cytoskeletal system. When we consider the association of Diva with actin and tropomyosin, oocyte-specific Diva may play a role in modulating the cytoskeletal system during oocyte maturation.

6.Outcome of Hemodialysis Treatment on 200 Cases of Chronic Renal Failure.

Regular hemodialysis has been established as a way of treatment for end stage renal failure. In adults, hemodialysis 5 hours each time, three times weekly with one square meter hemodialyzer is now widely accepted as a standard, and there are many reports on long term follow up studies. Quite a large difference are present, however, in our community mainly originated from patient's poor compliance in frequent dialysis with short interdialysis interval. The author analyzed 200 cases of chronic renal failure who have received hemodialysis treatments during the 5 year period from July 1976 to June 1981 at the hemodialysis unit of the hanyang University Hospital and the following results are obtained; 1. Actual one year survival rate was much higher in thrice(87.1%) than one(35.1%) or twice(54.1%) weekly dialysis. 2. Long term(more than 6 months) complications were also more frequent in once(85.7%) or twice(70.2%) weekly treatments than in thrice(22.2%). The predominant complications were congestive heart failure, pericarditis, and infections. 3. Those who recieved three times weekly dialysis had better rehabilitation grades than the patients groups of twice or once weekly treatment. 4. Of peridialysis distressful symptoms, nausea, vomiting dizziness were less frequent in the group three times a week dialysis. In contrast, headache, hypotension, muscle cramps, and weakness were not significantly related with frequency of hemodialysis. 5. Those who voided a large amount of urine output had better survival and less frequent requirement of blood transfusions. 6. Main causes of death were due to patient poor compliance, hyperkalemia and cerebrovascular acidents. 7. Economic problems were the major cause of dialysis interruption.

Sarcopenia is defined the loss of muscle mass and strength with aging. Although the etiology and the pathogenesis, the diagnosis of sarcopenia are obscure, the sequel of sarcopenia, i.e. morbidity and mortality, metabolic complications, are a major public health problem. Particularly, aged people in Korea are increasing rapidly more than expected; therefore the interest of sarcopenia is also increasing rapidly. This article provides an overview of the pathogenesis, and diagnosis, epidemiology, clinical results.

9.Osteoporosis and Decrease in Bone Mineral Density Have associated with the Reduced Quality Of Life

OBJECTIVES: There are lack of study for the association between osteoporosis and reduced quality of life in Korean. In this study, the association between osteoporosis and reduced quality of life were analyzed. METHODS: We used the data from the fourth Korea National Health and Nutrition Examination Survey (KNHNES), and 891 women were enrolled who were over the age of 50. Bone mineral density was measured by dual-energy X-ray absorptiometry. The quality of life were measured EuroQol-5 Dimension (EQ-5D). RESULTS: The quality of life of osteoporosis patients, were reduced in all dimensions of EQ-5D, except EuroQol;anxiety and depression. However, EuroQol; anxiety depression side (P=0.05) and VAS (P=0.039) of EQ-5D had significant difference among osteoporosis, osteopenia, and normal group even after adjusted with age, weight, waist circumference, and blood levels of vitamin D. In addition, bone density and EQ-5D utility values had positive association even after adjusted with age and weight, waist circumference, blood levels of vitamin D (R2=0.137, B=0.148, P=0.011). CONCLUSIONS: The elderly women with osteoporosis were significantly associated with decreased quality of life. Thus, the quality of life should be considered in treating of osteoporosis patients.
