2.Osteochondroma in the Rib: A Case Report Reviewing Magnetic Resonance Imaging Findings

Osteochondroma is a benign bone tumor that is most commonly found in the extremities and is prevalent in adolescents, with a peak incidence before the age of 20 years. Herein, we present a rare case of a 33-year-old man with an asymptomatic tumor on the right anterior rib that was pathologically confirmed to be a rib osteochondroma after surgical excision. We describe the computed tomography and magnetic resonance imaging (MRI) procedures and highlight the importance of MRI as a critical diagnostic tool for osteochondroma.
3.Accuracy of 18F FDG PET after Surgery and Radiotherapy in Head and Neck Cancers.

PURPOSE: The purpose of this study was to evaluate the diagnostic accuracy of [18F]FDG PET in the diagnosis of recurrent head and neck cancer after the completion of surgery and radiotherapy in patients with head and neck cancers. MATERIALS AND METHODS: In fifty-nine patients with head and neck cancers, whole body [18F]FDG PET studies were performed. According to the different therapeutic modalities, patients were divided into four groups (Group I; pre-treatment, Group II; surgery, Group III; radiotherapy, Group IV; both surgery and radiotherapy). [18F]FDG PET images were compared with clinical, CT and histopathologic findings. RESULTS: For detection of metastatic lymph nodes in 14 patients of pre-treatment group (group I), the sensitivity and specificity of PET were 100% (10/10) and 75% (3/4), and those of CT were 80% (8/10) and 100% (4/4). For detection of recurrence in 45 patients of post-treatment group, overall sensitivity and specificity of PET were 96.2% (25/26) and 78.9% (15/19) [(100% and 75% in group II, 80% and 50% in group III, and 100% and 100% in group IV)] without significant difference from pre-treatment group (p>0.1). In detecting recurrence, the sensitivity and specificity of [18F]FDG PET were 90.9% (10/11) and 20% (1/5) in 16 patients who underwent [18F]FDG PET within 2 months after the completion of treatment. The specificity of these patients was significantly lower than that of 29 patients (100% of sensitivity and specificity) who underwent [18F]FDG PET 2 months after treatment (p<0.05). CONCLUSION: [18F]FDG PET is an accurate diagnostic modality for the detection of recurrence in head and neck cancer. Post-therapy [18F]FDG PET should be obtained at least 2 months after the completion of surgery or radiotherapy.

4.The Incidence of Pediatric Trampoline Injury: Statistics from a Single Institution.

PURPOSE: Recently, there has been increasing number of trampoline injuries at our pediatric orthopedic clinic and emergency room. Therefore, this study is to analyze the prevalence of the trampoline injuries with emergency room surveillance. MATERIALS AND METHODS: Between September 2015 and February 2017, all patients under the age of 15 years, who visited emergency room of a teaching hospital with trauma and referred to the department of orthopedic surgery, was included; the cause of trauma was analyzed. The severity of injury was also evaluated and compared according to the cause of injury. RESULTS: There were a total of 1,807 patients under the age of 15 years who visited our emergency room during the study period. There were a total of 71 trampoline injuries (3.9%). There were 28 patients below the age of 5 years, and among them, 14 minor injury were recorded; 43 patients were over the age of 5 years and with 4 minor, 20 moderate, and 19 severe injuries. In the age between 5 and 15 years, the prevalence of mild injury was lower in trampoline injury compared with those of other sports injuries (9.3% vs. 34.0%) and the prevalence of severe injury was higher in trampoline injury (44.2% vs. 26.3%) (p=0.002). CONCLUSION: Our findings indicate that the incidence of trampoline injury was lower than other country, but it was more severe than injuries from other sports. With increasing incidence of trampoline injuries, parents should be more mindful that trampolines are not safe and stricter supervision may be necessary.

5.MR Findings of Spinal Angiolipoma: A Case Report.

Angiolipomas are usually found in the soft tissues of the extremity, trunk, or neck. Spinal angiolipomas are rare, accounting for between 0.14% and 1.2% of spinal axis tumors, and the majority are extradural. We report a case of surgically proven extradural spinal angiolipoma with a high vascular content and involving the thoracic level.

6.Trichilemmal Carcinoma of the Upper Eyelid: A Case Report.

We report a very rare case of trichilemmal carcinoma (TLC) involving the upper eyelid. To the best of our knowledge, this is the first report of trichilemmal carcinoma of the upper eyelid in Korea. A 51-year-old man presented to our hospital complaining of a bloody discharge from his left upper eyelid. He had a soft and lobulated mass on the palpebral conjunctiva. An incisional biopsy revealed trabecular growth of tumor cells with clear cytoplasm, prominent nucleoli, frequent mitoses, and foci of trichilemmal keratinization. Immunohistochemically, the lesion was positive for p53 and negative for CD 34. A diagnosis of TLC was made, and total excision of the mass and reconstruction of the eyelid were performed. Trichilemmal carcinoma is a rare malignant tumor, though it appears to be an indolent neoplasm with no metastatic potential. The treatment of choice for trichilemmal carcinoma of the eyelid is complete excision with tumor-free margins due to the locally invasive nature of the lesion.

8.Comparison of Intubation Efficiency by Suction Device in a Hemorrhagic Airway Manikin Model: Yankauer Suction Tip vs. Polyvinyl Chloride Catheter Suction Tip

PURPOSE: Tracheal intubation in a hemorrhagic airway is a difficult procedure because the visibility can be obscured by blood or vomitus. Several devices and methods have been developed to overcome such obstacles, but they are not available at all practical sites. Therefore, this study was conducted to assess the time and success rate of tracheal intubation according to the type of suction tip used in a hemorrhagic airway manikin model. METHODS: This study was a randomized crossover manikin simulation study. We recruited 18 emergency physicians to perform intubation with suction using a Yankauer suction tip and a polyvinyl chloride (PVC) catheter suction tip in a hemorrhagic airway manikin model. We then measured the time and success rate of intubation for each suction tip. RESULTS: The mean intubation time using the Yankauer suction tip was 42.3 seconds, and the mean intubation time using the PVC catheter suction tip was 91.1 seconds (p < 0.001). Two cases of esophageal intubation occurred when the PVC catheter suction tip was used (success rate, 88.9%). In contrast, there was no esophageal intubation when the Yankauer suction tip was used (success rate, 100%) (p=0.217). CONCLUSION: Intubation of a hemorrhagic airway manikin model can be performed in a shorter time when a Yankauer suction tip is used than when a PVC catheter suction tip is used.

9.The Long-Term Observation of Intraocular Pressure after Intravitreal Injections of Bevacizumab, Triamcinolone, and Combination of Both.

PURPOSE: The present study investigates the onset and frequency of increased intraocular pressure (IOP) after injections of intravitreal Bevacizumab, Triamcinolone, and a combination of both drugs. METHODS: Patients were classified into three groups: Bevacizumab group (group A), Triamcinolone group (group B), and combined drug group (group C), irrespective of the underlying causes. The IOP was measured 30 minutes prior to followed by one day, one week, one month, two months, three months, six months, and one year after injection. The measured IOP at each time point was compared with the pre-injection IOP and the differences in IOP among the three groups were statistically analyzed. The relationships between various factors possible of increasing IOP were also analyzed. RESULTS: A total of 259 subjects were enrolled in the present study. An IOP increase of more than 5 mm Hg was observed in 25 eyes (15%) in group A, 34 eyes (40%) in group B, and 40 eyes (50%) in group C. There was no statistically significant mean IOP change after injection in group A, but in groups B and C there was an increase in mean IOP up until two and three months after injection, respectively. However, eyes in group A with a history of glaucoma showed statistically significant increases in IOP. CONCLUSIONS: The onsets and frequencies of increased IOP were different with the different drugs. Proper follow-up for increased IOP after injection is necessary based on the type of drug used.

10.Effects of an Intravitreal Bevacizumab Injection Combined With Panretinal Photocoagulation on High-Risk Proliferative Diabetic Retinopathy.

PURPOSE: To investigate the short-term effects of panretinal photocoagulation (PRP) combined with an intravitreal injection of Avastin(R) (bevacizumab) as an adjuvant to high-risk proliferative diabetic retinopathy (PDR). METHODS: The data was collected retrospectively from the eyes of high-risk PDR patients, which were divided into two groups. One eye was treated with only PRP (PRP only group) and the fellow eye of same patient was treated with both PRP and intravitreal bevacizumab injection (Adjuvant group). Best corrected visual acuity (BCVA), IOP (intraocular pressure), and new vessel (NV) size in fluorescein angiography were recorded immediately and at the six-week follow-up visit. Adverse events associated with intravitreal injection were investigated. RESULTS: Of 12 patients with high-risk PDR, five were male and seven were female. There were no statistically significant BCVA or IOP changes after treatment in either group (p=0.916, 0.888). The reduction of NV size was found in both groups, but NV size in the adjuvant group showed a greater decrease than that of the PRP only group (p=0.038). Three patients had adverse events after intravitreal injection. Two patients had mild anterior uveitis and one patient had a serious complication of branched retinal artery obstruction (BRAO). CONCLUSIONS: Intravitreal bevacizumab injection with PRP resulted in marked regression of neovascularization compared with PRP alone. One serious side effect, BRAO, was noted in this study. Further studies are needed to determine the effect of repeated intravitreal bevacizumab injections and the proper number of bevacizumab injections as an adjuvant.
