3.Nurses' Job Analysis in a Regional Emergency Center Using the DACUM

Purpose:
This study aimed to establish the role of and to analyze the job of nurses in a regional emergency center using the Developing a Curriculum(DACUM) process, a job analysis technique.
Methods:
The DACUM committee was organized, and three workshops were held from June 1st to July 4th, 2016. One hundred two nurses in a regional emergency center participated in a survey from October 3rd to 10th, 2016. The frequency, importance and difficulty of each tasks and task elements in the DACUM were investigated on a Likert scale.
Results:
The roles of nurses in a regional emergency center were identified and the DACUM chart was developed with 6 duties, 29 tasks and 153 task elements. According to the survey, some tasks such as ‘CPR,’ ‘acute respiratory distress care,’ and ‘drug administration’ showed high frequency and importance or difficulty in all nursing units in common, and different results reflected the characteristics of each nursing unit were also found.
Conclusion
The results showed the feature of the regional emergency center. Based on these results, it is necessary to develop educational programs for practice.
4.The Effect of Premedication using a Jet-injector in Pediatric Patients.

BACKGROUND: This study was designed to evaluate the efficacy and safety of midazolam premedication administered by a jet-injector in pediatric patients. METHODS: Children undergoing outpatient surgery were randomized into three groups: intravenous induction with thiopental sodium (control group, n = 20), oral midazolam premedication (PO-med group, n = 20) or midazolam premedication using a jet-injector (Jet-med group, n = 20). In the PO-med and Jet-med group patients, anesthetic induction was performed by sevoflurane inhalation and an intravenous catheter (IVC) was inserted after the children had been anesthetized by sevoflurane inhalation. For the control group patients, an IVC was placed in the preoperative holding area. Agitation scores were recorded in the preoperative holding area and recovery room. Anesthesia times and the views of the medical staff concerning the technique benefits were also noted. Patients and parents were interviewed on the following day. RESULTS: Maximum agitation scores in the preoperative holding area and during separation with parents were significantly lower in the PO-med and Jet-med groups. The induction time was significantly longer in the PO-med and Jet-med groups. No statistically significant differences were found for the recovery characteristics. Though interviews with patients and parents produced similar results for patients in each group, the medical staff satisfaction levels were significantly higher for patients in the PO-med and Jet-med groups. CONCLUSIONS: The findings of this study suggest that the application of the jet-injector for midazolam premedication may be clinically useful in children who do not have an IVC. The use of the jet-injector could be a substitute for the oral route for midazolam administration.

5.Clinical Guidelines for the Diagnosis and Treatment of Cushing's Disease in Korea.

Cushing's disease (CD) is a rare disorder characterized by the overproduction of adrenocorticotropic hormone due to a pituitary adenoma that ultimately stimulates excessive cortisol secretion from the adrenal glands. Prior to the detection of pituitary adenomas, various clinical signs of CD such as central obesity, moon face, hirsutism, and facial plethora are usually already present. Uncontrolled hypercortisolism is associated with metabolic, cardiovascular, and psychological disorders that result in increased mortality. Hence, the early detection and treatment of CD are not only important but mandatory. Because its clinical manifestations vary from patient to patient and are common in other obesity-related conditions, the precise diagnosis of CD can be problematic. Thus, the present set of guidelines was compiled by Korean experts in this field to assist clinicians with the screening, diagnoses, and treatment of patients with CD using currently available tests and treatment modalities.

7.Amiodarone Induced Toxic Hepatitis: A case report.

Although amiodarone is a widely used and effective long-term antiarrhythmic drug, it is known to have life-threatening adverse effects, including acute hepatotoxicity. Although the occurrence of acute hepatotoxicity after amiodarone administration is rare, it can be fatal. However, this phenomenon has not been well studied. Here, we describe a 39 year old woman with atrial fibrillation and supraventricular tachycardia who developed acute toxic hepatitis within 8 days of amiodarone administration at the recommended dosage. The drug was withdrawn and the laboratory findings progressively returned to normal.

The intestinal lesion of Behcet's colitis shows aphthoid or punched-out ulceration. But these morphological characteristics are variable and the clinical course of Behcet's colitis is also diverse. There have been many case reports since 1962, but there is few study for clinical course or for morphological characteristics of intestinal lesion of Behcet's colitis. So we retrospectively investigated the clinical course of Behcet's colitis according to colonoscopic findings. Thirty-two patients with Behcet's colitis were investigated retrospectively from January 1984 to July l994. The patients who had at least two major stigmata of Behcet's syndrome with intestinal lesions were included, and whose colonoscopic films were available and whose follow-up studies were done. The studies used at their follow-up were colonoscopy and/or barium enema. In our study the patients ranged in age from 15 to 64 years old, with mean age of 37 years old; 28% were in 3rd decade. The mean follow-up period was 33 months.

10.What are the Characteristics of Obese Adults without Metabolic Complications?.

BACKGROUND: It is known that various subtypes of obesity exist and there has been a growing interest in the various subtypes of obesity, especially in the unique "metabolically healthy but obese" subtype. The purpose of this study was to analyze the features of obese adults without metabolic complications and to provide data which can be used in strategy formation for the effective treatment of obesity. METHODS: A total of 4,561 obese adults with a BMI greater than or equal to 25 kg/m2, were recruited from a health promotion center of a university hospital. Sociodemographic and life style information were gathered by self-questionnaire. We classified them into 2 groups-one having no features (metabolically healthy obese; MHO) and the other having 1~5 features (metabolically unhealthy obese; MUHO)-using the ATP III definition for metabolic syndrome, and used the multiple logistic regression analysis to examine the association between various factors and MHO. RESULTS: Age and BMI were related with MHO independently. Adjusted odds ratio for the MHO men was 2.17 compared with women (P<0.001). Adjusted odds ratio for the MHO regular exercise group was 1.66 compared with the no exercise group (P<0.007). Additionally, those who exercised more had a greater probability of being in the MHO group. Adjusted odds ratio for the MHO in the large income group was 1.88 (P=0.007) compared with those in the small income group. As was seen with exercise, there was a direct relationship between higher income and the probability of being in the MHO group. CONCLUSION: Men compared with women, large income compared with small income, and regular exercise compared with no exercise were features of obese adults which increased their probability of being categorized as "metabolically healthy but obese".
