1.Nursing Needs of Parents with Hospitalized Child.

The main purpose of this study is to identify nursing needs of parents who have hospitalized child. Research design is a descriptive survey. The subjects for the study were 79 parents who have hospitalized child at a pediatric ward of one of the general hospital attached to a university in J city. The data was collected by researchers using a structured questionnaire. The questionnaire was Liken type 5 point scale, composed of 5 categories with 61 items. The data was analyzed by SPSS/PC. The results of the study were as follows : 1) Mean score of nursing needs of subjects were 3.93 at 2nd day of admission and 3.99 at 7th day of ad mission. 2) Among the categories, the highest nursing need at the 2nd day and 7th day of admission was 'direct nursing', in desending order, 'facilities and environment', 'education and counseling', 'nursing assessment'. The lowest nursing need was 'reference'. 3) Differences between the nursing needs of subjects at 2nd day and 7th day were as follows ; (1) By categories, there was a significant difference only in the 'reference' category(t=2.74, P=.008). (2) By items, there were significant differences in items of 'to check necessary materials(t=2.31, P=.024)', 'to understand family function and family relationship(t=2.12. P=.041), 'to set up study room(t=2.22, P=.030)', and 'to mediate parent's meeting group(t=3.89, P=.000)'. The above result indicated that nursing needs of parents with hospitalized child were above average, especially very high in items about disease process, and items directly associated with treatment and nursing care. So, nurses have to focus on information about the patient's state of disease, treatment, test and procedure, and in efficiently giving direct nursing care to implement more effective care for the hospitalized children and their parents. And some future research is needed to identify the difference of degree of nursing needs of parents with hospitalized child according to admission duration using a different sample and a longer sampling interval.

3.Clinical Experience of Complete Neurologic Recovery from Severe Hypoxic Ischemic Encephalopathy after Cardiac Arrest.

Prediction of individual outcome after cardiopulmonary resuscitation is of major medical, ethical, and socioeconomic interest but uncertain. We experienced the case thats the patient got complete neurologic recovery after the 123th day firm cardiac arrest, who had been suspected to go with poor prognosis because she got the findings of Glasgow Coma Scale 4, severe diffuse encephalopathy on encephalogram and generalized tonic-clonic seizure at the 4th day. Recently, a 29 year-old women who sustained from respiratory arrest induced presumably by sedative and anticonvulsant therapy for control of seizure that happened during local lidocaine anesthesia far mamoplasty was transfered to our emergency medical center from local private plastic office. Arrest time was about 20 minutes. On hospital arrival, she had a pulseless bradyasystole and no respiration, but spontaneous circulation was restored at 10 minutes artier CPR started. We started cerebral oriented resuscitation including mild hypothermia(34degrees C), hemodilution, calcium channel blocker infusion. On hospital day 4, patient's glasgow coma scale(GCS) was 4. On hospital clay 7, Brain Magnetic Resonance Imaging(MRI) showed high signal intensity on T2WI, involving the bilateral basal ganglia. After contrast administraton, marked enhancement can be seen at the lesion site. Patient's glasgow coma scale(GCS) increased step by step to 5 on 8th day, 7 on 14th day, 10 on 15th day, 13 on 17th day, 15 on 20th day. 40 days later the patient was discharged with minor neurologic abnormality including hand tremor, dysphonia, amenorrhea and Mini Mental State Examination(MMSE) score(26). Long-term Follow up revealed that all neurologic functional abnormality inducting hand tremor, dysphonia, amenorrhea and MMSE score(26) is completely recovered on 123th day after episode of cardiopulmonary arrest.

4.Inflammatory Myofibroblastic Tumor of the Breast: A Case Report.

An inflammatory myofibroblastic tumor is an uncommon type of tumor composed of spindle-shaped myofibroblast admixed with mature plasma and inflammatory cells. It is most common in the lungs and in various organs; however, a few cases of inflammatory myofibroblastic tumor of the breast have been reported in the literature. To date, there are no typical radiologic findings but an ill-defined marginated mass which was a suspicious malignancy has been reported in the literature. Here, we describe the mammographic and ultrasonographic findings of a case of an inflammatory myofibroblastic tumor of the breast with a spiculated margin which was strongly suspect as being a malignancy in a 36-year-old woman.

5.Retroperitoneal Tumors Mimicking Adrenal Tumor.

Periadrenal retroperitoneal tumors are frequently misdiagnosed as adrenal tumors because of its rarity and anatomical vicinity. Thus, there are only very few cases reported and little information available in the literature. Preoperative diagnoses of theses tumors are challenging. The definitive diagnosis is based on postoperative pathological findings. We report two cases of periadrenal retroperitoneal tumors that mimicked adrenal neoplasms. Final diagnoses were mature cystic teratoma and papillary renal carcinoma, respectively. Although periadrenal retroperitoneal tumors are rare, it should be considered in the differential diagnosis of adrenal incidentalomas.

8.Interleukin-8 and MCP(Monocyte Chemoattractant Protein)-1 expression by the Human Dental Pulps in cultures stimulated with Substance P.

The induction of the IL-8 and MCP-1 by the stimulation of Substance P and TNF-alpha (IL-8 agonist) and the specificity for SP using Spantide (SP antagonist) in the dental pulp tissues was measured quantitatively. In addition, the secretion of the IL-8 in the human dental pulp tissue 36 hrs after the stimulation of SP was observed after the stimulation of SP qualitatively. According to this study, the results were as follows: 1. There was the significant IL-8 induction at 36 h after SP (10-4M) stimulation of the pulp tissue comparing with the unstimulated dental pulp tissues (p < 0.05). IL-8 immunostaining was weakly detected along the periphery of the pulp tissue after Mock stimulation and IL-8 immunostaining was detected around the fibroblast in the pulp tissue 36h After SP (10-4M) stimulation, 2. The secretion of MCP-1 from the dental pulp tissues comparing with Mock stimulation was induced at 36 hrs after TNF-alpha (40 ng/ml) stimulation, but no induction with SP(10-4M). TNF-alpha (40 ng/ml) did not induce the IL-8 secretion from the pulp tissue, weak IL-8 immunostaining was detected along the periphery of the pulp tissue. 3. Spantide (10-5M) inhibited IL-8 induction from the pulp tissues 36 h after SP (10-4M) stimulation. These results suggest that SP significantly induces IL-8 recruiting neutrophils in localized human dental pulp tissue. MCP-1 appears to be less involved in the early establishment of pulpal inflammation in response to irritation such as mechanical insult of dentin. SP may have positive relation with the inflammation of the human dental pulp tissues.

9.A Case of Burkitt's Lymphoma with Bilateral Renal Enlargement.

Burkitt's lymphoma is a distinct pathologic entity characterized as a diffuse undifferentiated malignant lymphoma of B-lymphocyte origin. We experienced a case of Burkitt's lymphoma with bilateral renal enlargement in which a 3year-old male patient was admitted with complaints of abdominal distention and facial edema. Laboratory examination revealed positivity for CD 19, CD 10, CD 20 and c-myc on flow cytometry, bilateral renal enlargement(10x12cm in longitudinal length) on abdominal ultrasonogram and CT, malignant lymphoma of Buritt's type with prominent nucleoli and cytospin of cerebrospinal fluid. Initially he showed tumor lysis syndrome and clinical stage D by Zigler(stage IV by Murphy) with CNS involvement. After initial management with hydration, urine alkalinization and allopurinol, combination chemotherapy had been applied with the craniospinal radiotherapy according to the CCG 503 II regimane, with achievement of complete remission. Thus we report a case of Burkitt's lymphoma with bilateral renal enlargement with a biref review of literatures.

10.Metastatic Clear Cell Renal Cell Carcinoma to the Thyroid Gland.

Clear cell renal cell carcinoma (RCC) is the most common histological subtype of malignant renal tumors. Although RCC frequently metastasizes to many organs, it rarely metastasizes to the thyroid gland without evidence of other organ involvement. We report a case of a metastatic renal cell carcinoma to the thyroid gland, which was misdiagnosed preoperatively as a follicular neoplasm from the fine needle aspiration cytology.
