4.A Case of Pemphigus Vegetans.

We experienced a 54-year-old female patient who had multiple vegetaing plaques and, peripherally, a few pustules in her both axillae. On the skin biopsy specimen of the vegetating plaque, it showed acanthosis, suprabasal acsntholysis, papillo-matosis, downward proliferation of the epidermis, and eosinophilic microroabscess in the epidermis. The indirect immunofluorescence study disclosed anto-antibody positive to intercellular substance of the lip of guinea pig, while direct. immuno-fluorescence of the perilesional skin reealed no specific findings. The skin lesion had improved with topical and systemic steroid therapy.

5.A Case of A cute Fevrile Neutrophilic Dermatosis.

A 36-year-old man has suffered from multiple, well defined, variable sized, and elevated papulonodular patches with oozing, scale, and crust on the face. He also presented multiple folliculitis-like eruptions on upper chest, back, and both limbs. He had no leukocytosis, but elevated ESR and fever during development of skin lesions. Histologically a dense iaflammatory cell infiltra,tion composed predominantly of polymorphonuclear neutrophils and few lymphoid cells were seen in the dermis and subcutis. The symptoms and signs did not spond to dapsone and tetracycline, but disappeared on orally administered predniselone.

6.The Effect of CO2 on Body Temperatures during Cooling and Rewarming of Cardiopulmonary Bypass.

Background: It is well known that the addition of CO2 gas during cardiopulmonary bypass (CPB) can increase cerebral blood flow (CBF). In this study, we tried to find whether the increase of CBF by CO2 gas could be expressed on the measurement of body temperatures. Methods: This study included 22 adult patients who underwent CPB with moderate hypothermia and were assigned randomly to two groups by adding CO2 gas (CO2 group and Air group). We measured each tympanic (TM), nasopharyngeal (NP), oral, forehead, finger, rectal and CPB arterial line temperatures of CPB circuit for 20 times during cooling in every one minute for 20 minutes and during rewarming every two minutes for 40 minutes. Results: During cooling and rewarming, the TM, NP and oral temperatures changed more rapidly than the rectal and finger temperatures. In TM temperature, as time changed, the percent changes of temperature varied more prominently in the CO2 gas group than in the Air group. The statistical differences were significant especially during 13 minutes to 20 minutes in cooling and 8 minutes to 20 minutes during rewarming. Conclusion: From this data, we can postulate the effect of CO2 gas on CBF indirectly by measuring several sites of body temperatures.

7.Efficacy of Fluvastatin in Patients with Hypercholesterolemia

Background: Fluvastatin is the first entirely synthetic 3-hydroxy-3-methylglutaryl-coenzyme A(HMG-CoA) reductase inhibitor. Clinical data indicate that this agent exhibits the proven efficacy of its class and also has some theoretical advantages in safety for long-term use because of its unique pharmacololgic property consistent with hepatoselectivity(i.e., low systemic exposure). This study is to evaluate efficacy and safety of fluvastatin in hypercholesterolemic patients in Korea. Methods: An open clinical trial with fluvastatin was conducted in 31 subjects who continued to have high blood cholesterol levels of 6.21 mmol/L(240 mg/dl) or greater after 1 month of lipid-lowering diet plus single blind placebo period. Fluvastatin was administered for 8 weeks with the initial dose of 20 mg per day and if serum cholesterol levels did not fall below 5.20 mmol/L(200 mg/dl) after 4 weeks the dose was increased to 40 mg per day for the second 4 weeks. 8.A Case of Kaposi's Sarcoma of the Stomach.

Kaposi's sarcoma is a rare tumor comprising 0.1 per cent of all malignancies worldwide. There is, however, an increased ineidence following renal transplantation, immunosupression and in the acquried immunodeficency syndrome(AIDS) Kaposi's sarcoma has been shown to involve every organ of the body except the brain. Gastrointesinal involvement is the most common extracutaneous site of involvement. Gaatrointesinal Kaposis sarcoma is preaent in approximately half of patients with cutaneous Kaposis sarcoma and the acquired immune deficiency syndrome(AIDS). Although usually asymptomatic, gastrointestinal Kaposi's sarcoma may cause pain, bleeding, diarrhea, obstruction, intussusception, perforation. malabsorption, and protein-losing enteropathy. Three distinct gastroscopic appearances of Kaposi's sarcoma have been described: maculopapular, polypoid, and umbilicated nodular lesions. We report a case of Kaposis sarcoma of the stomach.

9.A Case of Metastatic Melanoma of the Stomach.

The potential of malignant melanoma to metastasize to all parts of the body is well known. Metastatic melanoma of the stomach may present with vague gastrointestinal symptoms, abdominai pain, or gastrointestinal bleeding. When gastrointestinal symptoms occur in a patient with known melanoma, gastric metastases should be considered. Melanoma metastatic to the stomach develops multiple small ulcerating masses. These sharply delineated submucosal lesions have been described as having a "bull's eye" or "target" configuration. Barium X-ray study, endoscopy, cytology study, and biopsy may yield the diagnosis. The prognosis is poor. We report a case of metastatic melanoma of the stomach with the review of the literature.

10.A Case of Zosteriform Cutaneous Metastasis.

We report a case of zosteriform cutaneous metastasis from lung cancer in a 72- year-old male patient who have asymptomatic confluent or discrete hard papulonodules over the right upper arm and right side of chest. Histopathologic examinations of the cutaneous lesion revealed neoplastic cell, which showing glandular structure, infiltrating in the drmis and subcutaneous layer. Also histopathologic findings of metastatic lesion of skull and olecranon are similar to them. Fie died after three months from the appearance of cutaneous metastasis
