1.Significance of Ultrastructural Electron Dense Deposits on Glomerular Capillary Loops in IgA Nephropathy.

To evaluate the clinical and histopathological significance of electron dense deposits on capillary in IgA nephropathy, we reviewed and compared the clinical, laboratory, and pathological features of the patients with IgA nephropathy without loop extension of electron dense deposits(Group I, 91 cases) and IgA nephropathy with loop extension(Group II, 17cases) by ultrastructural examination using transmission electron microscope. IgA nephropathy associated with liver disease, Henoch-Schonlein purpura, systemic lupus erythematosus and the other IgA nephropathies associated with systemic diseases were excluded. The results were as follows; 1) There was no significant difference in age distribution. 2) Generalized edema was more common in group II. 3) Nephrotic ranged proteinuria(>3 g/24hr urine) was more prominent in Group II(52.9%) than Group I(8.8%). 4) Among the groups, segmental or mild deposits on the loops were noted in 13 cases, and severe and generalized deposits in 4 cases. Subendothelial deposits were noted in 6 cases, subepithelial deposits in 3 cases, subendothelial with intramembranous deposits in 1 case, subendothelial with subepithelial deposits in 1 case, intramembranous with subepithelial deposits in 2 cases, and subendothelial, subepithelial and intramembranous deposits in 4 cases. 5) The other associated ultrastructural changes of group II were diffuse effacement of foot processes with microvillous transformation, swelling or vacuolar degeneration of podocytes and glomerular endothelium. 6) According to the WHO morphologic criteria, the grade of Group II was significantly higher than Group I. From the above results, it can be concluded that the extension of electron dense deposits along the capillary loops in the cases of IgA nephropathy is highly correlated with proteinuria in the nephrotic ranged. It seems to be a poor prognostic indicator in view of the facts that it correlats with high histopathologic grading.

6.A Questionnaire Study Concerning Circumcision of Elementary School Boys in the City of Gwangju.

PURPOSE: Circumcision is widely practiced in Korea, but its implementation is still controversial, especially among younger parents. Using a questionnaire study, we tried to determine the rate of circumcision among elementary school boys and to determine parental concerns about phimosis and circumcision. We then compared our findings with previously published literature. MATERIALS AND METHODS: Between May and June 2007, we randomly selected five elementary schools and distributed self-administered questionnaires to the parents of 2,712 boys from these schools. Questions were related to children's circumcision status, age at the time of circumcision, and opinions concerning circumcision, such as its necessity and benefits. Completed questionnaires were collected and analyzed. RESULTS: The response rate was 66.2%. The overall circumcision rate of boys in elementary school was 15.4%. Some 40.3% of those boys underwent circumcision in the neonatal period, and 37.3% underwent the procedure between ages 9 and 12 years. The circumcision rate was significantly higher in boys in the higher income bracket group(29.5%) compared to boys in the middle and low income bracket groups(15.4% and 12.8%, respectively). Most boys were circumcised based on the intent of their parents(64.2%). Some 44.5% of parents believed that circumcision was necessary, while 36.3% of parents believed it was not necessary. The most common motive for circumcision was "hygiene benefits"(80.2%). CONCLUSIONS: The rates of actual circumcision practice and of approval of circumcision were lower in this report compared to previous reports. It seems that more clinical research on circumcision is needed. Physicians should provide information about circumcision to boys and their parents.

7.Expression of Sialosyl Tn Mucin Antigen in Gastric Adenocarcinoma and Its Relationship with Prognostic Factors.

Sialosyl Tn mucin antigen (STn) is a carbohydrate antigen of tumor associated mucin formed by the premature 2~6 sialation of N-acetylgalactosamine. STn has been expressed in several tumor types and showed prognostic significance in colonic carcinoma. The authors evaluated the expression of STn immunohistochemically and correlated its expression with clinicopathologic variables in 100 gastric cancers. In early gastric cancer, STn was expressed in 24 cases out of 50 cases (48%). In advanced gastric cancer, STn was expressed in 48 of 50 (96%). The difference in STn expression between advanced gastric cancer and early gastric cancer was statistically significant. The difference in STn expression between tumors with lymph node metastasis and those without lymph node metastasis, between tubular adenocarcinoma and signet ring cell carcinoma, and between intestinal type and diffuse type adenocarcinoma was statistically insignificant in early or advanced gastric adenocarcinoma. These results suggest that the STn expression plays a role in the tumor progression in both early and advanced gastric adenocarcinomas.

8.Serum Leptin Levels in Children with Type I Diabetes Mellitus .

PURPOSE: Leptin is a highly hydrophobic 16-kDa protein encoded by the ob gene which is expre- ssed in adipocytes and participates in the regulation of food intake and energy expenditure. The aims of the present study were to examine the comparison of leptin levels between type I diabetes mellitus (DM) patients who received exogenous insulin therapy and healthy children, and to find out correlating factors with leptin level in type I DM. METHODS: The study was conducted on 34 patients with type I DM and 29 healthy children. Body weight, height, leptin level and body fat percent were measured in both groups. The glycosylated hemoglobin value, C-peptide, insulin level and serum cholesterol were also measured in type I DM. RESULTS: There was no statistically significant difference in mean age, sex distribution and body mass index between the experimental groups, but body fat percent in type I DM was significantly lower than that of healthy children. The mean leptin levels in male (9.4+/-6.3ng/mL) and female (17.8+/-10.2ng/mL) type I DM were significantly higher than in male(6.3+/-5.7ng/mL) and female (10.2+/-0.9ng/mL) healthy children (male : P=0.045, female : P=0.005). Serum leptin levels were strongly correlated with the body fat percent (Pearson coefficient) (r=0.712). CONCLUSION: Serum leptin level in type I DM strongly correlated with the body fat percent. Despite lowered body fat percent in type I DM, the leptin level was significantly higher than values in healthy children. The mean leptin level in type I DM were significantly higher than value in healthy children, but body fat percent of type I DM was lower than value in healthy children who were adjusted for sex, age and body mass index.
